ABSTRACT Chronic kidney disease (CKD) represents one of today's main public health problems. Serum creatinine measurement and estimation of the glomerular filtration rate (GFR) are the main tools for evaluating renal function. There are several equations to estimate GFR, and CKD-EPI equation (Chronic Kidney Disease - Epidemiology) is the most recommended one. There are still some controversies regarding serum creatinine measurement and GFR estimation, since several factors can interfere in this process. An important recent change was the removal of the correction for race from the equations for estimating GFR, which overestimated kidney function, and consequently delayed the implementation of treatments such as dialysis and kidney transplantation. In this consensus document from the Brazilian Societies of Nephrology and Clinical Pathology and Laboratory Medicine, the main concepts related to the assessment of renal function are reviewed, as well as possible existing controversies and recommendations for estimating GFR in clinical practice.


RESUMO A doença renal crônica (DRC) representa um dos principais problemas de saúde pública da atualidade. A dosagem da creatinina sérica e a estimativa da taxa de filtração glomerular (TFG) são as principais ferramentas para avaliação da função renal. Para a estimativa da TFG, existem diversas equações, sendo a mais recomendada a CKD-EPI (Chronic Kidney Disease - Epidemiology). Existem ainda algumas controvérsias com relação à dosagem da creatinina sérica e da estimativa da TFG, uma vez que vários fatores podem interferir nesse processo. Uma importante mudança recente foi a retirada da correção por raça das equações para estimativa da TFG, que superestimavam a função renal, e consequentemente retardavam a implementação de tratamentos como diálise e transplante renal. Neste documento de consenso da Sociedade Brasileira de Nefrologia e Sociedade Brasileira de Patologia Clínica e Medicina Laboratorial são revisados os principais conceitos relacionados à avaliação da função renal, possíveis controvérsias existentes e recomendações para a estimativa da TFG na prática clínica.

The search for prognostic markers in breast cancer has bumped into a typical feature of these tumors, intra and intertumoral heterogeneity. Changes in the expression profile, localization of these proteins or shedding to the surrounding stroma can be useful in the search for new markers. In this context, classification by molecular subtypes can bring perspectives for both diagnosis and screening for appropriate treatments. However, the Triple Negative (TN) subtype, which is already the one with the worst prognosis, lacks appropriate and consistent molecular markers. In this work, we analyzed 346 human breast cancer samples in tissue microarrays (TMA) from cases diagnosed with invasive breast carcinoma to assess the expression and localization pattern of Maspin and their correlation with clinical parameters. To complement our findings, we also used TCGA data to analyze the mRNA levels of these respective genes. Our data suggests that the TN subtype demonstrates a higher level of cytoplasmic Maspin compared to the other subtypes. Maspin transcript levels follow the same trend. However, TN patients with lower Maspin expression tend to have worse overall survival and free-survival metastasis rates. Finally, we used Maspin expression data to verify possible relationships with the clinicopathological information of our cohort. Our univariate analyses indicate that Maspin is related to the expression of estrogen receptor (ER) and progesterone receptor (PR). Furthermore, Maspin expression levels also showed correlation with Scarff-Bloom-Richardson (SBR) parameter, and stromal Maspin showed a relationship with lymph node involvement. Our data is not consistently robust enough to categorize Maspin as a prognostic marker. However, it does indicate a change in the expression profile within the TN subtype.

A deeper understanding of gas emissions in milk production is crucial for promoting productive efficiency, sustainable resource use, and animal welfare. This paper aims to analyze ammonia and greenhouse gas emissions in dairy farming using bibliometric methods. A total of 187 English-language articles with experimental data from the Scopus and Web of Science databases (January 1987 to April 2024) were reviewed. Publications notably increased from 1997, with the highest number of papers published in 2022. Research mainly focuses on ammonia and methane emissions, including quantification, volatilization, and mitigation strategies. Other gases like carbon dioxide, nitrous oxide, and hydrogen sulfide were also studied. Key institutions include the University of California-Davis and Aarhus University. Bibliometric analysis revealed research evolution, identifying trends, gaps, and future research opportunities. This bibliometric analysis offers insights into emissions, air quality, sustainability, and animal welfare in dairy farming, highlighting areas for innovative mitigation strategies to enhance production sustainability. This research contributes to academia, enhancing agricultural practices, and informing environmental policies. It is possible to conclude that this research is a valuable tool for understanding the evolution of research on gas emissions in dairy cattle facilities, providing guidance for future studies and interventions to promote more sustainable production.

