[Growth and tolerance in infants fed an infant formula based on hydrolyzed rice proteins]. / Efficacité sur la croissance et tolérance d'une préparation pour nourrissons à base de protéines de riz hydrolysées.

UNLABELLED: The objective of this study was to assess the efficacy in terms of growth and tolerance of an infant formula based on hydrolyzed rice proteins. PATIENTS AND METHODS: Healthy infants, born at term, less than 1 month old, and exclusively fed an infant formula based on hydrolyzed rice proteins until their diet was diversified, were included in this open-label, multicenter study. The main outcome measure was daily weight gain during the study period. The infant's weight, height, body mass index (BMI), and the data concerning tolerance (digestive disorders, allergy manifestations) were collected at inclusion in the study, at 2 and 4 months, and before diversifying the infant's diet between 4 and 6 months. The growth parameters were compared to the WHO standards by calculating the Z-score. RESULTS: Seventy-eight infants were included. The mean daily weight gain over 5 months was 23.2 ± 4.3 g/day, identical to the WHO standards (22.2 ± 1.8 g/day, P = 0.09). During the study period, the Z-scores for weight, height, and BMI varied between +1.1 and -0.5 SD according to the WHO standards. Formula acceptance and tolerance were both good. CONCLUSION: The infant formula studied, based on hydrolyzed rice proteins, was well tolerated and led to normal growth over the first few months of life, comparable to the WHO standards.

Keratomycosis after cataract surgery.

PURPOSE: To evaluate cases and results of keratomycosis that developed after cataract surgery. SETTING: Hospital de Guipúzcoa, San Sebastián, Spain. METHODS: This retrospective study comprised 8 patients who developed keratomycosis soon after cataract surgery. RESULTS: Culture analysis revealed 7 cases of Aspergillus fumigatus and 1 of Aspergillus flavus. After medical treatment with antifungal agents, 6 cases resolved and 2 required evisceration. CONCLUSIONS: The presence of fungi in corneal ulcers that develop after cataract surgery should be considered. Initiation of early treatment determines the prognosis. Among the therapeutic options, collagen shields soaked in amphotericin B may be effective.

Análisis de la implementación de unas reglas de decisión clínica: reglas del tobillo de Ottawa / Analysis of the implementation of a clinical decision rules: Ottawa ankle rules

Introducción: Las Reglas del Tobillo de Ottawa (RTO) son reglas de decisión clínica que permiten disminuir la variabilidad en la práctica clínica y hacer un uso correcto de las radiografías (Rx). A pesar de su seguridad, ventajas y de haber sido validadas en diferentes ámbitos, actualmente están escasamente implementadas. Objetivo: Analizar la implementación de las RTO y la sensibilidad de la detección de fracturas. Metodología: Diseño: Prospectivo, con aplicación de las RTO y medición del resultado. Ámbito: Urgencias hospitalarias. Sujetos: Adultos atendidos con una lesión traumática aguda de tobillo, desde el 23/11/1998 al 23/06/1999. Intervención: Solicitud de la Rx en función de los criterios de las RTO. Seguimiento telefónico a los pacientes que no se les ha solicitado la Rx. Resultados: De los 883 pacientes con traumatismo agudo de tobillo atendidos, se valoró la aplicación de las RTO en 678 (76,8 por ciento). La implementación de las RTO ha permitido ahorrar un 18,5 por ciento de las Rx de tobillo y un 6,8 por ciento de las Rx de pie. Las RTO presentaron una sensibilidad del 100 por ciento (IC 95=92,3-100 por ciento) y una especificidad del 21,4 por ciento (IC 95=17,7-25,8 por ciento) Conclusiones: 1- Consideramos adecuado aunque mejorable el uso de las RTO en nuestro servicio, a pesar de que el índice de implementación sea menor de lo esperado. 2- La sensibilidad del 100 por ciento demostrada en nuestro estudio confirma la eficacia y validez clínica de las reglas, permitiendo hacer un uso eficiente de las Rx (AU)
