A Bulgarian team, within the frames of a multinational study on diabetic vascular disease, sponsored by WHO, studied a group of 473 diabetic patients by the inquiry method and by the determination of blood cholesterin and creatinine and urine albumin. The results were compared with those of the whole group (6695 patients from 14 countries) and with each national group separately. A higher percentage of insulin-treated patients was established in the Bulgarian group as compared with the whole group, fewer hypertonics, lower mean cholesterol values, a little higher average body weight and considerably lower percentages of smokers. The various forms of macroangiopathy among the Bulgarian group proved to be in percentages, approaching the average percentages for the whole group or little lower than them (e.g. for the category "probable infarctions" and "cerebral hemorrhages"). Only for the category "IHD" the affection percentage among the Bulgarian group significantly surpassed that of the whole group. Since the Bulgarian group, as regards the possible risk factors, is in a more favourable condition, the higher IHD percentage is admitted to be due to the circumstances that patients with graver states have been included in the group. That is suggested by the higher percentage of patients (52%), insulin-treated. The severity of diabetes is concluded to be a significant factor in the development of macroangiopathy.

The therapeutic effect of pentoxyphilin substance--Trental, was followed up in 102 diabetics (35 females and 67 males) aged from 34 to 79 via a broad complex of clinical and paraclinical methods. Favourable results were found in 90.4 per cent of the treated; excellent effect--in 42.3 per cent of the treated; very good--in 22.4 per cent and good--in 28.3 per cent; only 9,6 per cent of the total number with a short-term treatment (average of 27 days) failed to respond with a sufficient effect. Pentoxyphilin is very well tolerated by the patients, not deteriorating the carbohydrate metabolism; has a high effect coefficient upon the diabetic macroangiopathy; that concerns all stages of arterial lesions in the lower limbs. As regards microangiopathy our experience is still poor, thus making definite conclusions is rather risky, in spite of the encouraging results so far. The preparation is worthy of being widely applied in the clinical practice both in the treatment and prophylaxis of diabetic macroangiopathy.

Forty seven patients and 26 healthy subjects controls were followed up in the course of 8 years. Twenty fine from the diabetic patients were without vaso-active medication and 22--treated with various vaso-active drugs (Hydrosarpan 711, Eleparon, Radecol, Padutin, Trental, etc.). The clinical approach was made use of for the periodical controls as well as non invasive methods with 60 channel ECG "Hellige" apparatus, electronic device "Dopton" oscillograph "Gesenius--Keller", double rheograph "Shulfrid", tastotherm P-60 "Braun", contact skin thermography according to Trikoir. A complex of functional tests was also applied. A statistically significant decrease (P < 0,001) ws established to develop in 83,9 per cent of the diabetic patients without vaso-active medication versus 47.6 per cent of those with medication and 11.5 per cent in the controls. The lesions of the lower limbs get significantly ahead (P < 0.05) of the coronary lesions both in frequency and evolution. The existing so far pessimism about the therapeutic effect of diabetic macroangiopathy is admitted not to be fully reasonable.

The effect of pentoxiphyline substance--Trental, production of Höchst, was studied in 37 diabetics (12 females and 25 males) aged from 36 to 78. Besides according to clinical manifestations, the therapeutic effect was objectivized in dynamics through the oscillography "Gesenius-Keller", double-rheography "Schufrid", skin thermometer--Tastotherm P 60 "Braun" and 6-canal ECG apparatus "Hellige". Favourable results, evaluated according to all mentioned methods were found in 89.2 per cent of the treated 37 patients; excellent effect in 15 of them 40.5%); very good--in 8 (21.6%); good--in 10 (27.1%) 1 and without effect--only 4 patients (10.8%). Trental is concluded to deserve a wide application in the clinical practice for the peripheral diabetic angiopathy treatment in patients with diabetes mellitus.

RESUMO

On the base of three observations, derived from a series of systematically followed up diabetics, the authors describe certain rare findings and peculiarities of the diabetic osteoarthropathy, being of diagnostic interest. The parallel X-ray, scintigraphic, functional and thermography investigations are of special significance, wherefrom an enhanced fixation of radionucleids and local hyperthermia in zones, coinciding with the roentgenologically established bone lesions, were found. The importance of the complex dynamic follow up of the osteo-articular changes in the late degenerative syndrome of diabetes is stressed upon.

RESUMO

The therapeutic effect of swine insulin preparation Lente, "Pharmachim" production was studied in 50 diabetics. Its hypoglycemizing activitis compared with that of standardized insulin Lente--"Novo", extract of bovine pancreas. The Bulgarian preparation was found to be very similar to the preparation Lente-Novo in its pharma-codynamic properties and could successfully be used in the diabetes melitus treatment. Insulin Lente Pharmachim is presumed to have lower antigenicity as an extract of swine pancreas, which is actually its great advantage for the therapeutic practice.

The investigation is realized with 50 diabetics, considered suitable for oral therapy that revealed excellent or satisfactory effect from the Glybenclamide treatment (GCL), applied separately or in combination with butyl biguanide. GCL was established to give far better results than the rest oral antidabetic drugs, permitting a better metabolic syndrome compensation. GCL effect is more manifested than that of tolbutamide. The better therapeutic effect of GCL is obvious as compared with chloropropamide and butyl biguanide, but not so well manifested as with tolbutamide. The therapeutic GCL effect is better in almost all followed up ages, the most advanced incld., which is not cogent for the rest of the oral preparations. Side effects and severe hypoglycemic incidence, reported by other authors, were not observed during the investigation.
