BACKGROUND: Transcatheter aortic valve implantation (TAVI) has established itself as the preferential strategy to approach severe aortic stenosis. Information on procedural improvements and nationwide results obtained with the technique throughout the past decade are unknown. OBJECTIVES: To assess the temporal variation of the demographic profile, procedural characteristics, and in-hospital outcomes of patients undergoing TAVI procedures at the Rede D'Or São Luiz. METHODS: Observational registry comprising 29 national institutions, comparing the characteristics of the TAVI procedures performed from 2012 to 2017 (Group 1) to those performed from 2018 to 2023 (Group 2). The statistical significance level adopted was p < 0.05. RESULTS: This study assessed 661 patients, 95 in Group 1 and 566 in Group 2, with a mean age of 81.1 years. Group 1 patients had a higher prevalence of New York Heart Association functional class III or IV and STS risk score > 8%. In addition, they more often underwent general anesthesia, transesophageal echocardiographic monitoring, and access through femoral dissection. Group 2 patients had a higher success rate of the TAVI procedure (95.4% versus 89.5%; p = 0.018), lower mortality (3.9% versus 11.6%; p = 0.004), and less often needed permanent pacemaker implantation (8.5% versus 17.9%; p = 0.008). CONCLUSIONS: The 10-year temporal trends analysis of the TAVIDOR Registry shows a reduction in patients' clinical complexity over time. Furthermore, the advance to minimalistic implantation techniques, added to the technological evolution of the devices, may have contributed to the favorable outcomes observed among those whose implantation occurred in the last 5 years studied.


FUNDAMENTO: O implante percutâneo de bioprótese valvar aórtica (TAVI) consolidou-se como opção terapêutica da estenose aórtica de grau importante. Dados sobre as características evolutivas dos procedimentos e dos resultados obtidos com a técnica ao longo da última década, em escala nacional, são desconhecidos. OBJETIVOS: Analisar a tendência temporal referente ao perfil demográfico, características dos procedimentos e desfechos hospitalares de pacientes submetidos a TAVI na Rede D'Or São Luiz. MÉTODOS: Registro observacional envolvendo 29 instituições nacionais. Comparou-se características dos procedimentos realizados de 2012 a 2017 (Grupo 1) e de 2018 a 2023 (Grupo 2). Foram considerados significantes os resultados com valor de p < 0,05. RESULTADOS: Foram analisados 661 casos, 95 pertencentes ao Grupo 1 e 566 ao Grupo 2. A média de idade foi 81,1 anos. Observou-se no Grupo 1 maior prevalência de pacientes em classe funcional III ou IV e escore de risco > 8%. Foi mais frequente o emprego de anestesia geral, monitorização ecocardiográfica transesofágica e via de acesso por dissecção. Maior taxa de sucesso do procedimento (95,4% versus 89,5%; p = 0,018) foi aferida em implantes efetivados a partir de 2018, assim como menor mortalidade (3,9% versus 11,6%; p = 0,004) e necessidade de marcapasso definitivo (8,5% versus 17,9%; p = 0,008). CONCLUSÕES: A análise temporal de 10 anos do Registro TAVIDOR demonstra uma queda na complexidade clínica dos pacientes. Além disso, o avanço para técnicas de implante minimalistas, somadas à evolução tecnológica dos dispositivos, podem ter contribuído para desfechos favoráveis dentre aqueles cujo implante ocorreu no último quinquênio.

Frugivore bats are important seed dispersers in forests and their abundance are associated with the presence of zoochoric plants. In this context, the aim of our study was to investigate the association of the frugivore bat S. lilium with the diaspores of the zoochoric plant S. mauritianum, a common arboreal species present in forest fragments of southern Brazil. We also investigated the diet of the species based on seed content present in feces of individuals. Bats were mist-netted from November 2017 to April 2018 in a fragment of Atlantic Forest. The proportion of immature and mature diaspores of S. mauritianum was estimated in the same area where bats were sampled, and feces were sampled from captured individuals. In total, 61 individuals of S. lilium were captured, and 795 seeds were sampled from their feces. The abundance of S. lilium was significantly associated with the proportion of immature diaspores of S. mauritianum. We identified seeds of two botanical families: Solanaceae (89%) and Moraceae (11%) in the fecal samples. Our findings support the view that S. lilium is a legitimate disperser of S. mauritianum, and that its ecological function is probably a result of co-adaptation.

