RESUMO

In response to a long history of problems with defining and measuring fatigue, the University of Kansas School of Nursing established a Center for Biobehavioral Studies of Fatigue Management to facilitate the study of fatigue in diverse populations. The purpose of this article is to review past efforts to define and measure fatigue and the conceptual problems relevant to currently used measures of fatigue. Several distinct characteristics and corresponding measures of fatigue are identified and a definition and framework for the study of fatigue are discussed. Future research on fatigue must attend to the conceptual distinctions among various measures and the measures of fatigue most appropriate to the goals of a study.

RESUMO

Traditional ethical concerns in research tend to focus on serious misconduct such as fabricating data and gross violations of informed consent. In this two-part article, we focused on some of the less serious and more subtle ethical quandaries, inherent and common in most social science research. It is our thesis that these issues are important and warrant more attention and careful thought than they have been accorded in the scientific literature and, perhaps, by investigators at large. In Part 2, we provided examples of how we, and others, dealt with some of these issues in actual studies. However, we also must emphasize the individual and reflective nature of confronting ethical issues in research. We found many benefits in using a team approach to address ethical dilemmas during implementation of our studies. Doing so kept everyone honest with themselves and each other, shared the burden of responsibility among team members for the decisions made, maximized the use of team members' strengths, and led to much better resolutions of the issues from both a scientific and an ethical perspective. Maintaining scientific rigor while balancing and deliberating about the ethical implications of decisions is a challenge for all research teams. It requires learning how to be vigilant and responsive when faced with the many subtle, but ever-present, scientific and ethical quandaries encountered when doing what is often assumed to be nonthreatening social research. There are no fixed, prescriptive rules for what to do in every study. Rather, a reflective process, considering the specific issues of each unique study, is required.

A major challenge in the rehabilitation of individuals with schizophrenia is to establish a system of monitoring disease status so interventions can be instituted to prevent disease exacerbation and to promote healthier functioning. Monitoring illness status has primarily focused on indicators of illness. The aim of the present study was to determine what individuals with schizophrenia described as evidence of wellness to generate descriptors for a wellness questionnaire. Twenty participants diagnosed with schizophrenia and living in a community setting were interviewed about their descriptors of wellness. Coding of transcriptions of the interviews yielded two major categories of wellness: Statements related to the reduction or absence of troublesome (illness) indicators and statements related to the presence of helpful or positive (wellness) indicators. Three subcategories, thoughts, feelings, and actions or behaviors emerged under both major categories. The majority of wellness descriptions focused on the presence of wellness and were actions or behaviors.

RESUMO

Suicide is a significant public problem, directly or indirectly affecting the lives of many Americans each year. There have been many different explanations for suicide over the centuries. Many who commit suicide suffer from medically identifiable depression. Clinicians must be alert for subtle self-destructive behaviors that reflect depression. Currently, in the United States, teenagers are increasingly at risk for suicide. Parents and teachers are in key positions to promote healthy living environments for this age group, recognizing suicidal behaviors and taking them seriously. This is the first step toward intervention.

RESUMO

This paper reviews the problem of substance abuse during adolescence. By high school graduation, drugs and alcohol have become a part of the life-style of American youths. Although substance use may not cause problems for the majority, when there is abuse, it has a major impact on the adolescent's family as well as the adolescent himself. Nurses are in prime positions to identify the teenage substance abuser and provide treatment for him and his family.
