PURPOSE: Early detection of mucosal neoplastic lesions is crucial for a patient's prognosis. This has led to the development of effective optical endoscopic diagnostic methods such as narrow band imaging (NBI) and autofluorescence (AFI). Independent of each other, both of these methods were proven useful in the detection of mucosal neoplasias. There are limited reported data comparing both methods for oropharyngeal cancer diagnostics. The aim of the study was to compare NBI and AFI endoscopic visualization of signs in identifying tonsillar squamous cell carcinoma (SCC) and assessing its extent and to determine whether the score was related to the evaluator's experience. METHODS: Patients with tonsillar SCC underwent endoscopic pharyngeal examination using NBI and AFI. Fiftyseven video sequences of examinations of lesions proven to be SCC were evaluated by three reviewers. The accuracy of determination of lesion extent and visualization of its endoscopic signs of malignancy were evaluated. RESULTS: Endoscopic visualization of tumour spread was significantly better using AFI than NBI (p = 0.0003). No significant difference was found between NBI and AFI in the visualization of endoscopic malignancy determining signs (p = 0.1405). No significant difference was found among the three reviewers in the visualization of tumour spread and for identifying malignancy-determining signs in NBI endoscopy or AFI endoscopy. CONCLUSIONS: The results show that AFI obtained better results for assessing the extent of tonsillar cancers than NBI. Both methods were proven to be equal in the visualization of endoscopic malignancy-determining signs. Both are useful even for less experienced evaluators.

Food animal welfare is an issue of great concern, as society has a responsibility for animals under human care. Pork is the most consumed meat worldwide, with more than a billion pigs being slaughtered globally every year. Still, in most countries, sows are restrained in farrowing crates throughout lactation. In these crates, sows are confined with bars to an area that is just slightly larger than their body. Thus, moving and turning around, grooming, or expressing other natural behaviors are typically impossible. In this study, we utilized a simple and practical modification of conventional farrowing crates to designed farrowing pens, by removable confinement bars, which provide the flexibility to change the housing system from one to another. Our objective was to examine the parameters of production and hair cortisol concentrations after different restraint periods during lactation. Analyses included data from 77 sows and their 997 piglets. Sows were housed in farrowing crates, but the confinement bars were removed after different periods, from 3 days post-farrowing to full restraint. For certain analyses, sows were grouped into Short or Long Restraint groups (3-10 days vs 13-24 days, respectively). Multiple linear regression revealed that for any additional day in restraint of the sows, piglets' weaning rate decreases by 0.4% (P < 0.05). Moreover, the total number of weaned piglets per litter was higher in the Short Restraint group as compared to the Long Restraint group (10.4 ±â€¯0.3 vs 9.7 ±â€¯0.3, respectively; P < 0.05). Accordingly, total litter weight on the weaning day tended to be higher in the Short Restraint group (68.8 ±â€¯2.2 vs 64.9 ±â€¯1.8 kg; P = 0.1210). The requirement for medical treatments during lactation (e.g., antibiotics, NSAID) tended to be less frequent in the Short Restraint group (Sows: 21.9% vs 40%; P = 0.1219. Piglets: 2.4% vs 17.1%; P = 0.0609). Hair cortisol as a marker for chronic stress during lactation decreased when the restraint period was shortened in both sows and piglets. Our analysis revealed that sows' hair cortisol is a significant mediator between the restraint of the sow and its piglets' hair cortisol (Sobel test; P < 0.05). For every day of sows' restraint, sows' hair cortisol increased by 0.5 pg/mg, and for any additional unit of sows' hair cortisol, piglets' hair cortisol increased by 0.36 pg/mg. In conclusion, sustainable swine farming management can be beneficial for both animals and farmers; limiting sow restraint during lactation is expected to reduce stress, enhance welfare and production, and potentially improve the economics of swine operations.

Previous studies have reported that miners (and other workers) exposed to high levels of diesel engine exhaust (DEE) have an increased risk of lung function decline. The main objective of this study was to evaluate associations between exposure to different components associated with DEE in relation to lung function across a 12-h working shift. Eighty underground gold miners and twenty surface miners completed spirometry and questionnaires at the beginning and end of their 12 h work shift. Personal exposure to elemental carbon (EC), volatile organic compounds (VOCs), nitrogen dioxide (NO2), particle size and particle number were monitored during their shift. Multiple regression models were used to examine the associations between DEE and lung function, adjusting for a range of covariates. Underground miners were exposed to higher levels of EC, VOCs, NO2, and particle number and larger mean particle size than surface miners. Cross-shift reduction in Z-score value of FEV1/FVC in underground miners was statistically significantly greater than those of surface miners. The cross-shift change in Z-score value of FEV1/FVC was associated with exposure to higher concentration of EC and particle number, but not with VOCs, NO2 and particle size. Occupational exposure to diesel engine exhaust in current Australian gold mines is substantial. Exposures were higher in underground miners and had a negative association with their lung function over a single 12-h shift.

