RESUMO

A novel image-reconstruction method is proposed for the processing of data acquired at random spatial positions. The images are reconstructed and updated in real time concurrently with the measurements to produce an evolving image, the quality of which is continuously improving and converging as the number of data points increases with the stream of additional measurements. It is shown that the images converge to those obtained with data acquired on a uniformly sampled surface, where the sampling density satisfies the Nyquist limit. The image reconstruction employs a new formulation of the method of scattered power mapping (SPM), which first maps the data into a three-dimensional (3D) preliminary image of the target on a uniform spatial grid, followed by fast Fourier space image deconvolution that provides the high-quality 3D image.

BACKGROUND: Postpartum hemorrhage (PPH) is the leading cause of maternal death worldwide. The World Maternal Antifibrinolytic trial showed that antifibrinolytic tranexamic acid (TXA) reduces PPH deaths. Maternal anemia increases the risk of PPH. The World Maternal Antifibrinolytic-2 trial is now assessing whether TXA can prevent PPH in women with anemia. Low red blood cell (RBC) counts promote fibrinolysis by altering fibrin structure and plasminogen activation. OBJECTIVES: We explored interactions between RBCs and TXA in inhibiting fibrinolysis. METHODS: We used global fibrinolytic assays (ball sedimentation and viscoelasticity) to monitor the lysis of fibrin containing plasminogen and tissue-type plasminogen activator. We applied a fluorogenic kinetic assay to measure plasmin generation in fibrin clots and scanning electron microscopy to study fibrin structure. RESULTS: According to parallel-line bioassay analysis of the fibrin lysis-time data, the antifibrinolytic potency of 4-128 µM TXA was increased in the presence of 10% to 40% (v/v) RBCs. Global fibrinolysis assays showed that the joint effect of RBCs and TXA was about 15% larger than the sum of their individual effects in the inhibition of fibrinolysis. In plasminogen activation, TXA added the same increment of inhibition to the effect of RBCs at any cell count in the fibrin clot. Regarding fibrin structure, TXA thickened fibrin fibers, which impaired plasminogen activation, whereas RBCs promoted fine fibers that were more resistant to plasmin. CONCLUSIONS: The antifibrinolytic potency of TXA is enhanced in fibrin formed in the presence of RBCs through inhibition of plasminogen activation and fibrin lysis, which correlates with modifications of fibrin structures.

We present the design and the performance evaluation of a new interconnect for large-scale densely packed electronically scanned antenna arrays that utilize a high-speed digital board-to-board vertical connector. The application targets microwave tissue, imaging in the frequency range from 3 GHz to 8 GHz. The tissue-imaging arrays consist of hundreds of active antenna elements, which require low-reflection, low-loss, and low-crosstalk connections to their respective receiving and transmitting circuits. The small antenna size and the high array density preclude the use of coaxial connectors, which are also expensive and mechanically unreliable. Modern board-to-board high-speed connectors promise bandwidths as high as 12 GHz, along with high pin density, mechanical robustness, and low cost. However, their compatibility with the various transmission lines leading to/from the miniature printed antenna elements and microwave circuitry is not well studied. Here, we focus on the design of the transitions from coplanar waveguide transmission lines to/from a high-speed vertical connector. The performance of the interconnect is examined through electromagnetic simulations and measurements. Comparison is carried out with the expensive sub-miniature push-on sub-micro coaxial connectors commonly used in miniature radio-frequency electronics. The results demonstrate that high-speed vertical connectors can provide comparable performance in the UWB frequency range.

INTRODUCTION: The composition of thrombi determines their structure, mechanical stability, susceptibility to lysis, and consequently, the clinical outcome in coronary artery disease (CAD), acute ischemic stroke (AIS), and peripheral artery disease (PAD). Fibrin forms the primary matrix of thrombi intertwined with DNA, derived from neutrophil extracellular traps (NETs), and von Willebrand factor (VWF) bridging DNA and platelets. Here we examined the relative content of fibrin, DNA and VWF in thrombi and analyzed their interrelations and quantitative associations with systemic biomarkers of inflammation and clinical characteristics of the patients. PATIENTS, METHODS: Thrombi extracted from AIS (n = 17), CAD (n = 18) or PAD (n = 19) patients were processed for scanning electron microscopy, (immune)stained for fibrin, VWF and extracellular DNA. Fibrin fiber diameter, cellular components, fibrin/DNA and fibrin/VWF ratios were measured. RESULTS: Patients' age presented as a strong explanatory factor for a linear decline trend of the VWF content relative to fibrin in thrombi from CAD (adjusted-R2 = 0.43) and male AIS (adjusted-R2 = 0.66) patients. In a subgroup of CAD and PAD patients with dyslipidemia and high (above 80%) prevalence of atherothrombosis a significant correlation was observed between the VWF and DNA content in thrombi (adjusted-R2 = 0.40), whereas a 3.7-fold lower linear regression coefficient was seen in AIS patients, in whom the fraction of thrombi of atherosclerotic origin was 57%. Independently of anatomical location, in patients with atherosclerosis the VWF in thrombi correlated with the plasma C-reactive protein levels. CONCLUSIONS: The observed interrelations between thrombus constituents and systemic inflammatory biomarkers suggest an intricate interplay along the VWF/NET/fibrin axis in arterial thrombosis.

