RESUMO

Chronic kidney disease (CKD) affects approximately 14% of adults in the United States and is present in at least 10% of the population worldwide. Blood glucose and blood pressure control are imperative to adequately manage CKD as they are the only primary prevention measures for the condition. Recent changes in CKD evaluation and medication therapies that modify disease progression and aid in managing complications such as anemia of CKD have emerged, including a newly approved mineralocorticoid receptor antagonist and hypoxia-inducible factor-prolyl hydroxylase inhibitor, respectively. This focused update on CKD evaluation and management will review the most recent evidence and approved agents to support patients with CKD, including a review of glomerular filtration rate measurement methods such as CKD-EPI 2021 and utilization of cystatin C, Kidney Disease Improving Global Outcomes (KDIGO) guidelines, American Diabetes Association (ADA) guidelines, and primary literature supporting the use of newer agents in CKD. Checklists for managing blood pressure and blood glucose, CKD-mineral bone disorder, and anemia of CKD targeted for pharmacists are also provided. Additionally, a discussion of Centers for Medicare & Medicaid (CMS) coverage of agents approved for managing complications of CKD is included.

BACKGROUND: Oral squamous cell carcinoma (OSCC) is the most prevalent subtype of oral cancer. Detecting oral potentially malignant disorders (OPMDs) in their early stages is crucial to prevent their advancement into OSCC. One of the primary factors contributing to OSCC is tobacco use, which can lead to increased production of cytokines. Among these cytokines, interleukin-6 (IL-6), an immune molecule involved in inflammation, may serve as a valuable indicator for assessing the progression of OPMDs and OSCCs. AIMS: The aim of this study is to assess the levels of IL6 in both serum and saliva using the enzyme-linked immunosorbent assay (ELISA) technique and to determine the prognostic value of these measurements in individuals with oral leukoplakia and OSCC. MATERIALS AND METHODS: The research involved 45 participants, who were categorized into three groups: OSCC (15), leukoplakia (15), and a control group consisting of healthy individuals (15). Saliva and serum samples were collected from each individual within all three groups and analyzed using the ELISA method. Subsequently, the gathered data underwent statistical analysis for evaluation. RESULTS: There were elevated levels of IL-6 in both saliva and serum among individuals with OSCC in comparison to those with leukoplakia and the healthy control group, and this difference was statistically significant. The analysis of ROC (Receiver Operating Characteristic) curves demonstrated that salivary IL-6 was a more effective indicator than serum IL-6 for detecting the advancement of OSCC. As the histological grade of differentiation increased in both OSCC and leukoplakia cases, there was a corresponding rise in salivary IL-6 levels. CONCLUSION: Both salivary and serum IL-6 levels have the potential to serve as valuable prognostic biomarkers for oral leukoplakia and OSCC which shows possible involvement of IL-6 in the development and progression of these conditions. Salivary IL-6 is a superior prognostic marker compared to serum IL-6 due to its non-invasive nature which makes it a useful tool for mass screening.

OBJECTIVE: The objective was to review the safety and efficacy of daprodustat, a hypoxia-inducible factor-prolyl hydroxylase inhibitor (HIF-PHI) in the treatment of anemia of chronic kidney disease (CKD). DATA SOURCES: A literature search was conducted in MEDLINE, EMBASE, and ClinicalTrials.gov using the keywords "daprodustat," "GSK1278863," and "hypoxia-inducible factor-prolyl hydroxylase inhibitors" from January 2010 through November 2023. STUDY SELECTION AND DATA EXTRACTION: Literature was included if it evaluated pharmacology, pharmacokinetics, efficacy, and/or safety of daprodustat in human subjects and was reported in English. The manufacturer's product monograph was also utilized. DATA SYNTHESIS: Daprodustat significantly increased hemoglobin levels in CKD patients on dialysis (difference 0.18 g/dL) and not on dialysis (difference 0.08 g/dL) over 52-week treatment periods compared with erythropoiesis stimulating agents (ESA) in Anemia Studies in CKD: Erythropoiesis via a Novel PHI Daprodustat (ASCEND)-D and ASCEND-ND, respectively. First occurrence of major adverse cardiovascular events (MACEs) was similar between daprodustat and ESAs in both trials. RELEVANCE TO PATIENT CARE AND CLINICAL PRACTICE IN COMPARISON TO EXISTING DRUGS: Daprodustat can be used in patients with CKD on dialysis and already receiving an ESA for at least 6 weeks to further increase serum hemoglobin levels without increasing the risk of MACE. Adverse effects of daprodustat that may occur more than ESAs include headache, emesis, and thrombosis. CONCLUSIONS: Daprodustat is a novel oral, non-iron therapy for treatment of anemia of CKD. It was Food and Drug Administration approved in 2023 in patients already receiving dialysis for at least 4 months but not in non-dialysis patients. Long-term data for safety and additional benefits are pending.

