RESUMO

A total of 58 patients with essential hypertension were examined. 27 of them had a combination of essential hypertension and chronic obstructive pulmonary disease (COPD), 31 persons had isolated essential hypertension. The control group included 13 healthy individuals. The daily monitoring of blood pressure with a BPLab apparatus and the analysis of peripheral and central aortic pressure were performed. Comparison of groups was carried out using the Mann-Whitney test. The combination of hypertension and COPD was associated with increased daily and average daily peripheral and central aortic pressure compared with isolated essential hypertension and the control group. During daily monitoring of peripheral blood pressure the "dipper" type predominated in patients with isolated essential hypertension (41, 95%) and "non-dipper" type in those with comorbid disorders (44, 44%). The analysis of nocturnal blood pressure reduction in the aorta in patients with essential hypertension and COPD revealed the increased frequency of "non-dipper" type (55, 55%). The analysis of the central aortic pressure in patients with isolated essential hypertension showed the decreased incidence of "dipper" type (21, 87%). It is concluded that daily monitoring of the central aortic pressure permits to identify abnormal profiles with a higher frequency than the study of peripheral blood pressure.

RESUMO

A hundred and thirty-four hypertensive patients were examined, 75 of them had concomitant first-degree chronic obstructive pulmonary disease (COPD). As compared with the control group, the patients were found to have elevated levels of primary, secondary, and intermediate products of serum lipid peroxidation and lower activities of catalase of red blood cells and their peroxide resistance. All changes were most pronounced in concomitant COPD.

In order to reveal peculiarities of function of cardiac ventricles in functional class (FC) II-III chronic heart failure (CHF) Doppler echocardiography (DE) and myocardial tissue Doppler echocardiography (TDE) were carried out in 108 middle aged, elderly and old patients. Patients with signs of FCIII CHF had pronounced impairment of global contractile function of left and right ventricles (LV and RV) with RV ejection fraction (EF) higher than LV EF. These patients had lower parameters of central hemodynamics, high parameters of pulmonary hypertension, increased thickness of RV free wall, greater percentage of irreversible LV and RV myocardium in response to deep breath, more pronounced derangements of RV and LV diastolic filling. In patients with FC II of CHF and moderate RV dysfunction its pump function is determined by degree of impairment of systolic function, diastolic filling, while in patients with FC III of CHF and pronounced dysfunction of RV myocardium its pump function is to a lesser degree determined by ventricular contractile function, but becomes dependent on diastolic filling of RV and the state of LV.

RESUMO

The results of investigation of blood serum interleikins in hypertensive patients in combination with early stages of chronic obstructive pulmonary disease (COPD) are presented. The increase of interleikin 1beta, tumor necrosis factor-alpha in hypertensive patients of the I - II stages in comparison with healthy person was marked. In the presence of accompanied COPD of the I stage more increased number of cytokins was determined. Changes of interleukin-4 patients of both groups were not found.

RESUMO

Cytomegalovirus (CMV) infection is widely spread among population. While immunocompetent patients suffer rarely from this virus, it can lead to a lethal outcome in immunocompromised patients. An electron microscopic study has detected fibroblastic morphological changes of a definite cytodestructive character. The nuclei of some fibroblasts have chromatine condensation. A clear zone arising due to vacuolization near this inclusion may reflect nuclear rearrangement leading to further CMV metamorphosis of the cell. This metamorphosis is characteristic of the changes developing in the cells of different parenchymatous organs.

RESUMO

Eighty one patients with various biliary diseases and 36 apparently healthy individuals were examined. There were relatively elevated lipid peroxidation (LPO) products (E 232/220 and E 278/220) in biliary diskynesia and higher concentrations of LPO intermediate products in cholecystitis. In cholelithiasis, the levels of LPO products were lower than those in cholecystitis. E 232/220 and 278/220 may be used for early diagnosis of biliary dysfunctions. Selenium deficiency observed in the Transbaikal region decreases antiradical protection and increases the incidence of biliary diseases.

RESUMO

Changes in the phase structure of the systole and diastole of both ventricles of the heart with relation to the presence or absence of cardiac failure were studied in 68 patients with myocardial infarction. Four variants of such changes were defined. It was established that the right ventricle could be a factor of the compensation of cardiac activity in myocardial infarction. Change in the phase structure could be of prognostic value with regard to the development of cardiac failure. The appearance of the right ventricular high diastolic pressure syndrome against a background of a decrease in left ventricular contractility was shown to be an early sign of congestion in the pulmonary circulation and a prognostic criterion of the appearance of clinically marked left ventricular insufficiency in the nearest future requiring adequate therapy. Marked asynchronism in ventricular activity contributes to the development of cardiac failure in myocardial infarction.
