Aim: Orthognathic surgery aims to correct facial skeletal deformities and the correct condylar positioning is very important for stable results. The aim of the present study was to verify the occurrence of changes in the postoperative condylar positioning in artificial skulls with a skeletal Class II maxillomandibular relationship submitted to bilateral sagittal split osteotomy when the method of cephalometric data transfer was used. Methods: Ten skeletal Angle class II polyurethane skulls were used with metallic markers in the articular surfaces of the temporomandibular joint and mandibular condyles. The skulls were submitted to preoperative and postoperative cone beam computed tomography before and after the bilateral sagittal split osteotomy. To verify the condylar positioning, measurements between the distances of the markers at the temporal bones and mandibular condyles were taken in the coronal and sagittal views by the DISTANCE tool of the iCat Vision software. All measurements were obtained by one examiner in the preoperative and postoperative CBCTs, tabulated and submitted to statistical analysis by the Wilcoxon test with a level of significance of 5% (p<0,05). After 15 days of the completion of the first data collection, all measurements were redone to determine the random and systematic error by the Intraclass Correlation Coefficient. Results: With the exception of the average of the lateral-medial distance (from the measurements between the medium left markers only), the averages of the anterior-posterior distances (only in the left posterior and lateral right markers) and the vertical average (only in the central markers) showed no statistically significant differences between the preoperative and postoperative distances of the metallic markers. Conclusion: Even when using the method of cephalometric data transfer, variation of the condylar positioning occurred between the preoperative and postoperative periods. This variation occurred only in a few points of the mandibular condyles

PURPOSE: Surgically assisted rapid maxillary expansion (SARME) is a procedure routinely performed to correct transverse maxillary deformities and can be performed with or without pterygomaxillary disjunction (PD). The aim of the present study was to measure the effect of the amount of expansion and stability of SARME with or without PD. PATIENTS AND METHODS: We designed and implemented a double-blind, randomized clinical trial. The patients were randomly assigned to 2 groups: group 1, SARME without PD; and group 2, SARME with PD. Cone-beam computed tomography scans were performed at 3 points: baseline (T0), after maxillary expansion (T1), and at the end of the retention period (T2). Dental and bone expansion and dental inclination at the maxillary canine and first molar regions were assessed. Two-way repeated measures analysis of variance was used to evaluate the differences between the 2 groups at the 3 evaluation periods (T0, T1, and T2), using a level of significance of P < .05. RESULTS: A total of 24 patients underwent maxillary surgical expansion (group 1, n = 12; and group 2, n = 12). Both techniques promoted a significant transverse dental expansion in the first molar at T2 (with PD, 5.4 mm; vs without PD, 6.4 mm; change, -6.18 mm to 1.48 mm). However, no statistically significant differences were observed between the 2 groups. The tipping molars at T2 remained at a higher level in the SARME, no PD group than in the SARME, PD group (with PD, 2.3°; vs no PD, 4.6° for 3 teeth; change, -12.72° to 5.57°; and with PD, 1.6° vs without PD, 3.6° for 14 teeth; change, -9.96° to 9.83°). CONCLUSIONS: SARME with and without PD is a reliable method for obtaining maxillary expansion, with slight differences in the patterns of skeletal and dental alterations.

Osteogenesis imperfecta (OI) is a rare hereditary condition caused by changes in collagen metabolism. It is classified into four types according to clinical, genetic, and radiological criteria. Clinically, bone fragility, short stature, blue sclerae, and locomotion difficulties may be observed in this disease. OI is often associated to severe dental problems, such as dentinogenesis imperfecta (DI) and malocclusions. Radiographically, affected teeth may have crowns with bulbous appearance, accentuated constriction in the cementoenamel junction, narrowed roots, large root canals due to defective dentin formation, and taurodontism (enlarged pulp chambers). There is no definitive cure, but bisphosphonate therapy is reported to improve bone quality; however, there is a potential risk of bisphosphonate-related osteonecrosis of the jaw. In this study we report a case of OI in a male pediatric patient with no family history of OI who was receiving ongoing treatment with intravenous perfusion of bisphosphonate and who required dental surgery. In addition, we discussed the clinical and imaging findings and briefly reviewed the literature.

