Potato gun ocular injury.

PURPOSE: This study aimed to identify a dangerous new weapon capable of causing damage to the ocular and periocular regions. METHODS: The authors report two patients who had penetrating ocular injury in the past year because of homemade recreational potato guns. RESULTS: In one 14-year-old boy, projectiles from the firing of a potato gun resulted in orbital and cranial injuries that were life threatening with widespread fractures, marked disruption of facial structures, a cerebrospinal fluid fistula requiring bifrontal surgical repair, and loss of one eye. In a separate accident with a different potato gun, a 14-year-old boy who was wearing glasses at the time of injury had a sight-threatening perforating corneal laceration. CONCLUSION: Practitioners must be aware of the existence of these new, homemade unregulated devices. Information about the use and construction of these guns is widespread on the Internet, but no injuries resulting from these guns currently are documented in the medical literature.

Corneal transplant infection by Paecilomyces lilacinus.

Paecilomyces lilacinus was cultured from an ulcerated corneal transplant, sections of which, following surgical removal, revealed dense mycelial growth throughout the corneal remnant and penetrating Descemet's membrane. PAS-stained sections showed, in addition to the hyphae, numerous small, ovoid elements consistent with conidia of P. lilacinus. Infection apparently had spread to the transplant from the recipient's cornea. The fungal isolate proved resistant, in vitro, to amphotericin B, 5-fluorocytosine, and pimaricin, but sensitive to ketoconazole and moderately sensitive to miconazole. Following a second transplant and intensive antifungal therapy, the infection appears to have been eliminated.

Conjunctival bandage.

In those instances of corneal disease where a traditional hydrophilic bandage lens cannot be fitted, using the combination of a hydrophilic bandage lens and an overlying conjunctival flap is a relatively simple surgical procedure. It has the advantage of easy removability of the bandage lens while eliminating the possibility of accidental dislodgment of the bandage lens.

Unilateral optic neuropathy following vincristine chemotherapy.

Two subjects with acute lymphatic leukemia have been observed to have unilateral optic neuropathy five to seven days following administration of Vincristine (2.0 mg/sq m) during maintenance chemotherapy. Both subjects were in mission at the time their ocular problems developed. After discontinuance of Vincristine therapy, marked vision improvement in the affected eye occurred over the ensuing months. It is presumed that Vincristine therapy was the cause of toxic optic neuropathy in these patients.
