RESUMO

AIM: To evaluate clinical efficacy of combined anterior prosthetic hernia repair in treatment of large and giant ventral hernias. MATERIAL AND METHODS: Patients with large and giant hernias have been analyzed. In the main group (n=675) combined methods of hernia gates repair were applied, in control group (n=257) - stretching repair including prosthesis deployment. Surgeons (n=22) were interviewed for learning curve, safety, limitations and reliability of combined methods. RESULTS: Combined method of hernia repair in patients with large and giant hernias reduces overall postoperative morbidity (p<0.001), wound complications (p<0.05) and incidence of recurrent hernia (p<0.001). Questionnaire data showed the possibility of learning for the method by the most of surgeons to treat these patients. As a result, some practical recommendations are presented for successful procedures and satisfactory results of treatment.

RESUMO

AIM: To evaluate the biocompatibility of additive materials for personified endoprostheses of hand joints in vivo. MATERIAL AND METHODS: We tested a material based on titanium that was implanted into muscles and bone tissue in experiment on rabbits. Follow-up was 30 and 90 days. RESULTS: Implantation into muscle tissue is accompanied by reaction against foreign body followed by fibrosis without concomitant inflammation. Induction of osteogenesis and trabecular structures remodeling were detected after implantation into bone tissue. CONCLUSION: Biocompatibility of tested titanium-based material was confirmed.

RESUMO

Titanium and polypropylene mesh endoprostheses were implanted into the rat abdominal cavity and the populations of cells migrating to their surface were analyzed. On both materials, the cells were presented mainly by macrophages that proliferated, activated, and fused to form multinuclear cells. Contact with the foreign surface triggered superoxide anion generation and myeloperoxidase synthesis by macrophages (these processes were more intense on polypropylene implants than on titanium ones) and stimulated production of nitric oxide by macrophage. It was hypothesized that the effects of free nitrogen and oxygen radicals lead to oxidation and destruction of the polypropylene endoprosthesis surface.
