RESUMO

AIMS: Mycoplasma hominis and Ureaplasma urealyticum are potentially pathogenic bacterial species that are frequently isolated from the urogenital tract of women. These pathogens could be responsible for various genitourinary diseases and have been associated with adverse pregnancy outcomes and female fertility problems. The aim of this study was to analyse the presence of M. hominis and U. urealyticum in the cervical canal of uterus of women with and without fertility problems. METHODS: Endocervical swabs obtained from women with reproductive problems and fertile women were tested by both cultivation and polymerase chain reaction. The antimicrobial susceptibility to the azithromycin, ciprofloxacin, doxycycline and erythromycine of the isolated strains of M. hominis and U. urealyticum was also tested by the microdilution broth method. RESULTS: A total of 111 women with fertile problems were examined. U. urealyticum was detected in samples from 44 (39.6%) women. M. hominis was detected in significantly fewer samples, i.e. only from 9 (8.1%) samples. From these, 6 (5.4%) women were positive for both microorganisms. The fertile group consisted from 23 women. The presence of U. urealyticum was detected in 8 (34.7%) of them. M. hominis was detected only in the mixture with U. urealyticum in 3 (13.0%) cases. The most effective antibiotic against both species in our study was doxycycline. CONCLUSION: The results show slightly higher incidence of M. hominis and U. urealyticum in the genitourinary tract of women with fertility problems compare with control group. The potential negative effect of these species on the reproduction ability of women was not observed.

RESUMO

Recent knowledge on vitamin D has shown that its active form not only regulates calcium and phosphate metabolism but also has significant antimitotic and cell differentiation effects. It can inhibit proliferation, angiogenesis and metastatic potential in cancer tissue. Insufficient vitamin D plasma levels are found in 20- 60% of cancer patients at dia-gnosis. By many authors, vitamin D deficiency is associated with higher aggressivity of tumor and shorter survival of patients. Even in the absence of clinical studies showing benefit of supplementation on outcome, clear recommendations are currently available for treatment of vitamin D deficiency. Owing to the high prevalence of vitamin D insufficiency in cancer patients and significant risks of its further decrease after antitumor therapy, it should become standard of care to examine 25- hydroxyvitamin D serum levels and correct vitamin D insufficiency in cancer patients.

Sixty males and 2 females with the mean age of 58.1 years ranging from 39 to 80 years with atherosclerotic lesions of the arteries of lower extremities were studied. The claudication distance was subdivided into two segments: the first included the distance up to the onset of pain and the second one when the patients had to stop walking because of the severity of pain. The systemic systolic pressure and the poststenotic ankle pressure over both arteries on both calves by a Doppler ultrasound apparatus were measured and the individual pressure gradients were calculated. It was demonstrated that both claudication intervals were related to the poststenotic pressure as well as to the pressure gradient. The stepwise regression analysis showed a significant relationship between both distances and the lowest value of all ankle pressure measurements over four tibial arteries. The closest correlation (R = 0.98) was recorded between the initial and final claudication distance and it persisted even after the elimination of all pressure parameters.

The authors examined the relationship between the ankle pressure, or pressure gradient on the one hand, and the walking distance and different actions, i.e. equalting and standing on tip toes, on the other hand. In 56 male patients with obliterating atherosclerosis, the first stage showed both pressure parameters as closely correlated to the final claudication distance, but not to the actions indicated above. At the second stage, 62 persons showed both the initial and final claudication distances as dependent on the poststenotic pressure as well as on the pressure gradient. Stepwise regression analysis showed a significant dependence of both distances on the lowest pressure measured on four crural arteries. The closest correlation [R = 0.98] was found between the initial and final claudication distances which persisted even after elimination of all pressure parameters.