Metabolites of polycyclic aromatic hydrocarbons measured in human samples are often used as biomarkers of exposure to diesel engine exhaust (DEE). The aim of this study was to assess the changes in urinary levels of 1-aminopyrene (1-AP) and 1-hydroxypyrene (1-OHP) and their relationship with Elemental Carbon (EC), as a component of diesel engine exhaust exposure, among a hard-rock gold-mining population. Urine samples were collected at the beginning and end of a 12-hour work shift from 100 underground and above ground gold miners. Miners were fitted with personal exposure monitoring equipment to quantify exposure to DEE, measured as Elemental Carbon (EC), across their 12-hour work shift. General linear regression assessed associations of the post-shift urinary 1-AP and 1-OHP concentrations with EC, controlling for age, gender, the pre-shift biomarker level, Body Mass Index (BMI), days on current shift, time in mining, smoking status and second-hand smoke exposure. The concentrations of 1-AP and 1-OHP increased significantly across a 12-hour mining work shift. Moreover, consistent with the sensitivity analysis, the concentration of 1-AP was significantly associated with EC after adjustments. Urinary 1-OHP, but not 1-AP was significantly associated with current smoking. Urinary 1-AP may be a more robust and specific biomarker of DEE than 1-OHP.

BACKGROUND: The biological role of visfatin in humans, especially in eutrophic and healthy children, is not understood yet, except for its link to obesity-related disorders in adolescents and adults. OBJECTIVES: To determine the physiological values of serum visfatin concentrations, and visfatin mRNA expression in subcutaneous (SAT) and visceral adipose tissue (VAT), and to correlate them with anthropometric/metabolic data in prepubertal healthy boys. METHODS: The study included 59 healthy boys, age 1-10 years, hospitalized for elective surgery, divided according to age into group I (1-3 years old), group II (3-7 years old) and group III (7-10 years old). Anthropometric and biochemical measurements, and the visfatin serum and mRNA level in SAT and VAT were determined in all patients. RESULTS: Visfatin mRNA expression was higher in SAT compared with VAT in all three studied groups. Highest visfatin mRNA was found in SAT of group III compared with group II (P = 0.030). VAT visfatin mRNA expression negatively correlates with body weight (P = 0.039), waist circumference (P = 0.027) and morning glucose level (P = 0.007). CONCLUSION: Lack of changes in serum visfatin level despite the changes in visfatin mRNA expression of adipose tissue suggests paracrine effect of visfatin rather than endocrine. Negative correlation of visfatin VAT mRNA expression with anthropometric parameters indicates important role of VAT visfatin in maturation and in glucose metabolism.

In the search for suitable scaffold materials for tissue regeneration, silk fibroin has become one of the most promising candidates due to its biocompatibility and good physical properties. To facilitate bone formation in osteochondral defects, it is often combined with a bone promoting additive. Here we demonstrate using HRTEM analysis how the release of Ca2+ ions from bioactive glass or Ca-salts results in the reduction of ß-sheet domain size that effectively controls a scaffold's properties, such as degradation and mechanical stiffness. We show that these changes already occur in silk fibroin solution prior to scaffold preparation and are caused by a decrease in zeta potential that forces fibroin molecules into tighter packing resulting in higher scaffold crystallinity, smaller ß-sheet domains and higher interconnectivity. The reduction of ß-sheet domains improves the elastic modulus and allows faster degradation despite the higher crystallinity. Ca2+ was also shown to be beneficial to the formation of hydroxy-apatite sheets on the fibroin surface.

This study reports on the selective adsorption of whole plasma proteins on hydrothermally (HT) grown TiO2-anatase coatings and its dependence on the three main surface properties: surface charge, wettability and roughness. The influence of the photo-activation of TiO2 by UV irradiation was also evaluated. Even though the protein adhesion onto Ti-based substrates was only moderate, better adsorption of any protein (at pH = 7.4) occurred for the most negatively charged and hydrophobic substrate (Ti non-treated) and for the most nanorough and hydrophilic surface (HT Ti3), indicating that the mutual action of the surface characteristics is responsible for the attraction and adhesion of the proteins. The HT coatings showed a higher adsorption of certain proteins (albumin 'passivation' layer, apolipoproteins, vitamin D-binding protein, ceruloplasmin, α-2-HS-glycoprotein) and higher ratios of albumin to fibrinogen and albumin to immunoglobulin γ-chains. The UV pre-irradiation affected the surface properties and strongly reduced the adsorption of the proteins. These results provide in-depth knowledge about the characterization of nanocrystalline TiO2 coatings for body implants and provide a basis for future studies on the hemocompatibility and biocompatibility of such surfaces.

