RESUMO

Mycotic aneurysm secondary to tuberculous infection (TB) of the aorta is a rare and life-threatening disease. We report a case report of a 78-year-old woman with a tuberculous mycotic aortic aneurysm (TBAA). Early diagnosis and a combination of surgical intervention (aortic reconstruction and extensive excision of the infected field) and prolonged antituberculous drug therapy provide long-term survival without evidence of recurrence after tuberculous aortic involvement.

Diagnostic and therapeutic percutaneous endovascular procedures have become more and more common in recent years, and so also the number of local complications has increased. After such procedures a simple clinical examination may show the presence of an inguinal mass, but does not permit a diagnosis, while Color Doppler and Duplex Scanner can make a differential diagnosis between hematoma, pseudoaneurysm, arteriovenous fistula or other disease. Color Doppler is ubiquitously used to diagnose such complications as it offers a low-cost, easy-to-use method, only minimally uncomfortable for the patient. This ultrasound system can provide both anatomic and haemodynamic information. Our study highlights the diagnostic possibilities offered by the Color Doppler and Duplex Scanner and details, using many illustrations and examples, how the most common complications such as hematoma, pseudoaneurysm, arteriovenous fistula and thrombosis are imaged. Hematoma appears as a hypoechogenic zone, with no color inside, as flow is not present. Pseudoaneurysms, unlike hematoma, maintain a connection with an injured blood vessel, and so they show blood flow both inside the lesion and in the communicating channel, with a typical pattern. The arteriovenous fistula is a vascular channel created, after a percutaneous procedure, between an artery and an adjacent vein that have both been damaged. An endovascular thrombus is directly shown as a luminal defect of flow. Other less common complications are discussed and illustrated.

RESUMO

Lymphadenopathy is fairly widespread among the population and recognize multiple causes, often benign and self-limiting. We report on case about one young women with Kikuchi-Fujimoto disease or histiiocytic necrotizing lymphadenitis. This is a rare self-limiting lymphadenopathy which is usually diagnosed in young women. It is a self-limiting condition, usually resolving within 2-3 months, a low recurrence rate of 3% to 4% has been reported, of unknown origin, that does not require any specific management.

RESUMO

The common hepatic artery aneurysms (HAA) are uncommon atherosclerotic lesions and often they are clinically silent. Their diagnosis is often difficult prior to rupture, due to nonspecific clinical presentation. We report a case of a giant aneurysm of the common hepatic artery causing obstructive jaundice through compression of the biliary tract. The presence of the lesion was confirmed with the use of spiral computed tomographic angiography. Hepatic artery aneurysm is a lesion that should be considered in cases of unexplained obstructive jaundice.

RESUMO

Acute cytomegalovirus infection (CMV) may be responsible of venous thromboembolism, especially in immunocompromised patients. In addition, an association between CMV infection and thrombotic events in immunocompetent hosts has been sporadically reported. We present a case of a previously healthy woman with venous thromboembolic event during acute CMV infection and the presence of anti-cardiolipin IgM antibodies.
