Proctitis y úlcera ano-rectal como presentación inicial en pacientes con Viruela símica

Objetivo: Describir hallazgos clínicos, quirúrgicos e imagenológicos de la patología anorrectal en pacientes con Viruela Símica. Materiales y Métodos: Reporte de serie de casos clínicos, observacional, en pacientes con Viruela Símica tratados en un centro de salud privado desde agosto 2022 a diciembre 2022. Resultados: El 100% de los casos pertenecía al sexo masculino, la edad promedio fue de 32 años, todos los pacientes mantuvieron relaciones sexuales con otros hombres, la mayoría debuto con síntomas rectales, incluyendo proctalgia, prurito anal y heces diarreicas con mucosidad. El diagnóstico se confirmó mediante PCR (reacción de polimerasa en cadena) y el tratamiento fue sintomático, con mejoría en todos los pacientes en un seguimiento de 10 días. Discusión: La Viruela Símica es una enfermedad viral que se encuentra, principalmente, en regiones remotas de África Central, con brotes esporádicos en países africanos. Está relacionada con el virus de la viruela y se transmite, principalmente, del contacto con animales infectados o fluidos corporales. La enfermedad presenta síntomas similares a la gripe, seguidos del desarrollo de una erupción cutánea que progresa a pústulas y costras. No hay tratamiento específico para la viruela símica, el manejo medico alivia los síntomas. El brote de viruela símica en 2022 presentó características novedosas y atípicas en su forma de presentación. Conclusión: Los síntomas rectales asociados a una historia clínica compatible con viruela símica se deben considerar como diagnóstico diferencial a la proctitis por esta etiología específica y emergente.

Objective: To describe clinical, surgical and imaging findings of anorectal pathology in patients with monkeypox. Materials and Methods: Observational clinical case series report in patients with monkeypox treated in a private health center from August 2022 to December 2022. Results: 100% of the cases were male, the average age was 32 years old, all patients had sexual intercourse with other men, most debuted with rectal symptoms, including proctalgia, anal itching, and diarrheal stools with mucus. Diagnosis was confirmed by PCR (polymerase chain reaction) and treatment was symptomatic, with improvement in all patients at a 10-day follow-up. Discussion: Monkeypox is a viral disease found mainly in remote regions of Central Africa, with sporadic outbreaks in African countries. It is related to the smallpox virus and is transmitted primarily from contact with infected animals or bodily fluids. The disease presents with flu-like symptoms, followed by the development of a skin rash that progresses to pustules and scabs. There is no specific treatment for monkeypox; medical management alleviates the symptoms. The monkeypox outbreak in 2022 presented novel and atypical characteristics in its presentation. Conclusion: Rectal symptoms associated with a clinical history compatible with monkeypox should be considered as a differential diagnosis of proctitis due to this specific and emerging etiology.

Nonablative transurethral Erbium:YAG laser treatment for chronic prostatitis/chronic pelvic pain syndrome: A prospective comparative study.

AIMS: This prospective study aimed to compare the clinical outcomes between the use of Erbium:YAG (Er:YAG) laser in a nonablative mode, to the use of the pharmacological treatment of oral tadalafil for the treatment of chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS). METHODS: The laser group received two sessions of Erbium:YAG laser, administered intraurethrally in a long, nonablative train of long pulses (SMOOTH™ mode), applied at the level of the male prostatic urethra. Tadalafil group received oral tadalafil at a dose of 5 mg/day, consecutively for 2 months. Effectiveness was assessed using the International Prostate Symptom Score (IPSS) questionnaire, VAS (visual analogue scale) pain score, and maximum urethral flow at follow-up visits up to 12 months after initiating treatment. Adverse effects were recorded after each treatment and follow-up sessions. RESULTS: The results show a significant decrease in the IPSS score in both groups up to the 12-month follow-up. The increase in Q-max was evident up to 3-months follow-up in the tadalafil group and up to 6 months in the laser group. The decrease in the VAS pain score was also significant in both treatment groups, lasting up to 3 months in the tadalafil group and up to 6 months in the laser group. CONCLUSIONS: The nonablative Er:YAG SMOOTH™ laser seems to be a promising treatment for this widely occurring condition. More studies are needed to confirm its safety and efficacy.

Caracterización de los adolescentes comisores del intento suicida del Consejo Popular Siboney. Octubre de 2010 a Marzo de 2011 / Characterization of the adolescents with suicide attempt in the Popular Council of Siboney. October, 2010 to March, 2011

Se realizó un estudio cuantitativo con el objetivo de identificar los factores que a nivel personal están asociados a la comisión del intento suicida en un grupo de adolescentes del consejo popular Siboney perteneciente al policlínico docente René Vallejo Ortíz de Bayamo en el período de Octubre de 2010 a Marzo de 2011. El universo está compuesto por 16 adolescentes que a su vez constituyen la muestra con edades comprendidas entre 12 y 19 años comisores de actos suicida. La importancia de nuestra investigación radica en el análisis psicológico presente en esta etapa como proceso que dinamiza la conducta suicida en el adolescente, así como su percepción del funcionamiento familiar en el cual se encuentra inmerso. Para el desarrollo de la investigación los objetivos fueron evaluados mediante la aplicación de las siguientes técnicas: análisis documental, entrevistas semiestructuradas a (adolescentes y familia), observación, FF-sil, Rotter y 10 deseos. Como resultado se obtuvo que en la comisión del intento suicida de estos adolescentes influyeron de manera significativa la percepción que ellos tienen de su funcionamiento familiar y las necesidades presentes(AU)

It was performed a quantitative study with the objective to identify the factors that were personally associated with the suicide attempt in a group of adolescents from the Popular Council Siboney, belonging to René Vallejo Ortiz Polyclinic in Bayamo during the period October 2010 to March 2011. The universe was formed by 16 adolescents who also constituted the sample in ages between 12 and 19 years with suicide attempt. The significance of our research consisted on the psychological analysis at this stage as a process that rushed the suicidal behavior in adolescents and their perception of the family functioning in which they were immersed. For the development of this research the objectives were assessed through the application of the following techniques: documentary analysis, semi-structured interviews (adolescent and family), observation, FF-sil, Rotter and 10 wishes. As a result it was found that in the suicide attempt of these adolescents there was a significant influence in their perception of their family functioning and present needs(EU)

Trombolisis en pacientes con trombosis de la prótesis valvular mecanica: presentación de tres casos

Se presentan tres casos, masculinos, cuyo elemento común desencadenante de la trombosis valvular mitral protésica fue el suspender el anticoagulante (Warfarina). Dos se manifestaron clínicamente por insuficiencia cardíaca severa y uno por embolismo cerebral. A los tres se les realizó tratamiento trombolítico con estreptoquinasa recombinante, la dosis inicial empleada fue de 250000 UI, administradas en 30 minutos por vía endovenosa periférica, continuándose con 100000 UI por hora durante 72 horas. Luego se continuó con heparina en infusión continua y Warfarina vía oral. Las manifestaciones iniciales de insuficiencia cardiaca desaparecieron y el ecocardiograma evolutivo mostró movimiento de excursión amplia del disco de la prótesis mitral. Según la literatura revisada y la buena evolución de nuestros casos la terapia trombolítica debe ser considerada como primera opción, permitiendo preparar el paciente para la cirugía si fuese necesario y protegerlos de embolias sistemáticas(AU)
