Las lesiones por humedad representan un importante y grave problema en personas encamadas durante periodos de tiempo prolongados. En las unidades de cuidados intensivos, dichas lesiones suponen un importante reto para los profesionales de enfermería. Su prevención y tratamiento temprano con apósitos específicos es una tarea importante para disminuir la morbilidad del enfermo en cuidados intensivos. (AU)

In intensive care units, these injuries represent an important challenge for nursing professionals. Background: The continuous increase of people with chronic diseases is one of the greatest challenges for healthcare systems worldwide. Population growth and life expectancy means that an increasing number of people with chronic diseases and dependency need some kind of assistance to meet their needs. Determining these subjective unmet needs helps to understand the situation of these people. The aim of this study was to explore the perceptions of chronic patients over 65 years of age from the day-care center toward subjective health needs that are not being met by the socio-health system. Methods: Qualitative exploratory-descriptive study. Through convenience sampling, we selected people with chronic diseases and dependency who used day-care centers and met the inclusion criteria. Focus groups were performed. The data were transcribed and a thematic analysis was carried out using Atlas.ti software. Results: The topics resulting from the analysis were classified into dissatisfaction of biological/physiological needs, psychological needs, social needs, and other issues that arose in both groups of participants which referred to the types of needs previously indicated. The issues related to social and psycho-social needs stood out. Conclusions: People with chronic diseases and dependency have their physiological needs covered with the help they receive, but their situation of dependency generates additional costs that worsen their economic situation. However, their greatest need is due to the loneliness they feel and the feeling they have of "being a burden" on their families.

Constitutional complex chromosomal rearrangements (CCRs) are rare events that typically involve 2 or more chromosomes with at least 3 breakpoints and can result in normal or abnormal phenotypes depending on whether they disturb the euchromatic neighborhood. Here, we report an unusual balanced CCR involving chromosomes 1, 9, and 10 that causes an unbalanced karyotype in a severely affected toddler. The CCR was initially reported as a maternal 2-way translocation but was reclassified as a 3-way translocation after a microarray analysis of the propositus revealed the involvement of another chromosome not identified by G-banding in his phenotypically normal mother. FISH assays on maternal metaphase cells confirmed that the 1qter region of der(1) was translocated to der(10), whereas the 10qter segment was translocated to der(9), which in turn donated a segment to der(1). Subsequently, this CCR was also identified in her phenotypically normal father (the patient's grandfather). Thus, the patient inherited the previously unreported pathogenic combination of der(1) with a loss of 1q43→qter (including AKT3, ZBTB18, HNRNPU, and SMYD3) and der(9) with a gain of 10q25.2→qter (including FGFR2), leading to a compound phenotype with key features of the 1q43→qter deletion and distal 10q trisomy syndromes. Our observations suggest that the loss of SMYD3 accounts for cardiac defects in a subset of patients. Moreover, due to recurrent miscarriages in this family, our findings allowed improved genetic counseling.

Background: Assuring quality training for future nursing professionals is essential to preserving population health and socio-economic development. Quality assurance in the European Higher Education Area places students in a leading role to transform and improve higher education programs. Therefore, an innovative way of reviewing strengths and weaknesses of the nursing education program of a Spanish university has been developed. Objectives: The aim of this paper was to explore the perceptions and opinions of nursing students and newly-qualified nurses regarding the contents of the nursing curriculum in order to improve its quality. Methods: Descriptive and exploratory qualitative research was carried out involving 12 newly-qualified nurses and 12 student nurses. Semi-structured interviews and focus groups were performed. Results: Based on the thematic analysis, two themes emerged: improving clinical practices and reviewing the theoretical curriculum. Conclusions: Among the improvements suggested by the participants, the most relevant ones were establishing a clear structure of learning contents in the practicum, and redistributing the European Credit Transfer and Accumulation System ECTS credits in various courses of the study program. However, additional evidence is needed prior to proceeding with any changes.

