RESUMO

This paper describes the methodology and results, and draws conclusions from a large-scale source apportionment study undertaken in a large urban conurbation in the northwest of England. Annual average oxides of nitrogen (NOx) emission and ambient air pollution contributions have been estimated for road traffic sources. Ground level air pollution concentrations were estimated over a 1552-km(2) area with a resolution of up to 20 m, using emissions estimates and the second generation ADMS-Urban Gaussian dispersion model. Road traffic emissions were split into car and motorcycles; heavy and light goods vehicles; and buses to represent domestic users; commercial users and bus companies. Car related emissions were split further in to journey lengths under 3 km; journeys between 3 and 8 km; and journeys over 8 km to represent journeys which could be either walked or cycled; journeys for which a bus can easily be used and other journeys. These source sections were chosen so that the relevant authorities could target key groups in terms of reducing air pollution. The results confirm that the areas most likely to exceed air quality objectives are typically close to main arterial routes and close to urban centres and that the major culprits of road traffic related air pollution are goods vehicles and car journeys over 8 km. The paper also discusses the implications of the results and suggests how these can be used in the assessment of actions to reduce air pollution concentrations.

This paper describes a comparison of three different sets of road traffic emission factors released by the UK government for use in air quality review and assessment. The air quality management process of review and assessment began in 1997 in the UK. During this period of ongoing review and assessment, a number of changes have been made to the emission factors provided by the government. The use of different sets of emission factors during the assessment process has lead to some inconsistencies between results from neighbouring local authorities and also between different modelling exercises undertaken by the same local authorities. One purpose of this study has been to compare three different sets of emission factors, including the most recent set, and to some degree highlight the uncertainty associated with the use of factors, such as the shift of emphasis in terms of emissions from cars to heavy goods vehicles. The most recently released emission factors are the most comprehensive to date, and theoretically more accurate than previous sets due to the larger database of emission measurements that they have been based on. Therefore, the most recent set of emission factors have been additionally used in a validation exercise between modelled and monitored data. Comparison has been undertaken with monitoring data at a variety of urban background, urban centre and roadside sites. This work has shown some differences between the predicted trends in emission factors and measured trends in ambient air pollution levels, especially at roadside sites, indicating an under-prediction of the air pollution contribution from road traffic.

AIMS: Dietary supplement (DS) use is actively promoted among old people but there is little evidence in favour of DS use or information about the demographic, health and cognitive characteristics of DS users. METHOD: We examined 176 healthy, old people without dementia all born in 1921 and living independently in the community. IQ scores aged about 11 years were available for all subjects. DS users were more often female, had a lower BMI and were taking fewer prescribed medications than non-users. RESULTS: Usual dietary intake, as measured by food frequency questionnaire, did not differ between DS users and DS non-users. DS users were seen to have higher Vitamin C (p<0.05), alpha-carotene (p<0.05) and lower gamma-tocopherol (p<0.001) and homocysteine (p<0.01). DS users did not differ from DS non-users in years of education, indices of occupational code, current socio-economic category or parameters of cardiovascular or respiratory functions. DS users had higher (p<0.05) childhood IQ scores but did not differ in current Mini-Mental State Examination (MMSE) score or performance on Raven's Progressive Matrices (RPM) either before or after adjustment for childhood IQ. CONCLUSIONS: DS users may enjoy somewhat better general health than non-users but the source of this difference is unknown. Possible health benefits of DS use merit further study.

OBJECTIVE: To compare different statistical methods for assessing the relative validity of a self-administered, 150-item, semi-quantitative food-frequency questionnaire (FFQ) with 4-day weighed diet records (WR). DESIGN: Subjects completed the Scottish Collaborative Group FFQ and carried out a 4-day WR. Relative agreement between the FFQ and WR for energy-adjusted nutrient intakes was assessed by Pearson and Spearman rank correlation coefficients, the percentages of subjects classified into the same and opposite thirds of intake, and Cohen's weighted kappa. SUBJECTS: Forty-one men, mean age 36 (range 21-56) years, and 40 women, mean age 33 (range 19-58) years, recruited from different locations in Aberdeen, Scotland. RESULTS: Spearman correlation coefficients tended to be lower than Pearson correlation coefficients, and were above 0.5 for 10 of the 27 nutrients in men and 17 of the 27 nutrients in women. For nutrients with Spearman correlation coefficients above 0.5, the percentage of subjects correctly classified into thirds ranged from 39 to 78%, and weighted kappa values ranged from 0.23 to 0.66. CONCLUSIONS: Both Spearman correlation coefficients and weighted kappa values are useful in assessing the relative validity of estimates of nutrient intake by FFQs. Spearman correlation coefficients above 0.5, more than 50% of subjects correctly classified and less than 10% of subjects grossly misclassified into thirds, and weighted kappa values above 0.4 are recommended for nutrients of interest in epidemiological studies.

BACKGROUND: Prevention policies do not have an upper age limit, and as the overwhelming majority of older people continue to reside in the community there is a growing role for community dietetics and primary care team members in the promotion of healthy eating. METHOD: The multi-method project ascertained the dietary beliefs and practices of older people residing in high-income, low-income and rural localities of Scotland. One hundred and fifty-two people aged 75 years and over were interviewed using a semistructured interview schedule and 24-h food recall questionnaire. RESULTS: An analysis of the food recall questionnaire demonstrated that the diets of the elderly appear to differ little from the Scottish population as a whole. In all groups there was an under consumption of fruits and vegetables reported. Findings from the interviews demonstrated that dietary beliefs were found to be firmly rooted in childhood and lifetime experiences. Participants defined healthy eating as 'proper meals', 'proper foods', and a variety of foods eaten in moderation. These definitions were based upon the consumption of fresh foods which would be considered healthy. Changing and conflicting advice on health and nutrition was contrasted with personal experiences. Few knew of the role of the dietitian or community dietitian. CONCLUSIONS: This study demonstrates a contrast between stated beliefs and actual consumption patterns. Access to food, and the cost and quality of foods impacted upon food practices. The role of the community dietitian should be promoted. Advice on healthy eating must work with contemporary practices and beliefs building upon positive aspects of diet and eating and involving the food industry, retail sector and health services.

An increase in dietary intake of B from 0.25 to 3.25 mg/d has been reported to increase plasma oestradiol and testosterone and decrease urinary Ca excretion in postmenopausal women. Consequently, it is suggested that the higher level of B intake could reduce bone loss associated with the menopause and cessation of ovarian function. The present study was designed to investigate the effect of a B supplement on bone mineral absorption and excretion, plasma sex steroid levels and urinary excretion of pyridinium crosslink markers of bone turnover in healthy postmenopausal volunteers. The women were accommodated in a metabolic unit, given a low-B diet (LBD; 0.33 mg/d) for 3 weeks and were asked to take a B supplement of 3 mg/d in addition to the LBD for a further 3 weeks. Changing B intake from 0.33 to 3.33 mg/d had no effect on minerals, steroids or crosslinks. However, the LBD appeared to induce hyperabsorption of Ca since positive Ca balances were found in combination with elevated urinary Ca excretion. This phenomenon may have inhibited or obscured any effect of B.
