RESUMO

A Harmonização Orofacial (HOF) é especialidade odontológica reconhecida pelo Conselho Federal de Odontologia (CFO) e, no reconhecimento dessa especialidade, discussões internas e externas à classe odontológica surgiram. O presente artigo tem como objetivo levantar reflexões acerca do processo judicial n° 1003948-83.2019.4.01.3400 julgado pelo Tribunal Regional Federal (Seção Judiciária do Distrito Federal - 8ª Vara Federal) referente a HOF enquanto especialidade odontológica. Inicialmente, são discutidos os documentos que norteiam o ensino dos cursos de graduação e pós-graduação em odontologia. Em seguida, faz-se uma reflexão sobre o processo de reconhecimento da HOF como especialidade odontológica. Por fim, discute-se acerca do citado processo judicial, os principais pontos elencados pelas partes envolvidas no processo e eventuais consequências do desfecho desse processo para a classe odontológica. O reconhecimento da HOF enquanto especialidade odontológica não seguiu os trâmites que outras especialidades odontológicas seguiram e gerou debates educacionais e formativos sob uma perspectiva ética e legal. Ademais, tal reconhecimento fomentou debates relacionados ao conflito dos procedimentos dessa especialidade odontológica com procedimentos de especialidades médicas. Tais conflitos chegaram ao Poder Judiciário, que interveio em favor da Odontologia, contudo, isso não impede que o cirurgião-dentista seja responsabilizado profissionalmente pela realização de procedimentos que fogem ao escopo da odontologia


Orofacial Harmonization (HOF) is dental specialty recognized by the Federal Council of Dentistry (CFO) and, in recognition of this specialty, internal and external discussions of the dental class have arisen. This article aims to raise reflections on judicial process number 1003948-83.2019.4.01.3400 judged by the Federal Regional Court (Judicial Section of the Federal District - 8th Federal Court) referring to HOF as a dental specialty. Initially, the documents that guide the teaching of undergraduate and graduate courses in dentistry are discussed. Then, a reflection is made on the recognition process of HOF as a dental specialty. Finally, it discusses the judicial process number 1003948-83.2019.4.01.3400, the main points listed by the parties involved in the process, and the possible consequences of the outcome of this process for the dental class. The recognition of HOF as a dental specialty did not follow the procedures that other dental specialties followed and generated educational and training debates from an ethical and legal perspective. Furthermore, such recognition has fostered debates related to the conflict between procedures in this dental specialty and procedures in medical specialties. Such conflicts reached the Judiciary, which intervened in favor of Dentistry, however, this does not prevent the dental surgeon from being held professionally responsible for carrying out procedures that fall outside the scope of dentistry

Conventional methods for pathogen detection in water rely on time-consuming enrichment steps followed by biochemical identification strategies, which require assay times ranging from 24 hours to a week. However, in recent years, significant efforts have been made to develop biosensing technologies enabling rapid and close-to-real-time detection of waterborne pathogens. In previous studies, we developed a plastic optical fiber (POF) immunosensor using an optoelectronic configuration consisting of a U-Shape probe connected to an LED and a photodetector. Bacterial detection was evaluated with the immunosensor immersed in a bacterial suspension in water with a known concentration. Here, we report on the sensitivity of a new optoelectronic configuration consisting of two POF U-shaped probes, one as the reference and the other as the immunosensor, for the detection of Escherichia coli. In addition, another methos of detection was tested where the sensors were calibrated in the air, before being immersed in a bacterial suspension and then read in the air. This modification improved sensor sensitivity and resulted in a faster detection time. After the immunocapture, the sensors were DAPI-stained and submitted to confocal microscopy. The histograms obtained confirmed that the responses of the immunosensors were due to the bacteria. This new sensor detected the presence of E. coli at 104 CFU/mL in less than 20 min. Currently, sub-20 min is faster than previous studies using fiber-optic based biosensors. We report on an inexpensive and faster detection technology when compared with conventional methods.

