Introducción: la Hiperplasia gingival hereditaria es un agrandamiento de los tejidos gingivales poco frecuente, asociado a un componente genético, que se hace evidente en el momento del recambio dentario. Puede generar diastemas, maloclusiones, dolor a la masticación, caries dental, enfermedad periodontal, retardo en el recambio dentario, complicaciones fonéticas, de deglución y un importante compromiso estético y psicológico. Objetivo: mostrar el manejo multidisciplinario en el tratamiento de la Hiperplasia gingival hereditaria asociada a hipertricosis. Presentación del caso: paciente de sexo masculino de 9 años de edad, con antecedentes familiares de Hiperplasia gingival e hipertricosis; al examen clínico constatamos hipertricosis y agrandamiento gingival generalizado que compromete el brote dentario, la estética y la función, y tiene repercusión psicológica. Se realiza el tratamiento periodontal quirúrgico del maxilar (Fase I), bajo anestesia general, con el propósito de descubrir las coronas dentarias; posteriormente, se le instauró tratamiento de ortodoncia con aparatología fija. A los 6 meses, después del tratamiento, se constatan las encías sin evidencia clínica de recidiva de Hhiperplasia gingival y mejoramiento en el posicionamiento dentario. Conclusiones: la atención multidisciplinaria (quirúrgico-ortodóntico-periodontal) es fundamental para lograr el brote y alineamiento dentario y obtener resultados estéticos y funcionales óptimos en el tratamiento de la Hiperplasia gingival hereditaria.


ABSTRACT Introduction: hereditary gingival hyperplasia is a rare gingival enlargement associated to genetic background, which becomes evident at the time of eruption of permanent dentition. This condition may produce malocclusions, painful chewing, dental caries, periodontal diseases, delayed permanent eruption, phonetic and swallowing problems, as well as important aesthetic and psychological disturbances. Objective: to show the multidisciplinary management in the treatment of hereditary gingival hyperplasia. Case presentation: a 9 years old male patient with family history of hereditary gingival hyperplasia and hypertricosis. On physical examination he showed hypertricosis and generalized gingival enlargement reaching the dental edge, with aesthetic and functional damage leading to psychological disturbances. A surgical periodontal procedure of the maxilla (phase I) was given under general anesthesia in order to uncover the dental crowns. After recovery, fixed orthodontic braces treatment was applied. Six months later, no clinical signs of gingival enlargement were found and dental positioning had improved. Conclusions: a multidisciplinary approach is basic in order to achieve a proper dental alignment and eruption, with the subsequent aesthetic and functional improvement in the treatment of hereditary gingival hyperplasia.

OBJECTIVE: To determine the prevalence and severity of dental fluorosis, as well as factors associated with its occurrence, in seven cohorts of children before and after the implementation of a fluoridated salt program in 1991. MATERIAL AND METHODS: A cross-sectional study was carried out in 1,373 children aged from 6 to 12 years in Campeche, Mexico. Data were collected by means of a questionnaire administered to mothers and a dental examination of the children. Modified Dean's criteria were used to diagnose dental fluorosis. A multivariate logistic regression model was used to evaluate the relationship between dental fluorosis and independent variables. RESULTS: The prevalence of dental fluorosis was 51.9%. The most common degree of dental fluorosis was very mild, with 84.7%, followed by mild, moderate, and severe with 13.1%, 1.7%, and 0.6%, respectively. The multivariate model adjusted by number of additional sources of fluoride, age at the beginning of use of toothpaste, and level of schooling of the mother, showed that children born in 1990 (OR = 1.74; CI 95% = 1.36-2.22), 1991 (OR = 4.03; CI 95% = 2.58-6.28), and 1992 (OR = 10.41; CI 95% = 5.77-18.78) were more likely to have dental fluorosis than those born in the period 1986-1989. The frequency of toothbrushing (OR = 1.63; CI 95% = 1.37-1.95) was also associated with dental fluorosis. CONCLUSIONS: A close relationship was found between exposure to toothpaste and dental fluorosis. Implementation of the fluoridated salt program greatly increased the risk of fluorosis.

