RESUMO

CdSe fractional monolayer quantum dots (FMQDs) embedded in a ZnSe matrix were produced by atomic layer epitaxy with a nominal coverage of 0.5 monolayer. They have a thickness of a/2, where a is the strained perpendicular lattice constant of cubic CdSe. Their photoluminescence spectra at low temperature exhibit narrow and intense excitonic emission around 2.759 eV. Based on the experimental excitonic emission energy and applying the factorized-envelope approximation, we have estimated that the lateral dimensions of these FMQDs are around 4-5 nm and their density is [Formula: see text] cm-2.

Background Older age and inappropriate prescribing is related to a greater rate of emergency department visits and hospitalisations. Objective To assess the efficacy of an interprofessional collaboration programme in which a review of the medication of older patients seen in the emergency observation unit was carried out. Setting Emergency departments at four Spanish hospitals. Method Randomised, controlled study. Patients over 65 years of age presenting to the emergency department were randomised to a control or an intervention group. In the intervention group, a pharmacist reviewed the patients' chronic medication and identified any potentially inappropriate prescriptions based on the STOPP/START criteria. Each case was discussed with the emergency specialist and a recommendation to modify the treatment was sent to the general practitioner. Main outcome measure Rate of emergency visits and hospital admissions. Results The adjusted rate ratio of emergency visits and hospital admissions was 0.808 (95% CI 0.617 to 1.059) at 3 months, 0.888 (95% CI 0.696 to 1.134) at 6 months and 0.954 (95% CI 0.772 to 1.179) at 12 months. There was a statistically significant reduction at 3 months in two of the hospitals that participated in the study [adjusted rate ratio at 3 months was 0.452 (95% CI 0.222 to 0.923) in hospital 3 and 0.567 (95% CI 0.328 to 0.983) in hospital 4]. Conclusion Overall, the intervention did not reduce the number of emergency visits and hospital admissions. However, a significant effect was observed in centres were a high acceptance rate of treatment recommendations was achieved.

BACKGROUND: The perinatal environment has a role in the establishment of altered metabolic and inflammatory responses, and could be modulated by microRNAs regulating immune and metabolic processes. OBJECTIVE: To analyze the expression profile of four circulating microRNAs and cytokine serum concentrations in neonates born to overweight and obese women. METHODS: Pregnant women were included and grouped by pregestational body mass index (21 with normal weight, 10 overweight and 10 obese women). A peripheral blood sample was obtained from newborn infants and used to determine circulating miRNAs expression and cytokine serum concentrations. RESULTS: There were significant differences in the expression of three microRNAs between newborns of pregestational obese women and newborns from pregestational normal weight women: miR-155 (p = 0.03), miR-181a (p = 0.02) and miR-221 (p = 0.04). A significant reduction in IL-1ß (p = 0.005) expression was also found in newborns of overweight women; although this cytokine was also diminished in newborns of obese women, this was not statistically significant. An association between IL-1ß concentrations and miR-146a and miR-221 expression was also observed. CONCLUSIONS: Expression of miR-155, miR-181a and miR-221 differs in infants born to obese women compared with infants born to normal weight women. Changes in microRNA expression could participate in the epigenetic foetal programming of metabolic disorders in children born to obese women.

Retinoblastoma (Rb) is a pediatric intraocular malignancy and probably the most robust clinical model on which genetic predisposition to develop cancer has been demonstrated. Since deletions in chromosome 13 have been described in this tumor, we performed next generation sequencing to test whether recurrent losses could be detected in low coverage data. We used Illumina platform for 13 tumor tissue samples: two pools of 4 retinoblastoma cases each and one pool of 5 medulloblastoma cases (raw data can be found at http://www.ebi.ac.uk/ena/data/view/PRJEB6630). We first created an in silico reference profile generated from a human sequenced genome (GRCh37p5). From this data we calculated an integrity score to get an overview of gains and losses in all chromosomes; we next analyzed each chromosome in windows of 40 kb length, calculating for each window the log2 ratio between reads from tumor pool and in silico reference. Finally we generated panoramic maps with all the windows whether lost or gained along each chromosome associated to its cytogenetic bands to facilitate interpretation. Expression microarrays was done for the same samples and a list of over and under expressed genes is presented here. For this detection a significance analysis was done and a log2 fold change was chosen as significant (raw data can be found at http://www.ncbi.nlm.nih.gov/geo/accession number GSE11488). The complete research article can be found at Cancer Genetics journal (Garcia-Chequer et al., in press) [1]. In summary here we provide an overview with visual graphics of gains and losses chromosome by chromosome in retinoblastoma and medulloblastoma, also the integrity score analysis and a list of genes with relevant expression associated. This material can be useful to researchers that may want to explore gains and losses in other malignant tumors with this approach or compare their data with retinoblastoma.