Introducción: En el mundo, 5,4 millones de personas fallecen al año por enfermedades relacionadas con el tabaco, y es la adolescencia una de las etapas más vulnerables para desarrollar el hábito de fumar. Objetivo: Identificar factores asociados al inicio del consumo de tabaco en los estudiantes de la Pontificia Universidad Javeriana, Bogotá. Métodos: Se aplicó una encuesta de carácter voluntario, creada a partir del cuestionario de consumo de tabaco desarrollado por el Center of Desease Control and Prevention, a estudiantes de pregrado, mayores de edad, reclutados en los lugares de la Universidad más concurridos por fumadores. Resultados: La edad promedio de inicio del consumo en la muestra fue de 14,3 años (±1,9). Los factores más frecuentes fueron presión social (34,6%), curiosidad (24,7%) y estrés (17,6%). Conclusión: La poca resistencia a la presión del entorno es la principal causa del inicio del hábito de fumar.


Introduction: All over the world 5.4 million people die due to tobacco related diseases per year. Considering that teenagers are highly impressionable by media in tobacco consumption, they constitute the population of interest in order to diminish the use of tobacco and the morbidity associated with its use. Objective: To identify factors associated at the beginning of tobacco consumption. Methodology: Descriptive cross-sectional study using a poll created from a questionnaire on tobacco by Center of Disease Control and Prevention, with a sample of smokers, over 18, selected in the most popular smoking areas at Universidad Javeriana, Bogota ­ Colombia. Results: The most frequent factors related to the beginning of tobacco consumption were: Peer pressure (34.6%), curiosity (24.7%) and stress (17.6%). The average starting smoking age was 14.3 (±1.9). Conclusion: The low capacity of opposing social pressure at parties and recreational events showed as the most important factor at the beginning of a smoking habit.

RESUMO

Desde el proyecto de Cooperación Internacional para el Desarrollo "Mejora del nivel de salud de la población saharaui mediante el fortalecimiento de la formación continuada de los profesionales sanitarios y docentes", se realiza una expedición a los campamentos de refugiados saharauis, ramificada en las vertientes sanitaria, académica, institucional y sociosanitaria. Su objetivo principal es promocionar la salud de la población asentada en los campamentos. Como resultado, se obtuvo el cumplimiento del objetivo principal de la expedición, promocionando la salud a partir de la educación. La experiencia permite concluir que la responsabilidad de transmitir y concienciar al conjunto de la población recae, casi de manera exclusiva, al personal sanitario, a las promotoras de salud y al colectivo femenino en general, por lo que es necesario continuar ofreciéndoles apoyo


From the International Development Cooperation project "Improvement of the health of the Sahrawi people by strengthening the continuing education of health professionals and teachers", it made an expedition to the Saharawi refugee camps, branched takes place in the health, academic, institutional and socio aspects. Its main objective is to promote the health of the population is living in the camps. As a result, it compliance with the main objective of the expedition was obtained from promoting health education. The experience lead to the conclusion that the responsibility of transmitting and educate the whole population lies almost exclusively, medical personnel, the promoters of health and the general female population, so it is necessary to continue providing support

Para determinar la prevalencia de enteroparásitos en individuos seropositivos a los virus HTLV-I/II, se seleccionaron aleatoriamente 75 donantes que asistieron a los Bancos de Sangre de dos instituciones sanitarias del municipio Maracaibo, Venezuela. Se contactaron personalmente 50 individuos seropositivos a virus HTLV-I/II (grupo muestra) y 25 seronegativos (grupo control), quienes aportaron una muestra fecal para diagnóstico parasitológico. Estas fueron sometidas al examen al fresco con solución salina fisiológica (SSF) al 0,85%, tinción temporal de Lugol y técnica de concentración con formol-éter (Ritchie). Además se realizó cultivo en Agar Nutritivo (método de Arakaki) para investigar la presencia de larvas de Strongyloides stercoralis. Se determinó una prevalencia general de enteroparásitos en el grupo muestra de 54% y 44% en el grupo control. Predominó el monoparasitismo entre todos los individuos estudiados. Las especies identificadas fueron: Blastocystis spp. 47,3% y 15,4%, Entamoeba coli 21% y 15,4%, Endolimax nana 13% y 53,8%, Complejo Entamoeba 10,5% y 15,4%, Giardia intestinalis 5,2% y 0%, y Entamoeba hartmanni 2,6% y 0% en el grupo muestra y grupo control, respectivamente. No se detectó Strongyloides stercoralis ni otros helmintos intestinales, en los individuos estudiados. Blastocystis spp. fue el más frecuente en los pacientes seropositivos (diferencia estadísticamente significativa) que en el grupo control, lo que pudiese relacionarse con una mayor persistencia de este cromista en pacientes coinfectados con HTLV-I/II. Se puede concluir que el estatus serológico del individuo HTLV-I/II positivo parece no influir en la presencia o adquisición de parásitos intestinales.


