RESUMO

Bronchofiberoscopy is one of the basic diagnostic tools in pulmonary care. It is therefore of interest to assess the effects of this procedure on circulatory system as well as the risk of possible cardiovascular complications. The study included 56 patients for whom bronchoscopy was a planned diagnostic procedure (9 patients had additionally BAL). Each patient's examination protocol included, among others, ECG by Holter method, the measurement of arterial pressure and 12-lead ECG performed before and after the examination. The patients were divided into two groups: those with ventricular arrhythmia diagnosed by ECG at rest and those without arrhythmia signs in ECG done prior to bronchofiberoscopy. The analysis of data revealed that bronchofiberoscopy was associated with considerable increase of arterial pressure in both groups of patients as well as with increased ventricular arrhythmia, particularly in patients with previous history of arrhythmia. Additional load upon the respiratory system exerted by BAL did not have a significant effect on the circulatory system.

RESUMO

32 patients, in whom bronchoscopy was performed due to the removal of foreign body from the lower respiratory tract were described. Age of the patients clinical symptoms and a time lapse between aspiration and removal of foreign body from the lover respiratory tract and localization of foreign body in the particular bronchus were analysed. We conclude, that rigid bronchoscopy is still more useful and safe method of foreign body removing from the lower respiratory tract.

The study was performed in 18 patients (15 F and 3-M) aged 35-57 yers (mean 46 +/- 7) diagnosed as idiopathic lung fibrosis. Ten patients (group A) recived standard treatment (prednisone 10-15 mg/day) and supplementary home oxygen therapy (mean 16.4 h/day) for a period of 4 years. During exacerbations antibiotics, theophylline and expectorants were prescribed. Eight patients (group B) receiving the same treatment without home oxygen, served as controls. At study entry lung function test (TLC, VL, Dlco), blood gases, hematocrit, hemodynamic parameters of the pulmonary circulation and thermodilution cardiac output were measured in all patients. The measurements were repeated after 4 years of follow-up. At follow-up examination no differences in pulmonary function parameters were shown between the groups. Mean pulmonary artery pressure and pulmonary vascular resistance were significantly lower in patients receiving home oxygen therapy (group A) as compared with those treated pharmacologically only (group B). Best effects of oxygen therapy were observed in patient with lower pulmonary artery pressures at study entry.

The aim of this study was to assess the effect of chronic home oxygen therapy on cardiac function in patients with idiopathic interstitial lung fibrosis. Ten patients (Group A) received standard pharmacological treatment (prednisone 10-15 mg/day) and supplementary home oxygen therapy (mean 16.4 h/day) for the period of 4 years. Eight patients (Group B) received pharmacological treatment alone for the same period of time. Left ventricular ejection fraction was measured with a scintillation probe. Right and left ventricular function was also assessed by means of echocardiography. All measurements were taken at the beginning of study and repeated at the end of the 4 year follow-up. Anatomical and functional deterioration was slower in group A than in group B. Statistically significant differences between the groups at follow-up were found with respect to the right ventricle-early inflow velocity and tricuspid E/A ratio. Changes in left ventricular function were also different between the groups. A significant decrease in ejection fraction and mitral E/A ratio was observed in group B, while the corresponding changes in group A were not significant. These results suggest that chronic home oxygen therapy attenuates cardiac dysfunction in patients with idiopathic interstitial lung fibrosis.

RESUMO

The effect of Salbutamol (Polfa) inhalation on small airway function was studied in 20 patients with stage II sarcoidosis and 8 healthy volunteers. MEFV values and dynamic compliance were assessed. A significant statistical increase of FEV1, MEF50, Cdyn20 and Cdyn60 were seen only in healthy volunteers. In patients with sarcoidosis the assessed parameters were not effected by the Salbutamol inhalation.

The effect of hypobaric hypoxia on right ventricular pressure, right ventricular mass and hematocrit was assessed in Wistar rats divided into 4 groups. Group I comprised 20 control animals, kept in normal conditions. Groups II, III and IV consisted of 34-40 rats each at the beginning of the experiment. The animals were exposed to intermittent hypobaric hypoxia (380 mmHg, 10-12% O2, temperature 22 degrees C) 8 hours a day, 6 days a week, for 5 consecutive weeks. In group II no medication was given, in group III verapamil (Isoptin-Knoll) was administered in the drinking water at 30 mg/kg body mass/day, and in group IV nifedipine (Corotrend--Siegfried) was applied at 6 mg/kg body mass/day. Control animals were injected with i.p. nembutal (35 mg/kg) and then a polyethylene cannula was inserted in the right ventricle via external jugular vein, with consecutive measurement of right ventricular pressure. Hematocrit was measured in the arterial blood. After the animals had been sacrificed, the heart was isolated, right ventricle was excised and its mass was assessed as the ratio to mass of left ventricle-interventricular septum block. In groups II, III and IV right ventricular pressures were measured on days 10, 20 and 30 of the exposure to hypoxia. The hematocrit and ventricular mass were obtained at termination of the experiment. In the control group, mean right ventricular pressure was 28.5 +/- 3.5 mmHg, hematocrit 43.4 +/- 1.75% and right/left ventricular mass ratio was 0.267 +/- 0.03.(ABSTRACT TRUNCATED AT 250 WORDS)
