RESUMO

One of the main challenges of tissue engineering in dentistry is to replace bone and dental tissues with strategies or techniques that simulate physiological tissue repair conditions. This systematic review of in vitro studies aimed to evaluate the influence of the addition of nanohydroxyapatite (NHap) to scaffolds on cell proliferation and osteogenic and odontogenic differentiation of human mesenchymal stem cells. In vitro studies on human stem cells that proliferated and differentiated into odontogenic and osteogenic cells in scaffolds containing NHap were included in this study. Searches in PubMed/MEDLINE, Scopus, Web of Science, OpenGrey, ProQuest, and Cochrane Library electronic databases were performed. The total of 333 articles was found across all databases. After reading and analyzing titles and abstracts, 8 articles were selected for full reading and extraction of qualitative data. Results showed that despite the large variability in scaffold composition, NHap-containing scaffolds promoted high rates of cell proliferation, increased alkaline phosphatase (ALP) activity during short culture periods, and induced differentiation, as evidenced by the high expression of genes involved in osteogenesis and odontogenesis. However, further studies with greater standardization regarding NHap concentration, type of scaffolds, and evaluation period are needed to observe possible interference of these criteria in the action of NHap on the proliferation and differentiation of human stem cells.

Resumen Introducción: El personal de otorrinolaringología presenta una elevada exposición al virus SARS-CoV-2, debido a los procedimientos que lleva a cabo. Es fundamental tomar las medidas de protección adecuadas. Determinar la seroprevalencia nos dará un mejor panorama sobre la exposición, contagios y efectividad de medidas de protección adoptadas. Objetivo: Determinar la prevalencia de resultados serológicos positivos en personal médico que presta servicio en la Cátedra de Otorrinolaringología de marzo del 2020 a marzo de 2021. Material y Método: Estudio observacional, descriptivo, corte transversal, retrospectivo con asociación cruzada. Muestreo no probabilístico de casos consecutivos. La población total fue de 38 médicos del Servicio de Otorrinolaringología del Hospital de Clínicas. Resultados: La media de edades fue 37,4 años, 63,5% refirió haber atendido a paciente conocido portador de COVID-19. Un 42% refirió no haber cumplido con todas las medidas de protección personal, mientras que un 23,7% de los sujetos de estudio dio positivo para IgG, interpretándose como infección previa por COVID-19. Conclusión: Más de las dos terceras partes de los médicos refirió dar consulta a paciente COVID-19 positivo. Casi la cuarta parte de los médicos resultó ser positivo para COVID-19 según la prueba de serología anti-N. No se halló asociación entre consulta ni cirugía a pacientes portadores de COVID-19 y el contagio al personal médico.


Abstract Introduction: Otolaryngology personnel have a high exposure to SARS-CoV-2 virus due to the procedures they perform. It is essential to take appropriate protective measures. Determining seroprevalence will give us a better picture of exposure, contagion and effectiveness of protective measures adopted. Aim: To determine the prevalence of positive serological results in medical staff serving in the otolaryngology department from March 2020 to March 2021. Material and Method: Observational, descriptive, cross-sectional, retrospective, retrospective study with cross-association. Non-probability sampling of consecutive cases. The total population was 38 physicians of the Otolaryngology Service of the Hospital de Clinicas. Results: The mean age was 37.4. 63.5% reported having seen a patient known to be a COVID-19 carrier, while 42% reported not having complied with all personal protection measures. A 23.68% of the study subjects tested positive for IgG, interpreting previous COVID-19 infection. Conclusion: More than two-thirds of the physicians referred to giving consultation to COVID-19 positive patients. Almost a quarter of the physicians were positive for COVID-19 according to the Anti-N serology test. No association was found between consultation or surgery of patients with COVID-19 and infection of medical personnel.

