RESUMO

OBJECTIVE: This study aims to evaluate dental decay incidence and DMF index, plaque index and gingival index, and to assess the satisfaction/dissatisfaction in acrylic removal partial denture wearers (ARPD) and in partial edentate no wearers. It aims to improve the therapeutic management in prosthetic oral rehabilitation in DR Congo. MATERIAL AND METHODS: This prospective cohort interventional randomized study on 376 patients enrolled and divided into two groups (with and without denture prosthesis) was performed in University Clinics of Kinshasa, Kinshasa Medical Center and Clinic Glory during the period August 2008 to August 2010. RESULTS: Three hundred seventy-six patients (376) with 163 men (47 ± 16 years) and 213 women (42 ± 15 years) were followed. Of the 376 patients randomized, 189 were of "prosthesis" group and 187 were in the group "without aid". Both groups were regularly reviewed every 6 months for 2 years. Ten thousand four hundred and fifty-two (10452) teeth were examined (5149 teeth to the group "prosthesis" and 5303 for the teeth "without prosthesis" group. On hundred and twenty-nine (129) teeth that presented caries, 110 (82.96 per cent) are decayed teeth in the "prosthesis" group and 19 (17.04 per cent) are in the group "without prosthesis" (p = 0.00). The index Tooth Decayed, Missing, Filled (CAD) to start of the study for each group was 0.2. Two years after the index rose to 0.3 for the group "prosthesis" and remained steady (0.2) for the group "without prosthesis". The "prosthesis" group showed plaque index (PI) and gingival index (GI) above 0.99 (SD: ± 0.28) and 1.27 (SD: ± 0.43) than normal compared to those of the "no aid" group (0.46 ± 0.15 and 0.32 ± 0.12). The differences were statistically significant for both indices between the two groups (p = 0.00). Overall satisfaction rate for the "prosthesis" group was 26.4% while for the group "without prosthesis" satisfaction rate is 80.9%. CONCLUSION: This study shows that improved oral hygiene and regular check of the RPD acrylic resin carriers become a concern for establishing a true oral health policy.

RESUMO

The authors describe the early neglected oral communication of maxillary sinus and nasal cavity, causing reflux of liquid and sometimes solids through the nose, followed by immediate intrabuccal flow or posterior pharyngeal with fetidness. The objective of this work was to improve the therapy of immediate or delayed oral communication of maxillary sinus with nasal cavity and eventually and the quality of life.

RESUMO

Les auteurs rapportent le cas d'un patient de 69 ans; en rehabilitation prothetique pour edentement subtotal bilateral et biterminal. L'echec d'une premiere prothese posee en milieu non specialise; a conduit a la reprise du traitement dans le service de protheses aux cliniques universitaires de Kinshasa (CUK); avec bonne recuperation morphofonctionnelle. Cette observation illustre la carence en infrastructures et en personnel qualifie dans les soins prothetiques dans notre pays ; elle met en exergue les exigences techniques a respecter absolument pour un aboutissement satisfaisant du traitement

RESUMO

Une etude retrospective portant sur 3.360 dossiers relatifs aux extractions dentaires pratiquees au Centre Medico-Chirgical Militaire couvrant une periode de 5 ans; a montre que 140 cas avaient developpe une alveolite post-extractionnelle; soit 2;2 pour cent de l'ensemble. L'etude a surtout revele que les 92 pour cent d'alveolites observees etaient seches et que leur etiopathogenie serait imputable en grande partie aux bains de bouche a l'eau salee; pratiques de maniere abusive par les patients apres intervention; sur recommandation de certains praticiens. Une regression sensible de nombre d'alveolites est constatee depuis que la pratique des bains de bouche a l'eau salee a ete decommandee

RESUMO

Une incisive centrale permanente superieure est restee incluse jusqu'a l'age de 12 ans chez un sujet de sexe masculin. La cause en etait une retention de son homologue de lait. Une desinclusion a ete operee et obtenue avec succes grace a une traction lente au moyen d'un fil en acier de section 0;3 mm selon une technique tres simple. Cette incisive presentait egalement une dysharmonie orthodontique (rotation). Cette derniere fut corrige dans une deuxieme phase; toujours avec des moyens simples

