RESUMO

Folliculitis ulerythematosus reticulata (FUR) is an uncommon genodermatosis best classified as one of the keratosis pilaris atrophicans group of disorders. It is characterized by erythema and follicular plugging of the cheeks that evolves into honeycomb atrophy. Associated cutaneous and visceral abnormalities can occur. This report describes four patients with FUR, one with an unaffected twin, a finding not previously reported. The significance of an early and accurate diagnosis is emphasized.

Guinea pigs fed picryl sulfonic acid in aqueous solvents did not become unresponsive to contact sensitization with picryl chloride. In contrast, a substantial proportion of those fed picryl sulfonic acid, emulsified with triglyceride oil, were tolerant to subsequent immunization with picryl chloride. Feeding a related hapten (picryl chloride) in aqueous solvents was also considerably less effective in the induction of unresponsiveness to contact sensitization than feeding this hapten in vegetable oil. Animals fed picryl sulfonic acid in either aqueous or emulsified triglyceride solvents developed picryl-specific antibody after the initial feeding. However, those fed picryl chloride did not develop antibody and its role, if any, in the mechanism of unresponsiveness is unclear.

RESUMO

This report describes a patient with cytophagic histiocytic panniculitis in which the aggressive use of chemotherapeutic agents resulted in an apparent remission. The histiocytic nature of the process was confirmed by immunohistochemical techniques. There are many similarities with malignant histiocytosis, and it is believed that this entity should be regarded as a variant of malignant histiocytosis.

RESUMO

Three cases of pyoderma gangrenosum (PG) responsive to clofazimine are reported. The ulcers were recalcitrant to other forms of therapy, and the patients were incapacitated. Healing began within 2 or 3 weeks. To our knowledge, these are the first reported cases of PG responsive to clofazimine in the United States.

The infusion of 7,12 dimethylbenz(a)anthracene (DMBA) dissolved in DMSO:glycofurol into adult guinea pigs rendered most recipients unresponsive to the induction of contact sensitization with DMBA, either by injection in adjuvant or topical application. The results were similar when guinea pigs infused with DMBA dissolved in Upjohn fat emulsion, were challenged with DMBA in adjuvant, but studies attempting topical sensitization to demonstrate unresponsiveness were inconclusive. Mammary tumors appeared after a latent period of 8 to 11 mo in 4 guinea pigs infused with DMBA in fat emulsion in which topical sensitization with DMBA was attempted.

RESUMO

This report describes a case of secondary syphilis presenting with an atypical cutaneous eruption and acute spastic quadriparesis. The etiology of the cutaneous and neurologic manifestations was obscure until cerebrospinal fluid (CSF) studies revealed pleocytosis, elevated protein, and a positive CSF, VDRL, and fluorescent treponemal antibody (FTA). These findings, correlated with the histologic presence of plasma cells in the dermal infiltrate, indicated secondary syphilis with acute syphilitic meningitis as the cause of the patient's illness. Therapy with intravenous penicillin, 20 million units daily for 3 weeks, resulted in almost complete recovery.

The studies presented demonstrate that immunofluorescent techniques are capable of detecting picryl chloride erythrocyte protein conjugates formed in vitro and in vivo, following infusion of picryl chloride into normal, sensitized and tolerant guinea pigs. These experiments and the finding that picryl erythrocyte stromata prepared in vitro, and in vivo from infused recipients, have the capacity fo sensitize fresh animals, support the view that infused picryl chloride conjugates with the red cell membrane in vivo.

Recovery of living tubercle bacilli in a lesion of 35 years' duration is intriguing if not novel, and creates many possible rationalizations relative to the relentless progressive course of lupus vulgaris. That the histopathological picture is not always diagnostic is exemplified in this case.

RESUMO

Microscopical examination of biopsy specimens from cat-scratch skin test sites, performed in patients with cat-scratch disease (CSD), sarcoidosis, tuberculosis, and other granulomatous diseases, showed noncaseating granulomatous dermal inflammation in 11 of 12 patients with CSD and in some patients with sarcoidosis or tuberculosis. Biopsy of cat-scratch skin test sites may be a useful ancillary procedure in diagnosing CSD if other granulomatous diseases are excluded.

RESUMO

Guinea pigs receiving one large dose of picryl chloride by the intravenous or oral routes commonly develop circulating antibody demonstrable by passive cutaneous anaphylaxis or by active anaphylaxis. They often concommittantly become unresponsive to the induction of delayed contact hypersensitivity by intracutaneous injections. Erythrocytes obtained from guinea pigs after infusion or feeding of picryl chloride may be used to sensitize other animals when injected with adjuvant. It is concluded that guinea pigs may be anaphylactically sensitized to simple chemicals by the intravenous and oral routes if a sufficient dose is administered.
