Context: In the surgical treatment of primary hyperparathyroidism, intraoperative localization of the diseased glands is an essential issue. We aimed to evaluate the impact of the intraoperative frozen section during radio-guided focused parathyroidectomy and compare its efficiency with intraoperative gamma probe survey and measurement of intraoperative quick parathyroid hormone. Methods: All consecutive patients underwent radio-guided focused parathyroidectomy for primary hyperparathyroidism due to solitary parathyroid adenoma. The outcomes of parathyroidectomy were evaluated by serum levels of calcium and parathyroid hormone. The primary outcome was the surgical cure diagnosed by normal serum calcium levels in the sixth postoperative month. Results: A total of 55 patients with a mean age of 54.3±12.2 years were evaluated. The diagnostic accuracy of the preoperative localization tests was calculated as 92.7%. The intraoperative gamma probe survey impacted the operative plans of four patients (7.3%) by localizing the diseased gland in other positions. The accuracy of intraoperative quick parathyroid hormone and frozen section analysis was 100% for both. In all patients, serum calcium and PTH levels dropped to normal in the sixth postoperative month. There were no cases of the persistent or recurrent disease during the follow-up period. Conclusion: The results of the intraoperative gamma probe survey, intraoperative quick parathyroid hormone, and frozen section were concordant with the final pathology in all cases. In four patients (7.3%), intraoperative gamma probe survey impacted the operative plans. The intraoperative gamma probe survey may be used as the sole intraoperative method to locate the correct position of the solitary parathyroid adenoma.

BACKGROUND/OBJECTIVES: Structural and functional impairments of the Achilles tendon in diabetic patients has the potential to contribute to ulcer formation through altered foot mechanics. This study aimed to examine the biomechanical and histopathological alterations in Achilles tendon specimens from diabetic vs. non-diabetic individuals. MATERIALS AND METHODS: 42 Achilles tendon samples obtained from patients treated with below-knee or above-knee amputation for chronic diabetic foot ulcers (n=21) or for non-diabetic conditions (n=21) were included. A tensile test was performed for each tendon and a stress vs. strain graft was obtained to calculate following biomechanical parameters: elasticity (Young modulus), load, stiffness, toughness, energy, strain, elongation and tenacity. Groups were also compared with regard to histopathological findings (inflammatory cell infiltration, collagen organization, and degeneration). RESULTS: Non-diabetic tendons exhibited a superior biomechanical profile over diabetic tendons with regard to the following biochemical parameters: elasticity, maximum load, stiffness, toughness, load, energy, strain and elongation at break point, tenacity, and strain at automatic load drop (p<0.05 for all comparisons). Diabetic tendons had mild impairment of collagen organization and focal collagen degeneration, whereas neither diabetic nor non-diabetic tendons had inflammatory cell infiltration. CONCLUSION: The structural and functional alterations associated with diabetes adversely affect the biomechanical properties of the Achilles tendon, potentially acting together with neuropathy and ischemia in the development of diabetic foot ulcers.

Although sulfiting agents are commonly used in the fresh-cut potato industry, concerns about further regulatory restrictions on sulfite use and consumer fear of sulfite-treated foods have lead to increased research in alternative processing methods. The objective of this study was to determine processing and packaging techniques to achieve a safe and high-quality potato strips with a 4 wk minimum refrigerated shelf life. Potato strips were 1st blanched at low temperature (60 °C) in 0.5% CaCl(2) solution for 20 min and then 2nd blanched at high temperature (approximately 98 °C) in water for 5 min. Blanched strips were packaged in a near-aseptic environment or treated in-package with gaseous ozone, sodium metabisulfite (SM) solution, or FIT Fruit and Vegetable Wash™ (Procter and Gamble Co., Cincinnati, Ohio, U.S.A.) and then stored at 7 ± 1 °C for 28 d. No significant difference was observed in lightness of near-aseptically packaged fries, FIT-treated fries, and frozen fries; however, less color difference was determined in near-aseptically packaged fries and FIT-treated fries compared to frozen fries. Gaseous ozone treatment decreased color quality of potato strips, significantly. The highest after-frying peak force was observed in FIT-treated strips. There was no significant difference in oil absorption of refrigerated and unprocessed strips. These results indicate that either near-aseptic packaging or in-package FIT treatment are the better alternatives for blanched potato strips to extend shelf life and maintain quality. Practical Application: In this study, several processing conditions and packaging methods were evaluated to improve quality and extend shelf life of refrigerated potato strips. Results indicate that the combination of 2-step blanching and near-aseptic packaging was an effective nonchemical processing method, giving 28 d refrigerated shelf life. Similarly, FIT Fruit and Vegetable Wash™ proved to be an effective chemical alternative to sulfites and avoids the allergen concern.