Genes are frequently lost or gained in malignant tumors and the analysis of these changes can be informative about the underlying tumor biology. Retinoblastoma is a pediatric intraocular malignancy, and since deletions in chromosome 13 have been described in this tumor, we performed genome wide sequencing with the Illumina platform to test whether recurrent losses could be detected in low coverage data from DNA pools of Rb cases. An in silico reference profile for each pool was created from the human genome sequence GRCh37p5; a chromosome integrity score and a graphics 40 Kb window analysis approach, allowed us to identify with high resolution previously reported non random recurrent losses in all chromosomes of these tumors. We also found a pattern of gains and losses associated to clear and dark cytogenetic bands respectively. We further analyze a pool of medulloblastoma and found a more stable genomic profile and previously reported losses in this tumor. This approach facilitates identification of recurrent deletions from many patients that may be biological relevant for tumor development.

Thaumetopoea pityocampa causes allergies and skin and ocular lesions. No commercial tools are currently available for the clinical diagnosis of this allergy. We aimed to develop an in vitro method for the diagnosis of this allergy to avoid patients undergoing in vivo tests with insect extracts. Recombinant Tha p 2 was produced and used in an ELISA validated with 15 allergic patients. Subsequently, 42 subjects recruited from a random sampling cross-sectional study were analysed. The ELISA sensitivity and specificity were 93.3% and 100%, respectively, for the allergic patients and 71.4% and 95.3%, respectively, for the epidemiological study. The positive ELISA results correlated with the skin prick test areas with the whole body and the setae extracts. Professional exposure and short latency of symptoms onset were risk factors for a positive result in the ELISA. In conclusion, our ELISA is very useful for T. pityocampa allergy diagnosis and for epidemiologic testing.

Objective. To analyse survival in patients with pulmonary tuberculosis (PTB) and factors associated with such survival. Design. Study of a cohort of patients aged over 14 years diagnosed with PTB from January 1, 1998 to July 31, 2005. During 2004-2006 a home visit was made to each patient and, during 2008-2009, they were visited again. During these visits a follow-up interview was administered; when the patient had died, a verbal autopsy was conducted with family members. Statistical analysis consisted of survival tests, Kaplan-Meier log-rank test and Cox regression. Results. Of 305 studied patients, 68 had died due to PTB by the time of the first evaluation, 237 were followed-up for a second evaluation, and 10 of them had died of PTB. According to the Cox regression, age (over 45 years) and treatment duration (under six months) were associated with a poorer survival. When treatment duration was excluded, the association between poorer survival with age persisted, whereas with having been treated via DOTS strategy, was barely significant. Conclusions. In the studied area it is necessary that patients receive a complete treatment scheme, and to give priority to patients aged over 45 years.

Cada año se diagnostican aproximadamente nueve millones de personas con cáncer. El dolor es uno de los síntomas más comunes en este tipo de población. Su fisiopatología es múltiple y va desde el síntoma doloroso causado por la propia enfermedad, hasta el relacionado a procedimientos diagnósticos y/o terapéuticos, pasando por el asociado a enfermedades no oncológicas ligadas al cáncer. Esta revisión representa un análisis crítico de los estudios epidemiológicos sobre la prevalencia de dolor por cáncer en la población mundial. Se muestran grandes variaciones en cuanto a la prevalencia, debido quizá a aspectos metodológicos que dificultan la comparación de los resultados o, dicho de otra manera, por los diferentes criterios utilizados para conceptualizar y caracterizar el dolor por cáncer, los contrastes entre la población estudiada y los métodos de recolección de datos. Si a esto le agregamos que existen diferentes tipos de dolor y que la terapéutica puede diferir de un medio hospitalario a otro, no es raro que la validez de los reportes se limite y su uniformidad varíe considerablemente. La sociedad mexicana poco conoce sobre la prevalencia de dolor oncológico y sobre los prejuicios personales y socioeconómicos que conlleva esta temible enfermedad, por lo que, considerando los estudios existentes en la literatura, sugerimos que las pesquisas epidemiológicas en nuestro país deberán realizarse bajo estricto control metodológico, estudiando los diferentes grupos de edad, tipo de dolor, intensidad, diagnóstico oncológico, estadio clínico, terapéutica anticáncer, terapia analgésica farmacológica y no farmacológica, y los fármacos coadyuvantes (AU)