The advancement of technology has significantly transformed the livestock landscape, particularly in the management of dairy cattle, through the incorporation of digital and precision approaches. This study presents a bibliometric analysis focused on these technologies involving dairy farming to explore and map the extent of research in the scientific literature. Through this review, it was possible to investigate academic production related to digital and precision livestock farming and identify emerging patterns, main research themes, and author collaborations. To carry out this investigation in the literature, the entire timeline was considered, finding works from 2008 to November 2023 in the scientific databases Scopus and Web of Science. Next, the Bibliometrix (version 4.1.3) package in R (version 4.3.1) and its Biblioshiny software extension (version 4.1.3) were used as a graphical interface, in addition to the VOSviewer (version 1.6.19) software, focusing on filtering and creating graphs and thematic maps to analyze the temporal evolution of 198 works identified and classified for this research. The results indicate that the main journals of interest for publications with identified affiliations are "Computers and Electronics in Agriculture" and "Journal of Dairy Science". It has been observed that the authors focus on emerging technologies such as machine learning, deep learning, and computer vision for behavioral monitoring, dairy cattle identification, and management of thermal stress in these animals. These technologies are crucial for making decisions that enhance health and efficiency in milk production, contributing to more sustainable practices. This work highlights the evolution of precision livestock farming and introduces the concept of digital livestock farming, demonstrating how the adoption of advanced digital tools can transform dairy herd management. Digital livestock farming not only boosts productivity but also redefines cattle management through technological innovations, emphasizing the significant impact of these trends on the sustainability and efficiency of dairy production.

Background: The Direct Assessment of Functional Status (DAFS) is the only instrument validated in Brazil that assesses functionality directly with the patient. However, this clinical tool takes a long time to be administered. This limits its use in hospitals and outpatient clinics that require brief assessment instruments. Additionally, we need to count with a direct assessment because the number of older adults living alone is increasing and we thus lack reliable informants. Objective: This study aimed to present the development and content validity evidence of a direct complex functionality test for older adults, the Brief Instrument for Direct Functionality Assessment (BIDFA). Method: A total sample of 30 older adults and eight expert judges took part in the study stages. The BIDFA construction stages were: (1) literature review of functionality instruments; (2) development of seven ecological tasks to evaluate the performance of daily complex activities with the older adults; (3) content analysis by eight expert judges; (4) pilot study with 30 older adults; (5) the ecological analysis of items; (6) focus group analysis; and (7) final version of the BIDFA. Results: The BIDFA had evidence of content validity with an agreement index of 96.5%. The final version of BIDFA was left with six domains of complex functionality divided into semantic memory and time orientation; shopping skills; executive attention, math and finance skills; organization; planning and procedural memory; and problem-solving. The complex functionality score by BIDFA ranges from 0 to 100 points. Conclusion: The BIDFA was found to have good content validity by the expert judges and by the ecological analysis of the items by the older adults. The new instrument is expected to help assess the functional status of older adults, in an abbreviated context including complex functionality demands, with a wider range of total and subdomain scores.

Coinfection of HPgV-1 with hepatitis C virus (HCV) is common due to shared modes of transmission, with a prevalence of HPgV-1 viremia of approximately 20% among individuals with chronic HCV infection. The aim of the present study was to estimate the prevalence of HPgV-1 RNA and circulating genotypes in patients with hepatitis C from a health service located in the city of Belém, in the state of Pará, Northern Brazil. A total of 147 samples were included in the study from February to December 2019. Among the participants, 72.1% (106/147) were monoinfected with HCV, with detectable HCV viral RNA, and 27.9% (41/147) were coinfected with HCV/HPgV-1. The most frequently found genotypes were HPgV-1 genotypes 1 and 2 (36.6% and 63.4%), respectively. While for HCV there was a predominance of genotypes 1 and 3 (58.5% and 41.5%). No significant differences were found when comparing any risk, sociodemographic, or clinical factors between groups. Also, there was no statistically significant difference when relating the viral genotypes of both agents. This study indicated that the prevalence of infection by HPgV-1 is high in HCV carriers in Belém, Pará, and probably does not change the clinical course of HCV infection, however, further studies are still needed.

