RESUMO

Anaplastic thyroid carcinoma is a tumor that has a very ominous prognosis, and is characteristic of people older than 60 years. Generally occurs as a neck lump of rapid growth rate, arousing compressive symptoms on nearby structures. We report the case of a 29-year-old patient who presented an anaplastic thyroid carcinoma. He was treated with surgery, chemotherapy and radiotherapy, but died 10 months after being diagnosed.

A 39 year old patient diagnosed of severe aplastic anemia and treated with allogenic bone marrow transplantation and who presented chronic eosinophilic pneumonia eight months after the transplant is presented. The patient had no previous history of asthma or atopy. Conditioning was performed with cyclophosphamide and total body irradiation. Prophylaxis of the graft versus host disease was carried out with cyclosporin and short course of methotrexate. At day 30 mild graft versus host disease appeared which spontaneously resolved. A progressive increase in the number of eosinophils was observed from day +40 reaching 1.05 x 10(9)/l at day +180 coinciding with suspension of the cyclosporine. The patient remained asymptomatic with no evidence of chronic graft versus host disease. At 8 months following allogenic transplantation the patient developed three episodes of fever, cough, moderate dyspnea and pulmonary infiltrates. Respiratory tests showed a restrictive pattern. Bronchoalveolar lavage contained 20% of eosinophils. Upon lung biopsy alveolar infiltration by eosinophils, lymphocytes and mononuclear cells was observed. Diagnosis of chronic eosinophilic pneumonia was made with initiation of steroid treatment. A drastic response was observed. The patient remained asymptomatic without recurrence and without evidence of chronic graft versus host disease. This picture may have been caused by the donor eosinophils given that retrospective evaluation demonstrated a persistent moderate eosinophilia in the donor.

RESUMO

Ganglioneuroma is an exceptional benign tumour of the duodenum and only three cases have been described previously. We report the case of a 71-year-old male in whom this pathology was incidentally discovered, as it usually happens with this type of tumour. Within rare duodenal nonepithelial tumours, gangliocytic paraganglioma is a fairly well defined entity characterized by a proliferation of neurons, Schwann cells, and endocrine-like epithelial cells. Ganglioneuroma lacks this last type of cells and represents and even rarer finding within tumoral pathology of the duodenum. The histopathogenesis of this tumor is discussed and there are theories that defend an hamartomatous origin against a neoplastic nature.

Factors affecting stage I epidermoid cancer of the lung were studied in a series of 29 patients treated only by surgery and followed up for ten years. A set of 13 variables with a possible influence on prognosis were investigated. The application of the Cox Univariate Analysis to the different variables showed the grade of cell differentiation and the mitotic index to be predictors. In the Cox Multivariate Analysis, the proportional regression equation revealed two independently significant variables (p < 0.01), which were the Mitotic Index and Nuclear Area. Grouping patients on the basis of the prognostic variables indicated allows a better prediction for survival to be made for this series of patients.

RESUMO

A case of disseminated infection caused by Scedosporium inflatum is reported in a patient undergoing autologous bone marrow transplantation for acute myeloblastic leukemia. The fungus was isolated from a blood culture and necropsy studies showed a disseminated infection involving numerous organs. In vitro studies showed the isolate to be resistant to amphotericin B, 5-fluorocytosine, ketoconazole and miconazole.

RESUMO

OBJECTIVE: To assess the analgesic and anti-inflammatory value of sodium Naproxen in the treatment of otitis. DESIGN: A prospective study. There were interventions, but no controls. There was not a random distribution of the sample. SITE. Outpatient clinics in a Santander hospital. PATIENTS AND OTHER PARTICIPANTS: Patients presenting symptoms of acute or chronic otitis. INTERVENTIONS: A group of 20 patients with acute otitis and a second group of 15 with chronic otitis were treated normally; while 20 other patients suffering acute otitis and 17 with chronic otitis received in addition to normal treatment 550 mg of sodium naproxen every 12 hours. MAIN MEASUREMENTS AND RESULTS: The evolution of the clinical description was evaluated along the following parameters: perforation of the tympanum, otorrhea, pain, vertigo, migraines, hypoacusis and audiometries in acute cases of otitis; and all these and also tympanic biopsy in chronic cases. The results obtained indicated a statistically significant anatomical-pathological improvement in chronic cases of otitis treated with sodium naproxen; whereas in cases of acute otitis, these differences were observed in parameters such as hypoacusis, tympanometry, perforation of the tympanum and effectiveness of and tolerance towards additional treatment with sodium Naproxen. CONCLUSIONS: On the basis of the study's results as well as the high level of tolerance and absence of undesirable effects, we made a positive evaluation of the introduction of the aforesaid anti-inflammatory into the normal treatment of cases of otitis.

Nasal angiomyolipoma is a anatomoclinical disease that have been described a previous time. It's non epithelial tumour with vascular, muscular and lipomatous elements. It have higher incidence in renal tissue associated with tuberous sclerosis, that hasn't been demonstrated in this case.
