RESUMO

Urological complications of kidney transplantation remain a serious postoperative problem. They occur in 2-10% of cases. The authors present the data on 102 cadaveric and 74 syngenesioplastic transplantations of the kidney. 70 patients underwent surgery according to the alternative technique with ipsilateral nephrectomy and establishment of pyeloureteroanastomosis. Urological complications, lethal outcomes, destruction of the transplant occurred in 9.77, 11.11, 55.6% of the patients, respectively. Application of pyeloureteroanastomosis entailed complications in 4.28%, of ureteroneocystostoma in 13%, pyelopyeloanastomosis in 2 cases of 4. In urological complications pyeloureteroanastomosis often is the only feasible reconstructive operation saving the transplant. Its advantages make it treatment of choice in primary reconstruction of the urinary tracts in kidney transplant recipients with relatively low risk of urological complications.

RESUMO

The work has analyzed results of 91 transaortal endarterectomies and 66 prostheses of renal arteries with the autovein. A preliminarily arterialized autovein is proposed as an autotransplant. The arterialization is achieved by inclusion of the autovein in the peripheral arterial system where the vein is gradually transformed and within 10-12 weeks its structure and biomechanical properties become closer to those of the artery of the musculo-elastic type. Preliminary arterialization excludes dilatatory changes of the autovenous transport.

RESUMO

The analysis of case records of 24 injuried with crush syndrome is presented. The conclusion is drawn that it is necessary to start the treatment of such cases immediately after the removal of the traumatizing factor. These cases should be concentrated in medical institutions experienced in the use of program hemodialysis. The authors are convinced that hemodialysis should be applied when the organic nature of renal failure is revealed at the stage of oligoanuria. It has been shown that the fasciotomy incisions do not decrease the pronunciation of uremic intoxication, but they become the inlets for purulent infection in the injured extremity.
