RESUMO

OBJECTIVE: Of the work is a cross-cultural analysis of the characteristics of student response in a pandemic situation. MATERIAL AND METHODS: The sample consisted of students of NSMU and the Kazakh National University. Al-Farabi in the amount of 37 people aged 20 to 23; The battery of psychological methods was compiled by Spielberger and Khanina; Mississippi Post-Traumatic Stress Disorder Scale (civil version) and PSM-25 Psychological Stress Scale. RESULTS: 52.7% of respondents showed an average level of stress, 47.3% - a low level, which indicates insufficient psychological adaptation to psychological stress. High rates of personal and situational anxiety were diagnosed in 67.57% of cases; the average level of reactive anxiety - in 29.73% of cases and personal - in 27.03% of respondents. The majority of respondents showed average levels of post-traumatic stress reactions (50%, n=37), while a low level of post-traumatic stress reactions occurs in 11 (14.87%) people, a low level - in 17 (22.97%) people and increased - in 9 (12.16%) people. Stress level indicators tend to have significant differences: the frequency of occurrence of average stress indicators in the sample of students from Kazakhstan is higher (75.6%) compared to Russian students (24.32%) (p<0.05).Adapting to changing academic workloads in a pandemic situation does not contribute to the psychological well-being of students, as evidenced by the results of diagnosing anxiety and post-traumatic stress reactions. The results obtained emphasize the importance of taking into account cultural factors in stressful situations. CONCLUSION: Cross-cultural differences in the level of emotional response of students in a pandemic situation were revealed.

OBJECTIVE: To study the dynamics of personality features and features of self-release in drug addicts. MATERIAL AND METHODS: We examined 10 drug-dependent patients with whom psycho-correctional work was carried out, 10 drug-dependent patients with whom correctional work was not carried out, and 20 healthy young people. All participants were men aged 20 to 24 years. Personality characteristics were determined by Cattell's 16PF questionnaire and a set of psychological tests. Correction work was carried out by a psychologist (7 sessions, one hour each). In these cases, the patients were examined before and after psychocorrectional intervention. RESULTS: The high levels of sociability, self-esteem and expressiveness combined with low intelligence were identified in drug addicts compared with healthy people. Drug addicts are willing to accept themselves and are interested in their own personality. CONCLUSION: This study is of practical importance and can be used as a material for dealing with people with addictive behavior.

OBJECTIVE: To study symptoms of neurotic disorders and social masochism in chemical addicts. MATERIAL AND METHODS: Thirty outpatients of the Novosibirsk Regional Clinical Addiction Dispensary were studied. Corresponding scales and questionnaires, along with a social masochism questionnaire developed by the authors, were used. Results were compared to those of 25 healthy people. RESULTS AND CONCLUSION: A significantly higher level (p≤0.05) of the following neurotic disorders: sleep disturbances, hysterical neurotic disorder, anxious neurotic disorder, social maladaptation and depressive neurotic disorder as well as a subclinical level of anxiety and depression were observed in chemical addicts. Moreover, patients with chemical dependence revealed significantly higher (p≤0.05) indicators of aggression, victim behavior and social masochism (different types of aggression, victimization, irritability, negativism, resentment, guilt, self-harming behavior, hypersocial behavior, provoking and uncritical behavior, the role of the victim, negative perception and a general indicator of social masochism). The correlations between indicators of the affective sphere, manifestations of social masochism and neurotic disorders in individuals with chemical dependence were identified.

RESUMO

Current concepts of post-psychotic period rarely include posttraumatic context due to the conventional clinical opinion about psychotic disorders and transformation of the typical posttraumatic stress disorder symptoms. The authors presented the ways of coincidence of dissociative posttraumatic symptoms (dissociative spectra) and psychotic spectra, their reciprocal connection and different variants depending on psychosis dynamic type.

RESUMO

AIM: To study social-psychological predictors of different types of addictive behavior. MATERIAL AND METHODS: Thirty-six patients, aged 22-50 years, with different addictions (nicotine dependence, alcohol dependence, drug dependence, Internet addiction, extreme sports addiction) were studied. A questionnaire for assessment of personality accentuation, the Life Style Index, The world assumptions scale and the Rokeach Value Survey were administered. RESULTS AND CONCLUSION: Social-psychological predictors of the types of addictive behavior were identified.