OBJECTIVE: The deterioration of seminal parameters in men with varicocele suggests a vascular lesion producing progressive damage to the testes over time. The aim of this study was to analyze the relationship between the duration of infertility in infertile men with varicocele and the postoperative sperm counts to determine whether the infertility period is a parameter for predicting the pregnancy rate. METHODS: The medical records of 574 patients with palpable varicoceles were retrospectively reviewed. According to the duration of unprotected intercourse without conception (infertility period), the patients were divided into 4 groups: group 1, 0-3 years (n = 353); group 2, 3-6 years (n = 132); group 3, 6-9 years (n = 47); and group 4, >9 years (n = 42). The mean age of these groups was 28.5 +/- 5.3 years (range 19-58), 30 +/- 4.9 years (range 22-52), 31.6 +/- 4.1 years (range 25-42), and 36.4 +/- 4.2 years (range 29-50), respectively. The preoperative and postoperative total motile sperm counts (TMCs) and postoperative pregnancy rates among these groups were compared statistically, including multivariate analysis. RESULTS: An overall spontaneous pregnancy rate of 41.3% was achieved after microscopic varicocelectomy. The greatest pregnancy rate was achieved in group 1 (43.9%) and the lowest pregnancy rate in group 4 (31.7%; P < .05). The mean postoperative TMC in these groups was 34.9 +/- 3.5 x 10(6) and 24.6 +/- 1.7 x 10(6), the only statistically significant difference in terms of postoperative TMC and pregnancy among the 4 groups. CONCLUSIONS: According to our data, the pregnancy rates and TMCs after varicocelectomy correlate negatively with the infertility period. This finding supports the perspective that varicocele has deleterious effects on the testicular biology over time.

In this study, by using synthetic medium and Streptomyces mediterranai rifamycin was produced by fermentation in laboratory scale. By means of spectrophotometric determination and microbiological assay, production of antibiotic by the organism was proved.

Two different strains of Lactobacillus bulgaricus (ATCC-11842 and Microlife) and two strains of Streptococcus thermophilus (ATCC-19258 and Microlife) were grown anaerobically on 3% nonfat dry milk in pure and mixed culture using batch followed by fed batch culture. Samples were collected every 30 min. Concentrations of lactose, galactose, lactic acid, and other products present were measured using high pressure liquid chromatography. A spectrophotometric method from the literature was modified and used to determine microbial biomass concentrations. Relative cell numbers of the two organisms were measured microscopically in mixed culture. The results are presented in tabular form. S. thermophilus (Microlife) showed different growth characteristics compared to the other cultures. This culture utilized most of the galactose that was formed and produced greater amounts of lactic acid and biomass.

Obtaining accurate estimates of maximum specific growth rate, growth yield, and product yield is important for many fermentation processes. A systematic procedure is presented to select the exponential growth region and estimate the maximum specific growth rate using the covariate adjustment method with all the available measured variables (i.e. biomass, substrate, and product). The procedure is applied to data collected during growth of pure and mixed cultures of Lactobacillus bulgaricus and Streptococcus thermophilus on 3% dry milk under anaerobic conditions. The estimation procedure gives good estimates with relatively narrow confidence intervals even though biomass concentration is measured by an indirect method. The estimated values of maximum specific growth rate range from 0.2805 h(-1) for S. thermophilus (ATCC-19258) to 0.4672 h(-1) for S. thermophilus (Microlife). Growth and product yields are estimated using regression analysis and the data for the exponential growth region. The growth yields are compared to their theoretical maximum values.

