RESUMO

AIM: Bronchopulmonary dysplasia (BPD) remains the most common respiratory morbidity in immature infants. This review describes the diagnosis of BPD has evolved and summarises the therapeutic approaches that have made it possible to limit the incidence of BPD. METHOD: We reviewed the literature from the first definition of BPD by Northway in 1967 to the surfactant treatment policies that are currently in use, drawing on more than 50 papers up to 2017. RESULTS: Our review showed that improvements in neonatal survival have been associated with an increased risk of severe BPD, significant levels of long-term morbidity and the increased use of healthcare resources. These issues have encouraged researchers to explore potential new treatments that limit the incidence of BPD. Repeated surfactant instillation and the use of surfactant as a vehicle for budesonide are promising strategies for alleviating the burden of chronic lung disease. Ongoing research on surfactant or stem cell therapy may further improve the respiratory prognosis for prematurely born children. CONCLUSION: Considerable research has been carried out into the increase in BPD, which has resulted from improvements in neonatal survival. Key areas of research include repeated surfactant administration, using surfactant as a vehicle for budesonide and stem cell therapy.

High-flow nasal cannula oxygen therapy (HFNC) is a technique for noninvasive ventilation commonly used in pediatric intensive care units for respiratory distress, particularly in acute bronchiolitis. HFNC was introduced in the pediatric emergency department of the Reims university hospital for the treatment of infants with moderate to severe acute viral bronchiolitis. This retrospective observational study aimed to investigate the use of HFNC in a pediatric emergency ward, describing the groups of infants with acute viral bronchiolitis to be treated either with conventional oxygen therapy or with HFNC therapy. A total of 89 infants aged less than 6 months were included between December 2013 and April 2014. The choice of the type of oxygen used belonged to the pediatrician, according to his or her assessment of the clinical severity of the patient's condition, guided by a protocol of HFNC use for the treatment of moderate to severe bronchiolitis and limited by the availability of a single device in the department. Eighty-nine infants were included; 64% were boys. Fifty-seven infants (64%) had no medical history. At admission, 39 infants (46.4%) had tachypnea and 75 (84.3%) intercostal indrawing. RSV was found in 75 infants (97.4% of infants had a positive respiratory virology test). Seventy-six infants (85.4%) were treated with conventional oxygen therapy and 13 (14.6%) with HFNC. At admission, the mean weight for infants treated with HFNC was lower (P=0.03), the oxygen saturation level was 86.7% versus 93.5% (P=0.03) for conventional oxygen therapy. Moreover, modified Wood's score was higher and the mean PCO2 was 61.1 versus 49.9mmHg (P<0.01). Six infants treated with conventional oxygen therapy (7.9%) and six with HFNC (46.1%) were then transferred to an intensive care unit. HFNC therapy in the pediatric emergency ward seems to be an advantageous therapeutic option in the early treatment of infants with moderate to severe bronchiolitis, but further studies are needed to specify its indications and effectiveness.

Noninvasive ventilation (NVI) is increasingly used in paediatrics, although there is a high variety of practices and a paucity of published data in paediatrics. The last French consensus conference recognized a specific role of NVI for infants suffering from acute bronchiolitis with apnoea, and acute respiratory failure due to laryngotracheomalacia and cystic fibrosis. NVI is feasible and can be beneficial in paediatric acute respiratory failure during neuromuscular diseases. Like in adults, its place in other diseases such as acute bronchoalveolitis without apnoea, acute respiratory failure during neuromuscular diseases, status asthmaticus, acute respiratory distress syndrome (ARDS) and postextubation respiratory failure is growing, even though not always defined. All these adult recommendations have not been validated in the pediatric setting, and, thus, taking the paediatric characteristics into account is essential. In 2010, NVI had an important place in PICU, and must be managed by a trained team whose practice is regularly evaluated.

UNLABELLED: Resuscitation of infants who are born at the threshold of viability remains highly controversial. The purpose of our study was to evaluate the French neonatologists' attitude toward the birth of an infant at less than 26 weeks' gestation (WG). METHOD: This article reports the results of a survey sent by the Internet to one of the neonatologists in each of the NICUs in France. The answers showed the different attitudes at the time of the birth of an extremely premature infant born less than 26 WG. RESULTS: The respondents represented 57 % of the original sample. None resuscitates an infant born between 22 WG+0 days and 22 WG+6 days. During the 23rd WG, the majority answer (90 %) was "Resuscitation is never undertaken". During the 24th WG, the main answer (29 %) was "Resuscitation is undertaken if neonatal adaptation is sufficient". During the 25th WG, several answers were given, but only 6 % chose "Resuscitation is never undertaken". CONCLUSION: Analyzing the survey's answers indicates that the decision of whether to resuscitate these premature infants at the threshold of viability must be adjusted not only by gestational age, but also by many other factors.

RESUMO

The quality of noninvasive ventilation in pediatrics is interface-dependent. Several types of interfaces are currently available: nasal and oral masks, nasal pillows and helmets. Despite material improvements in material design, shape, size and components, interfaces are still not adapted for most children. The ideal interface must fit the child's characteristics and the disease requirements. For instance, a nasal canula is recommended for infants younger than 3 months of age. If necessary, nasal masks can be used as oronasal masks. Repeated and careful evaluations are indicated to ensure interface adequacy and to detect cutaneous injuries and facial deformities. Training is required for medical and paramedical personnel. Pediatrics studies, comparing interfaces, are needed to build evidence-based recommendations.
