RESUMO

Background Acute cholecystitis (AC) presents as inflammation of the gallbladder, predominantly attributed to gallstones obstructing the cystic duct. Another notable etiology is ischemic cholecystitis, often stemming from severe illnesses that compromise blood flow to the splanchnic system. Individuals with pre-existing cardiovascular conditions or undergoing cardiopulmonary surgery encounter elevated risks of gastrointestinal pathology, leading to heightened morbidity and mortality rates. In these cases, AC stands out as a significant concern, whether it originates from gallstones or is acalculous (ischemic). Methods We conducted a single-center, retrospective cohort study at the National Medical Center in Mexico City, Mexico. We included demographic, clinical, laboratory, preoperative, intraoperative, and postoperative data. Our main objectives were to describe the characteristics of our population and identify predictors of major complications following surgery for AC in patients with heart disease. Results Our study encompassed 18 patients diagnosed with both AC and cardiac disease, with acute myocardial infarction being the leading cause of admission, constituting 38.9% of cases. The clinical presentation was characterized by right hypochondrial pain, observed in all patients, while Murphy's sign was evident in 44.4% of cases. Additionally, various non-specific symptoms were present in varying percentages across the patient cohort. Concerning imaging studies, 88% of our cohort underwent ultrasound (USG), while computed tomography (CT) was utilized in 55% of patients. Among those who had USG, the most prevalent findings were gallbladder wall thickening in 56% and perivesicular fluid in 37.5%. In patients who underwent CT scans, the predominant observations included perivesicular fluid in 66% and perivesicular fat stranding in 55%. In terms of intraoperative findings, gallbladder empyema and necrosis were the most common findings identified in 55.6% of the cases. After surgery, 61.1% had no complications or developed mild complications, while 11.1% experienced moderate to severe complications without resulting in death; the mortality rate was 27.8% in our cohort. Overall, major complications or death were present in 38.9% of our cohort. In the postoperative data analysis, the use of mechanical ventilation and vasopressors showed a significant correlation with increased morbidity and mortality. Conclusions Our study contributes valuable information to the international literature, providing insights into the complications faced by the Mexican population in the context of heart diseases, particularly among patients suffering from cholecystitis. Within individuals with cardiac disease, the occurrence of AC requiring surgical intervention is associated with heightened morbidity and mortality rates, with our cohort experiencing rates as high as 38.9%. Consistent with findings in the international literature, these results underscore the critical importance of continuing the search for novel clinical or laboratory predictors for this high-risk population. While most parameters assessed in this study did not show any correlation with major complications, exceptions were observed in postoperative vasopressors and mechanical ventilation.

BACKGROUND: One of the most serious complications of cholecystectomy is bile duct disruption, which can be associated with concomitant vascular injury in up to 3.4%. The incidence, demographic characteristics and treatment are underreported worldwide. OBJECTIVE: To determine the incidence of vascular lesions in patients with a diagnosis of bile duct disruption secondary to cholecystectomy from January 1, 2015 to December 31, 2019, confirmed by preoperative CT angiography or intraoperative findings. METHOD: Retrospective, observational and analytical study of a series of cases between 2015 and 2019. Where a total of 144 cases of bile duct disruption were found, 15 cases (10%) with concomitant vascular injury. RESULTS: The most frequent vascular injury was of the right hepatic artery in 13 patients (87%). The level of biliary disruption most frequently associated was Strasberg E3 and E4 in 5 patients (36%), respectively. The treatment of vascular injury was ligation of the injured vessel in 11 patients (73%). The treatment established for the repair of biliary disruption was the hepatic jejunum anastomosis in 14 patients (93%). CONCLUSIONS: The presence of injury at the level of the right hepatic artery is the most frequent and its ligation did not show a significant impact on biliodigestive reconstruction, as long as an adequate technique is performed (Hepp-Couinaud).