The age estimation of an adult using methods accessible to the forensic routine is a goal pursued by forensic experts. Cameriere, Ferrante and Cingolani (2004) proposed the use of the pulp/tooth area ratio of canine teeth as a promising variable, but its reliability has shown conflicting results in the scientific literature. This article aimed to carry out a systematic review with meta-analysis to verify whether the pulp/tooth area ratio of canine teeth includes a variable that can be used alone to estimate dental age in adults. A systematic search was carried out in six databases using keywords related to the theme in Portuguese, English, and Spanish. The study selection process followed pre-established eligibility criteria. Assessments were carried out regarding risk of bias and publication bias of selected studies, and meta-analysis was carried out considering Pearson's correlation coefficient between pulp/tooth area ratio and chronological age as effect measure. Most selected studies showed low risk of bias; no publication bias was found when all studies were considered, and potential publication bias was found when outliers were removed. Despite the high heterogeneity among studies and the need for more research, it could be observed that the pulp/tooth area ratio has strong negative correlation with chronological age, and the pulp/tooth area ratio could be derived from both periapical radiographs and orthopantomographs. Therefore, it is suggested that there is scientific evidence that the pulp/tooth area ratio obtained from canine teeth is reliable for dental age estimation in adults.

OBJECTIVE: to analyze the social representations about sexually transmitted infections elaborated by undergraduate students. METHODS: a descriptive, qualitative study, in the light of the structural approach of Social Representation Theory, carried out with 160 young undergraduate students, in the second half of 2019, in the city of Rio de Janeiro. Data were collected using a sociodemographic characterization questionnaire, knowledge and practices for preventing sexually transmitted infections, analyzed using descriptive statistics and a form of free evocations with the inducing term STD, analyzed using prototypical and similarity analysis. RESULTS: the representation's possible central nucleus is composed of lexicons aids, disease and HIV; the peripheral system by syphilis, sex, condoms, gonorrhea, prevention, infection, carelessness, HPV, herpes, ignorance, treatment, fear, unprotected-sex and danger. FINAL CONSIDERATIONS: social thinking about sexually transmitted infections is characterized by their recognition as diseases, which require barrier prevention measures, associating with unsafe sexual practices that arouse fear.

OBJECTIVE: to discuss undergraduate students' sexual behavior from the perspective of social markers and cross-cultural care proposed by Madeleine Leininger. METHODS: descriptive-exploratory qualitative research, with a theoretical-philosophical foundation in the Transcultural Theory. Convenience sample was composed of 57 young people from two universities in Rio de Janeiro. The focus groups' content were analyzed lexically using the IRAMUTEQ software. RESULTS: four classes emerged: Young people's sexual scripts: between the fear of an unplanned pregnancy and the risk of exposure to sexually transmitted infections; Affective relationships: trust in steady sexual partners, apparent sense of security and disuse of condoms; Sexual practices, gender and cultural determinants: distinction in men's and women's role; Sexual partnerships, negotiation of condom use and vulnerability to sexually transmitted infections. FINAL CONSIDERATIONS: challenges are perceived for the attention to undergraduate students' sexual health, who verbalized risky sexual behaviors due to sociocultural vulnerabilities.

OBJECTIVE: To describe contents, structure and origin of social representations about falls by elderly people, the peridomiciliary structural conditions that predispose to falls, and to relate the implications of these empirical evidence on the routine of the elderly in the architectural context. METHOD: Convergent mixed method by triangulation. Qualitative approaches(structural, n=195 and procedural, n=40of the Theory of Social Representations) and quantitative (descriptive sectional, n=183) were used. Elderly people enrolled in primary care were interviewed at home in 2018. Analysis techniques: categorical-thematic, prototypical, statistical, and deductive according to Leininger. RESULTS: Categories of analysis: 1) Peridomicile: fall scenario and 2) Aging and vulnerability: risk of falls in peridomicile. The following environmental characteristics were precursors to falls: uneven floors, holes, unevenness and objects in the pathway. Feelings and behaviors allocated in the possible central core are associated, justifying falls, and determining their causes. CONCLUSION: There was an association between the peridomiciliary architectural environment and the predictive characteristics of the risk of falls.