The title compound (C14H23N3S, common name: cis-jasmone 4-ethyl-thio-semicarbazone) was synthesized by the equimolar reaction of cis-jasmone and 4-ethyl-thio-semicarbazide in ethanol facilitated by acid catalysis. There is one crystallographically independent mol-ecule in the asymmetric unit, which shows disorder of the terminal ethyl group of the jasmone carbon chain [site-occupancy ratio = 0.911 (5):0.089 (5)]. The thio-semicarbazone entity [N-N-C(=S)-N] is approximately planar, with the maximum deviation of the mean plane through the N/N/C/S/N atoms being 0.0331 (8) Å, while the maximum deviation of the mean plane through the five-membered ring of the jasmone fragment amounts to -0.0337 (8) Å. The dihedral angle between the two planes is 4.98 (7)°. The mol-ecule is not planar due to this structural feature and the sp 3-hybridized atoms of the jasmone carbon chain. Additionally, one H⋯N intra-molecular inter-action is observed, with graph-set motif S(5). In the crystal, the mol-ecules are connected through pairs of H⋯S inter-actions with R 2 2(8) and R 2 1(7) graph-set motifs into centrosymmetric dimers. The dimers are further connected by H⋯N inter-actions with graph-set motif R 2 2(12), which are related by an inversion centre, forming a mono-periodic hydrogen-bonded ribbon parallel to the b-axis. The crystal structure and the supra-molecular assembly of the title compound are compared with four known cis-jasmone thio-semicarbazone derivatives (two crystalline modifications of the non-substituted form, the 4-methyl and the 4-phenyl derivatives). A Hirshfeld surface analysis indicates that the major contributions for the crystal cohesion are from H⋯H (70.7%), H⋯S/S⋯H (13.5%), H⋯C/C⋯H (8.8%), and H⋯N/N⋯H (6.6%) inter-faces (only the disordered atoms with the highest s.o.f. were considered for the evaluation).

BACKGROUND: High-intensity interval training (HIIT) promotes similar or superior gains in physical fitness and health compared to continuous moderate-intensity training (MICT) in young individuals. However, the effectiveness of HIIT in older adults is uncertain. OBJECTIVE: This meta-analysis compared the effects of HIIT and MICT on cardiorespiratory fitness, body composition, vascular, metabolic, and hormonal variables, cognitive function, and quality of life in older adults. METHODS: PubMed, Embase, Cochrane, Web of Science, Scopus, and SportDiscus databases were searched from inception until December 2023. Trials included adults ≥ 60 y. Hedge's effect sizes (g) were calculated and study quality and features of training interventions were tested as moderators. RESULTS: Twenty-nine trials including 1,227 subjects (65.4 ± 3.3 y). HIIT and MICT elicited significant (p < 0.021) and similar (p > 0.156) 'gs' for VO2max (0.722 vs. 0.490), maximal workload/peak power output ratio (0.290 vs. 0.372),% fat (-0.297 vs. -0.262), glycemia (-0.273 vs. -0.334), systolic (-0.465 vs. -0.341), and diastolic blood pressure (-0.566 vs. -2.311). HIIT (p < 0.04) but not MICT (p > 0.170) produced significant 'gs' for fat mass (-0.245 vs. -0.103), waist circumference (-0.239 vs. -0.116), testosterone (0.339 vs. 0.170), and complex Stroop test (-0.595 vs. -0.250). Increases in the VO2max occurred only in controlled trials after HIIT but not MICT (Q = 6.286, p = 0.012; HITT-g = 1.068, p < 0.0001 vs. MICT-g = 0.109, p= 0.596). CONCLUSION: HIIT and MICT produced similar changes in most of the investigated variables. In trials with greater quality (controlled vs. non-controlled trials), cardiorespiratory gains were higher in HIIT vs. MICT. These findings suggest that HIIT might be suitable for improving fitness and health in older adults.

