Fecal glucocorticoid metabolites (FGMs) have been used as a non-invasive and indirect measurement of the complex stress response in a variety of species. Animals in facilities under managed care allow for the longitudinal study of FGMs in a controlled environment. Animal histories often include environmental, husbandry, and medical notes that can be matched to FGM concentrations to aid in the physiological validation of adrenal stimulation and response. The goal of this study was to demonstrate analytical validations using two enzyme-linked immunosorbent assays (EIA) to measure FGMs from northern sea otters (Enhydra lutris kenyoni) under human care (Seattle Aquarium, Seattle, WA, USA) and to determine baseline and stress response spike levels for individual sea otters. Individual variation was found among the four subjects in the study with fecal baseline levels ranging from 20.2 to 83.7 ng/g for cortisol-immunoreactive metabolites and 52.3 to 102 ng/g for corticosterone-immunoreactive metabolites. As a retrospective study, 39 percent of hormone peaks were associated with notes and most FGM spikes were associated with veterinary procedures or days in which enrichment items were provided and produced an excitatory response. Monitoring baseline FGMs levels and events associated with hormone peak values may provide insight into effective husbandry management to improve the overall welfare of sea otters and other marine mammals.

The dosimetric relationship between the human intake dose of a chemical contaminant (an "external dose") and its concentrations in bodily fluids such as blood and urine (related to an "internal dose"), often characterized by a dose-to-concentration ratio, has critical applications in exposure science, toxicology, and risk assessment, especially in the "new approach methods" era. However, there is a lack of a mechanistic, systematic understanding of how such a dosimetric relationship depends on fundamental chemical properties, such as partition coefficients and biotransformation half-lives. Here, we investigate this issue using a well-evaluated toxicokinetic model, which links external and internal doses by quantifying the absorption and elimination of chemicals. Results are visualized in a series of chemical partitioning space plots, whereby a chemical's dose-to-concentration ratio can be approximately predicted based on its partitioning between air, water, and octanol phases. Our results indicate that when taken in equal doses, chemicals with low volatility and moderate to high hydrophobicity exhibit the highest concentrations in the blood, and chemicals undergoing significant biotransformation tend to exhibit lower concentrations in comparison to their counterparts undergoing negligible biotransformation but possessing similar partitioning properties. Chemicals with high hydrophilicity have the highest concentrations in urine. Such revealed property dependence is similar for both adults and children and for individuals with normal body weights and with obesity. Overall, insights gained from this study are important in predicting blood and urinary concentrations from exposure information and in determining the exposure rate that produces the blood or urinary concentrations observed in biomonitoring studies.

Human use of marinescapes is rapidly increasing, especially in populated nearshore regions where recreational vessel traffic can be dense. Marine animals can have a physiological response to such elevated human activity that can impact individual health and population dynamics. To understand the physiological impacts of vessel traffic on baleen whales, we investigated the adrenal stress response of gray whales (Eschrichtius robustus) to variable vessel traffic levels through an assessment of fecal glucocorticoid metabolite (fGC) concentrations. This analysis was conducted at the individual level, at multiple temporal scales (1-7 days), and accounted for factors that may confound fGC: sex, age, nutritional status, and reproductive state. Data were collected in Oregon, USA, from June to October of 2016-2018. Results indicate significant correlations between fGC, month, and vessel counts from the day prior to fecal sample collection. Furthermore, we show a significant positive correlation between vessel traffic and underwater ambient noise levels, which indicates that noise produced by vessel traffic may be a causal factor for the increased fGC. This study increases knowledge of gray whale physiological response to vessel traffic and may inform management decisions regarding regulations of vessel traffic activities and thresholds near critical whale habitats.

