RESUMO

Personality disorders (PD) are recognized among key maladaptive factors in postoperative period of coronary artery bypass surgery (CABS). However studies on PD among CABS patients are predominantly under psychological approach and information on clinical features is very limited. A present investigation aimed at assessing clinical features and prognostic value of PD in late post operative period of CABS (from 1 to 3 years). Using psychiatric, cardiological and psychological methods, 48 patients (42 male, mean age 61.3 +/- 8.2 years) with diagnosis of PD and favorable cardiological status after CABS have been assessed. PD in post operative period is an important predictor of unfavorable social and occupational outcome of CABS. Three types of clinical course of PD were singled out: 1) hypohondric personality development; 2) "second life" personality development; 3) "denial of illness" reactions. Pronounced maladaptation was observed in type 1, most severe one--in type 2 and minimal maladaptation or its absence--in type 3. These types were differentiated by different repertoires of psychological defense mechanisms (PMD): type 1 was characterized by a limited number of them with a drift to mature mechanisms; PD of type 2 are featured by a narrow and rigid repertoire of defenses with predomination of immature mechanisms and in type 3 a maximum number of PMD with balanced involvement of mature and immature mechanisms was observed.

RESUMO

Myocardial contractility was examined in patients with type I diabetes mellitus without coronary disease by selective tolerance test with transesophageal heart stimulation using doppler cardiography. Nineteen diabetics aged 17 to 36 with the disease duration from 0 to 20 years were examined. Maximal ejection time before and after the test was assessed. The findings indicate impaired myocardial contractility in the examinees, though clinical signs of manifest circulation insufficiency were undetectable in all the patients. The authors come to a conclusion on the significance of diabetes duration and severity in development of myocardial injury.

RESUMO

Acupuncture points were exposed to helium-neon laser beam in 200 anginal patients. This therapeutic modality yielded good results: the number of attacks and nitroglycerin intake reduced, exercise tolerance grew, echocardiographic evidence showed a positive trend, hemodynamics improved, Spilberger's test values changed for the better.

RESUMO

An analysis of the results of polycardiographic examinations conducted during muscle work done by patients with ischaemic heart disease extends our knowledge of the functional state of the myocardium and is of certain prognostic importance; besides, such an analysis permits to make the prognosis of the effect of cardiac glycosides on the heart rhythm.

Veloergometry was practised in 35 patients with acute myocardial infarction at the time of their activiation (mainly on the 45-50th day of the disease or 7-8 days after they have been put on the feet). The investigation was aimed at detecting some signs of coronary insufficiency and cardiac incompetence, as well as of the limit of physical activity of such patients at the time of stimulated physical performance. The patients under examination demonstrated a significantly reduced amount of the work performed, which averaged 327 kgm. Among the patients complaints that led to discontinuance of the exercises were general fatigue and that of the feet in particular, dyspnoea and sensation of air deficiency, anginal attacks and deranged cardiac rhythm. In 28 cases changes in the terminal part of the ventricular ECG complex were recorded. The T-V1 greater than T-V6 syndrome was also analyzed before and after veloergometry. Of interest are the ECG changes in 2 groups of the patients, viz. the 1st without any ECG indications of transmural infarction and the 2nd one presenting such signs. Changes in the terminal part of the ventricular complex recorded in patients of the 2nd group, combined with dyspnoea, asphyxia and marked tachycardia during veloergometry suggest the presence in them not only of the coronary, but also of the left ventricular insufficiency.

The results of a single intravenous injections of strophanthin or digoxin and of a course-wise medication with cardiac glycosides in acute and chronic ischemic heart disease are contrasted. Investigations were carried out in 64 patients under cardiomonitoring with an automatic extrasystoles count. In the absence of extrasystoles or infrequent ones the cardiac glycosides provoked frequent extrasystoles in 10 per cent of the patients. The antiarrhythmic effect was recorded in 1/3 of the patients with infrequent extrasystoles. In cases of frequent extra systoles the positive result of the glycoside test justifies anticipating a beneficial influence of digitalization on the heart rate. In all the cases the results of the glycoside test determine in a large measure the individual prognosis.
