RESUMO

The intrinsically adaptive or maladaptive nature of certain activity patterns in fibromyalgia (FM) has been put into question. The role of contextual factors related to their influence on functional limitation is required. Perfectionism complicates the ability to cope of these patients. The aim of the study has been to analyze the moderating role of perfectionism between activity patterns and functional limitation. The sample were 228 women with FM. Moderation analyses were conducted with the PROCESS Macro. Activity avoidance and excessive persistence were associated with poorer functionality, regardless of perfectionism. Pain avoidance and task persistence were more strongly associated with FM impact in women with high or moderate levels of perfectionism. In a clinical setting adapting the recommendations given to patients according to their level of perfectionism would be justified. Pain avoidance might be inadvisable at high levels of perfectionism, and task persistence is recommendable especially when perfectionism is high.

Este trabajo identifica los factores psicosociales, clínicos y de estado de salud percibido que predicen la utilización de recursos sanitarios en la fibromialgia (FM), considerando el nivel asistencial en el que son atendidos los pacientes. Participaron 315 mujeres con FM de distintos centros de Atención Primaria (AP: n= 101) y Servicios de Reumatología (SR: n= 214) de la provincia de Alicante. El análisis de regresión «stepwise» mostró a las diferentes variables de autoeficacia como los mejores predictores del consumo de medicación en AP, y, junto con el afrontamiento centrado en la enfermedad, de la utilización de servicios sanitarios. En los SR, el estado de salud percibido fue el mejor predictor del consumo de medicación. En ambos niveles la intensidad del dolor no fue un predictor significativo del uso de recursos sanitarios. Estos datos ofrecen nuevas perspectivas para la reflexión sobre las estrategias de intervención en estos pacientes en cada nivel asistencial(AU)


This work identifies which psychosocial, clinical and perceived health status factors predict health-resource use in Fibromyalgia (FM), considering the patients’ assistance level. Participants were 315 FM patients from Primary Care (PC: n= 101) and Reumatology Services (RS: n= 214). Stepwise regression analysis showed that self-efficacy variables were significant predictors of medication intake in PC, and, along with Illness-focused coping, of the health service use. In RS, perceived health status was the best predictor of medication intake. In any case, Pain intensity significantly predicted the use of health resources. These data offer a new perspective for intervention in these patients, taking into account the level of assistance (AU)