Pseudomonas syringae pv. actinidiae (Psa) is responsible for the kiwifruit bacterial canker, the most severe disease of Actinidia spp. The use in agriculture of antibiotics and cooper-based compounds is increasingly being restricted, demanding for new sustainable alternatives to current agrochemicals. We aimed to characterize the anti-Psa potential of essential oils (EOs) of Mentha pulegium and Satureja montana and investigate if they elicit the plant-host hormonal defenses. The EOs were characterized through gas-chromatography with flame ionization detector (GC-FID) and mass spectrometry (MS). Pulegone (78.6%) and carvacrol (43.5%) were the major constituents of M. pulegium and S. montana EO, respectively. Only S. montana EO showed relevant anti-Psa activity in vitro. To evaluate if the EOs also elicited host defenses, in vitro shoots were treated with 2 mg shoot-1 of EO-solution and subsequently inoculated with Psa three days later. Shoots were analyzed 10 min, three days (and 10 min after Psa-inoculation), four and ten days after EO application. The up/down regulation of RNA-transcripts for hormone biosynthesis, Psa biofilm production and virulence genes were quantified by real-time quantitative PCR (RT-qPCR). Phytohormones were quantified by High-Performance Liquid Chromatography (HPLC). S. montana EO showed the most promising results as a defense elicitor, increasing 6-benzylaminopurine (BAP) by 131.07% and reducing indole-3-acetic acid (IAA) levels by 49.19%. Decreases of salicylic acid (SA), and gibberellic acid 3 (GA3) levels by 32.55% and 33.09% respectively and an increase of abscisic acid (ABA) by 85.03%, in M. pulegium EO-treated shoots, revealed some protective post-infection effect. This is the most comprehensive research on the Psa's impact on phytohormones. It also unveils the protective influence of prior EO exposure, clarifying the plant hormonal response to subsequent infections. The results reinforce the hypothesis that carvacrol-rich S. montana EO can be a suitable disease control agent against Psa infection. Its dual action against pathogens and elicitation of host plant defenses make it a promising candidate for incorporation into environmentally friendly disease management approaches. Nonetheless, to fully leverage these promising results, further research is imperative to elucidate the EO mode of action and evaluate the long-term efficacy of this approach.

Older adult maltreatment (OAM) is a global problem that has attracted increasing attention due to the ageing population and its severe impact on victim health. Thus, this study aims to analyse the prevalence of certain health conditions in people ≥ 60 years old whom physicians from a local healthcare unit suspected to be victims of maltreatment. The specific objectives are to determine the prevalence rates of health-related risk factors, traumatic injuries and intoxications, mental disorders, and physical disorders. We conducted a real-world, retrospective, observational, and cross-sectional study based on secondary data analyses of electronic health records and healthcare registers of patients at the Local Healthcare Unit of Matosinhos (2001-2021). Information was obtained based on codes from the International Classification of Diseases, codes from the International Classification of Primary Care, and clinical notes (according to previously defined keywords). We identified 3092 suspected victims of OAM, representing 4.5% of the total population analysed. This prevalence is lower than the known rates. We also found that some health risk factors, traumatic injuries and intoxications, mental health disorders, and physical disorders presented higher rates in the suspected victims than among the total population. In this age group, we cannot assume that these health problems are only related to a possible current victimisation process; they could also be associated with adverse childhood experiences or intimate partner violence, among other forms of violence, all of which can lead to cumulative effects on the victim's health. This evidence increases healthcare providers' responsibility in detecting and reporting all cases of suspected maltreatment.