BACKGROUND: The international literature emphasizes the importance of evaluating the knowledge of different groups such as teachers, students, dentists, physicians, parents, and athletes regarding dental injuries. In Brazil, community health workers are professionals who can reach a wide variety of people and who have a marked influence on prevention and health promotion strategies. The objective of this study was to investigate the knowledge and attitudes of community health workers regarding dental trauma. METHODS: A questionnaire consisting of 19 questions divided into 3 parts was applied: demographic characteristics, knowledge, and attitudes. Data from 206 respondents were analyzed using descriptive statistics and logistic regression models. RESULTS: Approximately 28% of the participants reported to have been called to assist individuals with dental trauma. Only 42 subjects had received education on dental injuries, with 34 of them being instructed by a dentist. In cases of tooth avulsion, only 1.9% of the health workers reported that they would search for the tooth and reimplant it. The most frequently indicated storage media for avulsed teeth were nonphysiological media (69.42%). CONCLUSIONS: The educational level of the community health workers somehow influenced their knowledge (P < 0.001) and attitude (P = 0.016) regarding dental injuries. Educational programs for this group are needed to improve the management of traumatic dental injuries.

BACKGROUND: Third molar surgery is considered the most frequently performed procedure in oral surgery. Although there are some indications for the removal of this tooth, such as the possibility of cystic degeneration or a tumor of the adjacent pericoronal follicle, there is no consensus in the literature about their prophylactic removal. OBJECTIVES: The aims of this study were to investigate the pathological alterations related to mandibular third molar dental follicles and to perform a narrative literature review. METHODS: A prospective clinical, radiographic, and histopathologic study was conducted with pericoronal follicles of third molars. After histopathologic evaluation and establishment of diagnosis, 2 groups were defined: G1 (pericoronal tissues with pathological alterations based on histopathologic analysis) and G2 (pericoronal tissues without pathological alterations based on histopathologic analysis). In addition, a systematic review of the literature was performed. RESULTS: One hundred thirteen specimens were analyzed. G1 was the most prevalent (P = 0.0004). Lesions were found in patients between 20 and 25 years of age (P < 0.004). The most prevalent histological diagnosis was the paradental cyst (47.7%; P < 0.0001). The narrative literature review showed that the majority of cases were mainly dentigerous cysts (P < 0.05). CONCLUSIONS: The mandibular third molars in young adults showed a direct relationship with age and a statistical propensity for the development of these cystic alterations, notably paradental cysts.

Despite the nomenclature suggested to be a tumor, torus palatinus (TP) is an overgrowth of the bone in the palatal region and represents an anatomic variation. Its prevalence varies among the population studied and its etiology is still unclear; however, it seems to be a multifactorial disorder with genetics and environmental involvement. Surgical removal of the TP is indicated in the following circumstances: (1) deglutition and speech impairment, (2) cancer phobia, (3) traumatized mucosa over the torus, and (4) prosthetic reasons. The aim of this case report is describe cases that occurred in two sisters, emphasizing the genetic etiology of this anatomic variation. In addition, intra-oral exam and computed tomography scan (axial, coronal and sagittal view) provided a detailed assessment of the TP and elimination of other possible diagnoses, furthermore allowed a better analyzes of the anatomic relation with adjacentes structures. No surgical removal was indicated for both cases.

Introdução: O tratamento da deficiência transversal de maxila, em adultos, exige a expansão de maxila cirurgicamente assistida. Diversas técnicas cirúrgicas são conhecidas para a realização desse procedimento, porém estas relacionam-se com complicações. Objetivo: Avaliar a incidência de complicações associadas ao procedimento de expansão de maxila cirurgicamente assistida. Material e Método: Trinta e três indivíduos com deficiência transversal de maxila foram submetidos ao procedimento de expansão pela técnica da osteotomia Le Fort I subtotal com degrau no pilar zigomático-maxilar, disjunção ptérigo-maxilar e osteotomia da sutura intermaxilar. Durante os períodos trans e pós-operatório, as complicações relacionadas ao procedimento e as distâncias interdentais foram registradas. Resultado: Doze homens e 21 mulheres, com idade média de 24,64 anos, submeteram-se ao procedimento. As mensurações das distâncias interdentais evidenciaram aumento das mesmas entre os períodos pré-operatório e pós-operatório de 2 meses. As complicações prevalentes foram sinusite (6%) e deslocamento associado à inclinação dental (6%). Conclusão: A expansão de maxila cirurgicamente assistida é um procedimento eficaz e de baixa morbidade para o tratamento da deficiência transversal de maxila em indivíduos adultos.