The biological activities of Porphyra sp., Gracilaria gracilis, Alaria esculenta and Saccharina latissima extracts prepared by enzymatic and ball milling-assisted methods and hot water were evaluated. Enzyme-assisted methods allowed the highest extraction yields. Alcalase-assisted extraction (EAA) was the most effective in the recovery of polyphenolic compounds and Porphyra sp. had the highest content. The efficiency of flavonoids extraction was highly dependent on the used method. Globally, Porphyra sp. and EAA extracts exhibited the highest antioxidant and chelating activities. The highest α-amylase inhibitory activity was determined in HW Porphyra sp. extract while EAA A. esculenta extract had the highest α-glucosidase inhibitory activity. The highest ACE inhibitory activity was obtained in EAA from S. latissima. None of the extracts showed antimicrobial activity against the tested bacteria. The results showed that Porphyra sp. and S. latissima are potentially useful as ingredient in functional foods and nutraceuticals.

Resumen Introducción: La teleconsulta puede ser de utilidad para evitar exposición de pacientes y optimizar recursos y dirigir a los que se consideran prioridad para atención presencial. Objetivo: Valorar el grado de satisfacción de la atención por teleconsulta en el Servicio de Otorrinolaringología del Hospital de Clínicas, San Lorenzo, durante julio de 2020. Material y Método: Diseño observacional, descriptivo, corte transversal, retrospectivo, muestreo probabilístico aleatorio simple. Se realizó encuestas sobre la calidad de atención por teleconsulta. Resultados: Se contactó 86 pacientes, 78 participaron. Edad promedio 32,2 años, 58,97% mujeres. 73% era de zona urbana, 27% rural. Los motivos de consulta más frecuentes fueron obstrucción nasal, hipoacusia y molestia en garganta. Un 66,67% no requirió ayuda para la consulta. 74,35% evitó asistir al médico durante la pandemia, el 51,72% por prevención o miedo. El 51,28% refirió empeoramiento de su dolencia durante la cuarentena. Se registró un alto nivel de satisfacción de la atención (9,41 ± 1,3). Un 82% refirió que esta modalidad puede ser implementada como opción, aunque el 70,5% requirió consulta presencial. Conclusión: La mayoría de los pacientes se encontraba en rango etario económicamente activo. La mayor parte de los pacientes no necesitó ayuda para acceder a la teleconsulta, reflejando la facilidad de acceso a esta. La teleconsulta disminuyó la necesidad de consulta presencial, el grado de aceptación del sistema de teleconsulta es elevada, traducidos en los altos índices de satisfacción y aceptación de la modalidad.


Abstract Introduction: Teleconsultation can be useful as a triage to prevent patient exposure to the COVID-19 virus, classifying infectious ones and perfecting hospital resources, to direct those considered priority for face-to-face care. Aim: To assess the degree of satisfaction of teleconsultation care. Material and Method: Observational design study, descriptive, cross-sectional, retrospective, simple random probabilistic sampling. Surveys were conducted on the quality of care of the teleconsultation service during July 2020. Results: 86 patients were contacted, 78 were surveyed. The average age was 32.2 years, 58.97% was female. 73% came from Asunción and metropolitan area, 27% of rural areas. Most common reasons for consultation were nasal obstruction, hearing loss and throat discomfort. 66.67% did not require help in carrying out the consultation. 74.35% avoided attending the doctor during the pandemic. 51.72% (30) for prevention or fear of infection. 51.28% reported worsening of their ailment during quarantine. There was an important level of attention satisfaction (9.41 ± 1.3). 82% reported that this mode of care can be implemented as an option in the service, although 70.5% require a face-to-face consultation. Conclusion: Most of the patients were in the economically active age range. Most of the patients did not required help for teleconsultation reflecting the ease of access to it. Teleconsultation reduced the need for face-to-face consultation, the degree of acceptance of the teleconsultation system is high, translated into the high rates of satisfaction and acceptance of the modality.