Agricultural systems are in need of low-cost, safe antibiotics to protect crops from pests and diseases. Peptaibiotics, a family of linear, membrane-active, amphipathic polypeptides, have been shown to exhibit antibacterial, antifungal, and antiviral activity, and to be inducers of plant resistance against a wide range of phytopathogens. Peptaibiotics belong to the new generation of alternatives to agrochemicals, aligned with the United Nations Sustainable Development Goals and the One Health approach toward ensuring global food security and safety. Despite that, these fungi-derived, non-ribosomal peptides remain surprisingly understudied, especially in agriculture, where only a small number has been tested against a reduced number of phytopathogens. This lack of adoption stems from peptaibiotics' poor water solubility and the difficulty to synthesize and purify them in vitro, which compromises their delivery and inclusion in formulations. In this review, we offer a comprehensive analysis of peptaibiotics' classification, biosynthesis, relevance to plant protection, and mode of action against phytopathogens, along with the techniques enabling researchers to extract, purify, and elucidate their structure, and the databases holding such valuable data. It is also discussed how chemical synthesis and ionic liquids could increase their solubility, how genetic engineering and epigenetics could boost in vitro production, and how omics can reduce screenings' workload through in silico selection of the best candidates. These strategies could turn peptaibiotics into effective, ultra-specific, biodegradable tools for phytopathogen control.

O atestado odontológico compreende um tipo de documento emitido pelo profissional que expressa a veracidade de um fato relacionado ao paciente constatado durante a prática clínica. O objetivo deste artigo foi apontar e analisar o tipo de crime que incorre o cirurgião-dentista que emite atestado falso. Trata-se de uma pesquisa transversal com abordagem descritiva realizada por meio do levantamento das provas de concursos públicos realizados no Brasil para o cargo de Perito Odontolegista ou nomenclatura correspondente. Foram considerados os concursos realizados nas unidades federativas do Brasil sem distinção quanto ao ano, sendo que a coleta das informações se deu nos meses de janeiro a abril de 2023. Conclui-se que, apesar de ocorrerem discrepâncias de interpretação muito pontuais na literatura odontológica quanto ao condizer entre os Artigos 299 e 302 do Código Penal Brasileiro, não há possibilidade legal de o cirurgião-dentista ser sujeito ativo do crime de falsidade de atestado médico, por ser crime próprio do profissional médico


The dental certificate comprises a kind of document issued by the professional that expresses the veracity of a fact related to the patient verified during clinical practice. The aim of this article was to point out and analyze the type of crime incurred by dentists who issue false certificates. This is a cross-sectional research with a descriptive approach carried out through the survey of public tenders held in Brazil for the position of Forensic Odontologist or corresponding nomenclature. It was considered the contests held in the Brazilian Federative Units without distinction to the year, and the information collection was performed from January to April 2023. It can be concluded that, although there are few interpretation discrepancies in the dental literature regarding articles 299 and 302 of the Brazilian Penal Code, there is no legal possibility for the dentist to be an active subject of the crime of falsity of medical certificate, as it is a crime specific to the professional medical doctor

A Odontologia Legal é a especialidade responsável por esclarecer demandas administrativas ou judiciais provenientes de diferentes esferas. Na esfera criminal, o processo de identificação humana engloba um conjunto de procedimentos que visam individualizar uma pessoa e, em diversas situações, a Odontologia Legal se destaca como um importante método de identificação humana. O objetivo desse trabalho foi apresentar um relato de caso em que a presença de um remanescente de guta-percha foi decisiva no processo de identificação humana. Após exame necroscópico em um cadáver e com a entrega do prontuário odontológico, foi realizada a comparação das informações recebidas do suspeito com aquelas coletadas no exame post mortem. Por meio dessa comparação, verificou-se que havia pontos coincidentes que permitiram que a identificação positiva da vítima fosse estabelecida, especialmente a presença, tanto no exame ante mortem quanto no post mortem, de um remanescente de guta-percha envolto por tecido cicatricial ósseo na região do dente 21. Os materiais dentários utilizados em procedimentos odontológicos, tais como o remanescente de guta-percha neste relato de caso pericial, são extremamente valiosos no processo de identificação humana


