RESUMO

OBJECTIVES: We performed a meta-analysis to examine the performance of second-trimester (14-24 weeks' gestation) isolated fetal pyelectasis as a marker for trisomy 21 and to calculate its associated weighted pooled likelihood ratios. METHODS: PubMed, Ovid MEDLINE and Cochrane databases were searched using the terms 'pyelectasis' and 'pelviectasis'. Studies were included if fetuses with isolated pyelectasis were reported separately from fetuses with other soft markers of aneuploidy and/or structural anomalies and if knowledge of the fetal karyotype was unknown at the time of ultrasound examination. RESULTS: Individual study statistics were pooled as weighted positive and negative likelihood ratios with 95% CIs, using a random-effects model. Ten observational studies were included (2148 cases of isolated pyelectasis). Isolated fetal pyelectasis was defined in seven out of 10 studies as a renal pelvis anteroposterior diameter of ≥ 4 mm. Isolated fetal pyelectasis was associated with pooled positive and negative likelihood ratios of 2.78 (95% CI, 1.75-4.43) and 0.99 (95% CI, 0.98-1.00), respectively. CONCLUSIONS: The detection of isolated fetal pyelectasis on mid-trimester ultrasound is associated with an increased likelihood of trisomy 21. If the finding of isolated fetal pyelectasis is used to adjust the trisomy 21 risk from maternal serum screening tests, a positive likelihood ratio of 2.78 should be used in the calculation.

Certain dog breeds, especially Collies, are observed to exhibit neurotoxicity to avermectin drugs, which are P-glycoprotein (P-gp) substrates. This neurotoxicity is due to an ABCB1 gene mutation (ABCB1-1Δ) that results in non-functional P-gp expression. A developed Abcb1a knock-in/Abcb1b knock-out mouse model expressing the ABCB1-1Δ canine gene was previously reported and mice exhibited sensitivity upon ivermectin administration. Here, model and wild-type mice were administered P-gp substrates doramectin, moxidectin, and digoxin. While knock-in/knock-out mice exhibited ataxia, lethargy and tremor, wild-type mice remained unaffected. In addition, no neurotoxic clinical signs were observed in either mouse type administered domperidone, a P-gp substrate with no reported neurotoxicity in ABCB1-1Δ Collies. Overall, neurotoxic signs displayed by model mice closely paralleled those observed in ivermectin-sensitive Collies. This model can be used to identify toxic P-gp substrates with altered safety in dog populations and may reduce dog use in safety studies that are part of the drug approval process.

Electric permittivity and density were measured in a nitrobenzene and octane mixture in the vicinity of the upper critical consolute point. Measurements were conducted in the one-phase region, at the critical concentration. The possibility of stirring in the course of measurements allowed us to check if the density and concentration gradients had any influence on the obtained results. No signs of the presence of the gradients mentioned above were found. Using the data obtained in the reported measurements, different methods of the fitting of the equation describing the permittivity anomaly were tested. The calculation of a reliable value of the critical amplitude, used to estimate the critical temperature shift under the influence of the electric field, was of particular interest. The derivative (∂T(c)/∂E(2)) was found to be (-3.9 ± 0.3) × 10(-16) K m(2) V(-2).

Dielectric properties of breast tissue obtained from 131 patients were investigated by an open-ended probe separated from the tissue by a dielectric film (plastic foil). The film decreases the measured capacitance, but the content of blood and body liquids has little effect on the measured quantity. We found that in cancerous tissue the electric capacity (related to the permittivity) measured at f > or = 10 MHz using the modified probe is higher than that in normal tissue. The dielectric assignment was confirmed by histopathology in 94% of investigated cases. High efficiency in differentiation between normal and cancer breast tissues and the feasibility of fulfilling of aseptic conditions (a film covering the probe) are promising in intra-operative cancer diagnostics.