Seiscentos primatas neotropicais foram submetidos a exames post mortem para avaliação da prevalência parasitária de helmintos gastrointestinais. Foram examinados 556 calitriquídeos (Callithrix spp.), 23 bugios (Alouatta guariba), 19 macacos-pregos (Sapajus nigritus), um mico-leão-dourado (Leontopithecus rosalia) e um mico-leão-da-cara-dourada (Leontopithecus chrysomelas). Do total de 600 animais, foram encontrados espécimes parasitos pertencentes aos filos Acanthocephala, Nemathelmintes e Platyhelminthes (classes Trematoda e Cestoda) em 110 primatas. A prevalência de primatas positivos para, pelo menos, uma espécie de helminto foi de 18,3% (110/600), sendo destes 83,6% (92/110) calitriquídeos, 8,2% (9/110) bugios, 6,4% (7/110) macacos-pregos, 0,9% (1/110) mico-leão-dourado e 0,9% (1/110) mico-leão-da-cara dourada. Em 80,4% (74/92) dos calitriquídeos foram encontrados nematoides Primasubulura sp. e em 1,1% (1/92) nematoides Trypanoxyuris callithrix, em 26,1% (24/92) acantocéfalos (Pachysentis sp.) e em 5,4% (5/92) digenéticos (Platynosomum sp.); em 77,8% (7/9) dos bugios foram encontrados nematoides (Trypanoxyuris minutus), em 11,1% (1/9) acantocéfalos (Pachysentis sp.) e em 11,1% (1/9) cestoides (Bertiella sp.); em 14,3% (1/7) dos macacos-pregos foram encontrados nematoides (Physaloptera sp.), em 28,6% (2/7) acantocéfalos (Prostenorchis sp.) e em 14,3% (1/7) digenéticos (Platynosomum sp.) e no mico-leão-da-cara-dourada foram encontrados acantocéfalos (Prostenorchis sp.). Foi realizado o georreferenciamento dos pontos de encontro dos cadáveres para pontuar a distribuição dos helmintos por região.(AU)


Six hundred neotropical primates underwent postmortem examinations to evaluate the parasitic prevalence of gastrointestinal helminths. Fifty-five callitrichids tamarins (Callithrix spp.), 23 howlers (Alouatta guariba), 19 nail monkeys (Sapajus nigritus), a golden lion tamarin (Leontopithecus rosalia) and a golden-faced lion tamarin (Leontopithecus chrysomelas) were examined. Parasitic specimens belonging to phylums Acantocephala, Nemathelmintes and Platyhelmintes (Trematoda and Cestoda Classes) were found. The prevalence of primates positive for at least one species of helminth was 18.3% (110/600), of which 83.6% (92/110) callitrichids, 8.2% (9/110) howler monkeys, 6.4% (7/110) capuchin monkeys, 0.9% (1/110) golden lion tamarin and 0.9% (1/110) golden faced lion tamarin. In 80.4% (74/92) of callitrichids nematodes (Primasubulura sp.) were found, and in 1.1% (1/92) nematodes (Trypanoxyuris callithricis), in 26.1% (24/92) acanthocephalus (Pachysentis sp.) and 5.4% (5/92) digenetics (Platynosomum sp.); in 77.8% (7/9) of howler monkeys presented nematodes (Trypanoxyuris minutus), 11.1% (1/9) acanthocephalus (Pachysentis sp.) and 11.1% (1/9) cestoids (Bertiella sp.); in 14.3% (1/7) of capuchin monkeys presented nematodes (Physaloptera sp.), 28.6% (2/7) acanthocephalus (Prostenorchis sp.) and 14.3% (1/7) digenetics (Platynosomum sp.) and in the golden-faced lion tamarin acanthocephalus (Prostenorchis sp.) were found. Georreferencing of the meeting points of the cadavers was performed in order to punctuate the distribution of helminths by region.(AU)

Squamous cell carcinoma (SCC) is a frequent malignant neoplasm of the skin that usually arises from areas of solar dermatosis. It is characterized by local invasiveness and regional lymph node metastasis, mainly in poorly differentiated tumours. Galectin-3 (Gal-3) is a lectin that is expressed in the nucleus or cytoplasm and has been identified as a prognostic tool for human neoplasms. The purpose of this study was to characterize Gal-3 expression in canine cutaneous SCCs and to investigate its relationship with tumour differentiation and cell proliferation indices. Immunohistochemical analysis of 50 SCCs for Gal-3 revealed no correlation between the localization or intensity of immunolabelling, or number of immunopositive cells, with histological grade of tumour or proliferative activity. The results suggest that Gal-3 expression is not a reliable prognostic marker for cutaneous SCC in dogs.