Forensic Dentistry is the specialty responsible for clarifying administrative or judicial demands from different spheres. In the criminal sphere, the human identification process encompasses a set of procedures that aim to individualize a person and, in several situations, and Forensic Dentistry stands out as an important method in human identification. The objective was to present a case report in which the presence of a gutta-percha remnant was important in the human identification process. After necroscopic examination and delivery of the dental records, the information received from the suspect was compared with that collected in the post mortem examination. Through this comparison, it was verified that there were coincident points that allowed the positive identification of the victim, especially the presence, both in the ante-mortem and in the post-mortem examination, of a gutta-percha remnant surrounded by bone scar tissue in the tooth region 21. Materials used in dental procedures, such as the gutta-percha remnant in this forensic case report, are extremely valuable in the human identification process

Hepatocellular carcinoma (HCC) is the third most common cause of cancer-related mortality worldwide. The Brazilian Society of Hepatology (SBH) published in 2020 the updated recommendations for the diagnosis and treatment of HCC. Since then, new data have emerged in the literature, including new drugs approved for the systemic treatment of HCC that were not available at the time. The SBH board conducted an online single-topic meeting to discuss and review the recommendations on the systemic treatment of HCC. The invited experts were asked to conduct a systematic review of the literature on each topic related to systemic treatment and to present the summary data and recommendations during the meeting. All panelists gathered together for discussion of the topics and elaboration of the updated recommendations. The present document is the final version of the reviewed manuscript containing the recommendations of SBH and its aim is to assist healthcare professionals, policy-makers, and planners in Brazil and Latin America with systemic treatment decision-making of patients with HCC.

The socioeconomic context and population vulnerability are directly associated with violence in the country. In Brazil, the Criminal Code provides the illicit act and punishes the aggressor who offends the physical aggression from a person. Interpersonal violence is an illegal act and is associated with the vulnerability of victims. Therefore, the study of the epidemiological profile of victims of interpersonal violence is needed to alert the population about their susceptibility. The present study aimed to design the epidemiological profile of victims of bodily injury from interpersonal violence in the region of Maringá, in the state of Paraná, and to study the characterization of dental injuries in terms of permanent weakness and deformity. Four thousand nine hundred sixty-two reports performed between 2018 and 2020 were analyzed using descriptive statistics. Data collected included: sex, age, marital status, the region affected by the bodily injury, type of dental trauma, and responses to "weakness" or/and "permanent deformity". During the period studied, there was a prevalence of female victims (57.8%), white skin color (80.2%), aged between 21 and 30 years (24.9%), single (54.9%), and the most affected region was the upper limbs (32.1%). Regarding bodily injuries with dental involvement, 67 cases were reported, male victims were prevalent (60.3%), and dental fracture was highlighted with 54.4% of examined injuries.The implementation of the Forensic Dentistry Centre took place in August 2019 at the Medico-Legal Institute of Maringá. Also, with the interruption of expert activities in 2020 - due to the COVID-19 pandemic ­ relevant outcomes regarding permanent weakness were observed. The epidemiological profile of victims is characterized by the female sex, white skin color, the age group from 21 to 30 years old, and singles. Regarding dental trauma, male victims, and dental fractures were prevalent. Furthermore, there was an increase in the classification of "permanent weakness", as well as a slight increase in cases of "further assessment required" for permanent deformity (AU).