Introduction: The treatment of adult transverse maxillary deficiency is the surgically assisted maxillary expansion. Several surgical techniques have been described for this and complications have been related to these procedures. Objective: the aim of this study was evaluate the incidence of complications associated with surgically assisted maxillary expansion. Material and Method: 33 individuals undergone surgically assisted maxillary expansion by subtotal Le Fort I osteotomy with a step in the zygomatic-maxillary buttress and associated to pterigomaxillary disjunction and osteotomy of intermaxillary suture. Operative complications, post-operative complications and the distance between the upper teeth were recorded. Result: 12 men and 21 women with an average age of 24.64 years undergone the procedure. The interdental distances increased from preoperative to 2 months post-operative time. The prevalent complications were sinusitis (6%) and teeth displacement and inclination (6%). Conclusion: Surgically assisted maxillary expansion is an effective and low morbidity procedure to treat transverse maxillary deficiency in adults.

INTRODUCTION: Ossifying fibroma (OF) is a well-defined benign neoplasm characterized by slow growth and predilection for the posterior mandible site, usually treated by conservative surgical approaches. In this context, Le Fort I Approach (LFIA) has been increasingly used providing good visualization and an easy access to the lesion. It also allows removal of the lesion quickly. Although there are case reports using LFIA for the management of benign neoplasms, odontogenic cysts and tumors of the middle third of face, to our knowledge, only one article describes the use of surgical LFIA for OF of the maxilla. Thus, the objective of this study was to report the second paper on the use of horizontal osteotomy of the maxilla in the resection of an extensive OF and conduct a brief literature review on this surgical approach. CASE REPORT: The following case report describes a 33-year-old man who was diagnosed with OF. Oral examination showed a painless swelling of hard consistency, covered by normal mucosa in edentulous alveolar ridge of the maxillary posterior region of the right side. Tomographic slices revealed that the image with mixed density had a centrifugal growth, with expansion of the buccal and lingual cortical with an upper displacement of the maxillary sinus. The patient was admitted to the hospital for surgical resection of the lesion via LFIA. Satisfactory functional results were obtained after 18-month follow-up period. In addition, no recurrence or clinical complaints were noted. DISCUSSION: LFIA shall be considered as a feasible option when planning surgery for removal of extensive benign lesions located in midfacial region because it is a predictable technique and routinely performed by the oral and maxillo-facial surgeons.

The 18F-fluorodeoxyglucose positron emission tomography/computed tomography (FDG-PET/CT) has become a standard procedure for the diagnosis, staging, and restaging in lymphoma patients. However, a relative high rate of false-positive results has been reported. We report a case of a 40-year-old man with a previous history of a nodal follicular lymphoma, stage IVA, treated with R-CHOP, which showed strong 18F-FDG uptake in the Waldeyer's tonsillar ring during his follow-up, being considered highly suspicious of relapsed lymphoma. A surgical removal of the palatine tonsils and adenoids was performed, which showed reactive follicular hyperplasia. Furthermore, bone marrow biopsy revealed absence of neoplasia. The patient is still in follow-up with no signs of recurrent lymphoma. This case illustrates that, despite the high sensitivity for the detection of recurrent lymphoma, 18F-FDG uptake should be interpreted with great caution and confirmatory studies should be performed before any therapy.