O seminoma é uma neoformação testicular originária de células germinativas de ocorrência comum em cães, com maior prevalência em animais senis. Em geral, o comportamento biológico do seminoma canino é benigno. Relata-se neste trabalho um caso de seminoma com metástase em região orbital em um cão com 14 anos de idade. O animal foi atendido com queixa de aumento de volume em órbita esquerda, com posterior detecção de nódulo testicular. A punção aspirativa por agulha fina da massa orbital sugeriu tratar-se de linfoma de alto grau, contudo o diagnóstico definitivo de seminoma difuso foi estabelecido pela avaliação histopatológica, a qual revelou tratar-se de neoplasia maligna pouco diferenciada, sendo o diagnóstico de seminoma difuso confirmado pelo exame imunoistoquímico. Relatos de seminomas metastáticos em cães são incomuns. Objetivou-se com este trabalho relatar um caso de seminoma anaplásico difuso em cão cujo foco principal de metástase ocorreu em região orbital, além de descrever e discutir as dificuldades diagnósticas encontradas.(AU)


Seminoma is a testicular neoformation originating from germ cells, commonly occurring in dogs. With higher prevalence in senile animals, the biological behavior of canine seminomas generally benign. This case reports seminoma with mestastasis in the orbital region in a 14-year-old dog. The animal was treated with a complaint of increased volume in the left orbit, and later a nodule in the testicle was discovered. Fine-needle aspiration of the orbit mass initially indicated a high-grade lymphoma. The definitive diagnosis of diffused seminoma was established by histopathological examination, resulting in poorly differentiated malignant neoplasia. Finally, the diagnosis was confirmed through immunohistochemistry, being the result compatible with diffused seminoma. Metastatic seminomas reported in dogs are quite uncommon. In this work we report a case of diffused anaplastic seminoma in dogs, where the main focus of metastasis was observed in the orbital region, and we also describe and discuss the difficulties encountered in the diagnostic.(AU)

It can be hypothesized that the body composition characteristics of different sheep breeds affect their nutritional requirements. However, no study has yet been carried out to determine the nutritional requirements for maintenance of Texel purebred lambs, despite their growing importance in sheep meat production globally. Our objective was therefore to determine the energy and protein requirements for maintenance of Texel lambs. Thirty-four Texel lambs were used, all intact males that were weaned at 50 days old, and confined in individual pens. Two experiments were conducted, as follows. In Experiment 1, a digestibility assay was performed to determine the dietary energy value, in a 3×3 double Latin square design, in which lambs were submitted to three levels of feed restriction (0%, 55% and 70% of ad libitum feed intake). In Experiment 2, the energy and protein requirements for maintenance of Texel lambs from 21 to 40 kg BW were determined using a randomized block design, in which lambs were also submitted to three levels of feed restriction (0%, 55% and 70% of ad libitum feed intake). The requirements for net energy for maintenance (NEm), metabolizable energy for maintenance (MEm), net protein for maintenance (NPm) and metabolizable protein for maintenance (MPm) were determined. The digestibility of dry matter, energy, protein and metabolizability were similar between food restriction levels, averaging 74.4%, 75.5%, 80.3% and 0.636, respectively. The NEm determined for growing Texel lambs was 263 kJ/kg of the metabolic fasting BW (FBW), the MEm was 417 kJ/kg0.75 FBW and the efficiency of use of MEm was 0.63. In addition, the NPm was 1.24 g/day per kg0.75 FBW and the MPm was 2.98 g/day per kg0.75 FBW. The energy requirements of Texel lambs are different from those reported in the literature, possibly due to differences between breeds, diets and environmental effects, whereas the protein requirements are different from literature mainly due to methodological differences; further studies are need to address these aspects that affects the nutritional requirements for raising sheep from different breeds in different environments.

