RESUMO

A breastfeeding mother is confronted with a wide variety of over-the-counter products, necessitating careful guidance from her health care professional. This article presents a discussion and comprehensive tables that cover, by category, over-the-counter medications that nursing mothers may need to take. These tables should greatly assist the health care professional in the important task of counseling the nursing mother about the proper use of over-the-counter medications. This information will enable health care professionals to interpret information on the topic of over-the-counter medications and breastfeeding; list medications, in specific over-the-counter categories, that are usually safe to take while breastfeeding; recommend over-the-counter medications of choice for a breastfeeding mother; and generally counsel a breastfeeding mother who is taking or expects to take over-the-counter medications.

RESUMO

A study was conducted to determine the organizational and professional values held by male and female physicians, nurses, and pharmacists serving in the U.S. Public Health Service. An existing instrument was modified to measure health professionals' organizational values and professional values. These values were analyzed by multivariate and multiple regression techniques to ascertain the relationship the independent variables sex, age, marital status, race, and practice type of the health professionals had with the levels of each measurement. Sex, age, marital status, race, and practice type appeared to affect the values held in varying degrees. Differences between organizational and professional values held by the health professionals will have an effect on future roles for officers in the U.S. Public Health Service. There will also be an impact on society as the nation decides the future direction for health care.

RESUMO

Felbamate (FBM) is a novel antiepileptic drug (AED) currently undergoing clinical evaluation in the United States. During a controlled clinical trial conducted at the National Institutes of Health Clinical Center, FBM was added to constant carbamazepine (CBZ) monotherapy. CBZ total concentrations were reduced during active FBM treatment (mean reduction 25%, range 10-42%, p less than 0.001). The effect was evident after the first week of treatment and reached a plateau in 2-4 weeks. To clarify the interaction mechanism, free and total concentrations of CBZ and its plasma metabolites were determined by high-performance liquid chromatography (HPLC) and ultrafiltration in four patients. In these patients, FBM treatment reduced CBZ concentrations and increased CBZ-epoxide (CBZ-E) concentrations (p less than 0.01). Free fractions of all compounds were unmodified. FBM appears to be capable of inducing CBZ metabolism. CBZ-FBM interaction may be clinically relevant.

We withdrew phenytoin from 17 inpatients maintained on combination therapy with carbamazepine for complex partial seizures and analyzed seizure occurrence in relation to plasma levels and time from initiation of withdrawal. The ratio of maximum to mean weekly seizure frequency did not vary with initial level or rate of withdrawal. The week with most frequent seizures began a median of 10 days after phenytoin levels became undetectable, and mean daily seizure frequency was higher at undetectable than at falling levels for the entire 2- to 10-week study period. Four patients had a total of 6 clusters of generalized tonic-clonic seizures; only 2 occurred while levels were falling and the other 4 at 3, 9, 28, and 42 days after reaching undetectable levels. Our data argue against the occurrence of withdrawal seizures in these patients and suggest that worsening of seizures following phenytoin discontinuation more likely reflects loss of therapeutic drug effect than a true abstinence phenomenon.

We studied the relation of plasma levels of carbamazepine (CBZ) and carbamazepine 10,11 epoxide (CBZ-E), and their ratio to drug toxicity and seizure control in 7 patients with complex partial seizures. CBZ-E/CBZ increased with increasing CBZ levels and was higher when patients were taking phenytoin or valproic acid. There were weak correlations between CBZ, CBZ-E levels, toxicity scores, and seizure control when patients were taking CBZ alone, but not when other drugs were given as well. There were no significant differences in the correlation with toxicity score or seizure frequency between CBZ, CBZ-E, or their sum. Measurement of CBZ-E levels did not provide additional information useful for monitoring clinical response to CBZ therapy.