ANTECEDENTES: Una de las complicaciones más serias de la colecistectomía es la disrupción de la vía biliar, la cual puede estar asociada con lesión vascular concomitante hasta en un 3.4%. Su incidencia, características demográficas y tratamiento son infrarreportados en todo el mundo. OBJETIVO: Conocer la incidencia de las lesiones vasculares en pacientes con diagnóstico de disrupción de la vía biliar secundaria a colecistectomía, del 1 de enero de 2015 al 31 de diciembre de 2019, confirmado por angiotomografía preoperatoria o por hallazgos transoperatorios. MÉTODO: Estudio retrospectivo, observacional y analítico de una serie de casos del 2015 al 2019. Encontrando 144 casos de disrupción de la vía biliar, 15 (10%) con lesión vascular concomitante. RESULTADOS: La lesión vascular más frecuente fue la de arteria hepática derecha, en 13 pacientes (87%). El nivel de disrupción biliar más habitualmente asociado fue Strasberg E3 y E4, en 5 pacientes (36%) cada uno. El tratamiento de la lesión vascular fue ligadura del vaso en 11 pacientes (73%). El tratamiento de la disrupción de vía biliar fue anastomosis hepático-yeyunal en 14 pacientes (93%). CONCLUSIONES: La presencia de lesión de la arteria hepática derecha es la más frecuente y su ligadura no tiene una repercusión significativa en la reconstrucción biliodigestiva, siempre y cuando se realice una técnica adecuada (Hepp-Couinaud).

ANTECEDENTES: La pandemia de COVID-19 ha ocasionado que los servicios de cirugía y de salud en todo el mundo tengan que reorganizarse y planear para poder brindar la mejor atención a los pacientes, con la protección necesaria para el personal de salud. Algunos de estos pacientes requerirán tratamiento quirúrgico, ya sea electivo o de urgencia. OBJETIVO: Reportar la experiencia inicial en el manejo de pacientes con COVID-19 que ameritaron tratamiento quirúrgico por los servicios de cirugía de un hospital de referencia. MÉTODO: Revisión de los protocolos quirúrgicos, equipo de protección personal usado por los equipos quirúrgicos y resultados del tratamiento de 42 pacientes sometidos a cirugía en un periodo de 4 meses. RESULTADOS: Fueron intervenidos 42 pacientes con COVID-19. Treinta pacientes tenían diagnóstico de infección por SARS-CoV-2 y en 12 casos el diagnóstico fue clínico y por imagen. Las cirugías más frecuentes fueron traqueostomía en 16 pacientes (38%) y laparotomías exploradoras en 8 pacientes (19%). La mediana de estancia posoperatoria fue de 17 días y la mortalidad durante los primeros 30 días fue del 26%. CONCLUSIONES: Es necesaria la reorganización de los departamentos quirúrgicos y del hospital para poder atender adecuadamente a los pacientes con COVID-19 y proteger al personal de salud. Los pacientes pueden presentan patologías que requieran tratamiento quirúrgico. Relacionado con la infección y la mayor frecuencia de comorbilidad, la mortalidad de estos pacientes es elevada. INTRODUCTION: the COVID-19 pandemic has caused a reorganization of hospital and general surgery departments worldwide to assure the best medical and surgical treatment of patients with this disease and protection of the health-related personnel. Some of them will require surgical treatment either elective or urgent. OBJECTIVE: report the initial experience in the management of patients with COVID-19 in a third level hospital. MATERIAL AND METHODS: a review of the surgical protocols, personal protection equipment used by the surgical teams, and results of the treatment of forty-two patients submitted to surgery. RESULTS: During four months (April-July 2020) forty-two patients with suspicion or confirmed infection of SARS-CoV2 underwent surgical treatment. The most common surgery was tracheostomy in 16 patients (38%) followed by exploratory laparotomy in 8 patients (19%). The median postoperative stay was 17 days and the thirty-day postoperative mortality rate was 26%. CONCLUSIONS: reorganization of the general surgery department and the hospital, favors adequate management and treatment of patients with COVID-19 and protection to the health-related personnel. Due to the usual co-existence of comorbidities and pulmonary complications the postoperative mortality of these patients is high.

