RESUMO

Severe injuries of the posterior eye segment mostly occur during perforation or rupture of the globe. The first treatment includes primary surgical repair of the ocular wound and prophylactic scleral buckling in case of involvement of the posterior segment. Thereafter, a vitrectomy should be performed to remove vitreous hemorrhage and intraocular foreign bodies and to achieve a stable attachment of the retina. Therefore, temporary perfluorocarbon and permanent silicone oil tamponade are used. A predictor of poor visual outcome is the presence of retinal and choroidal injury. In our patients, the most important prognostic factor was the initial visual acuity after the injury. Vitrectomy can significantly reduce the incidence of enucleation. Currently, better visual outcome is achieved by advances in surgical techniques.

UNLABELLED: In special forms of complicated secondary cataract, especially after a cataract operation in eyes with proliferative disease and after a cataract operation associated with pars plana vitrectomy and silicone oil instillation, surgical capsulotomy must be performed. METHODS: We developed a new surgical technique for posterior capsulorhexis via pars plana by using the high-frequency capsulotomy method developed by Klöti. We analyzed 14 consecutive patients undergoing late pars plana capsulotomy using a specially designed instrument for radiofrequency capsulorhexis. The indication for surgical capsulotomy was extreme secondary cataract; in 6 eyes fibrosis was associated with vascularizations. RESULTS: The high-frequency technique for posterior capsulotomy allowed reliable and precise tissue cutting. The cutting required minimal application of pressure to the capsule, and therefore damage to zonula fibers is minimized. Furthermore, use of the radiofrequency technique permitted hemostatic incision in vascularized membranes by inducing a fine coagulation margin. CONCLUSIONS: Posterior capsulorhexis using radiofrequency offers precise and effective tissue cutting if surgical capsulotomy is necessary.

RESUMO

Canaliculitis is a relatively rare dacryocanal infection which occurs most unilateral. It can easily be misinterpreted and not sufficiently treated. Typical agents of the canaliculitis are actinomyces, that can cause infections of the hollow spaces with formation of concrements. The clinical courses of two patients are shown. One of them was treated for a dacryocystitis for 3 years and the other one had been referred to the eye hospital for chalazion removal. Only a microbiologic examination including cultivation of the surgically obtained dacryolithes and secretion enabled us to a reliable proof of the actinomyces and to an appropriate therapy for canaliculitis.

RESUMO

The secured significance of position and extent of a cerebral tumour as well as its biological dignity with respect to the kind and frequency of the neuro-ophthalmological findings was the starting point of these investigations. In this retrospective study the findings of visual acuity, visual field and papillae of 204 patients operated on the cerebrum were determined and the significance of the morphological factors (position and size of the defect of the cerebral parenchyma, extent of the cerebral ventricles, degree of the cortical atrophy, influence of dignity) for the persisting ophthalmological deficiency phenomena was pointed out.

RESUMO

On the basis of 90 catamnestically pursued disease courses of surgically treated sphenoid bone wing meningioma of the years 1970 to 1987 the validity of the clinical-neuropsychiatric and neuro-ophthalmological examinations as well as the information value of the native X-ray pictures were ascertained. The aim of the work was to point out the possibility and necessity of an early clinical diagnosis by an intensified observation of the neuro-ophthalmological and neuro-psychiatric primary symptoms.