A modified version of the statistical dynamical diffraction theory (mSDDT) permits full-pattern fitting of high-resolution X-ray diffraction scans from thin-film systems across the entire range from fully dynamic to fully kinematic scattering. The mSDDT analysis has been applied to a set of model SiGe/Si thin-film samples in order to define the capabilities of this approach. For defect-free materials that diffract at the dynamic limit, mSDDT analyses return structural information that is consistent with commercial dynamical diffraction simulation software. As defect levels increase and the diffraction characteristics shift towards the kinematic limit, the mSDDT provides new insights into the structural characteristics of these materials.

The most collapsible part of the upper airway in the majority of individuals is the velopharynx which is the segment positioned behind the soft palate. As such it is an important morphological region for consideration in elucidating the pathogenesis of obstructive sleep apnea (OSA). This study compared steady flow properties during inspiration in the pharynges of nine male subjects with OSA and nine body-mass index (BMI)- and age-matched control male subjects without OSA. The k-ωSST turbulence model was used to simulate the flow field in subject-specific pharyngeal geometric models reconstructed from anatomical optical coherence tomography (aOCT) data. While analysis of the geometry of reconstructed pharynges revealed narrowing at velopharyngeal level in subjects with OSA, it was not possible to clearly distinguish them from subjects without OSA on the basis of pharyngeal size and shape alone. By contrast, flow simulations demonstrated that pressure fields within the narrowed airway segments were sensitive to small differences in geometry and could lead to significantly different intraluminal pressure characteristics between subjects. The ratio between velopharyngeal and total pharyngeal pressure drops emerged as a relevant flow-based criterion by which subjects with OSA could be differentiated from those without.

Antitumour necrosis factor (TNF) monoclonal antibodies have become an invaluable treatment against chronic inflammatory diseases such as rheumatoid arthritis (RA). However, due to increased risk of opportunistic infections, patients receiving antiTNF therapy should be closely monitored for serious infections. Here, we describe a case of acute Salmonella enteritidis infection of a joint arthroplasty that previously was functioning well, in a patient receiving infliximab treatment for RA. After prolonged antimicrobial chemotherapy and interrupted infliximab treatment, reimplantation of a new prosthesis was successfully performed two years after Salmonella septic arthritis. Therefore, because of the possibility of extraintestinal salmonellosis, screening for fecal colonization could be advisable in patients undergoing antiTNF treatment. Moreover, we emphasize the importance of appropriate counselling of these patients concerning food hygiene.


Los anticuerpos monoclonales del factor de necrosis antitumoral (FNT) se han convertido en un valioso tratamiento contra las enfermedades inflamatorias crónicas como la artritis reumatoide (AR). Sin embargo, debido al mayor riesgo de infecciones oportunistas, los pacientes que reciben terapia antiFNT se deben se monitoreados muy de cerca con respecto a la posibilidad de infecciones serias. Aquí describimos un caso de infección aguda por Salmonella enteritidis de una artroplastia de articulación que anteriormente funcionaba bien, en un paciente que recibía tratamiento con infliximab por RA. Después de prolongadas quimioterapias antimicrobianas y tratamiento interrumpido con infliximab, se realizó exitosamente la reimplantación de una nueva prótesis, dos años después de la artritis séptica por Salmonella. Por lo tanto, debido a la posibilidad de una salmonelosis extraintestinal, podría ser aconsejable el tamizaje de la colonización fecal en pacientes sometidos a tratamiento antiFNT. Por otra parte, hacemos hincapié en la importancia de aconsejar apropiadamente a estos pacientes con respecto a la higiene de los alimentos.

The importance of functionality-related characteristics (FRC) of hydroxypropyl methylcellulose (HPMC) described in the pharmacopeia monograph can be evaluated only for selected formulation or technological process. The aim of our work was to investigate the influence of apparent viscosity as one of the FRC for two batches of the same HPMC grade on the release properties of diclofenac sodium from HPMC matrix tablets. Our results show that two batches of HPMC differ in viscosity significantly and as a consequence, the significant differences were observed in the release profiles as well. HPMC-B sample has higher viscosity and therefore higher average molecular weight, thus the erosion and drug release were slower compared to HPMC-A sample with lower apparent viscosity. It can be concluded that batch-to-batch viscosity variation of the same HPMC grade can lead to the different release profiles; therefore the specification limits of some FRC should be postulated during the development of each individual formulation. However, the viscosity interval as FRC can not be generalized, because it is different for different tablet compositions.