The evaluation of hematological and plasma biochemical parameters and the subsequent establishment of reference intervals facilitate the diagnosis of the health status of animals. This work aimed to determine the blood parameters of wild specimens of the stingrays Potamotrygon motoro and Potamotrygon orbignyi from the lower Solimões River region, Amazonas, Brazil. One hundred forty-one stingrays were captured, 92 specimens of P. motoro and 49 of P. orbignyi, of both sexes and at different stages of development. No effect of sex was observed on the blood parameters of juvenile animals for both species. P. motoro neonates presented a distinct hematological and biochemical profile, with significantly lower hematocrit values, hemoglobina, number of erythrocytes, mean corpuscular hemoglobin concentration, monocytes, plasma glucose, total proteins, albumin, and globulin. On the other hand, total cholesterol and urea levels were significantly higher in this same group compared to juveniles of the same species. Comparison between species revealed lower values of triglycerides and total cholesterol in P. orbignyi of both sexes. The results obtained are pioneering for these Amazonian species in white water environments and will serve as a basis for evaluating the health status of wild stingrays. Thus, from the analysis of the blood of the P. motoro and P. orbignyi stingrays, it was possible to observe good health conditions.

Mercury (Hg) contamination on the Brazilian southeast coast has been highlighted, especially in relation to species of commercial importance. This study aimed to quantify the Hg concentration in species of mussels, fish, and crabs obtained from fishing colonies on the beaches of the west and south of the city of Rio de Janeiro, investigate the possible effect of seasonality, sex, and location sampling, and finally, the risk to consumer health in four different age groups. The difference between cooked and raw commercially available mussel samples was also verified. The main results highlight that the fish presented higher levels of Hg and that the mussel samples sold cooked presented lower levels of Hg when compared to the raw ones. For Micropogonias furnieri, Sardinella brasiliensis, and Callinectes spp., the season variable influenced Hg concentrations, while only for Merluccius merluccius was a difference between the sexes identified, with males having the highest values. Although Hg concentrations in animals were below the permitted limit, consumption of all species exceeded the monthly intake limit for this metal. For the hazard quotient (HQ) calculation, most species presented HQ > 1, especially when ingested by the child population. These results are fundamental for designing consumption strategies for these animals, in order to prioritize the acquisition of some species over others, depending on the location and/or season, for each age group, with the aim of maintaining seafood consumption with minimal risk to the population's health.

The equimolar and hydro-chloric acid-catalysed reaction between cis-jasmone and 4-methyl-thio-semicarbazide in ethano-lic solution yields the title compound, C13H21N3S (common name: cis-jasmone 4-methyl-thio-semicarbazone). Two mol-ecules with all atoms in general positions are present in the asymmetric unit. In one of them, the carbon chain is disordered [site occupancy ratio = 0.821 (3):0.179 (3)]. The thio-semicarbazone entities [N-N-C(=S)-N] are approximately planar, with the maximum deviation from the mean plane through the selected atoms being -0.0115 (16) Š(r.m.s.d. = 0.0078 Å) for the non-disordered mol-ecule and 0.0052 (14) Š(r.m.s.d. = 0.0031 Å) for the disordered one. The mol-ecules are not planar, since the jasmone groups have a chain with sp 3-hybridized carbon atoms and, in addition, the thio-semicarbazone fragments are attached to the respective carbon five-membered rings and the dihedral angles between them for each mol-ecule amount to 8.9 (1) and 6.3 (1)°. In the crystal, the mol-ecules are connected through pairs of N-H⋯S and C-H⋯S inter-actions into crystallographically independent centrosymmetric dimers, in which rings of graph-set motifs R 2 2(8) and R 2 1(7) are observed. A Hirshfeld surface analysis indicates that the major contributions for the crystal cohesion are from H⋯H (70.6%), H⋯S/S⋯H (16.7%), H⋯C/C⋯H (7.5%) and H⋯N/N⋯H (4.9%) inter-actions [considering the two crystallographically independent mol-ecules and only the disordered atoms with the highest s.o.f. for the evaluation].