The complex electric permittivity was measured in water + 2-hydroxypyridine (2HP) solid mixtures as a function of concentration, temperature and frequency. Just after freezing of diluted mixtures, (mole fraction of 2HP < 0.2) pronounced dielectric dispersion in the MHz region was observed. The dispersion disappears on cooling between -30 and -40 degrees C in a first-order phase transition. The dispersion was explained in terms of the movement of a guest molecule (2HP) in a clathratelike structure of ice.

This article presents a series of 49 km/h sled tests using the Hybrid III 6-year-old dummy in a high-back booster, a low-back booster, and a three-point belt. Although a 10-year review at a level I trauma center showed that noncontact cervical spine injuries are rare in correctly restrained booster-age children, dummy neck loads exceeded published injury thresholds in all tests. The dummy underwent extreme neck flexion during the test, causing full-face contact with the dummy's chest. These dummy kinematics were compared to the kinematics of a 12-year-old cadaver tested in a similar impact environment. The cadaver test showed neck flexion, but also significant thoracic spinal flexion which was nonexistent in the dummy. This comparison was expanded using MADYMO simulations in which the thoracic spinal stiffness of the dummy model was decreased to give a more biofidelic kinematic response. We conclude that the stiff thoracic spine of the dummy results in high neck forces and moments that are not representative of the true injury potential.

We present a quantitative experimental evidence for the influence of critical fluctuations on the high frequency dielectric relaxation in a binary critical mixture. The analysis clearly shows the presence of critical anomaly both for the maxima (epsilon(")(p)) and frequencies (f(p)) of loss curves peaks. For the temperature evolution of epsilon(")(p) similar relation as for the static dielectric permittivity was obtained. The temperature dependence of relaxation times can be portrayed by using the Eyring-type equation remote from the critical consolute point and shows the unusual precritical speeding up. The obtained behavior resembles that found recently in the isotropic phase of nematic liquid crystals [A. Drozd-Rzoska and S. J. Rzoska, Phys. Rev. E 65, 041701 (2002)]. Results presented agree also with the recent theoretical suggestion [S. Goodyear and S. C. Tucker, J. Chem. Phys. 111, 9673 (1999)] that relaxation in supercritical fluids may exhibit glasslike features.

Abnormal collagen metabolism is a hallmark of diffuse lung disease. Biochemical parameters which are correlated with collagen synthesis and degradation may be helpful to monitor fibrosis. In this study, we compared the sensitivity and specificity of procollagen type-III-N-propeptide (PIIINP) and collagen type-VI (C-VI) related antigen levels, as well as a ratio of both parameters (PIIINP/C-VI), in bronchoalveolar lavage fluid (BALF) from patients with diffuse and localized lung disease. We investigated 45 patients with diffuse lung disease (idiopathic pulmonary fibrosis (IPF), n = 21; sarcoidosis, n = 13; and lymphangitic carcinomatosis (LC), n = 11); 58 control subjects; and 92 patients with localized lung disease (bronchial carcinoma, n = 37; pulmonary tuberculosis, n = 31; and pneumonia, n = 24). C-VI and PIIINP were measured by immunoassay in concentrated BALF. Although the PIIINP and C-VI levels were increased in diffuse lung disease, the sensitivity of the individual parameters PIIINP and C-VI was low (IPF: PIIINP = 0.62, C-VI = 0.29; LC: PIIINP = 0.64, C-VI = 0.45; sarcoidosis: PIIINP = 0.69, C-VI = 0.15). When calculating the ratio of PIIINP/C-VI for each individual patient, we found a significant increase in this ratio in IPF (1.28 +/- 0.7), LC (2.34 +/- 1.2), and sarcoidosis (0.26 +/- 0.08) compared to both the controls (0.02 +/- 0.01) and other localized lung diseases (bronchial carcinoma 0.05 +/- 0.01; pulmonary tuberculosis 0.02 +/- 0.01), with the exception of pneumonia (0.18 +/- 0.06).(ABSTRACT TRUNCATED AT 250 WORDS)
