RESUMO

La formación basada en competencias es un proceso abierto y flexible de desarrollo de competencias laborales donde, a partir de su identificación y normalización, se establecen los diseños curriculares, que posibilitan garantizar un desempeño laboral más efectivo. Se sustenta en el desarrollo y valoración de diferentes actuaciones, una planificación innovadora y exige el inicio de cambios en las estrategias pedagógicas, los enfoques curriculares y en el papel tradicional asignado a docentes y educandos. Requiere la utilización de situaciones de aprendizaje combinadas con la orientación del aprendizaje hacia la solución de problemas. Se presenta una propuesta metodológica, que estructura el proceso en cuatro fases: Análisis y estudio previo; diseño curricular; implementación y evaluación curricular. Se explican las características principales de cada una de ellas. Este modelo propiciará la formación de un mejor especialista, con posibilidades de brindar un desempeño cualitativamente superior, que satisfaga las necesidades crecientes de nuestra población y pueda brindar una mejor cooperación médica internacionalista(AU)


The competence-based formation is an open and flexible process for the development of work competences in which, from its identification and standardization, curriculum designs are established which makes it possible to guarantee a more effective work performance. It is sustained on the development and assessment of different acts, an innovative planning and it demands the beginning of changes in pedagogical strategies, curriculum approaches and the traditional role assigned to teachers and students. It requires the use of combined learning situations with a learning orientation to problem solving. A methodological proposal is presented which structures the process in four stages: analysis and previous study; curriculum design; implementation and curriculum assessment. The principal characteristics of each of them are explained. The model will provide the formation of a better specialist, with possibilities to offer a qualitatively superior performance that satisfies the increasing needs of our population and affords a better internationalist medical cooperation(AU)

A partir de la aprobación de la Norma Cubana del Sistema de Gestión Integrada del Capital Humano y en correspondencia con la decisión del Ministerio de Salud Pública de comenzar a evaluar al personal médico en las siete especialidades médicas priorizadas, se plantea la necesidad de identificar y normalizar las competencias laborales que garanticen su implementación en el sistema nacional de salud, a fin de incrementar la calidad de los servicios que se brindan a la población. Una vez efectuada la identificación de las competencias laborales se hace necesario proceder a su normalización a fin de establecer el patrón que posibilite realizar su evaluación. Se presenta una propuesta metodológica para realizar la normalización de las competencias laborales en las especialidades médicas priorizadas. Este trabajo recaerá en los Grupos Nacionales de Especialidades Médicas de conjunto con los Grupos Provinciales Respectivos. Esta estandarización posibilitará poner de acuerdo a los profesionales médicos, los empleadores y las instituciones educacionales hacia la integración y enrumbamiento de sus respectivas responsabilidades. Se establece un modelo para el registro de la normalización de las competencias y sus elementos de competencias en el sistema nacional de salud(AU)


After the approval of the Cuban standard for the integrated management system of the human capital, and in line with the Ministry of public health's decision of evaluating the medical staff in the seven prioritized specialties, it is required to identify and to standardize the work competencies backing up the standard implementation in the health care system, in order to increase the quality of services offered to the population. Once the work competencies are identified, it is necessary to proceed to their standardization so as to set the pattern of evaluation. A methodological proposal for the work competency standardization in the priority medical specialties was submitted. The national groups of medical specialties and the respective provincial groups will be in charge of doing this work. This standardization will help to unify the criteria of the medical professionals, the employers, and the educational institutions towards integration and orientation of their respective responsibilities. A model was presented to register the standardization of the work competencies and their elements in the national health care system(AU)

RESUMO

The term persistent vegetative state (PVS) refers to the only circumstance in which an apparent dissociation of both components of consciousness is found, characterized by preservation of wakefulness with an apparent loss of awareness. Several authors have recently demonstrated by functional neuroimaging studies that a small subset of unresponsive "vegetative" patients may show unambiguous signs of consciousness and command following that is inaccessible to clinical examination at the bedside. The term "estado vegetativo" used in Spanish to describe the PVS syndrome by physicians came from the English-Spanish translation. The Spanish term "vegetativo" is related to unconscious vital functions, and "vegetal" is relative to plants. According to our experience, when a physician informs to patients' relatives that his/her family member's diagnosis is a "estado vegetativo", they understand the he/she is no more a human being, that there is no hope of recovery. The European Task Force on Disorders of Consciousness has recently proposed a new term, unresponsive wakefulness syndrome (UWS), to assist society in avoiding the depreciatory term vegetative state. Our group has embraced the use of the new term UWS and might suggest that we change our concept and use of the term MCS to minimally responsive wakefulness state (MRWS), or minimally aware wakefulness state (MAWS). Medical terms must be current and avoid any pejorative description of patients, which will promote our abilities to serve humankind and challenge neuroscientists to offer society new and realistic hopes for neurorehabilitation.