The increase of sheep meat competitiveness in international markets can be attributed to the rise of the quantity and the improvement of the quality of the edible portion of sheep carcasses. Usually, carcass yield is established after the slaughter of the animals. Yet, when carcass yield is determined in vivo, it can be both a costly and subjective method. This study proposes models for predicting the physical characteristics of lamb carcass using bioimpedance analysis (BIA) in live animals. Thirty-one Texel × Ile de France crossbreed ram lambs were slaughtered at 20, 26, 32 or 38 kg of BW. Before the slaughter, values of resistance (Rs) and reactance (Xc) were collected using a single-frequency BIA equipment (Model RJL Quantum II Bioelectrical Body Composition Analyzer). Then, BIA main variables such as body bioelectrical volume (V), phase angle (PA), resistive density (RsD) and reactive density (XcD) were calculated. After slaughter, cold carcass weight (CCW), cold carcass yield (CCY), subcutaneous fat thickness (SFT), soft tissue weight (STW) and soft tissue yield (STY) were also measured. Multiple regression analyses were carried out using the physical characteristics as dependent variables and the bioimpedance values as independent variables. Predictive performance of the models was assessed using leave-one-out cross-validation. The prediction model of CCW was obtained using the V, PA and RsD (R2 = 0.97), STW through the V, RsD and XcD (R2 = 0.97), CCY by Rs, Z and XcD (R2 = 0.69), STY by V and XcD (R2 = 0.67), and SFT only for XcD (R2 = 0.84). The results indicated that BIA has the potential to predict carcass characteristics of lambs at different body masses.

AIM: This was to evaluate the influence of root canal filling pastes on microshear bond strength (µSBS) of an adhesive system to primary dentine. METHODS: Human (32) primary molars were randomly assigned into four experimental groups (n = 8): zinc oxide eugenol paste (ZOE); iodoform paste (Guedes-Pinto paste); calcium hydroxide paste thickened with zinc oxide; and no filling paste (control). Flat dentine surfaces were covered with a 1 mm-thick layer of the pastes for 15 min at 37 °C. The pastes were mechanically removed from dentine surfaces, followed by rinsing and drying. After adhesive application (Adper Single Bond 2, 3M ESPE), starch tubes were placed over pre-treated dentine and filled with composite resin (Z250, 3M ESPE). The µSBS test was performed after 24 h of water storage at 37 °C. The failure mode was evaluated using a stereomicroscope. The µSBS values (MPa) were analysed with one-way ANOVA and Tukey post-hoc tests (α = 0.05). RESULTS: The lowest µSBS values were achieved when ZOE was used. No difference was found among other filling pastes compared with control group. All specimens showed adhesive/mixed failures. CONCLUSIONS: Zinc oxide eugenol paste negatively influenced the bond strength of adhesive systems to primary dentine. Iodoform-based Guedes-Pinto paste and calcium hydroxide paste thickened with zinc oxide did not influence the microshear bond strength values.