Background: Depressive symptoms have been associated with cognitive impairment after stroke, and women may be specifically affected. Objective: The aim of this study was to investigate gender-specific characteristics in the relationship between changes in depression severity and changes in cognitive performance after stroke. Methods: We prospectively evaluated 73 patients without a previous history of depression in the first and fourth months after a first ischemic stroke. The severity of depressive symptoms was assessed using the 31-item version of the Hamilton Rating Scale for Depression, and executive function, attention, working memory, and verbal fluency were assessed using a neuropsychological battery. Results: We included 46 (63.0%) men and 27 (36.9%) women, with mean ages of 55.2 (SD ± 15.1) and 46.8 (SD ± 14.7) years, respectively. We found significant improvement in the digit span forward and Stroop dots from month 1 to month 4 post stroke for both men and women. Women, but not men, presented a correlation between changes in phonemic verbal fluency and changes in the 31-item version of the Hamilton Rating Scale for Depression scores. Improvement in depression was correlated with improvement in verbal fluency, and worsening in depression was correlated with worsening in verbal fluency. Conclusions: Our results suggest that women might be more vulnerable to the relationship between depressive symptoms and cognitive performance, and improvement of depression may be necessary for women's improvement in phonemic verbal fluency from the first to the fourth month after a stroke. We did not adjust the results for multiple comparisons. Thus, our findings might be considered preliminary, and confirmatory studies, also focusing on specific characteristics of women that could explain these differences, are warranted.

PURPOSE: Invasive ductal breast cancer (IDC) is heterogeneous. Staging and immunohistochemistry (IH) allow for effective therapy but are not yet ideal. Women with Luminal B tumors show an erratic response to treatment. This prospective study with 81 women with breast cancer aims to improve the prognostic stratification of Luminal B patients. METHODS: This is a prospective translational study with 81 women with infiltrating ductal carcinoma, grouped by TNM staging and immunohistochemistry, for survival analysis, and their correlations with the chemokines. Serum measurements of 13 chemokines were performed, including 7 CC chemokines [CCL2(MCP1), CCL3(MIP1α), CCL4(MIP1ß), CCL5(Rantes), CCL11(Eotaxin), CCL17(TARC), CCL20(MIP3α)], 6 CXC chemokines [CXCL1(GroAlpha), CXCL5(ENA78), CCXCL8(IL-8), CXCL9(MIG), CXCL10(IP10), CXCL11(ITAC)]. RESULTS: Overall survival was significantly dependent on tumor staging and subtypes by immunohistochemistry, with a median follow-up time the 32.87 months (3.67-65.63 months). There were age correlations with IP10/CXCL10 chemokines (r = 0.4360; p = 0.0079) and TARC/CCL17 (Spearman + 0.2648; p = 0.0360). An inverse correlation was found between body weight and the chemokines Rantes/CCL5 (r = - 0.3098; p = 0.0169) and Eotaxin/CCL11 (r = - 0.2575; p = 0.0470). Smokers had a higher concentration of MIP3α/CCL20 (Spearman + 0.3344; p = 0.0267). Luminal B subtype patients who expressed lower concentrations of ENA78/CXCL5 (≤ 254.83 pg/ml) (Log-Rank p = 0.016) and higher expression of MIP1ß/CCL4 (> 34.84 pg/ml) (Log-Rank p = 0.014) had a higher risk of metastases. CONCLUSION: Patients with Luminal B breast tumors can be better stratified by serum chemokine expression, suggesting that prognosis is dependent on biomarkers other than TNM and IH.