In this paper, the practical application of a bio-inspired antenna for partial discharge (PD) detection in high voltage equipment was evaluated in order to validate the efficiency of using this technology for PD monitoring purposes. For this, PD measurements using the bio-inspired antenna were performed on operational 69 kV potential transformers (PT) in a real substation. After the field experiment, laboratory measurements using the IEC 60270 standard method and a bio-inspired antenna were performed, simultaneously, over the evaluated PT. The results obtained at the substation indicated suspicious frequencies of partial discharge activity in two out of three evaluated potential transformers, mainly for the frequencies of 461 MHz, 1366 MHz, 1550 MHz and 1960 MHz. During the laboratory tests, the presence of partial discharge activity over the suspicious potential transformers was confirmed with the detection of PD apparent charge levels above 20 pC. Finally, the frequency spectrum obtained from the PD signals detected by the bio-inspired antenna in the laboratory presented similar frequency values to those obtained during the practical application at the substation, making it a promising indicator for future defect classification studies using artificial intelligence.

The reaction of (2E)-N-phenyl-2-[(2E)-3-phenyl-2-propen-1-yl-idene]hydra-zine-carbo-thio-amide (common name: cinnamaldehyde-4-phenyl-thio-semi-carbazone) deprotonated with NaOH in ethanol with an ethano-lic suspension of PdII chloride in a 2:1 molar ratio yielded the title compound, [Pd(C16H14N3S)2]. The anionic ligands act as metal chelators, κ2 N 1 S-donors, forming five-membered rings with a trans-configuration. The PdII ion is fourfold coordinated in a slightly distorted square-planar geometry. For each ligand, one H⋯S and one H⋯N intra-molecular inter-actions are observed, with S(5) and S(6) graph-set motifs. Concerning the H⋯S inter-actions, the coordination sphere resembles a hydrogen-bonded macrocyclic environment-type. In the crystal, the complexes are linked via pairs of H⋯S inter-actions, with graph-set motif R 2 2(8), and building a mono-periodic hydrogen-bonded ribbon along [001]. The Hirshfeld surface analysis indicates that the major contributions for the crystal cohesion are: H⋯H (45.3%), H⋯C/C⋯H (28.0%), H⋯S/S⋯H (8.0%) and H⋯N/N⋯H (7.4%).

Mercury is an element with potential risk to fish and those who consume it. Thus, this study aimed to determine the levels of total mercury (THg), carry out a health risk assessment related to the consumption of the freshwater stingrays Potamotrygon motoro, and determine the physical and chemical properties of the water where stingrays occur. Stingrays of the species P. motoro were obtained from the Amazon River, and samples of the animals' musculature were collected to determine THg levels. Risk assessment was conducted using pre-established formulas of estimated monthly intake (EMI), maximum monthly intake rate (IRmm), and hazard quotient (HQ). Three population scenarios were evaluated, considering both sexes and differences between rural and urban areas. There was no relationship between weight and THg concentration nor between total length and THg concentration. Higher EMI values were observed in rural children; for the IRmm, male children had the lowest consumption levels. For the hazard quotient, there was a similarity between the three age groups when comparing the male and female sexes. In addition, the representatives of the rural area always had lower values than the urban area. Freshwater stingrays, like other elasmobranchs, can be crucial animal species because they act as sentinels in studies that assess harmful chemicals like mercury.

Objetivo: Descrever a presença da prática de exercício físico e de função física em um grupo de idosos durante a pandemia por COVID-19 no interior do Amazonas. Métodos: Trata-se de um estudo transversal realizado com idosos ativos que realizavam exercício físico por pelo menos 3 vezes na semana por 50 minutos no interior do Amazonas. Avaliou-se a prática de exercício físico antes e durante a pandemia e aplicaram-se duas questões investigativas referente a COVID-19. Para avaliar a capacidade funcional e física aplicou-se o Short Physical Performance Battery (SPPB). Resultados: A amostra foi composta por 63 idosos ativos da comunidade. Destes, 31,8% (21) eram analfabetos, 74,2% (49) aposentados e 75,8 % (50) de baixa renda. No SPPB 27,3% (18) apresentaram total de 10 pontos e 14,29% (9) dos idosos realizam exercício físico acima dos minutos recomendados pela Organização Mundial da Saúde (OMS). Conclusão: Neste estudo nenhum fator durante a pandemia interferiu na prática de exercício físico e nem na capacidade funcional dos idosos que residem no interior do Amazonas.