Objetivo: Describir el comportamiento biológico del CA 19-9 en colestasis para determinar su utilidad para el diagnóstico de cáncer pancreatobiliar en pacientes con ictericia de origen obstructivo. Sede: Hospital General de México, O.D. Diseño: Prospectivo, longitudinal y comparativo. Análisis estadístico: prueba t y Kolmogorov-Smirnov para muestras independientes; prueba de valor diagnóstico (sensibilidad, especificidad y valor global). Se realizó análisis con curvas ROC para identificar sensibilidad y especificidad con los diferentes puntos de corte. Pacientes y métodos: Se incluyeron 54 pacientes con diagnóstico de ictericia de origen obstructivo. Se dividieron en dos grupos de acuerdo al diagnóstico final, enfermedad maligna vs enfermedad benigna. Se realizó determinación sérica de CA 19-9 al ingreso y al resolver la colestasis y se correlacionaron los niveles con el diagnóstico final. Resultados: En pacientes ictéricos, con un punto de corte de 60 U/mL para distinguir entre enfermedad maligna y benigna, el CA 19-9 tiene una sensibilidad de 80% y una especificidad de 90%. Una vez resuelta la colestasis con punto de corte de 39 U/mL se obtiene una sensibilidad de 71% con una especificidad de 96%. La normalización del marcador después del drenaje biliar es altamente sugerente de patología benigna. La persistencia de niveles elevados (mayores de 60 U/mL) es altamente sugerente de malignidad con una sensibilidad de 58% y especificidad de 100%. Conclusiones: La colestasis sí modifica la sensibilidad y especificidad del CA 19-9 para el diagnóstico de neoplasias malignas pancreatobiliares, por lo que en presencia de ictericia de origen obstructivo el punto de corte de 60 U/mL ofrece una sensibilidad de 80% con una especificidad de 90% para distinguir entre enfermedad benigna y maligna. Una vez resuelta la colestasis, la persistencia de niveles elevados es altamente sugerente de malignidad.


Objective: To describe the biological behavior of the tumor marker CA 19-9 in cholestasis to determine its usefulness for the diagnosis of pancreatobiliary cancer in patients with obstructive-origin jaundice. Sede: General Hospital of Mexico, third level health care center. Mexico City. Design: Prospective, longitudinal, and comparative study. Statistical analysis: T and Kolmogorov Smirnov tests for independent samples; diagnostic value test (sensitivity, specificity, and global value). Analysis with ROC curves was performed to identify sensitivity and specificity at the different cutting points. Patients and methods: We included 54 patients with a diagnosis of obstructive-origin jaundice. They were divided in two groups according to the final diagnosis, malignant disease vs. benign disease. Serum CA 19-9 was determined at admittance and once cholestasis had been resolved, and the levels were correlated with the final diagnosis. Results: In jaundice patients, with a cut point of 60 U/mL to distinguish between malignant and benign disease, the CA 19-9 marker has a sensitivity of 80% and a specificity of 90%. Once cholestasis had been resolved with a cut point of 39 U/mL, sensitivity was of 71% with a 96% specificity. Normalization of the marker after bile drainage is highly suggestive of benign pathology. The persistence of high levels (higher than 60 U/mL) is highly suggestive of malignancy with a sensitivity of 58% and specificity of 100%. Conclusions: Cholestasis does modify the sensitivity and specificity of the CA 19-9 marker for the diagnosis of pancreatobiliary malignant neoplasms; therefore, in the presence of obstructive-origin jaundice, the 60 U/mL cut point offers a sensitivity of 80% with a specificity of 90% to be able to distinguish between malignant and benign disease. Once cholestasis has been resolved, persistence of high levels is highly suggestive of malignancy.