The significance of 3D printing has risen exponentially in biomedical and pharmaceutical applications. Its potential in the field of fabricating drug delivery systems, by virtue of processing biocompatible polymers, has been very lucrative. This work aims to tap the interstitial drug delivery kinetics that are often inaccessible through machine-specific infill patterns in additive manufactured tablets fabricated using PVA biopolymer as an excipient. In this regard, a myo-inositol containing tablet has been printed using Fused Deposition Modeling preceded by Hot Melt Extrusion drug loading route. Two machine-specific infill patterns were taken, namely straight and grid. Later, these two distinct patterns were juxtaposed to obtain novel hybrid infill patterns in the tablets. Then, these tablets and their filament were subjected to various thermal, mechanical, imaging and pharmaceutical characterization tests to assess the feasibility of the research attempt. Finally, dissolution tests were conducted to evaluate their dissolution behavior over a time period. The characterization tests proved the scientific viability of this attempt along with amorphous existence of drug in the polymeric filament. The dissolution results showed favorable drug release by achieving interstitial dissolution timings with surface area/volume (SA/V) ratio being found to be the principal factor.

A 46-year-old man with a prior history of cervical spondylosis and myelopathy needing cervical spinal surgery three years back presented to the emergency department with acute onset areflexic flaccid weakness of both lower extremities, with a sensory level at T10. Magnetic resonance imaging studies (MRI) of the cervical, thoracic, and lumbar spine ruled out significant cord compression, spinal cord ischemia, spinal shock, or findings to suggest transverse myelitis. CSF analysis showed normal albumin and protein; however, with the features of paraplegia with flaccidity, areflexia, absence of bowel and bladder symptoms, and MRI ruling out other possibilities, a diagnosis of Guillain-Barre syndrome (GBS) was made. The patient was treated with intravenous immunoglobulin (IVIG) and showed a clinical response, with improvement in strength in both lower extremities. This case is rare and unique, as it exhibits atypical features for a GBS case, including a sensory level and hyper-acute presentation, with the onset of weakness to a nadir within an hour. This case highlights the importance of awareness of such atypical GBS presentations so that the diagnosis is not missed and is appropriately managed for favorable patient outcomes.

Developing countries like India are rapidly transitioning from traditional energy sources to sustainable energy sources, due to the increase in demand and the depletion of fossil fuels. Grid-connected photovoltaic (PV) systems attract many investors, organizations, and institutions for deployment. This article studies and compares the performance evaluations of three 52-kW PV plants installed at an educational institution, SRMIST (SRM Institute of Science and Technology), in Tamil Nadu, India. This site receives an annual average temperature of 28.5°C and an average global horizontal irradiation of 160 kWh/m2/m. The prediction model for the 52-kW power plant is obtained using solar radiation, temperature, and wind speed. Linear regression model-based prediction equations are derived using the Minitab 16.2.1 software, and the results are compared with the real-time AC energy yield acquired from the three 52-kW plants for the year 2020. Furthermore, this 52-kW plant is designed using PVsyst V7.1.8 version software. The simulation results are compared with the energy yield from the plants in 2020 to identify the shortfall in the plant performance. The loss analysis for the plant is performed by obtaining the loss diagram from the PVsyst software. This study also proposes a methodology to study the commissioned PV plant's performance and determine the interaction between variables such as direct and diffused solar radiations, air temperature, and wind speed for forecasting hourly produced power. This article will motivate researchers to analyze installed power plants using modern technical tools.