OBJECTIVE: To evaluate the likelihood of caries increment in schoolchildren, based on their prior caries experience. MATERIAL AND METHODS: We undertook a longitudinal study in 452 six-to-nine year olds between 1999 and 2001 in Mexico, with dental exams conducted by two standardized examiners (kappa>0.85). The dependent variable was the DMFT increment, dichotomized as without increment, and at least one unit of increment. Independent variables estimated caries experience at baseline. Data were analyzed using non-parametric tests and generalized linear models (log-binomial) to calculate relative risk (RR) adjusted for age and sex. RESULTS: The percentage of caries-free children diminished by 20.5% from 1999 to 2001. DMFT index increased two-fold, from 0.25+/-0.70 in 1999 to 0.77+/-1.30 in 2001 (p<0.001). The overall risk for this sample was 24%. The DMFT increment was higher (p<0.001) in children with DMFT>0 and dmft>0 in 1999 (RR=1.89, 95% CI=1.37-2.62; RR=2.71, 95% CI=1.94-3.76, respectively). The likelihood for DMFT increment from the 1999 levels was: (1) 2.78 times higher (95% CI=2.06-3.76) if schoolchildren had caries in any of the first permanent molars and (2) 1.62 times higher (95% CI=1.20-2.19) if schoolchildren were affected by high severity caries at baseline. CONCLUSIONS: Both caries prevalence and mean DMFT had significant increments in 18 months. Dental caries in the primary (dmft) and permanent (DMFT) dentitions at baseline are goods indicators of subsequent caries development in this group of children in a medium income country. This relationship became stronger when the occurrence of caries in the first permanent molars was included.

El objetivo de este estudio fue determinar el estado de caries en los primeros molares permanentes para analizar la severidad del daño causado, la magnitud y distribución del problema, así como explorar su relación con factores socioeconómicos. En una muestra representativa de 3 615 escolares de 6 a 13 años de edad, 3 examinadores estandarizados (Kappa = .95) aplicaron el criterio de magnitud de la lesión cariosa (Gutiérrez, 1987). La prevalencia de caries fue del 80,3 por ciento, el CPOD para el grupo de 6 años fue de 0,11 (.444) y para los de 12 de 1,25 (1.78). La frecuencia de escolares con lesiones más severas de caries fue del 17,37 por ciento. El porcentaje de hombres con los 4 primeros molares afectados es de 1,47 por ciento en el grupo de 6 a 9 años y de 5,67por ciento en el grupo de 10 a 13 años; este porcentaje se incrementa de 1,11 a 6,5 por ciento en las mujeres en los mismos grupos de edad. Se encontró asociación entre la frecuencia de niños con lesiones severas y la actitud de la madre hacia la salud oral, número de hijos en la familia y escolaridad de la madre. Los resultados indican la persistencia de una proporción de la población con altos índices de caries y la necesidad de educación acerca de la importancia de este diente y de la salud oral(AU)


This study was aimed at determining the state of caries in the first permanent molars to analyze the severity of the damage caused, the magnitude and distribution of the problem, and to explore their relationship with socioeconomic factors. In a representative sample of 3 615 schoolchildren aged 6-13, 3 standardized examiners (Kappa = 95) applied the criterion of magnitude of the carious lesion (Gutierrez, 1987). The prevalence of caries was 80.3 percent, the CPOD for the group aged 6 was 0.11 (.444), whereas for those aged 12 it was 1.25 (1.78). The frequency of school children with more severe lesions was 17.37 % The percentage of men with the first 45 molars affected is 1.47 percent in the group aged 6-9, and 5.67 percent for those aged 10 - 13. This percentage increased from 1.ll to 6.5 percent among women from the same age groups. It was found association between the frequency of children with severe injuries and the mother's attitude towards oral health, number of children in the family and the mother's educational level. The results showed the persistence of high indexes of caries and the need of education about the importance of this tooth and of oral health(AU)