INTRODUCTION: Management of vascular infections represents a major challenge in vascular surgery. The use of cryopreserved vascular allografts could be a feasible therapeutic option, but the optimal conditions for their production and use are not precisely defined. AIMS: To evaluate the effects of cryopreservation and the duration of storage on the thrombogenicity of femoral artery allografts. METHODS: In our prospective study, eleven multi-organ-donation-harvested human femoral arteries were examined at five time points during storage at -80°C: before cryopreservation as a fresh native sample and immediately, one, twelve and twenty-four weeks after the cryopreservation. Cross-sections of allografts were perfused with heparin-anticoagulated blood at shear-rates relevant to medium-sized arteries. The deposited platelets and fibrin were immunostained. The thrombogenicity of the intima, media and adventitia layers of the artery grafts was assessed quantitatively from the relative area covered by fibrin- and platelet-related fluorescent signal in the confocal micrographs. RESULTS: Regression analysis of the fibrin and platelet coverage in the course of the 24-week storage excluded the possibility for increase in the graft thrombogenicity in the course of time and supported the hypothesis for a descending trend in fibrin generation and platelet deposition on the arterial wall. The fibrin deposition in the cryopreserved samples did not exceed the level detected in any of the three layers of the native graft. However, an early (up to week 12) shift above the native sample level was observed in the platelet adhesion to the media. CONCLUSIONS: The hemostatic potential of cryopreserved arterial allografts was retained, whereas their thrombogenic potential declined during the 6-month storage. The only transient prothrombotic change was observed in the media layer, where the platelet deposition exceeded that of the fresh native grafts in the initial twelve weeks after cryopreservation, suggesting a potential clinical benefit from antiplatelet therapy in this time-window.

Millimeter wave (MMW) technology is expanding rapidly into security screening for dangerous items concealed under clothing. It uses safe non-ionizing radiation and penetrates clothing well. We present a new planar system at Ka band for the three-dimensional simultaneous imaging of both sides of an inspected person where the images are produced in real time by a recently proposed generalized holographic reconstruction algorithm. Low-cost linear frequency modulation (LFM) radar technology is used along with a simple but efficient method for system calibration. Experimental characterization of the spatial resolution and the sensitivity of the system prototype has been carried out. It is established that the achieved spatial resolution is 6 mm or better if the item is not obscured by clothing and it may deteriorate to 7 mm depending on the clothing hiding the item. The spatial sensitivity is confirmed to be at least 2 mm.

A prototype of a bias-switched active sensor was developed and measured to establish the achievable dynamic range in a new generation of active arrays for microwave tissue imaging. The sensor integrates a printed slot antenna, a low-noise amplifier (LNA) and an active mixer in a single unit, which is sufficiently small to enable inter-sensor separation distance as small as 12 mm. The sensor’s input covers the bandwidth from 3 GHz to 7.5 GHz. Its output intermediate frequency (IF) is 30 MHz. The sensor is controlled by a simple bias-switching circuit, which switches ON and OFF the bias of the LNA and the mixer simultaneously. It was demonstrated experimentally that the dynamic range of the sensor, as determined by its ON and OFF states, is 109 dB and 118 dB at resolution bandwidths of 1 kHz and 100 Hz, respectively.