O contexto socioeconômico e a vulnerabilidade da população estão diretamente associados à violência no país. No Brasil, o Código Penal (CP) prevê o ato ilícito e pune o agressor que atentar contra a integridade física de outrem. A violência interpessoal constitui um ato ilícito e está associada à vulnerabilidade das vítimas. Sendo assim, o estudo do perfil epidemiológico de vítimas de violência interpessoal se faz necessário para alertar a população quanto à suscetibilidade dessas. O presente estudo objetivou traçar o perfil epidemiológico de vítimas de lesão corporal por violência interpessoal na região de Maringá, no estado do Paraná, e estudar a caracterização das lesões dentais quanto à debilidade e deformidade permanente. Foram analisados 4.962 laudos pertencentes aos anos de 2018 a 2020 por estatística descritiva. Dados coletados incluíram: sexo, idade, estado civil, região afetada pela lesão corporal, tipo de trauma dental e respostas para "debilidade" ou/e "deformidade permanente". Durante o período estudado, houve prevalência de vítimas do sexo feminino (57,8%), cor de pele branca (80,2%), com idade entre 21 e 30 anos (24,9%), solteiros (54,9%), e a região mais afetada foi a dos membros superiores (32,1%). Em relação às lesões corporais com envolvimento dental, 67 casos foram relatados. Vítimas do sexo masculino foram prevalentes (60,3%) e a fratura do elemento obteve destaque com 54,4% das lesões periciadas. A implementação do Núcleo de Odontologia Legal ocorreu em agosto de 2019 e, ainda com a interrupção das atividades periciais no ano de 2020 ­ devido à pandemia da COVID-19 ­ resultados quanto à debilidade permanente foram evidentes. O perfil epidemiológico das vítimas é caracterizado pelo sexo feminino, cor de pele branca, faixa etária de 21 a 30 anos e solteiros. Quanto aos traumas dentais, vítimas do sexo masculino e fraturas dentais foram prevalentes. Além disso, houve um aumento na classificação de "debilidade permanente", bem como um leve aumento nos casos de "necessidade de avaliação complementar" para deformidade permanente (AU).

BACKGROUND. Patients with nonmucinous rectal adenocarcinoma may develop mucinous changes after neoadjuvant chemoradiotherapy, which are described as mucinous degeneration. The finding's significance in earlier studies has varied. OBJECTIVE. The purpose of this study was to assess the frequency of mucinous degeneration on MRI after neoadjuvant therapy for rectal adenocarcinoma and to compare outcomes among patients with nonmucinous tumor, mucinous tumor, and mucinous degeneration on MRI. METHODS. This retrospective study included 201 patients (83 women, 118 men; mean age, 61.8 ± 2.2 [SD] years) with rectal adenocarcinoma who underwent neoadjuvant chemoradiotherapy followed by total mesorectal excision from October 2011 to November 2015, underwent baseline and restaging rectal MRI examinations, and had at least 2 years of follow-up. Two radiologists independently evaluated MRI examinations for mucin content, which was defined as T2 hyperintensity in the tumor or tumor bed, and resolved differences by consensus. Patients were classified into three groups on the basis of mucin status: those with nonmucinous tumor (≤ 50% mucin content on baseline and restaging examinations), those with mucinous tumor (> 50% mucin content on baseline and restaging examinations), and those with mucinous degeneration (≤ 50% mucin content on baseline examination and > 50% content on restaging examination). The three groups were compared. RESULTS. Interreader agreement for mucin content, expressed as a kappa coefficient, was 0.893 on baseline MRI and 0.890 on restaging MRI. Of the 201 patients, 156 (77.6%) had nonmucinous tumor, 34 (16.9%) had mucinous tumor, and 11 (5.5%) had mucinous degeneration. Mucin status was not significantly associated with complete pathologic response (p = .41) or local or distant recurrence (both p > .05). The death rate during follow-up was not significantly different (p = .21) between patients with nonmucinous tumor (23.1%), those with mucinous tumor (29.4%), and those with mucinous degeneration (9.1%). In adjusted Cox regression analysis, with mucinous degeneration used as reference, the HR for the overall survival rate for the mucinous tumor group was 4.7 (95% CI, 0.6-38.3; p = .14), and that for the nonmucinous tumor group was 8.0 (95% CI, 0.9-59.9; p = .06). On histopathologic assessment, all 11 patients with mucinous degeneration showed acellular mucin, yet 10 of 11 patients showed viable tumor (i.e., in nonmucinous portions of the tumors). CONCLUSION. Mucinous degeneration on MRI is not significantly associated with pathologic complete response, recurrence, or survival. CLINICAL IMPACT. Mucinous degeneration on MRI is uncommon and should not be deemed an indicator of pathologic complete response.

