RESUMO

UNLABELLED: Cytological and histopathological examinations performed in pregnant women allow quick for diagnosis of these pathological states. MATERIAL AND METHODS: 35 pregnant women with cervical dysplasia or carcinoma treated in Department of High Risk Pregnancy from 1996 to 2001 were included in the study. Diagnosis was based on the cytological and histological tests. RESULTS: We observed 10 cases of low-grade dysplasia and 8 cases of medium grade dysplasia. Both groups were qualified for vaginal delivery and treatment was started after delivery. In 12 pregnant women high-grade dysplasia was diagnosed (in 6 of them cervical conization between 16 and 26 week of pregnancy was performed). In 5 women cervical carcinoma in pregnancy was diagnosed. In 3 of them we performed cesarean section with radical operation between 34 and 40 week of gestation. CONCLUSION: Cytological and histological examination in pregnant women with cervical abnormalities should be obligatory because of high rate of dysplasia and cancer in pregnant women.

RESUMO

OBJECTIVE: An observation of an influence of long-term therapy of pregnant women affected by arterial hypertension with transdermal patches releasing GTN on stabilisation of blood pressure and newborn condition. STUDY DESIGN: The transdermal patches (Minitran S10) releasing GTN were used for the treatment of 27 pregnancies between 27-34 weeks admitted for hypertension above 150/100 mmHg. The average period of obtaining the proper blood pressure values, time of therapy, side effects of treatment and effects of nitrate tolerance were assessed. After delivery the newborns condition were estimated by Apgar score and their trophy was assessed after the centile chart. RESULTS: The diminishing of mean blood pressure values in range 135/85 mmHg was received after 8 to 48 hours of treatment in 25 of the examined women. The time of therapy was from 3 to 13 weeks without any serious side effects besides transient headaches eliminated by application of oral analgesic drugs. 24 newborns were delivered in good conditions--8-9 Apgar score. No side effects of long-term therapy with patches releasing GTN were observed among the newborns. CONCLUSIONS: The long-term transdermal treatment afford possibilities for pregnancy prolongation until the term of delivery and improves the foetal development conditions. The side effects of treatment are transient and affected only mothers.

RESUMO

OBJECTIVES: The problem of leiomyomas founded in pregnancy is reported. DESIGN: Diagnostic methods, indications and contraindications for the myomectomy and surgical technique are presented. MATERIALS AND METHODS: In years 1991-2000--185 pregnant women with leiomyomas diagnosed during pregnancy or caesarean section were treated. RESULTS: There were indications for the myomectomy during pregnancy in the group of 26 women, in next 55 women leiomyomas were diagnosed in the course of pregnancy but myomectomy was performed during caesarean section. In the other group of 104 pregnant women leiomyomas were found and enucleated first during caesarean section made on the other indications. CONCLUSIONS: It was observed that myomectomy during caesarean section and also in the course of pregnancy is a safe procedure when the indications and surgical technique are correctly made.