Anthropogenic debris including microparticles (<5 mm) are ubiquitous in marine environments. The Salish Sea experiences seasonal fluctuations in precipitation, river discharge, sewage overflow events, and tourism-all variables previously thought to have an impact on microparticle transport and concentrations. Our goals are two-fold: 1) describe long-term microparticle contamination data including concentration, type, and size; and 2) determine if seasonal microparticle concentrations are dependent on environmental or tourism variables in Elliott Bay, Salish Sea. We sampled 100 L of seawater at a depth of approximately 9 m at the Seattle Aquarium, Seattle, Washington State, United States, approximately every two weeks from 2019 through 2020 and used an oil extraction protocol to separate microparticles. We found that microparticle concentrations ranged from 0 to 0.64 particles L-1 and fibers were the most common type observed. Microparticle concentrations exhibited a breakpoint on 10 April 2020, where estimated slope and associated microparticle concentration significantly declined. Further, when considering both environmental as well as tourism variables, temporal microparticle concentration was best described by a mixed-effects model, with tourism as the fixed effect and the person counting microparticles as the random effect. Although monitoring efforts presented set out to identify effects of seasonality and interannual differences in microparticle concentrations, it instead captured an effect of decreased tourism due to the global Covid-19 pandemic. Long-term monitoring is critical to establish temporal microparticle concentrations and to help researchers understand if there are certain events, both seasonal and sporadic (e.g., rain events, tourism, or global pandemics), when the marine environment is more at risk from anthropogenic pollution. Baleen whale fecal samples have high potential for endocrine monitoring, which can be used as a non-invasive tool to identify the physiological response to disturbance events and describe population health and vital rates. In this study, we used commercial enzyme-linked immunosorbent assays to validate and quantify fecal steroid (progestins, androgens and glucocorticoids) and thyroid hormone metabolite concentrations in eastern North Pacific gray whales (Eschrichtius robustus) along the Oregon coast, USA, from May to October of 2016-2018. Higher mean progestin metabolite concentrations were observed in postweaning females, followed by pregnant females. Mean androgen, glucocorticoid and thyroid metabolites were higher in mature males. Progestin, glucocorticoids and thyroid fecal metabolites varied significantly by year, with positive correlations between progestin and androgen, and between glucocorticoid and thyroid metabolites. We also present two case studies of a documented injured whale and a mature male displaying reproductive competitive behavior, which provide reference points for physiologically stressed individuals and adult breeding males, respectively. Our methods and findings advance the knowledge of baleen whale physiology, can help guide future research on whale physiology and can inform population management and conservation efforts regarding minimizing the impact of anthropogenic stressors on whales.

BACKGROUND: Diabetes mellitus (DM) is risk factor for complications after orthopedic surgery. OBJECTIVES: We tested the hypothesis that anesthesia preoperative clinic (APC) referral for elevated glycosylated hemoglobin (HbA1c) reduces complication rate after total joint arthroplasty (TJA). PATIENTS AND METHODS: Patients (n = 203) with and without DM were chosen from 1,237 patients undergoing TJA during 2006 - 12. Patients evaluated in the APC had surgery in 2006 - 8 regardless of HbA1c (uncontrolled). Those evaluated between in subsequent two-year intervals were referred to primary care for HbA1c ≥ 10% and ≥ 8%, respectively, to improve DM control before surgery. Complications and mortality were quantified postoperatively and at three, six, and twelve months. Length of stay (LOS) and patients requiring a prolonged LOS (> 5 days) were recorded. RESULTS: Patients (197 men, 6 women) underwent 71, 131, and 1 total hip, knee, and shoulder replacements, respectively. Patients undergoing TJA with uncontrolled HbA1c and those with HbA1c < 10%, but not those with HbA1c < 8%, had a higher incidence of coronary disease and hypercholesterolemia than patients without DM. An increase in complication rate was observed in DM patients with uncontrolled HbA1c versus patients without DM (P < 0.001); the complication rate progressively decreased with tighter HbA1c control. More DM patients with preoperative HbA1c that was uncontrolled or ≥ 10% required prolonged LOS versus those without DM (P < 0.001 and P = 0.0404, respectively). CONCLUSIONS: APC referral for elevated HbA1c reduces complication rate and the incidence of prolonged hospitalization during the first year after surgery in diabetics undergoing TJA.

Acute mesenteric ischemia secondary to arterial occlusion (AMI) remains a highly lethal condition. To examine recent trends in management and associated outcomes, we examined our institutional experience over a recent 10-year period. All patients treated for AMI between January 1990 and January 2000 were identified (76 patients, 77 cases) and their medical records examined. At presentation, 64% demonstrated peritonitis and 30% exhibited hypotension. The interval from symptom onset to treatment exceeded 24 h in 63% of cases. Etiology was mesenteric thrombosis in 44 patients (58%) and embolism in 32 patients (42%). Thirty-five patients (46%) had prior conditions placing them at high risk for the development of AMI including chronic mesenteric ischemia (n = 26) and inadequately anticoagulated chronic atrial fibrillation (n = 9). Surgical management consisted of exploration alone in 16 patients, bowel resection alone in 18 patients, and revascularization in 43 patients, including 28 who required concomitant bowel resection. Overall, intestinal necrosis was present in 81% of cases. Perioperative mortality was 62% and long-term parenteral nutrition (TPN) was required in 31% of survivors. Peritonitis (odds ratio [OR] 9.4, 95% confidence interval [CI] 1.6, 54.0; p = 0.012 and bowel necrosis (OR 10.4, CI 1.9, 56.3; p = 0.007) at presentation were independent predictors of death or survival dependent upon TPN. We conclude that AMI remains a highly lethal condition due in large part to advanced presentation and inadequate recognition and treatment of patients at high risk.