Diabetic peripheral neuropathy (DPN) is a major complication of diabetes mellitus. Tight glycaemic management focused on lowering haemoglobin A1C and increasing time in the target glucose range along with metabolic risk factor management form the cornerstone of DPN prevention. However, there is limited evidence supporting the efficacy of glycaemic and metabolic control in reducing the symptoms and complications of DPN, including pain once painful DPN develops. DPN treatments include pharmacological agents and non-pharmacological interventions such as foot care and lifestyle modifications. Pharmacological agents primarily address pain symptoms, which affect 25-35% of people with DPN. First-line agents include the anticonvulsants pregabalin and gabapentin, the serotonin-norepinephrine reuptake inhibitors duloxetine and venlafaxine, and secondary amine tricyclic antidepressants, including nortriptyline and desipramine. All agents have unique pharmacological, safety and clinical profiles, and agent selection should be guided by the presence of comorbidities, potential for adverse effects, drug interactions and costs. Even with the current treatment options, people are commonly prescribed less than the recommended dose of medications, leading to poor management of DPN symptoms and treatment discontinuation. By keeping up with the latest therapy algorithms and treatment options, healthcare professionals can improve the care for people with DPN.

The aim of this work was to study on MgO-modified KOH activated biochar (AC) catalysts, in the pyrolysis of sawdust for the direct production of bio-jet fuels using a tandem micro-pyrolyzer. AC catalysts with various MgO contents (5 to 20 wt%) were synthesized using an impregnation method. The mesopores generated (4 to 18 nm) in the carbon has a great potential in the conversion of oxygenated to jet fuel. The importance of basic nature in the catalysts is demonstrated with the maximum bio-jet fuel yield of 29 % at 10 % MgO. Further, the temperature of 600 °C and a catalyst/sawdust ratio of 10 are identified as the optimal conditions. The nanosize of MgO and the synergism of acid and base sites seemed to enhance deoxygenation, via decarboxylation and decarbonylation, and oligomerization, which are required for jet fuel formation in high amounts from sawdust pyrolysis.

Many new technologies have been developed over the past decade, and these have substantially changed the way diabetes is managed. Continuous glucose monitoring is now the standard of care for many people living with diabetes, and among its numerous benefits, it has been shown to improve glycaemic outcomes and enhance quality of life. Older adults carry a high burden of diabetes and have a high risk of hypo-glycaemia and hypo-glycaemic unawareness, and continuous glucose monitoring can help to improve glycaemic management in this vulnerable population. Unfortunately, only a few trials have evaluated the effectiveness of continuous glucose monitoring in older adults. Certainly, the implementation of continuous glucose monitoring in older adults can come with many challenges, including logistical, educational and reimbursement barriers. This article will discuss the benefits of continuous glucose monitoring in older adults with diabetes, the clinical studies that support its use and the barriers to its optimal implementation in this population.

BACKGROUND: Undernutrition among adolescents is a major public health problem in developing nations including Ethiopia. Adolescents need to have good-quantity and good-quality nutrients to cope with this rapid growth and other health risks which increase nutritional demand. This study aimed to assess the prevalence and associated factors of stunting and thinness among school adolescents in Finote Selam Town, Northwest Ethiopia. METHODS: A school-based cross-sectional study among adolescent students aged 10-19 in public schools was carried out in Finote Selam Town from February 5 to March 27, 2018. A total of 397 school adolescents were included in this study. Stratified and simple random sampling techniques were employed to select study subjects. Pretested structured questionnaires were used to collect the data. Data were entered using Epi info version 7 and analyzed using SPSS version 20 and WHO AnthroPlus software. A multivariable logistic regression analysis was employed to identify factors associated with stunting and thinness. Crude and adjusted odds ratios with 95% level significance were used to measure the strength of association, and statistical significance was declared at a P value less than 0.05. RESULTS: The prevalence of stunting and thinness among school adolescents was 21.8% and 16.9%, respectively. About 9.7% of school adolescents experienced both stunting and thinness. Being male (P = 0.031), coming from rural area (P = 0.046), having a family monthly income of less than $28.37 (P = 0.044) and having less than four dietary diversity (P = 0.021) were significantly associated with stunting. Early adolescent stage (P = 0.034), being male adolescent students (P = 0.37), having a family monthly income of less than $28.37 (P = 0.016), having a family monthly income between $28.37 and $56.74 (P = 0.021) (35.25 Birr = 1 USD) and using well water (P = 0.045) were significantly associated with thinness. CONCLUSION: Undernutrition was a predominant problem in the study area. Sex, age, place of residence, dietary diversity, sources of drinking water and family monthly income were important factors associated with stunting and thinness among the respondents. Strategies to improve the nutritional status of adolescent students should be given much attention.