Each year, approximately, nine million people with cancer are diagnosed. Pain is one of the most common symptoms in this population. Its pathophysiology is multiple and varies from the painful symptoms caused by the disease itself until linked to diagnostic procedures and therapeutic, or else to that pain associated with non-oncological diseases linked to cancer. This review represents a critical analysis of epidemiological studies on the prevalence of cancer pain in the world population. Great variations are shown in prevalence, perhaps because of methodological issues that hinder the comparison of results. In others words, by different criteria used to conceptualize and characterize cancer pain, the contrasts among the study population and data collection methods. If we add to this that there are different types of pain therapeutics and may differ from one hospital to another, the validity and consistency of the reports are limited considerably. Mexican society little known about the prevalence of cancer pain and on personal and socioeconomic bias involved in this terrible disease. Whereas existing studies in the literature, we suggest that epidemiological investigations should be conducted under strict methodological control, studying the different age groups, type of pain, intensity, oncology diagnosis, clinical stage, used anticancer therapeutics, drug therapy and nonpharmacologic analgesic; without forgetting the adjuvant drugs associated with this management (AU)

OBJECTIVES: To analyse the proportion of multidrug-resistant tuberculosis (MDR-TB) in cultures performed during the period 2000-2002 in Los Altos, Selva and Norte regions, Chiapas, Mexico, and to analyse MDR-TB in terms of clinical and sociodemographic indicators. METHODS: Cross-sectional study of patients with pulmonary tuberculosis (PTB) from the above regions. Drug susceptibility testing results from two research projects were analysed, as were those of routine sputum samples sent in by health personnel for processing (n = 114). MDR-TB was analysed in terms of the various variables of interest using bivariate tests of association and logistic regression. RESULTS: The proportion of primary MDR-TB was 4.6% (2 of 43), that of secondary MDR-TB was 29.2% (7/24), while among those whose history of treatment was unknown the proportion was 14.3% (3/21). According to the logistic regression model, the variables most highly associated with MDR-TB were as follows: having received anti-tuberculosis treatment previously, cough of >3 years' duration and not being indigenous. CONCLUSIONS: The high proportion of MDR cases found in the regions studied shows that it is necessary to significantly improve the control and surveillance of PTB.

BACKGROUND: Chiapas is one of the Mexican states having the highest rates of Pulmonary Tuberculosis (PTB), due to the numerous factors impeding its management and control (poverty, poor housing and nutrition, shortage of health resources, among others). OBJECTIVES: To analyse the PTB mortality of a cohort of patients in Los Altos Region of Chiapas, who had been diagnosed with PTB from January 1, 1998 to December 31, 2002; and, to identify demographic, socioeconomic and health services utilization factors, associated with death from PTB. METHODS: Analysis of a cohort of patients aged over 14 years diagnosed with PTB in the above mentioned period (n = 431) in Los Altos region of Chiapas. The records of the Tuberculosis Programme were reviewed, and patients were located through a search attempting to locate them in their homes. Those found alive were interviewed and asked to provide sputum samples. In the case of deceased patients, a verbal autopsy was obtained from a member of their family. RESULTS: The records of the PTB Programme in the area were incomplete and erroneous in many cases. The results of the home follow-up visits were: 208 (48%) patients located alive, five of whom were still PTB positive (three with multi-drug resistance); 145 (34%) could not be located and 78 (18%) had already died. Apparently, in at least 40 cases, the deaths were associated with PTB. Of these forty, 33 (83%) died without having received any medical care. The factors associated with dying from PTB were: 45 and over years of age (OR = 1.3; 95% CI = 0.98-1.3), 0-3 schooling years (OR = 3.3; 95% CI = 1.1-9.6), engaged in agriculture (OR = 2.2; 95% CI = 1.1-4.4), not living in main villages of their municipality (OR = 1.2; 95% CI = 1.0-1.3), living in a rural community (OR = 2.7; 95% CI = 1.1-6.8), not having been treated in DOTS (OR = 1.2; 95% CI = 1.0-1.3) and having defaulted from treatment (OR = 11.5; 95% CI = 5.3-24.8). CONCLUSIONS: The high rate of mortality due to PTB observed constitutes a serious public health problem deserving attention. There is an urgent need to evaluate and restructure the Tuberculosis Programme in the studied area.