Prevalence of intestinal parasites in seropositive and seronegative to HTLV I/II viruses


The prevalence of intestinal parasites in individuals seropositive for the HTLV-I/II virus was studied. A total of 75 donors who attended the blood banks of two health institutions in Maracaibo, Venezuela were randomly selected: 50 patients seropositive for HTLV-I/II (study group) and 25 seronegative individuals (control group). The participants in the study were all contacted personally, after which they provided a fecal sample for parasitological diagnosis. The fresh samples were examined using 0.85% physiological saline solution (PSS), Lugol’s solution for temporary staining and the formalin-ether concentration technique (Ritchie). In addition, samples were cultured on nutrient agar (Arakaki method) to determine the presence of Strongyloides stercoralis larvae. Overall prevalences of 54% intestinal parasites in the study group and 44% in the control group were found. Monoparasitism prevailed among all the individuals studied. The species identified in the study and control groups were: Blastocystis spp. 47.3% and 15.4%, Entamoeba coli 21% and 15.4%, Endolimax nana 13.0% and 53.8%, Entamoeba complex 10.5% and 15.4%, Giardia intestinalis 5.2% and 0%, and Entamoeba hartmanni 2.6% and 0 %, respectively. Strongyloides stercoralis and other intestinal helminths were not detected in the individuals studied. Blastocystis spp. was significantly more common in seropositive patients than in the control group, which could be associated with a greater persistence of this chromista in patients co-infected with HTLV-I / II.It can be concluded that the positive serological status of HTLV-I / II infected individuals does not seem to influence either the presence or acquisition of intestinal parasites.

RATIONALE: Numerous medication development strategies seek to decrease nicotine consumption and prevent relapse to tobacco smoking by blocking glutamate transmission. Decreasing glutamate release by activating presynaptic inhibitory metabotropic glutamate (mGlu)2/3 receptors inhibits the reinforcing effects of nicotine and blocks cue-induced reinstatement of nicotine-seeking behavior in rats. However, the relative contribution of mGlu2 receptors in nicotine dependence is still unknown. OBJECTIVES: The present study evaluated the role of mGlu2 receptors in nicotine-taking and nicotine-seeking behavior using the novel, relatively selective mGlu2 positive allosteric modulators (PAMs) AZD8418 and AZD8529. RESULTS: Acute treatment with AZD8418 (0.37, 1.12, 3.73, 7.46, and 14.92 mg/kg) and AZD8529 (1.75, 5.83, 17.5, and 58.3 mg/kg) deceased nicotine self-administration and had no effect on food-maintained responding. Chronic treatment with AZD8418 attenuated nicotine self-administration, but tolerance to this effect developed quickly. The inhibition of nicotine self-administration by chronic AZD8529 administration persisted throughout the 14 days of treatment. Chronic treatment with either PAMs inhibited food self-administration. AZD8418 (acute) and AZD8529 (acute and subchronic) blocked cue-induced reinstatement of nicotine- and food-seeking behavior. CONCLUSIONS: These findings indicate an important role for mGlu2 receptors in the reinforcing properties of self-administered nicotine and the motivational impact of cues that were previously associated with nicotine administration (i.e., cue-induced reinstatement of nicotine-seeking behavior). Thus, mGlu2 PAMs may be useful medications to assist people to quit tobacco smoking and prevent relapse.