O objetivo desta pesquisa foi avaliar o efeito da suplementação privativa, com concentrado ou leguminosa, sobre as características da carcaça e dos componentes corporais de cordeiros lactentes mantidos em pastejo de azevém. Foram utilizados 27 cordeiros lactentes, distribuídos nos tratamentos que corresponderam aos sistemas de alimentação: cordeiros lactentes mantidos em pasto de azevém, cordeiros lactentes mantidos em pasto de azevém e suplementados com concentrado em comedouro privativo (creep feeding) e cordeiros lactentes mantidos em pasto de azevém e suplementados com leguminosa no pasto privativo (creep grazing). O concentrado era composto por milho, farelo de soja e calcário calcítico, e a leguminosa utilizada foi o trevo-branco. O delineamento experimental foi o inteiramente ao acaso. Os resultados foram submetidos à análise de variância, e as médias comparadas pelo teste de Tukey a 5% de significância. O conteúdo do trato gastrintestinal juntamente com bile e urina e a proporção de esôfago foram maiores (P<0,05) nos cordeiros mantidos em pasto de azevém. As proporções de fígado e intestino grosso foram maiores (P<0,05) nos cordeiros suplementados com concentrado. Os sistemas alimentares testados produzem carcaças com características semelhantes. A suplementação, com leguminosa ou concentrado, altera os componentes corporais de cordeiros lactentes mantidos em azevém.(AU)


This research objective was to evaluate the effect of private supplementation with concentrated or legumes, on the carcass characteristics and body components from suckling lambs kept on ryegrass pasture. Twenty seven suckling lambs were used, with about 17 days of age and weighing 9,91±0,594kg, which were distributed in the treatments that corresponded to feeding systems: suckling lambs kept on ryegrass pasture, suckling lambs kept on ryegrass pasture and supplemented with concentrated in private feeder (creep feeding) and suckling lambs kept on ryegrass pasture and supplemented with legume in the private pasture (creep grazing). The concentrate supplement was composed by corn, soybean meal and limestone, and was supplied ad libitum. The supplementary pasture was white clover legume. The experimental design was completely randomized, where the results were submitted to analysis of variance and means compared by Tukey test at 5% of significance level. The gastrointestinal+bile+urine content and the proportion of esophagus were higher (P<0,05) in lambs kept on ryegrass. The liver and large intestine proportions were higher (P<0,05) in lambs supplemented with concentrate. The tested alimentary systems produce carcasses with similar characteristics. The supplementation with legume or concentrate cause changes in body components of suckling lambs grazing ryegrass.(AU)

O objetivo deste estudo foi verificar a influência do tempo de armazenamento na qualidade microbiológica, físico-química e sensorial do pescado caranha (Piaractusmesopotamicus). Os pescados foram estocados em gelo durante zero, sete, 14, 21 e 28 dias e submetidos às contagens de micro-organismos mesófilos, psicrotróficos, coliformes a 35ºC e 45ºC, Salmonella sp. e estafilococos coagulase positiva. Foram realizadas análises de temperatura, pH, acidez, bases voláteis, proteínas, lipídeos, cinzas, umidade, prova de cocção, bem como análise sensorial. As contagens de mesófilos, psicrotróficos, coliformes a 35ºC e 45ºC aumentaram com o tempo de armazenamento. A presença de Salmonella sp. não foi constatada, enquanto a contagem de estafilococos coagulase positiva foi de <10 (est) UFC/g em todos os períodos de estocagem. Observou-se, ainda, que o tempo de armazenamento apresentou influência em todas as variáveis físico-químicas, exceto para temperatura. Na análise sensorial, foi constatado um aumento linear do índice de qualidade (IQ) ao longo do armazenamento, e a rejeição ocorreu aos 14 dias.(AU)


The objective of this study was to verify the influence of storage time on the microbiological, physical-chemical, and sensorial quality of the fish (Piaractusmesopotamicus). The fish were stored on ice for 0, 7, 14, 21, and 28 days and submitted to counts of mesophilic, psychrotrophic, coliform microorganisms at 35ºC and 45ºC, Salmonella sp. and coagulase positive staphylococci. Analyzes of temperature, pH, acidity, volatile bases, proteins, lipids, ashes, humidity, cooking test, as well as sensorial analysis were performed. Counts of mesophiles, psychrotrophic, coliforms at 35°C and 45°C increased with storage time. The presence of Salmonella sp. was not observed, whereas the coagulase positive Staphylococcus count was <10 (est) CFU/g in all storage periods. It was also observed that the storage time had influence on all physical-chemical variables, except for temperature. In the sensorial analysis, a linear increase of the quality index (IQ) was verified throughout the storage and the rejection occurred at 14 days.(AU)