AIM: To study the relationship between the family structure and characteristics of the response to stress in adolescent addicts. MATERIAL AND METHODS: The study included 65 patients who used psychoactive substances and 42 healthy controls. A set of psychological methods was used. RESULTS AND CONCLUSION: In the group of adolescent addicts, a low result was revealed on the scale 'family cohesion'. A high level of development of such mechanisms of psychological defense as denial, suppression, repression, compensation, substitution, intellectualization and reactive education was diagnosed in both groups. Significant differences were revealed only in repression. Such stress coping strategies as self-control, escape-avoidance and problem-solving were more often used in the main group while confrontational coping, distancing, seeking social support, accepting responsibility and positive reassessment were used in the control group. Lower indices of resilience in adolescent addicts compared to the control group.

AIM: To compare the prognosis of depression severity estimated by the physician and by the patient based on the treatment outcome. MATERIAL AND METHODS: One hundred and seven patients with depression were examined. Mental status was assessed with HАМ-D, SHAPS, CGI-S, CGI-I, PGI-S, PGI-I and VAS. A data analysis was performed. RESULTS: There were differences in the estimation of depression severity by psychiatrists and patients. Moreover, the scores on HАМ-D and CGI-S were not consistent when assessed by psychiatrists. As the severity of depression decreased and patient's state improved during the treatment with agomelatine (valdoxan), the assessments of the changes by the psychiatrist and the patient became similar. CONCLUSION: Agomelatine (valdoxan) is effective and tolerable in the treatment of depression of any severity. The differences between the psychiatrist's and patient's estimation of the depression severity at baseline using different psychometric scales can level the prognostic value of treatment outcome.

To evaluate advantages and shortcomings of Dumon's, T-shape and polyflex endotracheal stents in patients with tumor and scarry stenoses of the trachea and main bronchi, we analysed the results of stenting trachea and main bronchi in 180 patients for 11 years (1992-2002). We observed the highest rate of granulations and dislocations for polyflex stents in the lowest adhesion of the sputum to their walls. Long-term results of stenting in scarry tracheostenosis demonstrate a moderate efficacy of T-stents and low efficacy of Dumon's stents. Our final conclusion is that a key method of treatment of tracheal scarry stenoses is a circulatory resection of the stenosed zone. Tracheal stenting remains important in urgent necessity of the treatment of decompensated stenoses, in preparation for radical operation and in contraindications to the latter.

RESUMO

We consider low-dimensional dynamical systems exposed to a heat bath and to additional ac fields. The presence of these ac fields may lead to a breaking of certain spatial or temporal symmetries, which in turn cause nonzero averages of relevant observables. Nonlinear (non)adiabatic response is employed to explain the effect. We consider a case of a particle in a periodic potential as an example and discuss the relevant symmetry breakings and the mechanisms of rectification of the current in such a system.

RESUMO

A new approach is developed to study the relaxation of concentrations to equilibrium in reversible bimolecular reactions. For A+B <==> C, a general analytic expression is derived for the amplitude of the power law (t(-d/2)) asymptotics for arbitrary diffusion coefficients and concentrations of the reactants. Our formalism is based on the analysis of the time correlation functions describing the equilibrium fluctuations of the concentrations. This powerful and simple procedure can be readily used to study other bimolecular reactions such as A+B <==> C+D.

RESUMO

The authors analyze the condition of free-radical oxidation and activity of antioxidant system, clinical effectiveness of He-Ne laser therapy of patients with chronic pyelonephritis. It is shown that clinical manifestation of the disease is accompanied with activation of free radical oxidation and hypoactivity of antioxidant system. Endovascular laser hemotherapy stimulates antioxidant system, activity of superoxide dismutase, in particular. A good response and immunomodulation are achieved.

The author's investigations led them to the conclusion that clinical manifestation of chronic pyelonephritis is associated with destabilization of lipid bilayer of red cell membranes, renal tissue, activation of lipid peroxidation (LPO), rising lipid and LPO products excretion with the urine. 31 patients with chronic pyelonephritis received combined therapy comprising essential phospholipids. 25 control patients with the same disease were not given essentiale-forte. Finally, the latter had longer fever duration, more active inflammation. The former had also low urine excretion of oxalates and normal lipid composition of biological membranes.