The present study characterised gene expression associated with embryonic muscle development and placental vascularisation during early gestation in the pig and examined effects of Progenos supplementation in early pregnancy. Tissues were collected from commercial multiparous sows (n = 48) from Days 16 to 49 of gestation. In the placenta, qPCR revealed that vascular endothelial growth factor (VEGFA) expression did not change from Day 17 to 49 of gestation; however, KDR receptor and angiopoietin-1 and -2 expression were differentially regulated, with periods of high expression corresponding to two critical phases of angiogenesis in the pig. In the embryo, the pattern of myogenesis-related gene expression was consistent with available literature. A commercially available nutritional supplement Progenos (20 g day⁻¹ L-arginine) added to the diet of sows from either Day 15 to 29 (P15-29; n = 33), Day 30 to 44 (n = 29) or from Day 15 to 44 (n = 76) of gestation tended to increase (P = 0.058) embryonic growth rate compared with non-supplemented controls (n = 79) and angiogenin expression was higher (P = 0.028) at Day 30 of gestation in placentae from sows on the P15-29 Progenos treatment. These results are consistent with proposed beneficial effects of l-arginine on early embryonic development and placental vascularisation.

The key physicochemical properties of functional excipients should be identified, and the influence of their variability on the properties of the final dosage form should be evaluated during the development phase. Excipients produced by different manufacturers and/or by different manufacturing processes should have comparable properties. Hydroxypropyl methylcellulose (HPMC) with a high molecular weight is a functional excipient often used in solid matrix systems with prolonged release of active pharmaceutical ingredients (API). This study investigates whether HPMC manufactured by two manufacturers using different chemical procedures differs in particle-size distribution, particle shape, particle morphology, chemical composition, and dissolution of diclofenac sodium as a model drug. NIR spectroscopy was introduced and calibration models were developed to detect physical differences among HPMC batches from two different origins. The physical differences between HPMC samples were additionally confirmed with scanning electron microscopy (SEM), gas chromatography (GC) measurements, and dissolution testing of hydrophilic matrix tablets. Our results prove that, even if HPMC polymers manufactured from two different sources comply with the pharmacopeial specification, they significantly differ in physicochemical properties and thus influence the properties of the formulated dosage forms.

The expression of Ikaros family transcription factors and consequently their signalling pathway is limiting for hematopoietic and lymphocyte development in mice and human. Due to their importance, these transcription factors are highly homologous between species. As an initial approach to examining the possible involvement of Ikaros transcription factors in pathogenesis of rat lymphoid development, we analyzed the expression of all known Ikaros family members, Ikaros, Aiolos, Helios, Eos and Pegasus in the rat thymus. We established a semi-quantitative RT-PCR to detect mRNA of each transcription factor. For the first time we give evidence of the expression of Ikaros family transcription factors in the rat thymus. Further, we evaluated whether their mRNA expression was succumbed to changes when the rats were exposed to ethanol, as a known debilitating agent during development. Therefore we analyzed the thymus of adult rats whose mothers were forced to drink ethanol during gestation, to detect possible changes in thymus mRNA expression levels of Ikaros, Aiolos, Helios, Eos and Pegasus. We found that rats prenatally exposed to ethanol show a slightly higher expression of Ikaros family transcription factors in the adult thymus when compared to control rats, but these differences were not statistically significant. We further studied the distribution of the major lymphocyte subpopulations in the rat thymus according to CD3, CD4 and CD8 expression by four color flow cytometry. We found a higher incidence of CD3 positive cells in the double positive, CD4+CD8+ thymic subpopulation of rats prenatally exposed to ethanol when compared to non-exposed animals. Our findings indicate that ethanol exposure of pregnant rats might influence the development of CD3 positive cells in the thymus of the offspring but this result should be further tackled at the level of transcription factor expression.