Alzheimer's disease (AD) is one of the most prominent neurodegenerative diseases, which impairs cognitive function in afflicted individuals. AD results in gradual decay of neuronal function as a consequence of diverse degenerating events. Several neuroimmune players (such as cytokines and growth factors that are key players in maintaining CNS homeostasis) turn aberrant during crosstalk between the innate and adaptive immunities. This aberrance underlies neuroinflammation and drives neuronal cells toward apoptotic decline. Neuroinflammation involves microglial activation and has been shown to exacerbate AD. This review attempted to elucidate the role of cytokines, growth factors, and associated mechanisms implicated in the course of AD, especially with neuroinflammation. We also evaluated the propensities and specific mechanism(s) of cytokines and growth factors impacting neuron upon apoptotic decline and further shed light on the availability and accessibility of cytokines across the blood-brain barrier and choroid plexus in AD pathophysiology. The pathogenic and the protective roles of macrophage migration and inhibitory factors, neurotrophic factors, hematopoietic-related growth factors, TAU phosphorylation, advanced glycation end products, complement system, and glial cells in AD and neuropsychiatric pathology were also discussed. Taken together, the emerging roles of these factors in AD pathology emphasize the importance of building novel strategies for an effective therapeutic/neuropsychiatric management of AD in clinics.

Burnout remains an emerging challenge in healthcare. Burnout in these settings has been associated with compromised personal wellbeing, decreased work productivity, higher healthcare costs, decreased patient satisfaction, and deteriorating quality of patient care. As pharmacist roles on healthcare teams expand, health-system pharmacy leaders should be cognizant of burnout, including risk factors and identification. Coordinated efforts should foster pharmacist wellbeing and resilience, individualized work-related responsibilities to ensure professional ambitions are met, promotion of upper leadership expectation transparency, and cultivation of relationships within the pharmacy department.

The selective laser melting (SLM) process, a kind of metal additive manufacturing method, can produce parts with complex geometries that cannot be easily manufactured using material removal processes. With increasing industrial applications, there are still issues such as part quality and productivity that need to be resolved. In this study, maraging steel parts fabricated by synchronized three-spot scanning strategies, i.e., lateral spatial (LS) and spatial inline (SiL), are firstly presented. The LS and SiL represent the three-spot offset direction is perpendicular and parallel to the scanning direction, respectively. A laboratory SLM machine equipped with a fiber laser and three-spot module is used to fabricate the maraging steel parts with two scanning strategies, i.e., LS and SiL. The influence of these scanning strategies on the surface roughness, relative density, hardness, molten pool shapes, and microstructures are investigated. The relative density (~99.02%) and surface hardness (~34.0 HRC) are experimentally found to be higher than the SiL by the LS scanning strategy.

