RESUMO

Rotavirus vaccines work better in developed countries than in developing countries, leading to the question of whether the circulating strains are different in these two settings. In 2008, a clinical trial of the pentavalent rotavirus vaccine was performed in Nha Trang, Vietnam, in which the efficacy was reported to be 64 %. Although samples were collected independently from the clinical trial, we examined faecal specimens from children hospitalised for rotavirus diarrhoea and found that G3P[8] and G1P[8] were co-dominant at the time of the clinical trial. The aim of this study was to explore whether they were divergent from the strains circulating in the developed countries where the vaccine efficacy is high. Two G3P[8] and two G1P[8] strains that were regarded as representatives based on their electropherotypes were selected for full-genome sequencing. The genotype constellation was G1/G3-P[8]-I1-R1-C1-M1-A1-N1-T1-E1-H1. All but the VP4 genes, one of which belonged to the emerging P[8]b genotype (OP354-like VP4), clustered into one or more lineages/alleles with the strains circulating in developed countries, with ≥97.5 % nucleotide sequence identity. Additionally, 10 G1 and 12 G3 VP7 sequences as well as 31 VP4 sequences were determined. No amino acid differences were observed between the Vietnamese strains and strains in the developed countries that were likely to have affected the neutralisation specificity of their VP7 and VP4. In conclusion, apart from prevalent P[8]b VP4, virtually no differences were observed between the predominant strains circulating in Vietnam at the time of the clinical trial and the strains in the developed countries; hence, the lower vaccine efficacy was more likely to be due to factors other than strain divergence.

RESUMO

UNLABELLED: To describe a case of isolated infraorbital mass which had been present for the past 9 years in a young woman. Despite the size, the mass was successfully excised in total. METHOD: Case report. RESULT: Histologically, the tumour was found to be an isolated orbital myxoma. CONCLUSION: Isolated orbital myxoma is an exceedingly rare tumour of the orbit. The case shows the lesion could be excised in its entirety with good cosmetic result.

RESUMO

Corneal perforation is an ocular emergency that requires early intervention to avoid permanent visual loss from endophthalmitis or irreversible structural changes. Although tectonic keratoplasty is the best choice for patching a large perforation, a donor cornea is not always immediately available. Consequently, an alternative material is required while awaiting a donor cornea. We report the use of preserved bovine pericardium as a temporizing graft to maintain the ocular integrity in two patients with corneal perforation.

RESUMO

A 3-year-old girl with facial dysmorphic features suggestive of Cornelia de Lange syndrome was seen in the ophthalmology unit for a right leukocoria. The leukocoria was found to be caused by a large retinoblastoma and the right eye was enucleated. Chromosomal analysis revealed partial chromosome 13q deletion involving band 14 which is associated with a high risk of retinoblastoma. This case shows that patient with chromosome 13q deletion syndrome cannot be diagnosed based on dysmorphic features only. Chromosomal analysis is warranted in all infants with facial dysmorphism suggestive of Cornelia de Lange syndrome so that those with chromosome 13q deletion can be referred early for early detection of retinoblastoma.

Two experiments produced evidence that category relationships contribute to similarity ratings. In Experiment 1, participants gave similarity ratings with respect to a semantic category (VEGETABLE) and a set of exemplars, some of which were members of the category (e.g., BROCCOLI) and some of which were not (e.g., BANANA). A regression analysis was used to predict the similarity ratings in terms of numbers of common and distinctive features, as reported by other participants. Perceived similarity was greater for examplars that were members of the category, independently of feature overlap. Experiment 2 examined similarity ratings with respect to pairs of exemplars. In some cases, both exemplars were members of the same category (e.g., BROCCOLI/CUCUMBER). In other cases, one exemplar was a member of the category and the other was not (e.g., BROCCOLI/BANANA). A regression analysis was used to predict the similarity ratings in terms of numbers of common and distinctive features. Perceived similarity was greater when both exemplars were members of the same category, independently of feature overlap.

The range over which cells are sensitive to changes in oxygen concentration remains uncertain. Wilson and colleagues [Wilson, D.F. (1994) Med. Sci. Sports Exerc. 26, 37-43] have suggested that cytochrome oxidase is sensitive to oxygen concentrations below about 40 microM, but proposed that this sensitivity is obscured in intact cells because an increase in reduction state of cytochrome c acts to maintain oxygen consumption. We have tested this hypothesis in platelets, which are small cells (2-4 micrometer diameter, < 0.5 micrometer thick) that do not decrease their rate of oxygen consumption until oxygen concentrations fall below 2.5 microM. Contrary to the expectations of the hypothesis, the reduction state of cytochrome c, the concentration of NADH and the rate of glycolytic output are not changed as oxygen concentration declines from 40 microM down to 5 microM. Therefore, we conclude that at least some cell types contain mitochondria that are not capable of sensing oxygen above 5 microM by the mechanism proposed by Wilson and colleagues.

A system is described for the perifusion of cells with a perifusate containing oxygen at concentrations defined by the user. The metabolic competency of cells in this system was assessed by measuring total ATP turnover for human platelets. Human platelets, which are small (radius of 1-2 micron) anucleate cells involved in blood clotting, maintained ATP turnover over a 4-h period at 37 degrees C. Oxygen concentration in this system was controlled by adjusting the proportions of air-saturated and nitrogen-saturated perifusates. Examples of oxygen consumption and lactate output as a function of oxygen concentration are described.
