RESUMO

This study validated abbreviated methods for the presumptive identification of Staphylococcus lugdunensis and studied the antibiotic susceptibilities of 106 isolates. The combination of positive responses to ornithine and pyrrolidonyl arylamidase identified all S. lugdunensis isolates. Resistance to penicillin and methicillin was detected in 27 and 5% of isolates, respectively.

OBJECTIVE: To investigate the molecular epidemiology of carbapenem-resistant Acinetobacter baumannii-calcoaceticus complex isolates in a tertiary care hospital where the prevalence of carbapenem resistance among these organisms is high. DESIGN: The study was a prospective, observational study performed during an 8-month period (May 1 through December 31, 2004). A. baumannii isolates recovered from all clinical samples during the study period were included in the study. Antibiotic susceptibility testing was performed using the disk diffusion method, and all carbapenem-resistant strains were typed by a polymerase chain reaction-based typing method.Setting. An 800-bed hospital in Singapore. RESULTS: More than half of recovered isolates were clonally unrelated, with the remaining isolates grouped into 4 genotypes. CONCLUSIONS: The results of the study suggest that the high prevalence of carbapenem resistance among Acinetobacter organisms in this institution is not caused by the spread of a predominant clone and that other factors may need to be investigated.

The provision of rapid results from positive blood cultures is important for the clinical management of septicaemia. This study tested the accuracy of direct inoculation of biochemical tests from positive blood culture vials for the identification of members of the Enterobacteriaceae and Acinetobacter species. A hundred and eighty-one samples were included in the study, with 25 % subsequently excluded as a result of mixed colonial growth. The study method successfully identified 133 (98 %) isolates from 136 vials to genus level and was technically simple to perform, requiring an additional 3 min for the processing of each positive vial. The results of this study demonstrate that a direct inoculation method provides acceptable genus identification of Gram-negative bacilli in positive blood culture vials, with a potential saving of 24 h compared with traditional methods.

RESUMO

Laboratory criteria were used to select coagulase-negative staphylococci isolated from nonsterile body sites for further investigation. Fifty-seven percent of the study isolates were clinically significant, predominantly causing community-acquired soft tissue infections. There were species-related differences in virulence, with 91% of Staphylococcus lugdunensis isolates clinically significant compared with 11% of S. haemolyticus isolates.

Five cases of bacteraemia with Arcanobacterium haemolyticum in association with soft-tissue infections are reported from immunocompetent patients. A review of all A. haemolyticum isolates from our institution over a three-year period reveals that 94% were cultured from patients with soft-tissue infections, and 72% of all patients had concomitant diabetes mellitus. Arcanobacterium haemolyticum is an uncommon cause of bacteraemic soft-tissue infections, and diabetes mellitus may be an associated risk factor.