Heterogeneous catalysis has currently become an emerging tool for the design and development of sustainable manufacturing processes in order to obtain advanced intermediates, fine chemicals, and bioactive molecules. This field has been considered efficient and eco-friendly, as it investigates the utilization of non-hazardous metals for atom-economical reactions. Nanomaterials have created a significant impact on scientific and engineering advancements due to their tunable properties with superior performance over their massive counterparts. Due to the increased demand for heterogeneous catalysts in industries and academia, different transition metal oxides have been made into substantial nanostructures. Among them, titanium dioxide (TiO2) nanomaterials have received more attention on account of their chemical stability, low cost, dual acid-base properties, good oxidation rate and refractive index. Different modifications of TiO2 extend their applications as active catalysts or catalyst supports in diverse catalytic processes, such as photovoltaics, lithium batteries, pigments and others. One-dimensional (1-D) TiO2 nanostructures such as nanotubes, nanowires and nanorods have achieved greater importance owing to the unique properties of improved porosity, decreased inter-crystalline contacts, large surface-to-volume ratio, superior dispersibility, amplified accessibility of hydroxyl (-OH) groups and presence of good concentrations of Brønsted/Lewis acid sites. Since the discovery, 1-D TiO2 nanostructures have served good photocatalytic applications, but were less explored in organic transformations. While many articles and reviews have covered the applications of 0-D and 1-D TiO2 nanostructured materials (NSMs) in photoelectrochemical reactions and solar cells, there are other interesting applications of these as well. In contrast to the conventional multi-step processes that utilise the stepwise formation of individual bonds, one-pot conversions based on multicomponent reactions (MCRs) have acquired much significance in contemporary organic synthesis. This paper presents a critical review on history, classification, design and synthetic utility of titania-based nanostructures, which could be used as robust solid-acid catalysts and catalyst supports for MCRs. Further, to put ideas into perspective, the introduction and applications of MCRs for various organic transformations have been discussed.

C-C cross coupling reactions have been widely used for developing synthesis protocols for pharmaceuticals and agricultural products in the past few decades. Of all the reported C-C cross coupling reactions, the Suzuki-Miyaura reaction is preferred because of its mild reaction conditions, the commercial availability of associated reagents and the ease of removal of boron containing by-products. Recently, Corma and co-workers [Leyva-Perez et al., Angew. Chem., 2013, 125, 11768] reported water-stabilized three- and four-atom Pd clusters as highly active catalytic species for C-C coupling reactions. The present work focuses on developing detailed mechanistic insights into the Suzuki-Miyaura reaction with Pd3 and Pd4 clusters utilizing density functional theory calculations. The role of the base in the reaction was analysed in this study, which was found to lower the activation barriers of transmetalation over both Pd3 and Pd4. Free energy landscapes for Suzuki-Miyaura coupling of bromobenzene and phenylboronic acid over Pd3 and Pd4 clusters were developed. The highest free energy barriers of 34.7 and 30.4 kcal mol-1 were observed for the oxidative addition over Pd3 and Pd4, respectively, indicating the oxidative addition as the rate limiting step. Detailed energetics conclusively proved the active nature of small-atom Pd clusters for catalyzing the Suzuki-Miyaura reaction.

The traditional healthcare method follows 'one-size-fits-all' criteria, which shows some limitations in rendering proper results due to varied pharmacokinetic traits of different patients. Each patient shows a different result to a set of prescribed medicines with respect to other patient. In addition, this empirical criterion is also vulnerable to develop unfavourable side-effects due to overdose or under-dose. The advancements in the field of pharmacogenetic testing has paved the way for searching solutions to achieve the goal of 'personalized medicine'. This concept has the potential to provide more effective results as compared to the traditional technique. The technology of additive manufacturing or 3D printing can cater the need of the patient-centric principle. A wide range of 3D printing techniques provide the independence to work with a number of bio-materials. This paper carries out a systematic review methodology based on the application of this novel technology in the field of drug delivery. The materials, methods of drug administration, techniques used for printing have been elaborated along with the manufacturing of polypills with varied release profiles and geometries. Certain in vivo studies has also been reported to assess the feasibility of this technology. The concept of 4D printing in this direction has also been discussed. Finally, some excerpts on estimated future scope have also been illustrated.