Objective: Triple negative breast cancer (TNBC) has high relapse rates due to dysregulated inflammatory signaling pathways and significant changes in the tumor microenvironment, probably influencing the failure of several therapies. The Cysteinyl Leukotriene Receptor 1 (CYSLTR1), a leukotriene modulator of inflammation, has been shown to play an important role in cancer pathogenesis and survival but few studies have been reported on its role in breast cancer. Materials and Methods: The present work was conducted using publicly available platforms that have omics data to assess the clinical potential of CYSLTR1 expression and its prognostic validation in large cohorts of samples from breast cancer patients. Web platforms containing clinical information, RNA-seq and protein data were selected to perform in silico analyses of the potential marker CYLSTR1. Added together, the platforms included modules for correlation, expression, prognosis, drug interactions, and construction of gene networks. Results: Kaplan-Meier curves revealed that reduced levels of CYSLTR1 corresponded to an unfavorable outcome for overall survival (p<0.005) as well as relapse-free survival (p<0.001) in the basal subtype. Additionally, CYSLTR1 was downregulated in breast tumor samples compared to adjacent healthy tissue (p<0.01) and the basal subtype exhibited the lowest expression of CYSLTR1 relative to the other subtypes (p<0.0001). Furthermore, gene networking analysis showed strong associations of CYSLTR1 with two protein-coding genes (P2RY10 and XCR1) when tested on a TNBC dataset. Conclusion: Our data highlighted the relevance of CYSLTR1 since it may play an important role in TNBC therapy. However, further in vitro and in vivo studies should be directed towards validating our findings in an effort to improve our understanding of TNBC pathology.

Benznidazole (BZ) tablets are the currently prescribed treatment for Chagas disease. However, BZ presents limited efficacy and a prolonged treatment regimen with dose-dependent side effects. The design and development of new BZ subcutaneous (SC) implants based on the biodegradable poly-ɛ-caprolactone (PCL) is proposed in this study for a controlled release of BZ and to improve patient compliance. The BZ-PCL implants were characterized by X-ray diffraction, differential scanning calorimetry, and scanning electron microscopy, which indicated that BZ remains in its crystalline state dispersed in the polymer matrix with no polymorphic transitions. BZ-PCL implants, even at the highest doses, induce no alteration of the levels of hepatic enzymes in treated animals. BZ release from implants to blood was monitored in plasma during and after treatment in healthy and infected animals. Implants at equivalent oral doses increase the body's exposure to BZ in the first days compared with oral therapy, exhibiting a safe profile and allowing sustained BZ concentrations in plasma to induce a cure of all mice in the experimental model of acute infection by the Y strain of T. cruzi. BZ-PCL implants have the same efficacy as 40 daily oral doses of BZ. Biodegradable BZ implants are a promising option to reduce failures related to poor adherence to treatment, with more comfort for patients, and with sustained BZ plasma concentration in the blood. These results are relevant for optimizing human Chagas disease treatment regimens.

Dirofilariasis is a vector-borne disease frequent in many countries. Not only infected dogs, but also cats and wild canids (including wolves and foxes), represent important sources of infection for mosquitoes, which are the pathogen vectors. The disease is endemic in Mediterranean countries with increasing incidence in Italy, France, Greece and Spain, but limited epidemiological data is available from Portugal regarding its distribution and impact. Aiming to clarify this, canine whole blood samples (n = 244) from the north of Portugal were tested for Dirofilaria spp. antigens by use of a commercial rapid immunomigration test. Polymerase chain reaction (PCR) and the modified Knott test were also used to assess the presence of microfilariae. Results were also compared to assess the performance of each test used. Of the 244 animals tested, 118 (48.4%) were positive for Dirofilaria immitis (heartworm) in the serological adult worm rapid antigen detection test, and 36 (14.8%) had circulating microfilariae, identified as D. immitis. A combined positivity of 51.6% (126/244) was found. Results indicate that the risk of exposure to D. immitis in dogs is high in this region of Portugal, and that prophylaxis against the parasite is advisable to decrease the occurrence of canine infection and disease. The present study highlights the diagnostic value of serological and molecular tests in determining the prevalence of D. immitis.