OBJECTIVE: To determine the relationship between obesity, food insecurity and perceived stress in very low income Mexican American women. DESIGN: Cross-sectional baseline data analysis of a randomized clinical trial. SETTING: Texas-Mexico border region of South Texas. SUBJECTS: Very Low Income Mexican American Women. RESULTS: The relationship between obesity and food insecurity in a sample of very low income Hispanic women living in South Texas depends on the measure of obesity and the dimension of food insecurity. The only measure of food insecurity associated with all measures of obesity was often not having enough money to afford to eat balanced meals. Waist circumference was associated with the most dimensions of food insecurity, while BMI had the least associations. Finally, perceived stress was not significantly associated with BMI, waist circumference or percent body fat when adjusted for other covariates. CONCLUSIONS: We have found a strong and significant relationship between food insecurity related to having enough resources to eat a balanced diet and BMI, waist circumference, and percent body fat in low-income Mexican American women. While behavioural change is an important strategy for reducing obesity, consideration may need to be made as to how food access with high nutritional value, may be in and of itself a contributing factor in obesity in low income populations.

Se describe el reporte del caso de una paciente sana con incompetencia labial portadora de un implante dental en la zona de 1.2, a la cual se le realizó un injerto de tejido conectivo utilizando la técnica del «sobre¼ para mejorar un leve defecto estético existente. El injerto sufrió una necrosis, posiblemente debido a la condición de deshidratación a la que están sometidos los tejidos en estos pacientes que presentan incompetencia labial y respiración bucal. Se describe la resolución quirúrgica del caso mediante una segunda cirugía de injerto de tejido conectivo, y se entrega el manejo clínico para evitar la aparición de esta complicación.


We describe a case report of a patient with lip incompetence and a healthy carrier of a dental implant in the tooth 1.2, which was performed using a connective tissue graft technique for improving an existing slight cosmetic defect. The graft necrosis was possibly due to the dehydration that the tissues are subjected to in these patients with mouth breathing and labial incompetence. The surgical resolution of the case by a second connective tissue graft, and its clinical management to prevent the occurrence of this complication is presented.

El trauma dentoalveolar (TDA) constituye un conjunto de lesiones que comprometen los dientes o a sus estructuras periodontales. En Chile, desde el año 2007, la primera consulta/tratamiento de urgencia del TDA está cubierta por la Ley N 19.966, para todas las personas afiliadas al Fondo Nacional de Salud (FONASA) y a las Instituciones de Salud Previsional Privadas (ISAPRE) a través del Programa de Garantías Explícitas en Salud (GES). Escasos estudios nacionales se han realizado en TDA de adultos y ninguno en relación al impacto del GES en estas lesiones. Se realizó un estudio retrospectivo, descriptivo y transversal de TDA con los Datos de Urgencia de todos los pacientes adultos atendidos en el Hospital de Urgencia Asistencia Pública. Se compararon las variables clínicas, etiológicas, demográficas y sociales entre 2 períodos: pre-GES (1 de Julio de 2005 al 30 de Junio de 2006) y post-GES (1 de julio de 2012 al 31 junio de 2013). En los 2 períodos se observó una mayor frecuencia de TDA en el sexo masculino del grupo de 20­29 años, producidos en la mayoría de los casos por violencia interpersonal. Sin embargo se observó en el período post-GES una mayor consulta de TDA por Accidente vehículo-motorizado, presentándose lesiones de mayor gravedad. A pesar de la implementación del GES, se observó una alta frecuencia de TDA no tratados, esto podría deberse a la gravedad del estado sistémico del paciente (postergando el tratamiento de TDA), a la falta de insumos o a la inequidad en la entrega de recursos a los servicios de salud. Es necesario realizar más estudios y vigilancia de parte de la autoridad sanitaria para mejorar las garantías del GES en el tratamiento de los TDA.


Traumatic dental injury (TDI) is a group of injuries that affect hard dental tissues and/or periodontal structures. Since 2007 the first emergency treatment/consult of TDI, for both the National Health Fund (FONASA) and profit private insurer (ISAPRE) affiliates, is guaranteed in the Regulation of Explicit Health Guarantees (GES) established by Chilean Law 19.966. Few national TDI studies in adults have been carried out, and none in relation to the impact of GES in this type of lesion. A retrospective cross sectional study of emergency charts of all adult patients attended at the Hospital de Urgencia Asistencia Pública. Etiological, clinical, demographic and social variables were compared between 2 time periods, Pre-GES period (July, 1 2005 to June, 30 June 2006) versus Post-GES period (July, 1 2012 to June, 31 2013). A high incidence of TDI caused by interpersonal violence in males between 20 and 29 years old was observed in both periods. However, an increased TDI with more severe injuries caused by automobile accident was observed during the post-GES period. In spite of GES implementation, high frequency of non-treated TDI was seen in the present study, this could be due to the severity of the patient´s systemic condition (delaying the TDI treatment), a lack of resources and/or inequity in the delivery of these healthcare resources. More studies and surveillance programs by the Government are needed to improve TDI treatment guarantees, and as well as regular assessment of GES compliance.

