RESUMO

OBJECTIVE: Evidence from epidemiological and clinical studies strongly suggest that young women in the preconception period are a group at risk for thyroid disorders which may lead to further aggravation of pre-existing chronic thyroid disease and complications during pregnancy. MATERIALS AND METHODS: This paper is a literature review focusing on articles published in English between 2014-2017 searched in Medline database using terms 'young women', 'subclinical hyperthyroidism', 'subclinical hypothyroidism', 'nonthyroidal illness syndrome', 'obesity', 'depression'. CONCLUSIONS: Clinical assessment of young female patient including that of obtaining full medical history with a focus on the perceived changes in appearance, psychological symptoms and menstrual irregularities supported by laboratory tests indicative of metabolic status and characteristic changes in thyroid functions, may be the key to well-reasoned and justified individual therapeutic decisions.

RESUMO

Radioiodine treatment of hyperthyroid patients with autonomous thyroid nodule leads to cellular DNA damage not only in thyrocytes but also in peripheral blood lymphocytes. The purpose of this study was to evaluate DNA breakage and base damage in thyrocytes and lymphocytes in patients treated with 131-I. In all the patients thyroid scintiscan was performed using 131-I. Damage to DNA was estimated by comet assay. Samples were taken before radioiodine treatment, and 12 and 54 days afterwards. Our results indicate high diversity in the level of DNA damage among the individual patients. However, in all cases, after 54 days the level of DNA damage in lymphocytes was similar or even lower than that in the controls. In contrast, in hot nodule the DNA damage persisted until the 54th day after 131-I application. Differences in the type of DNA damage between thyrocytes and lymphocytes were also observed. In lymphocytes there was more base damage, whereas in thyrocytes single strand breaks prevailed. This may indicate different mechanisms of DNA damage induction and/or DNA repair.

RESUMO

UNLABELLED: 45 patients with hyper and hypothyroidism in the time 1989-1990 were observed. The Graves' disease was diagnosed in 29 and rather in the younger patients, but 16 had the toxic nodular goiter and those were elderly. In 27 the hypertension was secondary (symptomatic) and after the successful treatment of the hyperthyroidism was completely controlled. In 14 cases the hypertension was primary (essential) and the application of the hypotensive drugs was also necessary. Among 4 patients with primary hypothyroidism and associated hypertension and coronary insufficiency the early treatment by the thyroid preparation was successful: the blood pressure was lowered and the coronary insufficiency was improved; but if the replacement therapy was stopped and the hypothyroidism was relapsed, the blood pressure was increases and the coronary insufficiency was aggravated. CONCLUSIONS: 1. The secondary (symptomatic) hypertension associated with the hyperthyroidism may be controlled by successful treatment of the thyrotoxicosis, but the primary (essential) must be treated by the hypotensive drugs also. 2. The early treatment of the hypothyroidism may control the associated hypertension and the coronary insufficiency. 3. Graves' disease is associated mostly with symptomatic hypertension, in nodular toxic goiter in most of the cases the essential hypertension was established.

RESUMO

Aim of this report is a stress of over-hasty classification to the surgical treatment of goiter diagnosed as hyperactive. The authors suppose that the cause of such errors is an incorrect interpretation of the current diagnostic tests, especially scintigraphy, and not very careful clinical evaluation of cases diagnosed as hyperthyroidism, being vegetative neurosis which is sometimes accompanied by the arterial blood hypertension. Examples of patients misclassified from the surgical treatment after proper diagnosis based on the clinical symptoms confirmed with thyroid gland diagnostic test are given. The authors stress that the detailed knowledge on the clinical course of hyperthyroidism and proper interpretation of the diagnostic tests help to avoid such errors.