A second crystalline modification of the title compound, C12H19N3S [common name: cis-jasmone thio-semicarbazone] was crystallized from tetra-hydro-furane at room temperature. There is one crystallographic independent mol-ecule in the asymmetric unit, showing disorder in the cis-jasmone chain [site-occupancy ratio = 0.590 (14):0.410 (14)]. The thio-semicarbazone entity is approximately planar, with the maximum deviation from the mean plane through the N/N/C/S/N atoms being 0.0463 (14) Š[r.m.s.d. = 0.0324 Å], while for the five-membered ring of the jasmone fragment, the maximum deviation from the mean plane through the carbon atoms amounts to 0.0465 (15) Š[r.m.s.d. = 0.0338 Å]. The mol-ecule is not planar due to the dihedral angle between these two fragments, which is 8.93 (1)°, and due to the sp 3-hybridized carbon atoms in the jasmone fragment chain. In the crystal, the mol-ecules are connected by N-H⋯S and C-H⋯S inter-actions, with graph-set motifs R 2 2(8) and R 2 1(7), building mono-periodic hydrogen-bonded ribbons along [010]. A Hirshfeld surface analysis indicates that the major contributions for the crystal cohesion are H⋯H (67.8%), H⋯S/S⋯H (15.0%), H⋯C/C⋯H (8.5%) and H⋯N/N⋯H (5.6%) [only non-disordered atoms and those with the highest s.o.f. were considered]. This work reports the second crystalline modification of the cis-jasmone thio-semicarbazone structure, the first one being published recently [Orsoni et al. (2020 ▸). Int. J. Mol. Sci. 21, 8681-8697] with the crystals obtained in ethanol at 273 K.

The reaction between a racemic mixture of (R,S)-fixolide and 4-methyl-thio-semicarbazide in ethanol with a 1:1 stoichiometric ratio and catalysed with HCl, yielded the title compound, C20H31N3S [common name: (R,S)-fixolide 4-methyl-thio-semicarbazone]. There is one crystallographically independent mol-ecule in the asymmetric unit, which is disordered over the aliphatic ring [site-occupancy ratio = 0.667 (13):0.333 (13)]. The disorder includes the chiral C atom, the neighbouring methyl-ene group and the methyl H atoms of the methyl group bonded to the chiral C atom. The maximum deviations from the mean plane through the disordered aliphatic ring amount to 0.328 (6) and -0.334 (6) Š[r.m.s.d. = 0.2061 Å], and -0.3677 (12) and 0.3380 (12) Š[r.m.s.d. = 0.2198 Å] for the two different sites. Both fragments show a half-chair conformation. Additionally, the N-N-C(=S)-N entity is approximately planar, with the maximum deviation from the mean plane through the selected atoms being 0.0135 (18) Š[r.m.s.d. = 0.0100 Å]. The mol-ecule is not planar due to the dihedral angle between the thio-semicarbazone entity and the aromatic ring, which amounts to 51.8 (1)°, and due to the sp 3-hybridized carbon atoms of the fixolide fragment. In the crystal, the mol-ecules are connected by H⋯S inter-actions with graph-set motif C(4), forming a mono-periodic hydrogen-bonded ribbon along [100]. The Hirshfeld surface analysis suggests that the major contributions for the crystal cohesion are [(R,S)-isomers considered separately] H⋯H (75.7%), H⋯S/S⋯H (11.6%), H⋯C/C⋯H (8.3% and H⋯N/N⋯H (4.4% for both of them).