INTRODUCTION: Increasing evidence demonstrates that risk factors for chronic diseases are established during childhood and adolescence. Consensus about the need to increase prevention efforts makes the adoption of a healthy lifestyle seem desirable from early childhood onwards. After reviewing educational tools for children and adolescents aimed at promoting a healthy lifestyle, it was recognized that there was a need to develop a simple educational tool specifically designed for these age groups. METHODS: Development of the healthy lifestyle pyramid for children and adolescents. RESULTS: We propose a three-dimensional, truncated and staggered pyramid with 4 faces and a base, which introduces a completely new concept that goes beyond other published pyramids. Each of the faces is oriented towards achieving a different goal. Two faces (faces 1 and 2) are formulated around achieving a goal on a daily basis (daily food intake, face 1, and daily activities, face 2). Face 3 is an adaptation of the traditional food guide pyramid, adapted to children's energy, nutritional and hydration needs. Face 4 deals with both daily and life-long habits. On the base of the pyramid, there is advice about adequate nutrition alternating with advice about physical activity and sports. CONCLUSION: The Healthy Lifestyle Pyramid is specifically developed for children and adolescents according to current scientific knowledge and evidence-based data and includes easy-to-follow advice and full colour pictures. Following these guidelines should improve health and reduce risk factors, promoting enjoyable and appropriate development towards adulthood.

Cobalt alloy samples were bioactivated during investment casting. The cavities of the investment mold were previously coated with wollastonite. Additionally, before coating with wollastonite, some mold cavities were filled out with graphite rods to avoid a chemical reaction between the wollastonite powder and the investment material. Half of the cast samples were heat treated at 1220 degrees C for 1 h. To perform the in vitro bioactivity assessment, the cast and heat-treated samples were immersed in a simulated body fluid solution (SBF) for a period of 21 days. The surface of the samples before and after immersion in SBF was characterized by SEM, EDX, and XRD analyses. During the casting, particles of pseudowollastonite were embedded on the metallic surface. After immersion of the samples in SBF, a ceramic layer was formed on both the alloy obtained by using the investment mold and the alloy obtained by using the graphite-filled cavity. The ceramic layer was thicker on the alloy cast in the investment mold. The layer was identified as hydroxyapatite by XRD analysis, in all the cases. The heat-treated samples after immersion in SBF showed the formation of a thin homogeneous layer consisting of fine grains of apatite.

Objetivos: valorar la influencia de tratamientos crónicos con los antioxidantes vitamina E y tioprolina en: a) la supervivencia de ratones; b) la actividad neurológica de los animales envejecidos, y c) la disminución de actividades enzimáticas mitocondriales y el daño oxidativo mitocondrial asociados al proceso de envejecimiento. Material y método: los ratones recibieron desde la semana 28 de vida, y durante toda su vida, una suplementación en la dieta de vitamina E (5 g de acetato de dl-*-tocoferol/kg de comida) o de tioprolina (2 g de l-4-ácido tiazolidín carboxílico/kg de comida). Para evaluar la actividad neurológica los ratones se sometieron cada 2 semanas a 2 pruebas de comportamiento. En mitocondrias aisladas de cerebro se determinó el daño oxidativo, medido como proteínas o lípidos oxidados, así como por disminución de las actividades enzimáticas NADH-citocromo c reductasa, succinato-citocromo c reductasa, citocromo oxidasa y óxido nítrico sintasa mitocondrial (mtNOS). Resultados: la expectativa de vida de los ratones macho aumentó después de la suplementación con vitamina E en un 34-34% (vida media y longevidad máxima, respectivamente), y después de la suplementación con tioprolina en un 33-24%. La vitamina E y la tioprolina fueron efectivas en la disminución de los marcadores mitocondriales de daño oxidativo (TBARS y carbonilos proteínicos), y en el retardo de la disminución de las actividades enzimáticas y neurológicas asociadas al envejecimiento. Conclusiones: las actividades enzimáticas de mtNOS, NADH deshidrogenasa y citocromo oxidasa pueden usarse como indicadores de tratamientos efectivos del déficit neurológico asociado al envejecimiento


