RESUMO

The aim of the current study was to examine the mental well-being of healthcare personnel (HCP) working in COVID-19 units in Greece and to calculate the prevalence of burnout (BO) amongst them. A questionnaire based on the Maslach Burnout Inventory for Medical Personnel was utilized between February 21st, 2021 and March 5th, 2021. A total of 190 HCP responded to the questionnaire, of which 73.7% were nurses and midwives. The mean age of the participants was 38.3 (8.4) years. Overall, 71.6% of the participants had a high BO score, while 20.5% had a moderate and 7.9% had a low BO score. Night shifts in COVID-19 wards and job dissatisfaction were significantly associated with a high BO score (P = .03 and P < .0001, respectively). The majority of HCP working in COVID-19 wards in Greece is experiencing high levels of overall BO and emotional exhaustion.

Hemiplegic patients are prone to bone loss and alterations in fat and lean mass, which ultimately affect their rehabilitation status and propensity in bone fractures. The present study aimed to evaluate body composition and bone mineral density (BMD) in stroke patients within 1st year post-stroke. Fifty-eight hemiplegic patients (36 men and 22 women) were enrolled in this prospective study. Dual-energy X-ray absorptiometry was used to assess total-body and lower-extremity BMDs (g/cm(2)), lean mass (g), and fat mass (g) after 3, 6 and 12 mo of stroke that led to hemiplegia. The Modified Ashworth Scale and the functional ambulation category were used to evaluate spasticity and ambulatory category of patients, respectively. Both sexes exhibited total-body and paretic lower-limb BMD loss, fat mass gain, and lean mass waste during the 1st 12 mo poststroke, and in most cases, statistically significant differences were found between 3 and 6 mo; however, the pattern of changes was different between males and females. Therefore, it is suggested that disability because of hemiplegia led to alterations in muscle function, which triggered skeletal and body composition changes and rendered these patients particularly prone to increased fracture risk.

RESUMO

Central venous catheterization is widely used for hemodialysis for patients with renal failure. A number of short and long-term complications have been reported in literature. We describe a rare case of a lethal iatrogenic injury of the brachiocephalic artery (BA) in a 65-year-old woman following subclavian venous catheterization with a Hickman catheter. Contrast-enhanced CT-scan and reconstruction images demonstrated a pseudoaneurysm of the BA. Due to clinical deterioration, the patient underwent urgent sternotomy. Aspects concerning prevention and diagnosis of the above complication are analyzed.

To evaluate the loss of trabecular and cortical bone mineral density (BMD) and geometric parameters of bone strength expressed by stress-strain index (SSI) in the proximal and distal forearm and the alterations of BMD in the hip of hemiplegic patient and 12 mo after stroke. Sixty-seven hemiplegic patients (43 men and 24 women) with a history of single completed strokes associated with unilateral weakness were enrolled in this prospective study. All patients underwent bone densitometry measurements at 3, 6, and 12 mo after the initial episode of stroke. Both paretic and normal forearms were examined by peripheral quantitative computed tomography (pQCT) at the 4% and 20% sites of the forearm length and both hips were examined by dual-energy X-ray absorptiometry (DXA) including the area of femoral neck and greater trochanter. The diagnosis of stroke was confirmed by cranial computed tomography. Motor function was assessed by the functional ambulation category (FAC) and spasticity by the modified Ashworth scale (MAS). We found statistically significant trabecular and cortical bone density reductions during the course of our study in the forearm, which was more profound on the paretic side. Trabecular bone loss and SSI measured at 4% of the paretic forearm in the male group represented a 12-mo decrease of 14.01% and 28.61%, respectively, and in the female group 9.29% and 19.17%, respectively. Cortical bone and SSI measured at the 20% site of paretic forearm in the male group corresponded to a 12-mo decrease of 4.02% and 7.43%, respectively, and in the female group 2.59% and 6.97%, respectively. Paretic femoral neck and trochanter measurements in males showed a reduction of 11.76% and 10.38%, respectively, and in females 13.04% and 12.6%, respectively. A significant loss of BMD and bone strength was found during the first year after stroke in both trabecular and cortical bone at the forearm and at the neck and great trochanter on the paretic hip. Most prominent BMD reduction was evident in men compared with perimenopausal women in the same age.

To investigate alterations in the body composition of paraplegic men, 31 complete paraplegic men thoracic (T)4-T 12 neurological level of injury, 16 with paraplegia above (high), and 15 below (low) thoracic 7, were compared with 33 able-bodied men. Whole body dual X-ray absorptiometry was used to estimate regional (arms, legs) and total body bone mineral density (g/cm(2)), lean, and fat mass (g).The influence of the neurological level of injury and the duration of paralysis in relation with the above parameters were also investigated. Body mass index, bone mineral density, and lean mass were significantly decreased (p < 0.0005) and fat mass was increased (p < 0.05) in the legs and total body composition in paraplegics. Bone mineral density was significantly lower in high paraplegics' arms compared with low paraplegics (p = 0.028). The correlation of body mass index with fat mass was statistically significant in all paraplegics and controls (r = 0.57, p = 0.001 and r = 0.73, p = 0.0001, respectively) and in low paraplegics (r = 0.72, p = 0.004). Legs' bone mineral density and arms' fat mass were correlated with the duration of paralysis in all paraplegics (r = -0.46, p = 0.009 and r = 0.43, p = 0.020, respectively) and in high paraplegics (r = 0.73, p = 0.001 and r = 0.55, p = 0.042, respectively). Total fat mass was correlated with the duration of paralysis in high paraplegics (r = 0.5, p = 0.05). These results suggest body composition changes in paraplegics.

RESUMO

Splenic artery aneurysms represent approximately 60% of visceral arterial aneurysms. Their incidence at autopsy is 0.02 - 2 % in all age groups and rises to 10.4% above the age of 60. Although splenic artery aneurysms are the most common visceral aneurysms, giant splenic artery aneurysms >10 cm in diameter have rarely been reported. A recent review of the literature came up with 12 true giant splenic artery aneurysms >10 cm. A case of 12-cm splenic artery aneurysms is presented in this study and the imaging findings are described.

RESUMO

Two cases of allergic angina and allergic myocardial infarction (Kounis syndrome) following penicillin administration are described. The patients suffered from lung and mandible neoplasms and had previously received several courses of antineoplastic therapy without any sequelae. One patient had normal coronary arteries (type I variant of the syndrome) and the other had coronary artery disease with previous myocardial infarction (type II variant of the syndrome). The allergic reaction following penicillin administration seemed to have triggered the development of an acute coronary artery spasm in the first patient and an acute myocardial infarction in the second. This report shows that susceptible individuals expressing a magnified mast cell degranulation effect may be more vulnerable to coronary artery spasm and plaque erosion or rupture.

RESUMO

This is the first case reported of combined cervical emphysema and pneumomediastinum associated with unusual electrocardiographic and local neurologic findings. These may be the result of an increase in intracervical and intrathoracic pressure induced by the dissecting air.