Introdução. O estudo da anatomia da região naso-sinusal e de suas variações é de grande importância diagnóstica e para a definição de condutas de tratamento nas áreas da Odontologia e da otorrinolaringologia, principalmente em relação aos seios maxilares, regiões anatômicas diretamente relacionadas com os dentes superiores. Objetivos. Avaliar por meio da tomografia computadorizada de feixe cônico (TCFC) a prevalência das seguintes variações anatômicas do complexo ostiomeatal: concha bolhosa, células de Haller, desvios de septo nasal e aerações do processo uncinado e, ainda, avaliar a prevalência das afecções inflamatórias dos seios maxilares. A presença das variações anatômicas supramencionadas e de alterações apicais de dentes posteriores superiores serão correlacionadas com a presença das afecções inflamatórias dos seios maxilares. Material e Métodos. Foram avaliados 100 exames de TCFC de pacientes odontológicos escolhidos de forma aleatória de acordo com protocolos de aquisição que abrangiam as regiões anatômicas de interesse ao estudo. Todas as tomografias do estudo foram adquiridas através do tomógrafo i-Cat, modelo Classic e as imagens em formato DICOM foram visualizadas e analisadas através do software i-Cat Vision. Resultados. Os exames tomográficos avaliados eram de 100 pacientes, 43 do gênero masculino (média de idade de 23,7anos) e 57 do gênero feminino (média de idade de 25,8 anos). Todas as variações anatômicas em estudo foram visualizadas nos exames analisados. Oitenta e cinco indivíduos apresentaram uma ou mais de uma variação concomitantemente, enquanto 15 indivíduos não apresentaram qualquer tipo de variação anatômica. A maior prevalência foi de indivíduos que apresentaram duas variações anatômicas (38%). Desvio de septo nasal ocorreu em 67% dos pacientes. Considerando-se a possibilidade de ocorrência bilateral em cada paciente, concha média bolhosa, células de Haller e aeração do processo uncinado apresentaram, respectivamente, as seguintes...


Introduction. The study of anatomy nasal-sinus and its variations is of great importance for the diagnosis and definition of behavior treatment in the areas of dentistry and otolaryngology, particularly in relation to the maxillary sinuses, anatomical regions directly related to the upper teeth. Objectives. Evaluate by beam computed tomography (CBCT) the prevalence of the following complex anatomical variations ostiomeatal: concha bullosa, Haller cells, nasal septum deviation and airings of the uncinate process, and also to assess the prevalence of inflammatory diseases of the sinuses jaws. The presence of anatomical variations above and apical changes of maxillary posterior teeth will be correlated with the presence of inflammatory diseases of the maxillary sinuses. Materials and Methods. We evaluated 100 CBCT examinations of dental patients chosen at random according to acquisition protocols covering the anatomical regions of interest to the study. All scans were acquired through the study of the tomograph i-Cat, Classic model and the images in DICOM format were viewed and analyzed using the software i-Cat Vision. Results. CT scans were evaluated for 100 patients, 43 males (mean age 23.7 years) and 57 females (mean age 25.8 years). All anatomical variations in the study were displayed in the images analyzed. Eightyfive subjects had one or more concomitant variation, while 15 subjects did not show any anatomical variation. The highest prevalence was individuals who had two anatomical variations (38%). Septal deviation occurred in 67% of patients. Considering the possibility of bilateral occurrence in each patient, concha bullosa, Haller cells and aeration of the uncinate process showed, respectively, the following rates: 31.5%, 20% and 9%. The inflammatory diseases of the maxillary sinuses showed the following distributions and prevalence rates considering bilateral analysis: in 74 maxillary sinuses were not visualized inflammatory disorders (37%)...

Agenesia dentária é uma anomalia genética que caracteriza-se pela ausência total de um ou mais dentes. Ocorre em de 6% da população brasileira, geralmente no segundo pré-molar inferior e representa um desafio para a reabilitação, pois leva a alterações funcionais e estéticas. Nesse contexto, os implantes osseointegráveis ocupam lugar de destaque, dentre eles, os implantes de superfície SLActive®, que promovem aceleração no período de osseointegração, reduzindo esse tempo para 21 dias, além de melhor qualidade do osso neoformado ao redor dos implantes. Com altos índices de sucesso, mesmo quando submetidos a carga oclusal precoce ou imediata, tais implantes têm revolucionado a implantodontia moderna. O objetivo desse trabalho é relatar caso de agenesia dentária do segundo pré-molar inferior, reabilitado por meio de implante de superfície SLActive® e instalação da coroa protética definitiva na quinta semana pós-operatória.


Dental agenesis is a genetic abnormality characterized by total absence of one or more teeth. It affects about 6% of the population, affects preferably the second mandibular premolar and represents a challenge for rehabilitation because it leads to functional and esthetic problems. Currently, dental implants play a prominent role in rehabilitation, and the SLActive® implants stand out because they accelerate osseointegration, shorten it to 21 days, and ensure better quality of new bone around the implant. With high rates of success even when immediate or early loading is used, these implants have revolutionized modern Implantology. This report describes a case of dental agenesis of the second mandibular premolar treated using a SLActive® implant and a final prosthetic crown installed on the fifth postoperative week.