The reaction between the (R,S)-fixolide 4-methyl-thio-semicarbazone and PdII chloride yielded the title compound, [Pd(C20H30N3S)2]·C2H6O {common name: trans-bis-[(R,S)-fixolide 4-methyl-thio-semicarbazonato-κ2 N 2 S]palladium(II) ethanol monosolvate}. The asymmetric unit of the title compound consists of one bis-thio-semicarbazonato PdII complex and one ethanol solvent mol-ecule. The thio-semicarbazononato ligands act as metal chelators with a trans configuration in a distorted square-planar geometry. A C-H⋯S intra-molecular inter-action, with graph-set motif S(6), is observed and the coordination sphere resembles a hydrogen-bonded macrocyclic environment. Additionally, one C-H⋯Pd anagostic inter-action can be suggested. Each ligand is disordered over the aliphatic ring, which adopts a half-chair conformation, and two methyl groups [s.o.f. = 0.624 (2):0.376 (2)]. The disorder includes the chiral carbon atoms and, remarkably, one ligand has the (R)-isomer with the highest s.o.f. value atoms, while the other one shows the opposite, the atoms with the highest s.o.f. value are associated with the (S)-isomer. The N-N-C(=S)-N fragments of the ligands are approximately planar, with the maximum deviations from the mean plane through the selected atoms being 0.0567 (1) and -0.0307 (8) Š(r.m.s.d. = 0.0403 and 0.0269 Å) and the dihedral angle with the respective aromatic rings amount to 46.68 (5) and 50.66 (4)°. In the crystal, the complexes are linked via pairs of N-H⋯S inter-actions, with graph-set motif R 2 2(8), into centrosymmetric dimers. The dimers are further connected by centrosymmetric pairs of ethanol mol-ecules, building mono-periodic hydrogen-bonded ribbons along [011]. The Hirshfeld surface analysis indicates that the major contributions for the crystal cohesion are [atoms with highest/lowest s.o.f.s considered separately]: H⋯H (81.6/82.0%), H⋯C/C⋯H (6.5/6.4%), H⋯N/N⋯H (5.2/5.0%) and H⋯S/S⋯H (5.0/4.9%).

RESUMO

The hydro-chloric acid-catalyzed equimolar reaction between cis-jasmone and 4-phenyl-thio-semicarbazide yielded the title compound, C18H23N3S (common name: cis-jasmone 4-phenyl-thio-semicarbazone). Concerning the hydrogen bonding, an N-H⋯N intra-molecular inter-action is observed, forming a ring with graph-set motif S(5). In the crystal, the mol-ecules are connected into centrosymmetric dimers by pairs of N-H⋯S and C-H⋯S inter-actions, forming rings of graph-set motifs R 2 2(8) and R 2 1(7), with the sulfur atoms acting as double acceptors. The thio-semicarbazone entity is approximately planar, with the maximum deviation from the mean plane through the N/N/C/S/N atoms being 0.0376 (9) Š(the r.m.s.d. amounts to 0.0234 Å). The mol-ecule is substantially twisted as indicated by the dihedral angle between the thio-semicarbazone fragment and the phenyl ring, which amounts to 56.1 (5)°, and because of the jasmone fragment, which bears a chain with sp 3-hybridized carbon atoms in the structure. The Hirshfeld surface analysis indicates that the major contributions for the crystal cohesion are: H⋯H (65.3%), H⋯C/C⋯H (16.2%), H⋯S/S⋯H (10.9%) and H⋯N/N⋯H (5.5%).

Introduction: Symptoms related to mental health disorders became the background of the COVID-19 pandemic, and psychologists had to adapt to the demands, while they themselves were exposed to the pandemic and its stressors. Objectives: To identify demographic and professional characteristics of Brazilian psychologists in different phases of pandemic and their reported care practices, concerns, and symptoms. Methods: This was an observational study conducted online in four independent phases with no pairing among the samples (May/June 2020, n = 263; November/December 2020, n = 131; May/June 2021, n = 378; November/December 2021, n = 222). Depression, Anxiety and Stress Scale was used. The validity and reliability of the data obtained with the DASS-21 were attested to by confirmatory factor analysis. Basic lexographic and similarity analysis were conducted to obtain textual information. Prevalence of variables was estimated and compared between phases using the z-test (α = 5%). Similarity analysis was performed to identify the psychologists' concerns. Results: Most of participants were women and were self-employed or employed. There was rapid adjustment to remote work and more than 70% reported changes in their mental health since the onset of pandemic. One in four participants had a previous mental health disorder, and there was a high prevalence of symptoms such as anxiety, fear, and angst. The prevalence of professionals who reported not caring about their own mental health was significant. In 2020, one cluster (health) of concern was identified, while in 2021 there were three clusters (health, family, and COVID-19). The prevalence of depression, anxiety, and stress symptoms was high and did not change during the pandemic. Conclusions: Psychologists adapted to the demands of the population in the face of the pandemic. However, there was a high prevalence of mental health symptoms and a disregard for self-care among these professionals.