RATIONALE: Chronic nicotine administration decreases the functioning of the cystine-glutamate antiporter system x(c)- which is hypothesized to promote nicotine-taking and nicotine-seeking behaviors. N-acetylcysteine (NAC), a cystine pro-drug, increases the activity of the cystine-glutamate antiporter system x(c)-. Thus, NAC could potentially reverse nicotine-induced alterations in glutamatergic transmission and decrease nicotine taking and seeking. OBJECTIVES AND METHODS: To test this hypothesis in the present study, the effects of acute NAC treatment (30, 60, and 90 mg/kg, i.p.) on nicotine (fixed- and progressive-ratio schedules) and food (fixed-ratio schedule) self-administration were assessed in rats. In addition, the effects of acute NAC treatment on cue-induced reinstatement of nicotine- and food-seeking behaviors were investigated. Finally, the effects of repeated daily NAC administration (60 mg/kg, i.p., 14 days) on nicotine and food self-administration were assessed. RESULTS: Acute NAC administration decreased nicotine self-administration but not food responding under a fixed-ratio schedule of reinforcement. In addition, acute NAC administration showed a nonsignificant trend in attenuating nicotine self-administration under a progressive-ratio schedule that was similar to the dose-response function under the fixed-ratio schedule. Furthermore, repeated NAC administration decreased nicotine self-administration from day 6 to 14 compared with vehicle treatment, with no indication of tolerance development. By contrast, repeated NAC administration decreased food responding from day 6 to 8 compared with vehicle treatment and showed rapid development of tolerance. Finally, acute NAC administration attenuated cue-induced reinstatement of nicotine and food seeking. CONCLUSIONS: Altogether, these findings suggest that NAC may be useful in promoting smoking cessation in humans.

The metabotropic glutamate 2/3 (mGlu2/3) receptor agonist LY379268 ([-]-2-oxa-4-aminobicyclo [3.1.0] hexane-4,6-dicarboxylate) attenuates both nicotine self-administration and cue-induced nicotine seeking in rats. In this study, the effects of LY379268 (1 mg/kg) or saline pretreatment on nicotine-induced increases in nucleus accumbens (NAcc) shell dopamine were evaluated using in vivo microdialysis under different experimental conditions: (i) nicotine (0.4 mg/kg, base) was experimenter-administered subcutaneously to nicotine-naïve rats; (ii) nicotine was experimenter-administered either subcutaneously (0.4 mg/kg) or by a single experimenter-administered infusion (0.06 mg/kg, base) in rats with a history of nicotine self-administration (nicotine experienced) in the absence of a nicotine-associated context (ie, context and cues associated with nicotine self-administration); (iii) nicotine (0.06 mg/kg) was self-administered or experimenter-administered in nicotine-experienced rats in the presence of a nicotine-associated context. In saline-pretreated nicotine-naïve and nicotine-experienced rats, nicotine increased NAcc shell dopamine regardless of the context used for testing. Interestingly, LY379268 pretreatment blocked nicotine-induced increases in NAcc shell dopamine in nicotine-experienced rats only when tested in the presence of a nicotine-associated context. LY379268 did not block nicotine-induced increases in NAcc shell dopamine in nicotine-naïve or -experienced rats tested in the absence of a nicotine-associated context. These intriguing findings suggest that activation of mGlu2/3 receptors negatively modulates the combined effects of nicotine and nicotine-associated contexts/cues on NAcc dopamine. Thus, these data highlight a critical role for mGlu2/3 receptors in context/cue-induced drug-seeking behavior and suggest a neurochemical mechanism by which mGlu2/3 receptor agonists may promote smoking cessation by preventing relapse induced by the combination of nicotine and nicotine-associated contexts and cues.