INTRODUCTION: Fibrin structure and cellular composition of thrombi profoundly affect the clinical outcomes in ischemic coronary and peripheral artery disease. Our study addressed the interrelations of structural features of thrombi and routinely measured laboratory parameters. MATERIALS AND METHODS: Thrombi removed by thromboaspiration following acute myocardial infarction (n=101) or thrombendarterectomy of peripheral arteries (n=50) were processed by scanning electron microscopy and immunostaining for fibrin and platelet antigen GPIIb/IIIa to determine fibrin fibre diameter and relative occupancy by fibrin and cells. Correlations between the structural characteristics and selected clinical parameters (age, sex, vascular localization, blood cell counts, ECG findings, antiplatelet medication, accompanying diseases, smoking) were assessed. RESULTS: We observed significant differences in mean fibre diameter (122 vs. 135 nm), fibrin content (70.5% vs. 83.9%), fluorescent fibrin/platelet coverage ratio (0.18 vs. 1.06) between coronary and peripheral thrombi. Coronary thrombi from smokers contained more fibrin than non-smokers (78.1% vs. 62.2% mean occupancy). In the initial 24 h, fibrin content of coronary thrombi decreased with time, whereas in peripheral thrombi platelet content increased in the first 7 days. In coronaries, higher platelet content and smaller vessel diameter were associated with thinner fibrin fibres, whereas hematocrit higher than 0.35 correlated with larger intrathrombotic platelet occupancy. Smoking and dyslipidaemia strengthened the dependence of clot platelet content on systemic platelet count (the adjusted determination coefficient increased from 0.33 to 0.43 and 0.65, respectively). CONCLUSION: Easily accessible clinical parameters could be identified as significant determinants of ultrastructure and composition of coronary and peripheral thrombi.

This paper reports the progress toward a fast and reliable microwave imaging setup for tissue imaging exploiting near-field holographic reconstruction. The setup consists of two wideband TEM horn antennas aligned along each other's boresight and performing a rectangular aperture raster scan. The tissue sensing is performed without coupling liquids. At each scanning position, wideband data is acquired. Then, novel holographic imaging algorithms are implemented to provide three-dimensional images of the inspected domain. In these new algorithms, the required incident field and Green's function are obtained from numerical simulations. They replace the plane (or spherical) wave assumption in the previous holographic methods and enable accurate near-field imaging results. Here, we prove that both the incident field and Green's function can be obtained from a single numerical simulation. This eliminates the need for optimization-based deblurring which was previously employed to remove the effect of realistic non-point-wise antennas.

This study compared the inconsistent responding validity scales of the Personality Assessment Inventory (PAI) and the Psychopathic Personality Inventory (PPI)/PPI-Revised (PPI-R) in two correctional samples to determine the extent to which they overlap in identifying invalid profiles. Results revealed substantial differences in the way the inconsistent responding validity scales of these measures performed. In particular, the PAI identified far fewer participants as having responded inconsistently compared with the PPI/PPI-R. We discuss the implications of our findings for clinical practice, and potential concerns with the use of a single measure to identify inconsistent responding in clinical practice and research.

We undertook a secondary data analysis to study issues relevant to co-occurring mental health and substance disorder in a combined sample of offenders (N = 3,197). Using the Personality Assessment Inventory, we compared the frequency of depressive, traumatic stress, and personality disorder symptom elevations across offenders with and without substance problems, identified the extent to which co-occurring problems were accompanied by risk factors for suicide and aggression, and tested for gender differences. Offenders with substance problems were more likely than others to have increased mental health problems and risk factors for suicide or aggression. Women with substance problems, compared with men, had higher depression, traumatic stress, and borderline features, in addition to lower antisocial features. The frequency with which suicide and aggression risk factors were associated with mental health problems was generally similar across men and women. Measurement issues relevant to co-occurring disorder and risk assessment are discussed.

Progress curve analysis is a convenient tool for the characterization of enzyme action: a single reaction mixture provides multiple experimental measured points for continuously varying amounts of substrates and products with exactly the same enzyme and modulator concentrations. The determination of kinetic parameters from the progress curves, however, requires complex mathematical evaluation of the time-course data. Some freely available programs (e.g. FITSIM, DYNAFIT) are widely applied to fit kinetic parameters to user-defined enzymatic mechanisms, but users often overlook the stringent requirements of the analytic procedures for appropriate design of the input experiments. Flaws in the experimental setup result in unreliable parameters with consequent misinterpretation of the biological phenomenon under study. The present commentary suggests some helpful mathematical tools to improve the analytic procedure in order to diagnose major errors in concept and design of kinetic experiments.

Nonradiating electromagnetic sources are sources whose field is identically zero outside of their volume. They are undetectable unless the observation point is in direct contact with them. They are the basis of the theory of source equivalence, which studies the field invariance with respect to source transformations. In this work, we focus on the equivalent source transformations in a nonuniform medium and the implications in the theory of the electromagnetic vector potentials. We identify three types of nonradiating sources. Subsequently, we define the mathematical transformations of the sources, which preserve the field outside of their support (source invariance). We give complimentary expressions, which preserve the field inside the source support as well. We show that the nonuniqueness of the electromagnetic potentials is due to the nonunique solution to the inverse problem. The well known field gauge invariance follows from its source invariance. Also, the gauge-invariant transformation appears to be just one possibility in an infinite set of field-invariant vector-potential representations all related to the respective equivalent source transformations.