PURPOSE: Earlier imaging techniques for coronary artery disease (CAD) focused primarily on either morphological or functional assessment of CAD. However, dual-energy computed tomography (DECT) can be used to assess myocardial blood supply both morphologically and functionally. We aimed to evaluate the diagnostic accuracy of DECT in detecting morphological and functional components of CAD, using invasive coronary angiography (ICA) and single photon emission computed tomography (SPECT) as reference standards. METHODS: Twenty-five patients with known or suspicious CAD and scheduled for ICA were investigated by DECT and SPECT. DECT was performed during the resting state using retrospective electrocardiography (ECG) gating. CT coronary angiography and perfusion images were generated from the same raw data. All patients were evaluated for significant stenosis (≥50%) on both ICA and DECT coronary angiography, and for myocardial perfusion defects on SPECT and DECT perfusion. Comparison was done between ICA and DECT coronary angiography for detection of significant stenosis and between SPECT and DECT perfusion for detecting myocardial perfusion defects. RESULTS: Using ICA as reference standard, sensitivity, specificity, and accuracy of DECT coronary angiography in detecting ≥50% stenosis of coronary artery lumen were 81.6%, 97.8%, and 95.0%, respectively, by segment-based analysis and 92.1%, 96.1%, and 93.7%, respectively, by vessel-based analysis. Using SPECT as the reference standard, the sensitivity, specificity, and accuracy of DECT perfusion in detecting myocardial perfusion defects were 70.4%, 86.4%, and 80.6%, respectively, on per-segment analysis and 90.7%, 66.6%, and 84.7%, respectively, on per-territorial basis. CONCLUSION: DECT accurately detected coronary artery stenosis and myocardial ischemia using ICA and SPECT as reference standards. In the same scan, DECT can accurately provide integrative imaging of coronary artery morphology and myocardial perfusion.

The present study examined the effects of the pyrolysis environment on BTEX (benzene, toluene, ethylbenzene, and xylenes) production in the catalytic upgrading of yellow poplar pyrolysis vapors. Three different gas environments, N2, CH4, and pre-decomposed CH4 stream (10 wt%-Ni/5 wt%-La2O3-5 wt% CeO2-Al2O3), which is a mixture of H2 (55.62%) and CH4, were studied using two types of zeolite catalysts, HZSM-5, and 1 wt% Ga/HZSM-5. The BTEX yields were enhanced linearly in the order N2 < CH4 < CH4 ex-situ decomposition. The highest BTEX yield of 9.58 wt% was obtained under the CH4 ex-situ decomposition environment over 1 wt% Ga/HZSM-5. The methane and hydrocarbons derived from biomass were activated on highly dispersed (GaO)+ sites and transformed smoothly to BTEX by aromatization on the BrØnsted acid sites of Ga/HZSM-5. The hydrogen produced from methane decomposition also assisted in aromatics production through the hydrodeoxygenation of methoxyphenols, guaiacols and catechols.

Even if adolescence is a window of opportunity to break the intergenerational cycle of malnutrition, adolescents are the neglected age groups. Hence, information regarding the nutritional status of adolescents is lacking, making creating and implementing intervention programs difficult. This study aimed to assess the prevalence and determinants of thinness among school adolescents in Finote Selam Town, Northwest Ethiopia. A school-based cross-sectional study among adolescent students aged 10-19 in public primary and secondary schools was carried out in Finote Selam Town from February 05 to March 27, 2018. Stratified and simple random sampling techniques were employed to select study subjects. A total of 397 adolescent students were included in the study. Pretested structured questionnaires were used to collect the data. Data were entered using Epi Info version 7 and analyzed using SPSS version 20 and WHO AnthroPlus software. A multivariable binary logistic regression analysis was employed to identify factors associated with thinness. Crude and adjusted odds ratios with 95% level significance were used to measure the strength of association, and statistical significance was declared at p value less than 0.05. The prevalence of thinness among adolescents was 14.9%. Early adolescent stage (AOR = 4.81; 95% CI : 1.23, 18.51), being male adolescent students (AOR = 2.33; 95% CI : 1.60, 3.40), having less than 1000 birr family monthly income (AOR = 6.54; 95% CI : 3 : 82, 14.89), having 1000-2000 birr family monthly income (AOR = 3.47; 95% CI : 1.15, 7.45), and using well water (AOR = 3.82; 95% CI : 1.46, 10.04) were significantly associated with thinness at 95% confidence interval. The study revealed that prevalence of thinness was high in the study area. Sex, place of residence, and family monthly income were found to be important factors associated with thinness among the respondents.