PURPOSE: The present study aims to evaluate the serum concentrations of 25 hydroxyvitamin D[25(OH)D] in individuals aged ≥80 years, independent, free-living in Sao Paulo, Brazil (Lat 23.5 oS), and to investigate their associations with musculoskeletal system, physical performance and health markers. METHOD: This cross-sectional study included 212 community dwellers aged ≥80 years and evaluated serum 25(OH)D, PTH, calcium, albumin, phosphorus, creatinine, bone markers, and bone mineral density. Physical performance was evaluated with stationary march, Flamingo, and functional reach tests, questionnaires to assess falls and fractures in the previous year, energy expenditure (MET), and Charlson index. Physical activity was evaluated with the International Physical Activity Questionnaire. RESULTS: Vitamin D deficiency (<20 ng/mL) was observed in 56% and severe vitamin D deficiency (<10 ng/mL) in 13% of those individuals. Serum concentrations of 25(OH)D were significantly and positively associated with BMD total hip (p = 0.001), femoral neck (p = 0.011) and 33% radius (p = 0.046) BMDs, MET (p = 0.03) and functional reach test (p = 0.037) and negatively with age (p = 0.021), PTH (p = 0.004) and osteoporosis diagnosis (p = 0.012). Long-lived individuals with 25(OH)D ≥ 20 ng/mL had higher total hip and femoral neck BMDs (p = 0.012 and p = 0.014, respectively) and lower PTH (p = 0.030). In multiple linear regression analysis, age and osteoporosis diagnosis remained negatively associated with 25(OH)D levels (p = 0.021 and p = 0.001, respectively), while corrected calcium and cholecalciferol use remained positively associated (p = 0.001 and p = 0.024, respectively). CONCLUSION: We observed high vitamin D inadequacy prevalence in those Brazilian community dwellers' oldest old. Serum concentrations of 25(OH)D were positively associated with bone mass and dynamic balance, and negatively with PTH and osteoporosis diagnosis. Additionally, 25(OH)D ≥ 20 ng/mL was associated with better bone mass and lower PTH levels.

Introdução: Dispositivos externos compressivos após a punção radial para cateterismo cardíaco e intervenção coronária percutânea visam à hemostasia, ao conforto para o paciente e à segurança. O estudo objetivou avaliar pacientes submetidos a procedimentos coronários invasivos por via radial, nos quais se utilizou um curativo compressivo e de baixo custo, desenvolvido no serviço, avaliando a segurança dele e a ocorrência de complicações. Métodos: Indivíduos submetidos à coronariografia e à intervenção coronária percutânea, avaliados no momento de retirada do curativo compressivo e após 7 dias, por meio de questionário abrangendo comorbidades, quadro clínico, tempo do procedimento e de compressão, avaliação física de hematoma e dor, e Doppler vascular sonoro para avaliação de oclusão da artéria radial. Resultados: Foram avaliados 144 pacientes, sendo 138 com seguimento em 7 dias. Os eventos não diferiram entre procedimentos diagnósticos e terapêuticos. Na avaliação imediata à retirada do curativo, revelou-se incidência de 4,2% de oclusão da artéria radial, com dor referida em 23,6% dos pacientes, graduada em 2,9±1,7 pela Escala Visual Analógica (intensidade de zero a dez) e sem sangramentos graves. Hematoma ocorreu em um paciente (0,9%), com classificação III pelo critério EASY. Na avaliação de 7 dias, a incidência de oclusão da artéria radial foi de 2,2%, a dor foi referida em 11,1% da amostra (intensidade 1,8±0,8), e o hematoma foi evidenciado em 3,5%. Conclusão: O curativo compressivo mostrou-se um procedimento seguro, com baixa taxa de complicações e baixa taxa de dor local nos pacientes submetidos a procedimentos coronários invasivos pela via radial.

The objective of the study was to evaluate patients undergoing invasive coronary procedures by radial approach, in which a low-cost compressive dressing developed at the service was used, assessing its safety and the occurrence of complications. Methods: Patients undergoing coronary angiography and percutaneous coronary intervention, evaluated at the time of removal of compressive dressing and after 7 days, by means of a questionnaire addressing comorbidities, clinical picture, procedure and compression time, physical assessment of hematoma and pain, and a vascular Doppler ultrasound to evaluate radial artery occlusion. Results: A total of 144 patients were evaluated, 138 of whom were followed up within 7 days. Events did not differ among diagnostic and therapeutic procedures. In the immediate evaluation after removal of dressing, an incidence of 4.2% of radial artery occlusion was observed, with pain reported by 23.6% of patients, graded at 2.9±1.7 points in the Visual Analogue Scale (intensity of zero to ten), and no major bleeding. Hematoma occurred in one patient (0.9%), classified as type III according to the EASY criteria. In the 7-day evaluation, the incidence of radial artery occlusion was 2.2%, pain was reported in 11.1% of sample (intensity 1.8±0.8), and hematoma was evident in 3.5%. Conclusion: The compressive dressing proved to be a safe procedure, with a low rate of complications and a low rate of local pain in patients undergoing invasive coronary procedures via radial approach.