Aims: the aim of this study was to evaluate the influence of chronic treatments with the antioxidants vitamin E and thioproline on: 1) survival in mice; 2) the neurological activities of aged animals; and 3) the decreased mitochondrial enzymatic activities and oxidative damage associated with the ageing process. Material and method: mice received food supplemented with vitamin E (5 g dl-*-tocopherol acetate/kg of food) or with thioproline l-4-thioproline/kg (2 g l-4-thiazolidine carboxylic acid/kg of food) from 28 weeks of age and during their entire lifespan. To evaluate neurological activity the animals underwent two behavioural tests every 15 days from weeks 28 to 76 of age. Oxidative damage to isolated brain mitochondria was evaluated by determining protein and lipid oxidation products and mitochondrial enzyme activities: NADH-cytochrome c reductase, succinate-cytochrome c reductase, cytochrome oxidase, and mitochondrial nitric oxide synthase (mtNOS). Results: lifespan was increased in male mice by 34-34% (mean and maximal lifespan, respectively) after supplementation with 5 g vitamin E/kg food and by 33-24% (mean and maximal lifespan, respectively) after supplementation with 2 g thioproline/kg food. Vitamin E and thioproline were effective in decreasing the level of markers of oxidative damage (TBARS and protein carbonyls) in isolated mitochondria and in delaying the decreases in mitochondrial enzyme activities and the loss of neurological function associated with ageing. Conclusions: the activities of mtNOS, NADH dehydrogenase and cytochrome oxidase can be used as indicators of the effectiveness of treatments for age-dependent neurological impairment

Injuries to the superior gluteal nerve are very bad complications in hip surgery. An exact knowledge of its course may be helpful in avoiding such problems. Nineteen half pelvises from ten male and female adult cadavers were dissected. Dissections revealed that the nerve divided into two (89.48%) or three (10.52%) branches after leaving the pelvis. The more caudal branch was responsible for innervation of tensor fascia latae. The distance and the angle from the entry points of all branches of the superior gluteal nerve into the deep surface of the gluteus medium and minimus muscles to the mid-point of the superior border of the greater trochanter were measured. The branch that innerved the tensor fascia latae was also followed. These data were subjected to several statistical tests. Based on these findings, and in order to prevent nerve damage, we propose to define a 2-3 cm safe area above the great trochanter.

RESUMO

Acute fatty liver of pregnancy can produce a great variety of complications. Of these, infections are important. We describe two cases, one of them complicated with uterine sepsis. Both gave evidence of the necessity for multidisciplinary treatment, which reduces the maternal-fetal morbidity and mortality produced by this disease.

RESUMO

The axillopectoral muscle, usually called Langer's axillary arch instead of Langer's arm arch, is a supernumerary muscle and is the principal anatomic variation of the axilla. Three cases of the muscle were observed originating from latissimus dorsi crossing over the axillary neurovascular bundle and inserting deep to the insertion of pectoralis major or into the coracoid process. Clinicians should be aware of its existence as it can give rise to different pathologies. It should be recognised and excised to expose the axillary artery and vein in patients with trauma and to perform axillary lymphadenectomy or axillary bypass. It should be considered in the differential diagnosis of axillary masses or in a history of intermittent axillary vein obstruction. If the muscle causes problems its excision should be curative.

The purpose of this study was to investigate the prevalence of latex allergy in fruit-allergic patients, and to assess its clinical significance. Fifty-seven fruit-allergic patients and 50 non-fruit-allergic atopic patient controls were studied. All patients were questioned about conventional immediate symptoms after contact with latex products. Patients also underwent skin prick testing and determination of specific serum IgE to latex, as well as a screening test for environmental allergens. Immunologic latex sensitization occurred in 49/57 (85.9%) fruit-allergic patients, who showed a positive STP and/or CAP to latex, but in only two controls (P < 0.001). Six out of 57 (10.5%) fruit-allergic patients suffered from clinically relevant latex allergy. Symptoms included contact urticaria, angioedema, conjunctivitis, generalized urticaria, and moderate anaphylactic reactions. No control reported symptoms with latex products (P = 0.052). In all patients, clinical symptoms to fruits preceded a history of latex allergy. The fruits most associated were melon, peach, and banana. From our data, we conclude that there is a potential for allergic reactions to latex in patients with allergy to fruit. All patients with fruit allergy should be screened for individual risk of latex allergy.

RESUMO

Vegetable dyes can be recommended to patients sensitized to oxidative dyes due to their low allergenic power. The most important of these is henna which is used as a reddishbrown hair dye in some parts of the world. Different pathologies have been described caused by henna but the incidence of contact dermatitis appears to be extremely rare. In the present paper, we describe the case of a 30-year-old woman who developed allergic contact dermatitis following application of henna, but who did not work with the dye professionally.
