RESUMO

OBJECTIVE: Oral treatment with inositol hexaphosphate (InsP6) has shown to be efficient in decreasing adverse effects in patients with breast cancer under chemotherapy. This study was aimed at evaluating and comparing the efficacy of topical InsP6 in improving quality of life in women treated with anticancer drugs. PATIENTS AND METHODS: The study was a double-blind, randomized controlled trial (RCT) with allocation concealment of 20 patients in two groups, one (experimental) applied 4% topical formulation of InsP6 once a day, whereas the second one (control) a gel containing hyaluronic acid. InsP6 therapy started 6 weeks after lumpectomy. Blood tests were monitored in both groups and quality of life was assessed using standardized QLQ-C30 and QLQ-BR23. RESULTS: Patients who applied InsP6 on the breast significantly improved their quality of life and functional status reducing side effects compared to control group; moreover, after treatment, a significant difference between the two groups was observed in the white blood cells and platelets count values. CONCLUSIONS: Topical InsP6 treatment has demonstrated to be effective and safe in preventing and/or mitigating chemotherapy-induced side effects as well as the preserving quality of life in women with ductal breast cancer.

AIM: The aim of this study is to compare initiation and duration rates of breastfeeding in Molise region (Italy) to those targeted from World Health Organization (WHO), and to examine factors associated with infant health. METHODS: Trained caregivers administered interviews to 430 mothers whose infants were receiving the 2nd dose of compulsory immunisation in 4 primary health care centers (ASL) of Molise region, during October-December 2001. Type of breastfeeding was classified as predominant or partial, according to the WHO definitions. RESULTS: The rate of breastfeeding initiation was 92%, dropping to 30% when infants were 6 months of age. We found strong differences among the 4 health care centers involved in the study. Lacking of support after discharge, non-breastfed previous child, receiving infant formulas, breastfeeding on schedule, rooming-in not effected at hospital, both maternal and familiar smoking, were significantly associated with early breastfeeding cessation. The rate of iodine supplemented salt consuming was 40%; the rate of mothers who knew the importance of using folic acid in preconceptional age was 19%; only 1/3 of infants (34%) was placed back to sleep. CONCLUSIONS: Although we meet WHO goals regarding breastfeeding initiation, we don't about breastfeeding duration, yet. Moreover, the leading factors negatively linked to infant health, are not widely recognized. It is therefore necessary to promote the whole infant health, either supporting predominantly breastfeeding, especially in some ASL, or providing better knowledge about risk factors.

RESUMO

Lyme disease involves numerous anatomic areas and systems and is caused by a spirochete, Borrelia burgdorferi. In this article, we describe one case of monolateral chorioretinitis with multiple foci in a 22-year-old myopic man. The diagnosis of this disease was confirmed by positive specific anti-B. burgdorferi antibodies titrated with an enzyme-linked immunosorbent assay method and by serum-conversion occurring approximately six weeks after the end of therapy. Considering the potential effect of this disease on the world's population, we wish to draw attention to its occurrence because it frequently involves ocular structures. Prompt and adequate therapy guarantees complete healing and avoids both short- and long-term complications.

After describing the main clinical features of primary empty sella syndrome and the main hormonal alterations in chronic renal failure, the authors report the results obtained in a group of 33 patients undergoing dialysis three times a week. Each patient had a complete ophthalmologic examination, with electroretinography and visual-evoked potentials, a complete radiologic examination, including skull stratigraphy (particularly of the sellar and parasellar bony structures), and a computed tomographic scan. In the whole group, visual acuity, ocular and lid motility, and visual fields were normal. In two patients, a slight bilateral papilledema, associated with a P100 latency increase and normal morphology and visual-evoked potentials, was reported. Two patients had initial unilateral papillary pallor associated with P100 amplitude alterations. In all four of these patients, radiologic examination showed a markedly pathologic sellar widening with thickening and double contour image of the floor. The report of a primary empty sella was confirmed by computed tomography in all four cases. The authors discuss the possible etiopathogenic mechanisms of the neuroophthalmologic symptoms associated with this syndrome.