Anomalias coronarianas são menos frequentes em relação às doenças coronarianas adquiridas, como a aterosclerose, e têm sido implicadas como causa de eventos cardiovasculares. Este relato de caso descreve um achado incidental por angiografia de coronária direita anômala com origem no seio coronariano esquerdo e trajeto intra-arterial, após episódio de morte súbita abortada. A análise do Heart Team indicou como melhor tratamento a abordagem percutânea com implante de stent farmacológico em origem de coronária direita, procedimento este que foi realizado com sucesso. Este relato de caso e estudos recentes têm demonstrado que o tratamento percutâneo de anomalias coronarianas é uma opção segura para pacientes selecionados, levando em consideração quadro clínico, estudo anatômico e capacitação da equipe de hemodinâmica


Coronary anomalies are less frequent than acquired coronary diseases, such as atherosclerosis, and have been implicated as a cause of cardiovascular events. This case report describes an incidental finding on angiography of an anomalous right coronary artery with origin in the left coronary sinus and an intra-arterial course, after an episode of aborted sudden death. The Heart Team analysis indicated a percutaneous approach with drug-eluting stent implantation at the origin of the right coronary artery as the best treatment, and the procedure was successfully performed. This case report and recent studies have demonstrated percutaneous treatment of coronary anomalies is a safe option for selected patients, considering clinical presentation, anatomy study, and training of the cath lab team

RESUMO

Objetivo: Verificar o uso da fisioterapia aquática na reabilitação de pacientes com fibromialgia. Métodos: Trata-se de uma revisão integrativa da literatura, com a busca de artigos realizada nos meses de março e abril de 2020, nas bases de dados eletrônicos Physiotherapy Evidence Database (PEDro) e PubMed, utilizando-se os descritores indexados no Descritores em Ciências da Saúde (DeCS) e no Medical Subject Headings (MeSH). Os critérios de inclusão foram estudos com abordagens observacionais e intervencionistas, publicados entres os anos de 2010 e 2020, disponíveis em texto completo nos idiomas português e inglês. Excluíram-se artigos de revisão, anais de eventos, dissertações, teses e publicações duplicadas. Resultados: Foram encontrados 27 artigos. Após a leitura dos títulos e resumos, foram selecionados para leitura na íntegra e inclusos nesta revisão 8 estudos. Além do alívio da dor, foi verificado os efeitos da fisioterapia aquática em variáveis que otimizam a capacidade funcional e aumentam qualidade de vida. Conclusão: O uso da fisioterapia aquática na reabilitação de pacientes com fibromialgia promove efeitos benéficos como o alívio do quadro álgico, entre outros, influenciando positivamente na qualidade de vida dos indivíduos.

The objective of this study was to evaluate whether there is an interaction between pepper extract (PE) supplementation and evaporative cooling regimens on the performance, thermal comfort, and metabolism of lactating cows. The experiment was designed as a double 4 × 4 Latin square using eight multiparous Holstein cows (days in milk 147 ± 43.8 days). Treatments were a 2 × 2 factorial arrangement; two evaporative cooling regimens [sprinkler cycles of 30 s on and 4.5 min off (R5) and 30 s on and 9.5 min off (R10)] were combined with no inclusion of pepper extract (CT) or 800 mg/cow daily of PE. The inclusion of PE allowed a greater reduction in the surface temperature of the coat, and the cows remained for longer periods in the feeding area. There was an interaction between the use of PE and the climate regimen for surface temperature, which was lower for PER5. Cows experiencing greater intermittency in the spraying cycles (R10) spent 35% more time at the feeding area. A significant interaction was observed for milk production, with greater production for CTR5 compared to the other treatments. The feed efficiency for R5 was higher than that for R10. The R5 treatment combined with PE reduced water intake. There was no significant difference in serum parameters other than gamma-glutamyl transferase, with an interaction between treatments and greater activity for PER10, and total protein and albumin, which had cooling effects that were higher for R10. The two factors tested interfered in different and dissociated ways. The evaporative cooling strategies were effective, and the R5 treatment resulted in higher feed efficiency and milk production. The inclusion of PE enhanced heat reduction in cows when they were subjected to a cooling regime.

