RESUMO

Objective: Type 2 diabetes (T2D) and high blood pressure are the main causes of chronic kidney disease (CKD) in adulthood. Both metabolic and oxidative stresses driven by hyperglycemia as well as genetic factors have been suggested as pathogenic causes of renal failure. Some single nucleotide variants (SNVs) on gene coding KLOTHO (KL) have been implicated in several clinical scenarios including hypertension, diabetes, and cardiovascular disease. The aim of this study was to analyze the association of rs1207568 (-395G > A), rs953614 (+ 1062T > G) and rs564481 (+ 1818 C > T) SNVs with metabolic and renal function parameters in Mexican patients living with type 2 diabetes. Methods: A cross-sectional study was conducted in 637 Mexican patients with T2D, and/or hypertension without previous diagnosis of CKD. Anthropometric, metabolic, and renal function parameters were determined. Patients were genotyped for rs1207568, rs953614 and rs564481 SNVs and associations under a dominant genetic model were analyzed by logistic regression. Results: For rs9536314, G-allele showed to be protective for hypo-HDL-C, albuminuria, and CKD. Carriers of minor allele of rs564481 had low odds for high glucose levels. No differences in genotype nor allele frequencies between the patients and the reference population were observed. Conclusion: In Mexican patients living with type 2 diabetes, KL variant rs9536314 was found associated with low odds of hypo-HDL cholesterol, albuminuria and presence of CKD. Meanwhile the consensus of soluble KLOTHO measurement is reached, genetic variants in the KL gene could be considered as genetic markers for CKD susceptibility in patients at high-risk of vascular complications.

During the early stages of the development of the living multiorgan systems, genome modifications other than sequence variation occur that guide cell differentiation and organogenesis. These modifications are known to operate as a fetal programming code during this period, and recent research indicates that there are some tissue-specific codes in organogenesis whose effects may persist after birth until adulthood. Consequently, the events that disrupt the pre-established epigenetic pattern could induce shifts in organ physiology, with implications on health from birth or later in adult life. Chronic kidney disease (CKD) is one of the main causes of mortality worldwide; its etiology is multifactorial, but diabetes, obesity, and hypertension are the main causes of CKD in adults, although there are other risk factors that are mainly associated with an individual's lifestyle. Recent studies suggest that fetal reprogramming in the developing kidney could be implicated in the susceptibility to kidney disease in both childhood and adulthood. Some epigenetic modifications, such as genome methylation status, dysregulation of miRNA, and histone coding alterations in genes related to the regulation of the renin-angiotensin axis, a common denominator in CKD, may have originated during fetal development. This review focuses on epigenetic changes during nephrogenesis and their repercussions on kidney health and disease. In addition, the focus is on the influence of environmental factors during pregnancy, such as maternal metabolic diseases and dietary and metabolic conditions, as well as some sex differences in fetal kidney reprogramming during which dysregulation of the renin-angiotensin system is involved.

Aarskog-Scott syndrome (AAS), also known as faciogenital dysplasia (FGD, OMIM # 305400), is an X-linked disorder of recessive inheritance, characterized by short stature and facial, skeletal, and urogenital abnormalities. AAS is caused by mutations in the FGD1 gene (Xp11.22), with over 56 different mutations identified to date. We present the clinical and molecular analysis of four unrelated families of Mexican origin with an AAS phenotype, in whom FGD1 sequencing was performed. This analysis identified two stop mutations not previously reported in the literature: p.Gln664* and p.Glu380*. Phenotypically, every male patient met the clinical criteria of the syndrome, whereas discrepancies were found between phenotypes in female patients. Our results identify two novel mutations in FGD1, broadening the spectrum of reported mutations; and provide further delineation of the phenotypic variability previously described in AAS.

El síndrome de Griscelli (SG) es una enfermedad autosómica recesiva, caracterizada, según las variantes clínicas, por albinismo parcial o cabello platinado, inmunodeficiencia celular, hipogammaglobulinemia, pancitopenia y severo deterioro neurológico. El diagnóstico se realiza de acuerdo a los hallazgos histopatológicos de la biopsia de piel, las manifestaciones clínicas descriptas y el análisis molecular de los genes RAB27A y MYO5A. Se describe un caso de SG, confirmado mediante estudio molecular, en una familia mexicana con antecedentes de consanguinidad tío paterno-sobrina y una hermana mayor con característiscas similares, fallecida a los 4 años. El paciente, al nacer, había tenido diagnóstico de albinismo. Se presentó a la consulta con historia de infecciones frecuentes y fiebres recurrentes sin foco y se encontró bicitopenia y síndrome mieloproliferativo. Ante la sospecha diagnóstica, se realizó una microscopía del cabello, en la que se observó distribución del pigmento en cúmulos y en los frotis hemáticos se determinó la ausencia de inclusiones intracitoplasmáticas. Se estableció el diagnóstico de SG tipo 2 y se obtuvo una muestra de ADN para el estudio molecular del gen RAB27A. El examen confirmó una mutación homocigota no comunicada previamente, por lo que se dedujo que su hermana había tenido la misma afección y que el padre era portador obligado...(AU)


Griscelli syndrome (GS) is an autosomal recessive disease characterized by partial albinism or platinum hair, varying cell immunodeficiency, hypogammaglobulinemia, pancytopenia and severe neurological impairment, depending on clinical variants. The diagnosis is made with histopathological findings in skin biopsy, the clinical manifestations described and molecular analysis of the genes RAB27A and MYO5A. We describe a case of GS, confirmed by molecular study, in a Mexican family with a history of paternal uncle-niece consanguinity and a sister with similar characteristics, deceased at 4 years old. At birth, the patient was diagnosed as albinism. He presented to us with a history of frequent infections and recurrent fevers of unknown origin and a bicytopenia and a myeloproliferative syndrome were found. With diagnostic suspicion, a mycroscopic study of the hair was done and the findings were consistent with pigment distribution in clusters and in hematological smears the absence of intracytoplasmic inclusions was demonstrated. Therefore the diagnosis of GS type 2 was established and a DNA sample was obtained for RAB27A gene molecular study. The exam confirmed a previously unreported homozygous mutation in the gene RAB27A, so it was established that his sister had the same condition and the father was a forced carrier...(AU)
