RESUMO

This article describes a new technique for reconstruction of the umbilicus. The technique used simply originated from a few clinical cases in which we were confronted with an absent or destroyed umbilicus. Previously described techniques for neo-umbilicoplasty were unsatisfying or seemed too complex in our hands. We introduce this easy, safe and reproducible technique which in our experience resulted in a very acceptable newly formed umbilicus.

RESUMO

A technique of lipectomy for moderate degree of fatty deposition was described in 1976 by Fischer. Modifications of the technique have been popularized by Kesselring and Meyer (1978), and by Illouz in 1978. The technique can be either dry--direct suction--or wet--suction after local injection of hypotonic solution. The aspiration is carried out by preserving 1 or 2 cm of subcutaneous fat. Tunnels are created at different levels and the incision is placed at a distance from the mass to be removed. The technique can be used to treat unsightly fat deposits in many sites of the body, including the thighs, buttocks, abdomen, hips, arms, ankles, breasts, and submental regions. The results are based on a series of 2000 patients operated on, mostly in Paris since 1977. The results are good, longlasting, and the complications rare if the operation is performed by well trained plastic surgeons.

RESUMO

From June 1982 to June 1984, selective suction lipectomy was performed successfully in 9 patients with posttraumatic fat deposit deformity. Seven patients had minor deformities that were repaired under local anesthesia on an outpatient basis. Two other patients sustained thigh deformities after a moped and a skiing accident, respectively, and were operated on under general anesthesia. Encouraging results have been obtained using the blunt dissection technique of Illouz. Indications for use of this technique and probable future developments are also discussed.

RESUMO

Computerized tomography of the face offers a new technique for the evaluation of malignant tumors. It serves to ascertain exactly the extension of the malignant process and to establish the indication for surgery in the best possible conditions. A brief clinical description of the most frequent tumors of the face is given and the indications for this examination are discussed. The technical aspect of CT is also presented. In the light of four cases selected among patients who had been treated for squamous cell carcinoma of the face, the authors demonstrate the value of the examination. They believe that computed tomography is fully indicated in recurrent tumors of the face and in complex localisation such as the medial angle of the eye, the external auditory canal, the alar base of the nose or the orbits.

RESUMO

From 1971 to 1976, 72 patients over 60 years old underwent aortic valve replacement. At the moment of the operation 6 patients were older than 70 (8%), 25 patients were between 65 and 69 (35%) and 41 patients between 60 and 64 years old (57%). The early postoperative mortality was 5.5% and the late postoperative mortality 12.5%. The average observation period was 34 months. The frequency of death was comparable in the three age groups. The operative risk and functional postoperative recovery are unrelated to the age of the patients. The patients with combined aortic disease have the best prognosis.
