RESUMO

The histologic examination of 556 gastric biopsies performed in patients with chronic gastritis, revealed the presence of intestinal metaplasia in 81 cases (14.5%). This was correlated with the evolutive stage of gastritis, the maximum incidence being observed in the case of gastric atrophy (24.7%). The iterative examination of the patients with intestinal metaplasia, allowed the determination of its evolutive tendency. After an average period of 5 years, the occurrence of gastric malignoma was observed in 5 cases (6.3%) thus confirming the oncogenic risk of intestinal metaplasia and justifying the careful "surveillance" of these cases.

RESUMO

An early detection of the progress to chronic stages of the acute diseases has particular pathogenic and therapeutic implications in hepatology. A complex clinical, biologic and morphologic study has been carried out in 86 patients with persistent acute hepatitis, recently released after 4-8 weeks of hospitalization for acute viral hepatitis, who were submitted to sequential investigations and follow up for a mean period of 16 months (range 2-24 months). All the patients have shown signs of activity of the liver injury, more than three months after the viral hepatitis onset (GOT/GPT 134 +/- 41 KU/219 +/- 59 KU, gammaglobulins 24.2 +/- 2.4%) and a characteristic immune pattern. In some cases, the morphologic investigations (endo-histologic and infrastructural) have revealed elements of acute and chronic active hepatitis. In 70% of the case the disease had a favourable course, while 30% of them showed a tendency to chronicization and even to cirrhosis, within a period of two years.

From the about 3000 laparoscopies effected in our Department, 276 liver tumors (9.2%) were detected. Of these, 47 (17%) were primary adenocarcinomata (28), or adenocarcinomata grafted on cirrhosis (19). The large majority (195 cases: 70.6%) consisted of metastases of digestive (gastric, large intestine, pancreatic) or extradigestive (breast, ocular, genital) tumors. In 34 cases the tumor formations where represented by hydatic cysts, abscesses and hemangiomata (3 cases). The decisive contribution of peritonescopy to the diagnosis of the primary or the metastatic liver cancer as well as of other liver tumors is obvious.
