RESUMO

PURPOSE: The purpose of the paper is to present the possibility of treatment of periorbital capillary hemangiomas in infancy. MATERIALS AND METHODS: Doppler colour ultrasonography was used as a method of diagnosis and monitoring of the treatment. The material comprised 11 patients aged from 1 month to 3 years. In 6 cases local corticosteroid injections were applied, in 3 cases surgical treatment was performed, out of which 1 additionally received steroid. RESULTS: Results of the treatment were satisfactory in all patients. No serious complications were observed. CONCLUSIONS: Basing on these cases, the authors confirmed the opinion that intralesional corticosteroid injection is the method of choice in the treatment of periorbital infantile hemangiomas. The authors indicate the usefulness of Doppler colour ultrasonography in diagnosis, demonstrating the extent of lesions and monitoring the response to therapy.

RESUMO

PURPOSE: The lesions are difficult to visualize in image diagnostics. The paper aims at describing abilities of ultrasonography in the assessment of varices of the orbit. MATERIAL AND METHODS: The results of color Doppler examinations in 12 patients with moderate proptosis, in whom the presence of varices was suspected, are presented. RESULTS: The method allowed for identification and evaluation of the flow in the orbital region. The typical appearance of orbital varices and changes of the flow during the Valsalva's manoeuvre are described. CONCLUSIONS: The authors indicate a high usefulness of the method in the assessment of the orbital varices.

RESUMO

PURPOSE: Possibility of visualising the vessels within retinal detachment. MATERIAL AND METHOD: Examinations were performed using "Color Doppler" and "Power Doppler" methods on 46 eyes with linear hyperechogenic lesions within vitreous body found in US B-mode examination. RESULTS: The results show the possibilities of Doppler examination in visualising branches of the central retinal artery within the detached retina. The presence of vessels in the membranous proliferations was not found. CONCLUSION: Color Doppler ultrasonography can be used in localising the position of retina, especially when proliferative lesions and detachment coexist.

RESUMO

To determine the incidence and significance of late potentials in patients with mitral valve prolapse, we performed surface signal-averaged electrocardiography and 24-hour ambulatory electrocardiographic (ECG) monitoring in 41 patients with moderate to severe mitral valve prolapse on two-dimensional echocardiograms. Late potentials were defined as the presence of either a root mean square voltage of the last 40 msec of the QRS (RMS-40) of less than 20 mu v or a low-amplitude signal duration (LAS-40) of greater than 39 msec. Despite the absence of clinically significant ventricular tachycardia by history and on ambulatory ECG monitoring, 12 patients had late potentials on their signal-averaged electrocardiograms. Clinical characteristics could not differentiate patients with from patients without late potentials, and all patients were doing well at a mean follow-up of 34 months except for one noncardiac death. We conclude that late potentials on the surface signal-averaged electrocardiogram are a common and benign finding in patients with mitral valve prolapse and their clinical significance should be determined only in the presence of other findings.

38 patients with recurrent cardiotopic rhythm, treated by permanent "on demand" pacemaker, were investigated. 15 patients (group A) were observed for 14 days of pacing, 23 (group B) were observed from 7 to 68 months of pacing. Further, 17 persons were investigated--in 12 of them the endocardial electrode USCI had been placed in the right ventricle and left there for 24 hours. 10 persons were paced by endocardial bipolar USCI electrode from external pacemaker, the other two by unipolar electrode from permanent "on demand" pacemaker. In paced patients a negative deep T inversion and ST depression were observed. In persons paced for 24 hours the ST-T changes disappeared at the earliest 48 hours after pacing was stopped. Changes in repolarization caused by artificial depolarization should be taken into account when in patients with ventricular pacing and cardiotopic rhythm subendocardial infarction is suspected.

RESUMO

The ECG tracings in patients with recent and past myocardial infarction and permanent or temporary artificial pacing were analysed. The observations were made in 12 patients (8 with recent infarction and 4 with a past one). Anterior wall infarction was diagnosed in 4 subjects, anteroseptal in 3, inferior wall infarction was recognized in 4, and posterior wall infarction in one patient. In the stimulated complexes of persons with anterior and anteroseptal infarctions there appears a pathological Q deflection with ST segment elevation in leads I, aVL, V5. In the infarct of the inferior wall the ST segment elevation occurs in leads II, III, aVF without the presence of Q deflection. No differences were shown in the morphology of stimulated complexes in persons with recent and past myocardial infarction.
