RESUMO

A 34 years old male patient was referred to our clinic for restorative dental treatment. During detailed consultation and dental examination a relatively rare form of periodontal disease had been diagnosed. Intraoral examination included recording of dental and periodontal status. Based on patient's dental history, measurements of probing pocket depths (PPD), clinical attachment level (CAL), and also the X-ray findings, Localized Aggressive Periodontitis (LAP) unknown by the patient was diagnosed. After patient's consent the comprehensive treatment plan covered the dental prevention, periodontal non-surgical and surgical therapy and rehabilitation. The treatment started with oral hygienic instruction, motivation then supra- and subgingival scaling and rootplaning. Later extraction and elective root canal treatment were performed, followed by open flap periodontal surgery combined with hemisection of two molars. After a full mouth conservative restorative therapy, function and esthetics were restored by fix dental prostheses. This case is a good example to underline the importance of periodontal examination during the dental screening and dental status recording for each patients showing up at dental clinics. Otherwise in many cases this asympthomatic disease can remain undetected.

RESUMO

The aim of this review is to overview the development achieved in restorative therapy based on the publications in Fogorvosi Szemle during the past hundred years. The top quality of the restorative therapy was, and still has been the inlay, but nowadays the esthetic inlay -- fixed with adhesive material -- has priority over the gold inlay. The quality of amalgam fillings has been recently improved by using high-copper containing amalgam alloys. The development on the field of adhesive material and methods made it possible to give up the routine use of any conventional base material. The place of gingival wall of the cavity moved from subgingival position into supragingival position. The macro retention has been replaced by micro retention, and nowadays the cavity preparation prefers minimal-invasive techniques.

RESUMO

Present review makes an attempt to summarize the Hungarian endodontic literature of pulpal and periodontal diseases, published during the past 100 years. The experimental examinations and clinical studies make it possible to follow the changes in the methods and in the medicines, used in the field of pulpal treatment. The overview gives us information about the problem of disinfection, shaping, measuring, and obturation of root canal.

The newly developed self-etching, self-priming all-in-one adhesives are appealing to clinicians because they are simple and efficient to use. These single-application bonding systems contain chemically active compounds that can alter pulpal blood circulation when applied to deep dentin surfaces. Since adequate microcirculation and oxygenation are the basic requirements for tissue survival, the aim of this study was to investigate the immediate vascular effect of a new self-etching adhesive, Prompt L-Pop/composite and compomer version/(test group). The technique of vitalmicroscopy was used to record the changes in vessel diameter of the first lower incisor of 20 (10-10 in each group) male Sprague-Dawley rats (weighing 315 +/- 74/SE/g) prior to, and at 5, 15, 30 and 60 minutes after the investigated materials were applied to dentin. The application of saline served as the untreated control. The systemic arterial pressure remained unchanged throughout the experiments both in the control (110 +/- 8 mmHg) and in test animals (114 +/- 4 mmHg). In control rats, the diameter of the vessels was stable during the experiment. In the presence of Prompt L-Pop, the diameters decreased significantly during the experimental period (5 minutes: -11.15 +/- 5.03%; 15 minutes: -14.66 +/- 7.71%; 30 minutes: -13.35 +/- 5.79%; 60 minutes: -11.82 +/- 5.63% p < 0.05 in each cases). In this group, stasis developed in pulpal circulation was 1 out of 10 rats. The results from the rat model used in this study suggest that Prompt L-Pop may result in compromised pulpal microcirculation.

After crown preparation, exposed and untreated dentinal tubules can result in bacterial penetration into pulp. Treating the exposed dentin involves closing the tubules. Dentin sealers are often applied on a very thin dentin layer that covers the pulp chamber. In these cases, the sealers may have some effect on local micro-circulation through dentin. This study examined the acute effects of different dentin sealers on the vascular-diameter of pulpal vessels measured by vitalmicroscopic technique in rats. Gluma Desensitizer in Group 1 (n = 10), Seal & Protect with acid etching in Group 2 (n = 10) and Seal & Protect without acid etching in Group 3 (n = 10) were applied on a very thin layer of dentin in the left lower incisor of male Sprague-Dawley rats weighing 336 +/- 93SE/g. Saline served as the untreated control. After one-hour equilibration time, changes in vessel diameter were recorded with a digital camera connected to a microscope at baseline and at 5, 15, 30 and 60 minutes after the investigated materials were administered on dentin. The results were evaluated by ANOVA. In each group, diameter changes were averaged (compared to the baseline diameters) and standard errors of the mean were calculated for each examined time. The results suggest that Gluma Desensitizer caused the most severe pulpal vessel-diameter changes, followed by Seal & Protect with acid etching, while the least change was recorded in Seal & Protect without acid etching.

INTRODUCTION: Besides the main etiological factors other factors are also involved in development of caries. Bacteria (Streptococcus mutans, lactobacilli) and saliva (its buffering capacity, and rate of secretion) are also risk factors in the development of caries. MATERIAL, METHOD, RESULTS: The authors were the first in Hungary to use the Dentocult kits for assessing the number of lactobacilli in saliva, to use the Oricult-N plate for the determination of the number of Candida, and the medium containing Dentocult-SM plates in the assessment of the number of Streptococcus mutans colonies in saliva. Children in Fót were divided into two groups: children who consumed sorbitol-containing sweets, and another consisting of children who consumed sucrose-containing sweets. The number of lactobacilli, as well as the caries-index was determined. The proportion of children with high lactobacillus counts was significantly lower in the sorbitol group than in the sucrose group. Caries prevalence and salivary lactobacillus counts were determined in 140 second and third graders and 150 adults. The difference between caries-active and caries-inactive subjects was statistically significant in both age groups. A relationship between lactobacillus and Candida albicans counts was found in 396 children between the ages of 6 and 11 living in children's homes. The microbiological assessment of the saliva of 269 children, from seven different children's home, all between the ages of 7 and 12, who were included in the WHO's three-year xylitol caries-preventive program, was also done. Cariological, saliva and microbiological investigations were carried out on 349 adolescents secondary school students. The amount of bacteria and yeasts showed a strong relationship with the caries mean values. The saliva of 210 fixed orthodontic appliances wearing--using amine fluoride-containing products--children, was investigated. According to the results, amine fluoride has a pronounced effect on the mouth's microflora, especially on the number of lactobacilli and Candida.