A total of 117 patients with pulmonary interstitial diseases (PID) were examined. The functional activity of alveolar macrophages was assayed in the lavage fluid and in lung tissue biopsy specimens from the generation of active oxygen forms, the secretion of tumor necrosis factor, fibronectin, expression of c-fos- and c-sis-oncoprotein. The stereotypic value for various PID was the development of alveolitis running in 2 stages: 1) early one, including exudative inflammation and 2) late one, involving sclerotic changes up to the formation of the honeycomb lung. This results in the block of the blood-air barrier and progression of respiratory failure and hypoxia in patients. The morphogenesis of fibrosing alveolitis is formed of alveolar septal damages caused by etiological agents of various nature, which is frequently unclear, by active forms of oxygen, lipid peroxidation products, proteases, tumor necrosis factor, which are produced by activated alveolar macrophages and polymorphonuclear leukocytes. The alveolar macrophage that secretes growth factors, c-fos- and c-sis-oncoproteins plays the key role in the progression of sclerotic changes. Lung cancer may develop at the end of fibrosing alveolitis at the stage of the honeycomb lung.

Upon analysis of 550 cases of different chronic diffuse pulmonary diseases included in a group of interstitial diseases of the lungs (IDL) the authors came to the conclusion that IDL incorporate such variants as alveolitis, pulmonary vasculitis and pulmonary hemorrhages; granulomatosis covers exogenic allergic alveolitis, alveolitis in chronic active hepatitis; vasculitis group includes such rare diseases as necrotizing sarcoid granulomatosis vasculitis and lymphoid granulomatosis; fibrosing alveolitis--secondary alveolitis in sclerodermia systematica, rheumatoid arthritis, Sjogren's disease, chronic active hepatitis. Knowledge of IDL etiology (environmental, occupational, induced by radionuclides, drugs, viruses, fungi) with focus on drug affection of the lungs is thought of value. Biopsy and bronchial lavage findings are compared clinically and morphologically. Mechanisms of pulmonary fibrosis and approaches to inhibition of pulmonary fibrosis progression are outlined.

RESUMO

Clinicomorphological analysis covered 41 cases of idiopathic fibrosing alveolitis (IFA). The comparison of the degree of respiratory insufficiency, hypoxia, survival of the patients with cytogram of the bronchoalveolar lavage, morphological findings, generation of active oxygen forms by alveolar macrophages and leukocytes has provided evidence on an important role of alveolar macrophage in interstitial alveolitis and fibrosis in IFA. Three variants of chronic IFA are distinguished: aggressive, persistent and slowly progressive. The variants have different prognosis and need individual therapeutic approach.

RESUMO

Electron microscopic and immunohistochemical methods were employed to study composition of collagens in extracellular matrix and quantify oncoproteins c-sis and c-fos. It was established that interstitial pulmonary diseases of various etiology are characterized at early stages by alveolitis with activation of alveolar macrophages, at late stages by interstitial fibrosis with accumulation of collagens type I, III, IV and V and epithelial dysplasia which is likely to initiate bronchioloalveolar cancer.

It is established that active pyelonephritis and operative stress are accompanied with structural and functional changes in erythrocytic membranes: cholesterol and its ethers rose in quantity as well as difficulty oxidized phospholipids, cation adenosine triphosphatase activity changed. Being nonspecific, these changes seemed more pronounced in the active phase of chronic pyelonephritis. Minor structural rearrangement of membrane lipids in surgical patients indicated uneventful postoperative period. It is suggested that bacterial inflammation in the kidneys and urinary tracts may be related to the above shifts. The adjuvant use of dimephosphone produced a clear-cut membrane-stabilizing effect clinically reflected by reduced inflammation and improved renal function.

The paper examines the relationship between the clinical manifestations of pyelonephritis and the functional activity of enzymes of cation transmembrane erythrocyte transport (Mg(2+)-, N(+)-K(+)-, Ca(2+)-ATPases). An individual analysis ascertained that the patients who showed a low Ca(2+)-ATPase activity had marked signs of inflammation in the body, as evidenced by ESR, seromucoid and fibrinogen concentrations. These patients had more significantly depressed immune defense mechanisms as reflected by the levels of immunoglobulins, T-lymphocytes, complement, the neutrophil phagocytosis, and urinary IgA concentrations). Variations were also found in examining the excretion of a number of metabolites. There was a substantial decrease in urea, creatinine, titrated acid, phosphorus excretions in patients with deficient Ca(2+)-ATPase activity than in those with its high activity. It was concluded that there was a relationship between some clinical manifestations of pyelonephritis and the functional activity of enzymes of cation transmembrane transport. To treat metabolic disorders, membrane-protective agents are recommended to include into combined therapy.