RESUMO

Ayant observe qu'au sein de la population zairoise de Kinshasa; certaines personnes etaient porteuses de couronnes postiches en or sur l'une ou l'autre dent frontale; sans gene; les auteurs ont recherche des mobiles profonds de cette pratique et releve ses consequences eventuelles. Le travail repose sur 57 observations dont 41 concernaient des femmes contre 16 pour les hommes. L'examen clinique succint et les enquetes menees ont revele que: ces couronnes postiches en or sont exclusivement l'oeuvre des non chirurgiens-dentistes; les incisives laterales et canines sont des dents cibles; le but poursuivi par les porteurs de ces protheses est double (l'expression de leur fortune; l'or etant un metal precieux et brillant) et le snobisme. Les auteurs deconseillent cette pratique compte tenu de ses consequences nefastes sur l'occlusion; sur le parodonte marginal et de la responsabilite qui incombe a de telles couronnes dans l'eclosion de la carie sans oublier l'incompetence et la non-qualification des praticiens

RESUMO

Ce texte presente une etude menee au centre neuro-psychopathologique de Kinshasa au Zaire et concernant 98 malades des 2 sexes hospitalises qui revele que la grande majorite de ces sujets ont ont une hygiene buccale defectueuse. Outre les tartres; la carie dentaire et la gingivite qui constituent les pathologies orales les plus courantes; l'etude revele surtout une usure pathologique et inquietante des couronnes dentaires chez tous les malades : cette ablasion est imputable au bruxisme. Les auteurs estiment qu'il incombe aux psychiatres et neurologistes d'orienter periodiquement des malades mentaux vers les consultations odonto-stomatologiques

Oral lesions associated with HIV infection as classified by the WHO Collaborative Centre for Oral manifestation of HIV were studied in 83 heterosexual AIDS patients in Kinshasa, Zaire. One or more lesions were fungal (94%), bacterial (33%) and viral (23%). Neoplasm was found in 12% of cases while lesions of unknown etiology in 14%. As location of those lesions, the palate, lips, tongue and the buccal mucosae mostly predominate as sites. The findings from this study were clinically similar to those reported in AIDS in other areas.


PIP: From January 1988-September 1989, dental practitioners performed a comprehensive oral examination on 83 HIV positive patients at the Department of Infectious Diseases, University Clinic of Internal Medicine in Kinshasa, Zaire for a study on prevalence and clinical aspects of oral lesions associated with HIV infection. Women comprised 55.5% of these AIDS patients. They all had oral lesions: 94% fungal, 33% bacterial, 23% viral, 14% unknown origin, and 12% neoplasms. The majority of these oral lesions developed in 31-40 year olds. Further, the 21-30 year olds were more likely to have bacterial infections, especially aggressive periodontitis. Fungal infections occurred most often on the lips, palate, and tongue, while viral infections occurred mainly on the tongue. Kaposi's sarcoma only afflicted the palate. Pseudomembranous candidiasis was the leading fungal infection (32% of all oral lesions) then atrophic (22.8%) and hyperplastic (6%) types. 17% and 16% of all lesions included these bacterial infections: aggressive periodontitis and necrotizing gingivitis respectively. the leading viral infection was hairy leukoplakia (14%) followed by leukoplakia (8%), and herpetic stomatitis (4%). The unknown lesions included ulcers (12%) and a swollen salivary gland )2%). 12% of the examined AIDS patients, mostly 31-50 year olds, had oral Kaposi's sarcoma. They also had it on other parts of the body. Since HIV prevalence in Zaire ranges between 3-8%, all dentists should be cognizant of oral manifestations of HIV which may indeed be the 1st clinical indications of HIV. They should refer any patients with such lesions to a health facility with AIDS specialists for diagnosis and care.

RESUMO

The authors study hygienic techniques used by the Zaïre population of Kinshasa. The study was carried out among 1190 men and women with an average age of 46 and 40 respectively. The results show that most of the examined people brushed their teeth once a day. Three groups of traditional tools are used: A vegetable tool (root, creeper) A vegetable residue (wood embers) A mineral tool (sand) However, the majority of the examined people used a toothbrush.