The objective of this study was to evaluate the main scientific studies addressing the topic of compost barns in recent years, highlighting the main journals, authors, countries, organizations, and keywords associated with the publications and trends in this type of research through a bibliometric analysis. For this analysis, publications (articles and literature reviews) addressing compost barns were obtained from the Scopus and Web of Science databases. A total of 108 studies published between 2007 and April 2022 were included. A greater number of publications was observed starting in 2017, with 2021 having the largest number of publications. The Journal of Dairy Science was the most highly cited journal. Marcia I. Endres was the author with the greatest academic impact. The most influential country was the United States, followed by Brazil. Among the organizations that have published studies on compost barns, the Federal University of Lavras, and the University of Florence had the largest numbers of publications. In conclusion, this bibliometric analysis allowed us to evaluate the scientific evolution, research, and publishing tendencies of studies on the compost barn animal housing system, and the results make it possible to deduce current trends in scientific research and publications.

Background: Epidermal growth factor receptor (EGFR) overexpression has been considered a poor prognostic factor in breast cancer. Methodology: A prospective study of 206 women with breast cancer analysed by stages (I, II, III and IV) and by immunohistochemical subtype (Luminal A, Luminal B, HER2+ and triple-negative (TN)); 89 healthy controls with normal recent mammography were included. The EGFR measured in the serum (sEGFR) was detected by the Enzyme-Linked Immunosorbent Assay (ELISA) method (R&D Systems kit DY231) collected by blood before any treatment in patients. Kaplan-Meier method and Cox regression were carried out to obtain the prognostic value, considering significance if p < 0.05. Results: With a median follow-up of 36.6 months, 47 deaths occurred. Multivariable Cox regression showed difference of overall survival (OS) associated with sEGFR levels (sEGFR ≤ or > 47.8 ng/mL) in patients with TN cancers, but not of Luminal A, Luminal B or HER2+ subtypes; adjusted by stage, the death risk increased by approximately 415% [hazard ratio (HR): 5.149 (1.900-13.955), p = 0.001] for patients with sEGFR > 47.8 ng/mL compared to patients with a lower sEGFR value. There was no significant correlation of sEGFR with staging, histological tumour grade (G1/G2/G3), Ki67 (< or ≥14%) or body mass index. Conclusions: Increased sEGFR expression in patients with TN tumours is a significant predictor of lower OS and its quantification is inexpensive and straightforward.

The compost barn system has become popular in recent years for providing greater animal well-being and quality of life, favoring productivity and longevity. With the increase in the use of compost barn in dairy farms, studies related to the thermal environment and behavior are of paramount importance to assess the well-being of animals and improve management, if necessary. This work aimed to characterize the thermal environment inside a compost barn during the four seasons of a year and to evaluate the standing and lying behavior of the cows through images. The experiment was carried out during March (summer), June (autumn), August (winter), and November (spring). Dry bulb temperature (tdb, °C), dew point temperature (tdp, °C), and relative humidity (RH,%) data were collected every 10 minutes during all analyzed days, and the temperature and humidity index (THI) was subsequently calculated. In order to analyze the behavior of the cows, filming of the barn interior was carried out during the evaluated days. Subsequently, these films were analyzed visually, and in an automated way to evaluate the behavior of these animals. For the automated analysis, an algorithm was developed using artificial intelligence tools, YOLOv3, so that the evaluation process could be automated and fast. It was observed that during the experimental period, the highest mean values of THI were observed during the afternoon and the autumn. The animals' preference to lie down on the bed for most of the day was verified. It was observed that the algorithm was able to detect cow behavior (lying down or standing). It can be concluded that the behavior of the cows was defined, and the artificial intelligence was successfully applied and can be recommended for such use.