A response surface method of smoothing fermentation data with spline functions is presented. The available electron balance is used to optimally select the values of the smoothing parameters associated with the spline functions. The method is applied to six sets of anaerobic fermentation data in which pure and mixed cultures are grown in batch followed by fed batch culture. Lactobacillus bulgaricus and Streptococcus thermophilus are cultured on 3% dry milk. Measured concentrations of biomass, lactose, galactose, lactic acid, and other acid products are smoothed using spline functions. Values of specific growth rate, specific lactose consumption rate, specific galactose formation rate, and specific acid product formation rate are estimated and the consistency of the results is examined using the available electron balance. The results show that the method works reasonably well, but that an upper bound should be used for the smoothing parameters to obtain accurate estimates of the derivative quantities.

Point and 95% confidence interval estimates of the true growth yield and maintenance coefficient are reported for four pure cultures andtwo mixed cultures of Lactobacillus bulgaricus and Streptococcus thermophilus grown anaerobically on 3% nonfat dry milk using batch followed by fed batch culture. An improved method of data analysis, which includes separate analysis of the exponential growth region and a new method of selecting smoothing spline functions, is utilized. The parameter estimation results are compared with the theoretical maximum yields for one and two moles of ATP produced per mole of glucose and to the growth yield estimated from the exponential growth region analysis.

RESUMO

In this work, the methods and technology of vitamin B12 production were studied on laboratory scale. The microorganisms used for experiments were Streptomyces olivaceus IFO 3409, Streptomyces olivaceus CBS 355.53 and Streptomyces griseus CBS 161.45 which were brought from foreign countries. Vitamin B12 activity that have been obtained from fermentation experiments of each microorganisms are determined by microbiological assays using Lactobacillus leichmannii IFO 3073 (ATCC 4797) which is a test microorganism. As a result of these assays S. olivaceus IFO 3409 (2 micrograms/ml) was found as the most efficient strain were followed by S. olivaceus CBS 355.53 and S. griseus CBS 16.45 respectively.

In many anaerobic fermentation processes, high energy bonds in adenosine triphosphate (ATP) are produced when available electrons are converted from organic substrate into extracellular organic products such as ethanol. The true growth yield and maintenance parameters are directly related to the product formation kinetic parameters for these anaerobic processes. Methods are presented which allow all of the experimental measurements to be used simultaneously to estimate these parameters. Results are presented for several different anaerobic fermentations.

In the microbial production of useful products, it is important to understand the allocation of substrate energy for maintanance, growth, and product formation. Methods are presented to obtain point and 95% confidence interval estimates for the true growth yield parameter, true product yield parameter, and the maintenance parameter. Methods are presented which allow all data to be used simultaneously for those cases where more than the minimum number of measurements are made at each specific growth rate (or dilution rate). Three estimation methods and two forms of the energy allocation equations are investigated. Point estimates are similar for the three methods, but interval estimates are considerably larger for one of the three methods. The results depend on the form of the equations.

Microbial growth and product yields have been investigated for several cultures. Available electron and free energy balances may be used to examine the efficiency of microbial energetics. Energetic yields based on free energy analysis were obtained and compared with yields from available electron analysis. Estimates of true growth yields under anaerobic conditions have been obtained using statistical methods and the results were compared with theoretical maximum yields.

Electroencephalographic studies were done in rheumatic fever patients in order to determine the distribution of normal and abnormal EEG patterns in different clinical forms. The duration of EEG abnormalities related to rheumatic activity was compared with the duration of increased sedimentation rate and tachycardia. Rheumatic fever cases were divided into four groups: (1) acute active rheumatic carditis, (2) acute polyarthritis without carditis, (3) nonactive old rheumatic valvular heart disease, and (4) Sydenham's chorea. EEG findings were within normal limits in acute polyarthritis and in nonactive rheumatic valvular heart disease, but there were abnormal EEG findings in 29 per cent of chorea and in 94 per cent of active carditis cases. In active carditis all EEG changes returned to normal approximately within 8 weeks, whereas the sedimentation rate and tachycardia returned to normal within a shorter period.