A tomografia computadorizada de feixe cônico (TCFC) é uma ferramenta de diagnóstico por imagem com crescente utilização na Odontologia principalmente na implantodontia oral. Nessa especialidade, a sua utilização proporciona um melhor planejamento visto que medidas mais precisas de altura, largura, espessura e qualidade óssea podem ser obtidas. Além disso, as relações entre os potenciais sítios para instalação de implantes osseointegrados e estruturas anatõmicas nobres, especialmente seio maxilar, fossa nasal, forame mentual e canal mandibular, podem ser avaliadas com mais fidelidade. O objetivo deste trabalho foi apresentar uma série de casos onde a TCFC foi utilizada em pacientes submetidos à cirurgia de colocação de implantes osseointegrados. Em dois casos mostrou-se fundamental na determinação pré-operatória das dimensões dos implantes a serem instalados e escolha dos respectivos sítios anatômicos. Nos outros dois casos a TCFC apresentou-se relevante na resolução de complicações advindas da instalação de implantes que foram planejados com base apenas em exames convencionais (radiografias panorâmicas), situações em que houve deslocamentos acidentais para o interior do seio maxilar e do canal mandibular. Com base na série de casos relatados e na literatura consultada conclui-se que a TCFC é um exame de extrema importância na reabilitação oral através de implantes osseointegrados, desde o planejamento, prevenção de acidentes e resolução de eventuais complicações.


The cone beam computed tomography (CBCT) is a diagnostic imaging tool recently introduced in dentistry. In oral implantology its use provides a better planning as more precise measurements of height, width, thickness and bone quality can be obtained. In addition, the relationships among potential sites for implant placement and noble anatomic structures, especially the maxillary sinus, nasal cavity, the mental foramen and mandibular canal can be evaluated with more fidelity. The objective of this study was to present a series of cases in which the CBCT was used in patients submitted to implant placement surgery. In two cases the CBCT proved to be important in determining the preoperative dimensions of the implants and choose their anatomical sites. In the other two cases CBCT showed to be relevant to resolve the complications due to the implant installation that were planned based only in conventional exams (panoramic radioqraph), in these situations there were accidental implant displacement for inside the maxillary sinus and mandibular canal. Based on these report of a series of cases and in the literature used, it is concluded that CBCT is an exam extremely important in oral rehabilitation with implants, from planning, accident preventions and resolution of any complications.

Introdução: a Ortodontia como uma especialidade odontológica tem mostrado para a sociedade a sua grande relevância e seu impacto na promoção da saúde da população. Enquanto ciência, os avanços para tratamento e reabilitação de pacientes são incontestáveis,desde situações mais simples às mais complexas, proporcionando saúde e bem-estar a um número cada vez maior de pessoas. Por outro lado, o grande número de profissionais tem gerado preocupações e inquietações à medida que tratamentos podem ser conduzidos deforma inadequada. Objetivo: neste artigo será abordada a importância do diagnóstico de patologias dos maxilares por meio da descrição de quatro situações clínicas, nas quais pacientes sob tratamento ortodôntico, portadores de lesões visíveis ao exame radiográfico panorâmico,tiveram o diagnóstico destas lesões negligenciado ou desconsiderado.Conclusão: a radiografia panorâmica constitui importante meio de diagnóstico de patologias assintomáticas dos maxilares, devendo ser criteriosamente avaliada antes do início da terapia ortodôntica.A parceria com outras especialidades da Odontologia é fortemente aconselhada na rotina do ortodontista.


Introduction: orthodontics as a dental specialty has shown its relevance to society and a great impact on the promotion of public health. The advances in the treatment and rehabilitation of orthodontic patients, from mild to severe cases, provide health and wellness to a growing number of people. On the other hand, the growing number of orthodontic professionals has raised concems,as treatments can be conducted improperly Objective: in this articlewe discuss the importance of diagnosis of pathological changes of the jaws through the description of four clinical situations in which patients under went orthodontic treatment. Lesions which were visible on panoramic radiographs were overlooked or disregarded for these patients. Conclusion: the panoramic radiograph is an important modality of diagnosis of asymptomatic pathological changes of the jaws. The panoramic image must be carefully evaluated before the beginning of orthodontic treatment. Collaboration with other specialties of dentistry is strongly recommended for the orthodontist.
