RESUMO

Executive coaches use a disciplined process to enable people to uncover why they are getting their current results and stimulate them to identify new ideas to achieve different results in the future. Unlike mentors, coaches do not give direction or advice. A coach might offer examples of what others have done in similar situations but only in service to idea generation, not recommendations. Data is key. Coaches typically gather information through assessments or interviews to give clients new insights. Clients learn about their deficiencies and strengths, their brand, how they work with teams, and glean unvarnished advice. Mindset matters. Anyone coerced into coaching might be frustrated about their situation and, therefore, be less open to honestly discovering the source of their discomfort and uncovering new possibilities through coaching. Courage is crucial. Being open to coaching can be daunting, yet with a willing mindset, the insights and results can be compelling.

Outbreaks of serious infections associated with heroin use in persons who inject drugs (PWIDs) occur intermittently and require vigilance and rapid reporting of individual cases. Here, we give a firsthand account of the cases in London during an outbreak of heroin-associated anthrax during 2009-2010 in the United Kingdom. This new manifestation of anthrax has resulted in a clinical manifestation distinct from already recognized forms. During 2012-13, additional cases of heroin-associated anthrax among PWIDs in England and other European countries were reported, suggesting that anthrax-contaminated heroin remains in circulation. Antibacterial drugs used for serious soft tissue infection are effective against anthrax, which may lead to substantial underrecognition of this novel illness. The outbreak in London provides a strong case for ongoing vigilance and the use of serologic testing in diagnosis and serologic surveillance schemes to determine and monitor the prevalence of anthrax exposure in the PWID community.

RESUMO

A patient with an 18-year history of chronic lymphocytic leukaemia developed zygomycosis of the orbit, sinuses and nasal bones together with pulmonary fungal nodes due to Absidia corymbifera while on high dose steroids and four months after successful treatment of pulmonary aspergilloma with liposomal amphotericin B followed by oral voriconazole. He was treated successfully with extensive surgical debridement, intravenous liposomal amphotericin B and intravenous itraconazole.

PURPOSE OF REVIEW: Hepatitis B virus infection is prevalent worldwide and is a significant cause of morbidity and mortality particularly in Asia. Adults chronically infected with hepatitis B virus remain a significant potential source of sexually transmitted hepatitis B. The purpose of this article is to review the recent literature relating to hepatitis B virus transmission with particular emphasis on sexual transmission and efforts to prevent spread. RECENT FINDINGS: The introduction of hepatitis B virus vaccine and the implementation of universal childhood vaccination for hepatitis B in some countries have led to a dramatic reduction in the number of children with chronic hepatitis B. However, recent reports suggest that we are not as successful in preventing infection by sexual transmission. It is clear that sexual transmission of hepatitis B virus is still widespread and is a major problem in certain high-risk groups such as men who have sex with men, intravenous drug users, prisoners and sex workers. Significant problems remain with respect to education and vaccination within these groups. SUMMARY: Hepatitis B virus remains a major health burden but it is preventable by education and vaccination. Greater resources are required to expand vaccination to the at-risk, sexually active adult populations if the World Health Organization ideal of hepatitis B virus eradication is to be realized and the burden of hepatitis B virus-related morbidity and mortality contained.