RESUMO

INTRODUÇÃO E/OU FUNDAMENTOS: A valva Melody, indicada para revalvularização percutânea pulmonar, tem sido usada no Brasil desde 2013. Nos EUA a probabilidade de endocardite (EI) tardia é de 9,5% em 5 anos e 16,9% em 8 anos (mortalidade de 6,7%). Gradientes residuais (GR), faixa etária no implante e história prévia de EI têm sido implicados como fatores de risco. No Brasil, onde encontramos uma população heterogenea, não temos esta informação. Objetivo é determinar a incidência e os desfechos da EI tardia após implante de Melody no Brasil. MÉTODOS: Estudo de coorte observacional, retrospectivo, de pacientes submetidos a implante da Melody. Todos os centros implantadores foram contatados para reportar o número total de implantes e dos casos de EI, incluindo o momento do diagnóstico, manejo e o desfecho final. O diagnóstico de EI foi baseado em critérios clínicos, culturas, ecocardiograma e, por vezes, tomografia. RESULTADOS E CONCLUSÕES: 87 pacientes foram submetidos ao implante em 10 centros no Brasil. Em 9 anos de seguimento, 7 pacientes tiveram EI tardia (8,0%), com 2 óbitos (2,2% do total e 28% dos casos de EI). Em 3 pacientes o quadro foi agudo e em 4 sub-agudo. Um caso acometeu uma criança, sendo o restante em adultos, todos após 1 ano ou mais do implante. Um paciente possuía GR elevado previamente por "mismatch". A EI foi associada a piora importante do gradiente em 3 pacientes: um submetido a dilatação com balão e os outros 2 a troca valvar, com 1 óbito após cirurgia tardia. Os germes identificados foram S. aureus (1), S. viridans (1), Pseudomonas (1) e S. Cristatus (1). Em 3 pacientes as hemoculturas foram negativas. O manejo incluiu, além da antibioticoterapia, explante valvar em 3 (um óbito em paciente crítico, estenose grave e falência do VD), valvoplastia com balão em 1 e tratamento conservador em 3 (com 1 óbito devido a embolia pulmonar). Dos 7 casos de EI, em 4 identificamos fatores de risco que poderiam ter sido evitados: injeção intramuscular de anabolizantes, tratamento dentário sem profilaxia adequada, presença de piercing e tatuagem e história prévia de EI. Nos 4 casos curados nos quais a Melody foi mantida, observou-se estenose e/ou insuficiência discreta a moderada da valva. CONCLUSOES: A incidência de EI tardia na valva Melody no Brasil é significativa, mas não difere significativamente dos dados de literatura. Encontramos fatores de risco diferentes dos relatados previamente, muitos deles evitáveis. Além da incidência significativa, a EI pode se apresentar de forma aguda e ser letal.

OBJECTIVE: This study aimed to compare pain intensity, stiffness, functionality, central sensitization, and self-efficacy, between individuals with bilateral knee osteoarthritis and unilateral knee osteoarthritis. METHODS: We included sedentary participants with knee osteoarthritis. The diagnosis was defined by a specialist, in which there was a complaint of pain and/or altered function in the lower limbs (duration ≥3 months); morning stiffness; pain intensity ≥3; Kellgren-Lawrence 2-3° associated with X-ray; persistence of symptoms >3 months. We used the following tools: Western Ontario and McMaster Universities Arthritis Index, Numerical Pain Scale, Central Sensitization Inventory, and Pain Self-Efficacy Questionnaire. Intergroup comparisons were performed using the t-test. RESULTS: The sample consisted of 118 adult individuals, divided into two groups: bilateral knee osteoarthritis (n=59) and unilateral knee osteoarthritis (n=59). We observed a significant difference (p<0.05) and a large effect size (d≥0.8), in the comparisons between: stature, body mass index, physical function, central sensitization, and self-efficacy. CONCLUSION: Individuals with bilateral knee osteoarthritis have higher levels of central sensitization, impaired functionality, and a lower level of self-efficacy.
