RESUMO

OBJECTIVES: Uterine artery Doppler flow studies during the 11th and 14th week of pregnancy are important in the prediction of preeclampsia and intrauterine growth restriction in pregnant women as well as in the prevention thereof. METHODS: Our study on Doppler flow indices of the uterine arteries involved 168 patients examined in our clinic, with pregnancies ranging from 11 weeks + 0 days to 13 weeks + 6 days. RESULTS: There were 72 patients from 11 weeks + 0 days to 11 weeks + 6 days (42.86%), 43 from 12 weeks + 0 days to 12 weeks + 6 days (25.60%), and 53 from 13 weeks + 0 days to 13 weeks + 6 days (31.55%). The mean values of the Doppler indices were PI 1.75±0.79, 1.88± 0.81, 1.71±0.81, and 1.58±0.72 and RI 0.72±0.14, 0.75±0.14, 0.71±0.14, and 0.70±0.14 for the entire group and for the three intervals, respectively. There were 71 (42.26%), 33 (19.64%, with 18 cases or 54.55% on the right side), and 64 (38.10%) patients with bilateral, unilateral and absent uterine artery notching, respectively. The mean Doppler indices for the three aforementioned groups were 2.18±0.79, 1.63±0.72, and 1.33±0.57 for the PI, and 0.79±0.11, 0.71±0.14, and 0.66±0.14 for the RI, respectively. The indices for the 175 arteries with and 161 without notching, taken separately, in all patients, as well as for the uterine arteries with and without notching in patients with unilateral notching only were 2.16±0.76, 1.30±0.54, 2.08±0.66, and 1.17±0.43 for the PI, and 0.79±0.11, 0.65±0.14, 0.79±0.11, and 0.63±0.12 for the RI, respectively. CONCLUSIONS: The mean uterine artery PI and RI decrease from 11 weeks + 0 days-11 weeks + 6 days to 13 weeks + 0 days-13 weeks + 6 days. They also decrease from patients with bilateral uterine artery notching to those without notching. The frequency of uterine artery notching decreases with increasing gestational age. Our results are similar to those in literature.

We present the case of a 71-year-old right-handed male, admitted to the Department of Neurology, Emergency County Hospital of Arad, Romania, on November 2015, with a rare case of Klüver-Bucy syndrome (KBS), following an ischemic stroke of the right temporal lobe, which was previously diagnosed in December 2014 and was treated accordingly. At the moment of second hospital admission, the patient was found somnolent and confused at home, with traumatic signs of biting of the tongue and urine emission. A couple days after admission, our patient became alert and presented hypersexuality, hypermetamorphosis, increased oral tendency, behavior changes including apathy with loss of anger and fear, and a very increased appetite, transient visual agnosia and right-left disorientation. In the initial phase, the patient could not recognize any members of his family, but he had a tendency to touch everything within his reach and place it into his mouth. The KBS presented in this case, following an ischemic stroke of the right temporal lobe provides distinct and intriguing insights into the possible pathophysiology of this syndrome. Often disruption of consciousness during recovery period may hide the clinical manifestation of the syndrome.

RESUMO

We present the case of a 34-year-old female with cervical cancer and right ureterohydronephrosis. She was admitted to the Urology Clinic of the Emergency County Hospital of Arad, Romania, on November 11, 2015, after previously being diagnosed with stage IIIA cervical cancer in December 2014 and undergoing radio and chemotherapy and laparotomy for staging. At the moment of hospital admission, the patient suffered from fever and right lumbar pain, she was also diagnosed with grade IV right ureterohydronephrosis. The patient was operated at the Urology Clinic of Arad; the intervention involved a total hysterectomy with bilateral adnexectomy and a right obturatory necrectomy of the necrotic lymph nodes. However, the evolution was favorable. The purpose was to increase the patient's survival rate and increase the quality of her life by also applying one palliative method including a urethral stent. As a result, the patient's life was extended.

The dicopper(II) complex [Cu(2)(L)](4+) (L = alpha,alpha'-bis[bis[2-(1'-methyl-2'-benzimidazolyl)ethyl]amino]-m-xylene) reacts with hydrogen peroxide to give the dicopper(II)-hydroquinone complex in which the xylyl ring of the ligand has undergone a double hydroxylation reaction at ring positions 2 and 5. The dihydroxylated ligand 2,6-bis([bis[2-(3-methyl-1H-benzimidazol-2-yl)ethyl]amino]methyl)benzene-1,4-diol was isolated by decomposition of the product complex. The incorporation of two oxygen atoms from H(2)O(2) into the ligand was confirmed by isotope labeling studies using H(2)(18)O(2). The pathway of the unusual double hydroxylation was investigated by preparing the two isomeric phenolic derivatives of L, namely 3,5-bis([bis[2-(1-methyl-1H-benzimidazol-2-yl)ethyl]amino]methyl)phenol (6) and 2,6-bis([bis[2-(1-methyl-1H-benzimidazol-2-yl)ethyl]amino]methyl)phenol (7), carrying the hydroxyl group in one of the two positions where L is hydroxylated. The dicopper(II) complexes prepared with the new ligands 6 and 7 and containing bridging micro-phenoxo moieties are inactive in the hydroxylation. Though, the dicopper(II) complex 3 derived from 6 and containing a protonated phenol is rapidly hydroxylated by H(2)O(2) and represents the first product formed in the hydroxylation of [Cu(2)(L)](4+). Kinetic studies performed on the reactions of [Cu(2)(L)](4+) and 3 with H(2)O(2) show that the second hydroxylation is faster than the first one at room temperature (0.13 +/- 0.05 s(-1) vs 5.0(+/-0.1) x 10(-3) s(-1)) and both are intramolecular processes. However, the two reactions exhibit different activation parameters (Delta H++ = 39.1 +/- 0.9 kJ mol(-1) and Delta S++ = -115.7 +/- 2.4 J K(-1) mol(-1) for the first hydroxylation; Delta H++ = 77.8 +/- 1.6 kJ mol(-1) and Delta S++ = -14.0 +/- 0.4 J K(-1) mol(-1) for the second hydroxylation). By studying the reaction between [Cu(2)(L)](4+) and H(2)O(2) at low temperature, we were able to characterize the intermediate eta(1):eta(1)-hydroperoxodicopper(II) adduct active in the first hydroxylation step, [Cu(2)(L)(OOH)](3+) [lambda(max) = 342 (epsilon 12,000), 444 (epsilon 1200), and 610 nm (epsilon 800 M(-1)cm(-1)); broad EPR signal in frozen solution indicative of magnetically coupled Cu(II) centers].