Introduction: As a health science, dentistry seeks to raise self-esteem and improve patients' quality of life. However, the emergence of the aesthetic procedure "ear shut," which proposes correcting protruding ears without surgery, has raised ethical and legal doubts in the dental profession, especially concerning the limits of professional activity. Objective: To conduct a survey of the laws, norms, and resolutions on the area of performance of dental surgeons, as well as to discuss the limits and consequences of their extrapolation from the perspective of the procedure propagated as "ear shut." Method: A search was carried out for administrative norms on the Federal Council of Dentistry (CFO) websites and the Federal Council of Medicine and legal provisions on the Portal da Legislação website. Results: In the civil sphere, the disclosure of procedures such as the "ear shut" can characterize a promise of result and lead to judicial accountability. Exceeding professional boundaries constitutes an illegal exercise and a crime under the Brazilian Penal Code. In addition to not covering the area of clinical practice of dentists, at the administrative level, CFO Resolutions No. 198/2019 and No. 230/2020 emphasize that ear procedures are not part of the scope of procedures relevant to Dentistry and may lead to ethical infractions and consequent administrative proceedings. Conclusion: At present, based on the skills, prohibitions, rights, and duties of dentists, it can be stated that performing the "ear shut" by these professionals confronts civil, criminal, and administrative obligations.


Introdução: Como ciência da saúde, a Odontologia busca elevar a autoestima e melhorar a qualidade de vida dos pacientes. Entretanto, o surgimento do procedimento estético "ear shut", que propõe a correção da orelha em abano sem cirurgia, gerou dúvidas de ordem ética e legal na classe odontológica, sobretudo no que diz respeito aos limites de atuação profissional. Objetivo: Realizar o levantamento das leis, normativas e resoluções sobre a área de atuação dos cirurgiões-dentistas, bem como discutir os limites e as consequências de sua extrapolação sob a perspectiva do procedimento propagado como "ear shut". Método: Foi realizada uma busca de normas administrativas nos sites do Conselho Federal de Odontologia (CFO) e do Conselho Federal de Medicina, bem como de dispositivos legais no site Portal da Legislação. Resultados: Em âmbito cível, a divulgação de procedimentos como o "ear shut" pode caracterizar promessa de resultado e levar à responsabilização judicial. Ultrapassar os limites profissionais constitui exercício ilícito e constitui crime segundo o Código Penal Brasileiro. Além de não abranger a área de atuação clínica dos cirurgiões-dentistas, no âmbito administrativo, as Resoluções do CFO N° 198/2019 e N° 230/2020 enfatizam que procedimentos na orelha não compõem o escopo de procedimentos pertinentes à Odontologia, podendo levar a infrações éticas e consequente processo administrativo. Conclusão: No momento atual, com base nas competências, vedações, direitos e deveres dos cirurgiões-dentistas, pode-se afirmar que a realização do "ear shut" por estes profissionais confronta obrigações de ordem cível, penal e administrativa.

The Coronavirus Disease 2019 (COVID-19) has drastically overwhelmed most countries in the last two years, and image-based approaches using computerized tomography (CT) have been used to identify pulmonary infections. Recent methods based on deep learning either require time-consuming per-slice annotations (2D) or are highly data- and hardware-demanding (3D). This work proposes a novel omnidirectional 2.5D representation of volumetric chest CTs that allows exploring efficient 2D deep learning architectures while requiring volume-level annotations only. Our learning approach uses a siamese feature extraction backbone applied to each lung. It combines these features into a classification head that explores a novel combination of Squeeze-and-Excite strategies with Class Activation Maps. We experimented with public and in-house datasets and compared our results with state-of-the-art techniques. Our analyses show that our method provides better or comparable prediction quality and accurately distinguishes COVID-19 infections from other kinds of pneumonia and healthy lungs.