The ongoing trend of increasing air temperatures will potentially affect soil organic matter (SOM) turnover and soil C-CO2 emissions in terrestrial ecosystems of Maritime Antarctica. The effects of SOM quality on this process remain little explored. We evaluated (i) the quantity and quality of soil organic matter and (ii) the potential of C release through CO2 emissions in lab conditions in different soil types from Maritime Antarctica. Soil samples (0-10 and 10-20cm) were collected in Keller Peninsula and the vicinity of Arctowski station, to determine the quantity and quality of organic matter and the potential to emit CO2 under different temperature scenarios (2, 5, 8 and 11°C) in lab. Soil organic matter mineralization is low, especially in soils with low organic C and N contents. Recalcitrant C form is predominant, especially in the passive pool, which is correlated with humic substances. Ornithogenic soils had greater C and N contents (reaching to 43.15gkg-1 and 5.22gkg-1 for total organic carbon and nitrogen, respectively). C and N were more present in the humic acid fraction. Lowest C mineralization was recorded from shallow soils on basaltic/andesites. C mineralization rates at 2°C were significant lower than at higher temperatures. Ornithogenic soils presented the lowest values of C-CO2 mineralized by g of C. On the other hand, shallow soils on basaltic/andesites were the most sensitive sites to emit C-CO2 by g of C. With permafrost degradation, soils on basaltic/andesites and sulfates are expected to release more C-CO2 than ornithogenic soils. With greater clay contents, more protection was afforded to soil organic matter, with lower microbial activity and mineralization. The trend of soil temperature increases will favor C-CO2 emissions, especially in the reduced pool of C stored and protected on permafrost, or in occasional Histosols.

Advances in the early detection of cancer and the development of more effective treatments have resulted in a larger number of adolescents and young adults (AYAs), becoming cancer survivors. However, knowledge regarding their needs and if those needs are adequately addressed remains limited. The aims of this study were to: (1) better understand the needs of AYAs after cancer treatment; (2) analyse the importance of those needs; (3) determine which needs are not adequately addressed; and (4) test the hypothesis that AYA cancer survivors have different needs than adult survivors. Twenty-nine health oncology professionals, 17 AYA survivors and 12 relatives of AYA survivors participated in the Delphi study. The needs identified could be classified into six categories, and all were rated as highly important by all participants. The category perceived as least adequately addressed across the three groups was 'Counselling and psychological support.' The findings provide important new information regarding the needs of AYA cancer survivors that can inform the development of interventions to improve the quality of life of these individuals.

OBJECTIVES: The introduction of metagenomic sequencing to diagnostic microbiology has been hampered by slowness, cost and complexity. We explored whether MinION nanopore sequencing could accelerate diagnosis and resistance profiling, using complicated urinary tract infections as an exemplar. METHODS: Bacterial DNA was enriched from clinical urines (n = 10) and from healthy urines 'spiked' with multiresistant Escherichia coli (n = 5), then sequenced by MinION. Sequences were analysed using external databases and bioinformatic pipelines or, ultimately, using integrated real-time analysis applications. Results were compared with Illumina data and resistance phenotypes. RESULTS: MinION correctly identified pathogens without culture and, among 55 acquired resistance genes detected in the cultivated bacteria by Illumina sequencing, 51 were found by MinION sequencing directly from the urines; with three of the four failures in an early run with low genome coverage. Resistance-conferring mutations and allelic variants were not reliably identified. CONCLUSIONS: MinION sequencing comprehensively identified pathogens and acquired resistance genes from urine in a timeframe similar to PCR (4 h from sample to result). Bioinformatic pipeline optimization is needed to better detect resistances conferred by point mutations. Metagenomic-sequencing-based diagnosis will enable clinicians to adjust antimicrobial therapy before the second dose of a typical (i.e. every 8 h) antibiotic.