The semen evaluation techniques used in most commercial artificial insemination centers, which includes sperm motility and morphology measurements, provides a very conservative estimate of the relative fertility of individual boars. As well, differences in relative boar fertility are masked by the widespread use of pooled semen for commercial artificial insemination (AI) in many countries. Furthermore, the relatively high sperm numbers used in commercial AI practice usually compensate for reduced fertility, as can be seen in some boars when lower numbers of sperm are used for AI. The increased efficiency of pork production should involve enhanced use of boars with strong reproductive efficiency and the highest genetic merit for important production traits. Given that the current measures of semen quality are not always indicative of fertility and reproductive performance in boars, accurate and predictive genetic and protein markers are still needed. Recently, significant efforts have been made to identify reliable markers that allow for the identification and exclusion of sires with reduced reproductive efficiency. This paper reviews the current status of proteomic and genomic markers of fertility in boars in relation to other livestock species.

The effects of feed restriction (60% of anticipated feed intake; Restrict; n=60) during the last week of a 21-day lactation in primiparous sows compared with feeding at 90% of anticipated feed intake (Control; n=60) on sow metabolic state, litter growth and sow reproductive performance after weaning were compared. Metabolisable energy (ME) derived from feed was lower, ME derived from body tissues was higher and litter growth rate was reduced (all P<0.05) in Restrict sows during the last week of lactation. Treatment did not affect weaning-to-oestrus interval, pregnancy rate, ovulation rate, embryonic survival or the number of live embryos (P>0.05) at Day 30 of gestation: However, embryo weight was greater (P<0.05) in Control than in Restrict sows (1.55±0.04vs 1.44±0.04g, respectively). These data suggest the biology of the commercial sow has changed and reproductive performance of contemporary primiparous sows is increasingly resistant to the negative effects of lactational catabolism. Overall, catabolism negatively affected litter weaning weight and embryonic development of the next litter, but the extent to which individual sows used tissue mobilisation to support these litter outcomes was highly variable.

Expression of panels of candidate genes controlling myogenesis, angiogenesis and gender-specific imprinting of development were analysed in embryonic, placental and endometrial tissues recovered at Day 30 of gestation from a subset of primiparous sows that were either feed restricted (Restrict; n=17) or fed to appetite (Control; n=15) during the last week of the previous lactation. Embryos were also sex typed to investigate gender bias in response to treatments. Average embryonic weight was lower in the subset of Restrict compared with Control litters (1.38±0.07vs 1.59±0.08g, respectively) and the male:female sex ratio was higher (P<0.05) in embryos (litters) recovered from Restrict sows. Treatment affected (P≤0.05) the expression of embryonic and placental genes involved in insulin-like growth factor (IGF) 2 signalling, including IGF2, INSR and IGF2R. Embryonic expression of ESR1 was also affected by treatment (P<0.03) and sex×treatment interactions were observed for the expression of embryonic ESR1 (P<0.05) and placental ANGPT2 (P<0.03). At the molecular level, these results support the suggestion that changes in placental function are not the primary mechanism mediating detrimental effects of previous sow catabolism on early embryonic development in the feed-restricted lactational sow model. However, perturbations in the IGF2 system are implicated as mediators of these effects.

INTRODUCTION: Severe acute pancreatitis (pancreatitis with local and systemic complications) remains a therapeutical problem. Even more complicated is ESAP (Early Severe Acute Pancreatitis), which presents itself as a fulminant form (FAP--fulminant acute pancreatitis) and subfulminant form (SFAP--subfulminant acute pancreatitis). Severity of acute pancreatitis can be assessed with help of prognostic markers. Intensive conservative treatment as a part of "pancreatic unit" dominates the therapy. Couple things remain frequently discussed. First, indications for operative treatment, of which infectious complications of the necrosis and local intraabdominal complications are generally accepted. Second, optimal time of surgical intervention remains an issue discussed. Based on recommendations, patients should not be operated on in first two weeks of disease. AIM: Aim of the authors was to evaluate their own group of patients in terms of indication and timing of the surgical intervention. MATERIAL AND METHODS: Authors analyse group of patients with acute pancreatitis, hospitalised on IVth surgical clinic of Comenius University, Bratislava in years 2006-2007. There were 125 patients in this group, 47 of these were classified as SAP (severe acute pancreatitis). RESULTS: 13 patients were operated on, 12 for retroperitoneal abscessus, ACS (abdominal compartment syndrome) and septic state, one patient for FAP. Patients were operated on between 17th and 80th day with average 28.8 days after onset. Overall mortality was 9.6%, mortality for SAP 21.2% and 15.4% in operated patients. CONCLUSION: Authors conclude, that correctly timed surgical intervention does not aggravate mortality of patients with SAP, and ambition is to shift operation to time of necrosis and infection demarcation. Early indication in setting of failure of conservative treatment of ESAP remains discussed.