BACKGROUND: Conflicting data regarding the incidence of hepatocellular carcinoma (HCC) after cure of HCV infection with direct-acting antivirals (DAAs) remains. We investigated the incidence and risk factors to HCC after treatment with DAAs followed up for five years. METHODS: A total of 1075 HCV patients ≥ 18 years were treated with DAAs from 2015 to 2019 and followed until 2022. Ultrasonography was performed before DAAs and each 6 months thereafter. RESULTS: Of the total, 51/1075 (4.7%) developed HCC in the median of 40 (IQR 25-58) months: 26/51 (51%) male, median age 60 (IQR 54-66) years, alpha-fetoprotein (AFP) 12.2 (IQR 6.1-18.8) ng/mL, 47/51 (92.1%) cirrhotic 78.7%, 8/51 (15.7%) without sustained virological response (SVR). Seventeen percent had non-characterized nodules before DAAs. Cumulative HCC incidence was 5.9% in 5 years. Overall incidence was 1.46/100 patient-years (PY) (95% CI = 1.09-1.91), being 2.31/100 PY (95% CI = 1.70-3.06), 0.45/100 PY (95% CI = 0.09-1.32) and 0.20/100 PY (95% CI 0.01-1.01) in METAVIR F4, F3 and F2, respectively, and the main risks to HCC were non-characterized nodule, cirrhosis, high AFP values and non-SVR. CONCLUSION: HCV cure reduced risk for HCC, but it still occurred particularly in cirrhotic patients. Some risk factors can be identified to predict early HCC diagnosis.

ABSTRACT Hepatocellular carcinoma (HCC) is the third most common cause of cancer-related mortality worldwide. The Brazilian Society of Hepatology (SBH) published in 2020 the updated recommendations for the diagnosis and treatment of HCC. Since then, new data have emerged in the literature, including new drugs approved for the systemic treatment of HCC that were not available at the time. The SBH board conducted an online single-topic meeting to discuss and review the recommendations on the systemic treatment of HCC. The invited experts were asked to conduct a systematic review of the literature on each topic related to systemic treatment and to present the summary data and recommendations during the meeting. All panelists gathered together for discussion of the topics and elaboration of the updated recommendations. The present document is the final version of the reviewed manuscript containing the recommendations of SBH and its aim is to assist healthcare professionals, policy-makers, and planners in Brazil and Latin America with systemic treatment decision-making of patients with HCC.


RESUMO O carcinoma hepatocelular (CHC) é uma das principais causas de mortalidade relacionada a câncer no Brasil e no mundo. A Sociedade Brasileira de Hepatologia (SBH) publicou em 2020 a atualização das recomendações da SBH para o diagnóstico e tratamento do CHC. Desde então, novas evidências científicas sobre o tratamento sistêmico do CHC foram relatadas na literatura médica, incluindo novos medicamentos aprovados que não estavam disponíveis na época do último consenso, levando a diretoria da SBH a promover uma reunião monotemática on-line para discutir e rever as recomendações sobre o tratamento sistêmico do CHC. Um grupo de experts foi convidado para realizar uma revisão sistemática da literatura e apresentar uma atualização, baseada em evidências científicas, sobre cada tópico relacionado ao tratamento sistêmico e a apresentar os dados e recomendações resumidas durante a reunião. Todos os painelistas se reuniram para discutir os tópicos e elaborar as recomendações atualizadas. O presente documento é a versão final do manuscrito revisado, contendo as recomendações da SBH, e seu objetivo é auxiliar os profissionais de saúde, formuladores de políticas e planejadores no Brasil e na América Latina na tomada de decisões sobre o tratamento sistêmico de pacientes com CHC.
